[Synchronous double cancer of the esophagus and the urinary bladder: report of two cases].

An 82-year-old man was seen with the complaints of gross hematuria and dysphagia in September 1979. An invasive bladder tumor was found and TUR-Bt (Transitional cell carcinoma, G2, pT3NXMO ) was performed. Fluoroscopic examination revealed a large esophageal cancer (Undifferentiated squamous cell carcinoma, T2NXMO ) and irradiation was performed (Linac 4,600 rads). The patient's condition aggravated rapidly and he died in February 1980. A 76-year-old man irradiated for an esophageal cancer (Linac 6,540 rads) (undifferentiated squamous cell carcinoma, T1NXMO ) in March 1981. Eleven months later, bladder cancer was found and treated with TUR-Bt (Transitional cell carcinoma, G2, PTlmNXMO ) followed by intravesical instillation of carboquone and adriamycin. The patient was alive 1 year and 9 months after the diagnosis of the esophageal cancer. Sixteen cases of double cancer of the esophagus and urinary bladder were found in the Japanese literature. Eighteen cases including the above 2 cases were males and their ages ranged from 51 to 82 years. Sixteen bladder cancers were transitional cell carcinoma and 15 esophagus cancers were squamous cell carcinoma. Of 9 cases whose clinical course were described in detail, 3 were synchronous and 6 were metachronous. Radical surgery was performed for one or both of the two cancers in 5 cases, 4 of which were metachronous. Indication of surgery for the metachronous second cancer does not differ significantly from sporadic cancer when the first cancer has been managed successfully. However, the treatment for the synchronous double cancer of this type of combination is often forced to be restricted, since the prognosis of esophageal cancer is poor and surgical risk may be increased by two radical surgeries in such elderly patients.

Mapping the primate visual system with [2-14C]deoxyglucose.

The [2-14C]deoxyglucose method was used to identify the cerebral areas related to vision in the rhesus monkey (Macaca mulatta). This was achieved by comparing glucose utilization in a visually stimulated with that in a visually deafferented hemisphere. The cortical areas related to vision included the entire expanse of striate, prestriate, and inferior temporal cortex as far forward as the temporal pole, the posterior part of the inferior parietal lobule, and the prearcuate and inferior prefrontal cortex. Subcortically, in addition to the dorsal lateral geniculate nucleus and superficial layers of the superior colliculus, and structures related to vision included large parts of the pulvinar, caudate, putamen, claustrum, and amygdala. These results, which are consonant with a model of visual function that postulates an occipito-temporo-prefrontal pathway for object vision and an occipito-parieto-prefrontal pathway for spatial vision, reveal the full extent of those pathways and identify their points of contact with limbic, striatal, and diencephalic structures.

Local cerebral glucose utilization in the newborn macaque monkey.

The [14C]deoxyglucose method for quantitative determination of local cerebral glucose utilization was employed in newborn macaque monkeys. Values ranged from a high of 180 mumol/100 gm/min in the inferior colliculus to a low of 14 mumol/100 gm/min in the centrum ovale, a wider range than had been found in this laboratory for pubescent monkeys. Relatively low values in white matter overlapped with the generally higher values in gray matter. Rates for structures in the lower neuraxis were equal to or exceeded those reported for the mature animal, while those above the midbrain were generally lower. The auditory system was unique in having rates equal to, or exceeding, its mature levels in all parts of the pathway including the cortex. In the visual system, subcortical structures were at their mature levels while rates in the cortical areas were variably low; nevertheless, the striate cortex had differential rates marking the various laminae. It was possible to demonstrate the metabolic counterpart of the ocular dominance columns in two newborn animals that had had one eye occluded during the experiment. Rates for white matter were above their mature levels, with the differences being greatest in the brainstem and cerebellum. A high correlation between glucose utilization and local blood flow measured with [14C]antipyrine (r = 0.92) was found.

Investigation of genetic markers in a true hermaphrodite with chi 46,XX/46,XY.

We documented a new case of chi 46,XX/46,XY true hermaphroditism substantiated by the evaluation of chromosomal heteromorphism in banded preparations. The patient, a 12-year-old Japanese boy with ambiguous external genitalia, was seen because of abnormal breast development. Surgical exploration showed the right gonad to be an ovotestis and the left gonad to be an ovary. Cytogenetic studies revealed cell admixtures of 46,XX and 46,XY karyotypes in peripheral lymphocytes, skin fibroblasts, and gonadal fibroblasts. From the pedigree studies, the paternal double genetic contributions were evidenced by the differences of sex chromosomes and the blood group types for the ABO and MNSs systems in the two cell lines of the patient. The maternal double genetic contributions were confirmed by the inheritance of Q-fluorescent markers on chromosomes 13 and 22 and by alleles for the Kidd blood group system.

[Pharmacokinetics and clinical evaluation of amoxicillin sustained release preparation].

C-AMOX--long acting amoxicillin preparation (gastro coating 3, enterocoating 7)--has been investigated to give following results. A pharmacokinetic study of C-AMOX was conducted in 6 healthy male volunteers in postprandial state (30 minutes after light mean). The levels of 1.8--2.5 micrograms/ml in blood was detected for 6 hours after 500 mg dose orally. It was seemed to be long acting amoxicillin in blood levels. And, the calculated data of AUC of C-AMOX was 15.17 micrograms . hr/ml administered 500 mg dose, and 14.33 micrograms . hr/ml of AMPC twice 250 mg doses, respectively. The urinary excretion rate was 56.1% of the dose during 10 hours. Sixty-six patients with respiratory tract infections, urinary tract infections and others were treated with C-AMOX 0.5 g twice daily for 4--7 days mainly. Thirty one patients of all were cured excellent, 27 were good, 4 were fair and 4 were poor. The global efficacy rate was 87.9%. Only 1 patient was occurred diarrhea, and no abnormal laboratory finding was observed.

Suppression of spermatogenesis in patients with Behçet's disease treated with cyclophosphamide and colchicine.

Reproductive function was studied in 31 adult male patients with Behçet's disease during treatment with cyclophosphamide and/or colchicine for 1 to 64 months. Semen was obtained from 27 patients. Azoospermia or severe oligospermia was found in 13 of 17 patients receiving cyclophosphamide with or without colchicine, whereas the sperm count was almost normal in six patients treated with colchicine alone and in four patients receiving neither drug (control patients). Blood samples were available for 31 patients. The mean follicle-stimulating hormone level among 12 cyclophosphamide-treated patients was significantly higher than that of 6 colchicine-treated patients and 6 control patients. These results indicate that cyclophosphamide impairs spermatogenesis in adult men, whereas colchicine does not.

Comparative rates of local cerebral glucose utilization in the visual system of conscious albino and pigmented rats.

We measured local cerebral glucose utilization by means of the [14C]deoxyglucose technique in normal, conscious albino Sprague-Dawley and pigmented Norway rats in the ambient light of the laboratory. There were no differences between the two strains in any structures except those of the visual system. The rates of glucose utilization in all components of the visual system were lower in the albino than in the pigmented rats. The affected structures and percent differences were as follows: visual cortex (17%); stratum griseum superficialis (28%), stratum opticum (17%), and stratum lemnisci (10%) of the superior colliculus; dorsal nucleus (34%) and ventral nucleus (33%) of the lateral geniculate body; and posterolateral thalamus (10%). All differences were statistically significant (p < 0.05) except those in the striatum lemnisci and posterolateral thalamus. These results indicate that the known aberrant neurophysiologic function of the visual system of the albino rats is paralleled by alterations in metabolic activity of its component structures.
