Correlation between first morning sample and 24-hour urinary estriol excretion.

The estriol concentration of a morning sample of urine from high-risk pregnant women was correlated with the total 24-hour estriol excretion. One hundred seventy sample pairs compared the estriol concentration of the morning specimen with those of a 24-hour specimen of the previous day, while 110 sample pairs compared the value of morning specimen with those of a 24-hour specimen of the following day. Although there is highly significant statistical correlation between those sample pairs, they are of modest magnitude. The mean values of the sample pairs are significantly different. Also, the morning sample estriol concentrations are significantly more variable than those of the 24-hour specimen.

The adaptive mandible: a product of the relative osteo-neural growth.

The growth interrelation existing between the developing brain and its bony case (with the brain representing the morphogenetically determining structure to the shape and size of which the neurocranial bony envelope is adapted) is to be conceived just as a special case of a general phenomenon, the osteo-neural relation, which works throughout the entire vertebrate body. The morphological manifestations of this relation may be disclosed in the axial as well as in the appendicular skeleton. In the present communication the osteo-neural concept is applied to the morphogenesis of the facial skeleton with special reference to the mandible. The more or less elongated shape of the mandible in various species of vertebrates depends on the phylogenetically established higher or lesser degree of growth potentiality of the mandibular nerve. With the increasing size of the brain in the course of hominization the growth-in-length capability of the mandibular nerve is (compensatorily?) decreased with corresponding shortening of the mandible and with appearance of the chin. The proposed interpretation is supported by a model experiment. The highly characteristic shortening and angulation of the lower beak which may be produced experimentally in the chick embryo by a great variety of teratogens, is related most probably to the same underlying mechanism, viz., to the primary inhibition of the highly susceptible neural growth with the secondary adaptive deformity of the beak. Pierre-Robin-Syndrome (cleft palate and micrognathia) may be readily explained by a growth insufficiency of the palatine and mandibular nerves.

[Determination of serum digoxin: a path to better control of therapy?].

The usefulness of serum digoxin measurements in the diagnosis of digitalis toxicity was studied prospectively in hospitalized patients. Serum levels were increased above 2 ng/ml in 28 of 32 cases with digoxin-induced arrhythmias and/or gastrointestinal symptoms, whereas only 1 of 35 non-toxic patients had an abnormal serum concentration. There was, however, no direct relationship between the type of toxic manifestation and absolute values of serum digoxin.

[Bistability of pyruvate production by E. coli ML30 in continuous culture].

The pyruvate production of E. coli ML30 in continuous cultures was investigated. In a glucose mineralsalt medium with ammonium as the limiting substrate two stable stationary states (bistability) of pyruvate concentration were obtained. The bistability was limited to dilution rates lower than 0.3 h-1 and connected with a decrease of the yield coefficient (Y Glc) from approximately 0.45 to approximately 0.1.

Mesquite pollen--an aeroallergen in asthma and allergic rhinitis.

Skin reactions to mesquite pollen extract occurred in 62% of 100 consecutive patients tested at an adult allergy clinic despite absence of the plant within about a 50-mile radius. Radioallergosorbent test (RAST) studies confirmed that the skin tests represented specific IgE antibodies to the mesquite pollen. Nasal, conjunctival, and bronchial challenges indicated that mesquite provoked symptoms in a specific manner. The pollen was trapped at a considerable distance from its source. Mesquite surveys and climatologic data suggested a route and method of transport. Mesquite pollen is a potent allergen capable of evoking immediate hypersensitivity reactions in a susceptible population remote from the plant source.

A clinical trial of phenelzine in anxiety depressive and phobic neuroses.

A double-blind clinical trial of phenelzine and diazepam against placebo and diazepam in neurotic patients over a four-week period showed phenelzine to be superior to placebo on three rating scales in some groups of patients who completed the trial. The findings suggest that phenelzine may exert a beneficial effect on some as yet undefined features of anxiety and depression which were not revealed by a multiple regression analysis of clinical symptomatology or premorbid personality.

Significance of amniotic fluid corticosteroid levels in human pregnancies.

Retrospective analysis of 103 samples of amniotic fluid was performed. Twenty-four samples were obtained prior to midtrimester abortion, 31 were from patients who delivered spontaneously, 14 were from patients in whom labor was induced, and the remaining samples were obtained from patients who were delivered by cesarean section. In all normal pregnancies, there was a rising trend in corticosteroid levels in amniotic fluid as the gestation advanced. This was less marked in abnormal pregnancies. There is a suggestive fetal role in initiation of normal spontaneous labor, as reflected by a significant increase in amniotic fluid corticosteroid levels, in patients immediately prior to vaginal delivery as opposed to abdominal delivery. There was a significant correlation between the birthweight of babies born vaginally with the amniotic fluid corticosteroid levels, as compared to the weight of babies born by cesarean section.

[Congenital dislocation of the hip: an osteo-neural growth disturbance (author's transl)].

Congenital dislocation of the hip may be readily explained as the result of the osteo-neural growth-in-length disproportion. The theoretical concept is supported by experimental findings in frog tadpoles with "congenital" dislocation of the knee joint produced by osteolathyrogenic diet. In cleared specimens with selective staining of the nervous trunks according to Williams (1943) a growth inhibition of the sciatic nerve responsible for the proximal shift (dislocation) of the tibia may be noted. The tibia is adapted to the growth peculiarities of the sciatic nerve in an essentially similar way like the bony neurocranial capsule to the growing brain. The suggested pathogenetic mechanism appears to apply even to the congenital dislocation of the hip in man.

Pigmented iris angiography.

A small volume of concentrated fluorescein was used in a technique of fluorescein angiography in rabbits. The procedure is not accompanied by the usual risks of general anesthesia in rabbits. Angiographic findings demonstrated that the pigmented rabbit iris vessel pattern is similar to that found in albino rabbits.

Morphogenesis of the spinal canal, normal and stenotic.

The closest formative developmental dependence of the axial skeleton upon the morphogenesis of the intraspinal nervous structures is reflected in the roentgen features of the individual vertebrae as well as of the vertebral column in its entirety. The vertebroneural developmental events are characterized by a steady relative decrease in size of the nervous structures (the first to be laid down and huge in the embryonic period) under a corresponding increase in size of the skeletogenic tissues. There exists experimental evidence that the maintenance of the necessary developmental balance between the two tissues, the bony and the skeletogenic, is a function of the nervous substance. The tight spinal canal appears to result from a failure of the latter neural function leading to overgrowth of the bony structures, viz., to massive vertebrae and laminae encroaching upon the neural contents. Morphogenesis of the normal and tight lumbar spinal canal is discussed with special reference to the developmental interrelations between the cauda equina complex and the lumbar vertebrae.

The differentiation of paranoid from affective psychoses by patients' premorbid characteristics.

Various premorbid characteristics of a group of patients aged 50 or over suffering from paranoid psychoses (N = 54) were compared with those of patients of similar age suffering from affective psychoses (N = 57). (The diagnoses were those of the consultants.) The presence of deafness was investigated in the manner described by Cooper et al(1974). Using the discriminant function method, the diagnostic groups were highly significantly differentiated by six independent variables, which in combination predicted 40 per cent of the variance. These were: a 'schizoid personality' factor, the number of surviving children, precipitating events, deafness, family history and social class.
