Microcirculation in the conjunctival capillaries of healthy and hypertensive patients.

To study the impact of hypertension on conjunctival circulation, one group of healthy individuals and three groups of hypertensive patients (different stages of disease) were examined. Each participant had to undergo videomicroscopy to determine erythrocyte velocity in conjunctival capillaries, reactive hyperaemia and the diameter of the erythrocyte column within the vessels. Compared to healthy subjects, erythrocyte velocity and the diameter of the erythrocyte column were markedly increased in hypertensive patients. Furthermore the normalization of the erythrocyte velocity after a period of hypoxia was impaired in the hypertensives. These results indicate that changes in microcirculation due to hypertension can be detected by video microscopy.

Microcirculation and hemorheology of children with type I diabetes.

A group of 53 children, suffering from diabetes mellitus type I and a group of 12 healthy children were compared. In both groups the following data were determined: Haemoglobin A1C, retinal blood flow (video fluorescence angiography), erythrocyte velocity in finger nailfold capillaries (video capillaroscopy), capillary tortuosity, plasma viscosity, erythrocyte rigidity, and haematocrit. The degree of capillary tortuosity was significantly elevated and erythrocyte velocity was significantly diminished in the group of diabetic children in comparison to the healthy children. Regarding rheological data there was a significant decrease in erythrocyte deformability for the diabetic children. Diabetic children with good stabilization presented better rheological parameters than the poorly stabilized ones. Although these findings provided sufficient information, it will be necessary to cover a long follow up period to judge the prognosis of good and poorly stabilized diabetic children.

Quantification of characteristic blood-flow parameters in the vessels of the retina with a picture analysis system for video-fluorescence angiograms: initial findings.

Fluorescence angiograms of the retina were performed on 12 healthy, adult subjects between the ages of 20 and 52 years with a 30 degrees fundus camera (C. Zeiss), a low-light TV camera and a video recorder. Characteristic blood-flow parameters for the retina were obtained from the video recording using the picture-analysis system (Mikrovideomat 3, C. Zeiss). These included: the arm-retina time (ART), the arteriovenous passage time (AVP) and the arterial contrast-medium velocity (ACMV). Normal values for these parameters were derived for the four retinal quadrants and then discussed. In addition, the filling characteristics of venules were measured and discussed.

Rheologic findings in patients with acute central retinal artery occlusion.

The most important rheologic parameters were examined in six patients with acute occlusion of the central retinal artery. Blood samples were taken as part of the initial clinical examination on the day of the occlusion before the onset of therapy, and the following parameters were studied: (1) yield shear stress; (2) hematocrit; (3) plasma viscosity; (4) extent of erythrocyte aggregation; (5) erythrocyte deformability; (6) plasma fibrinogen concentration. A definite shear stress could be measured in all patients, something not possible in rheologically healthy volunteers. Hematocrit values were in the upper normal range; plasma viscosity was significantly increased. The extent of erythrocyte aggregation was greater than normal, erythrocyte deformability was markedly reduced, and the plasma fibrinogen was markedly higher than the normal level. Aside from the well-known pathologic findings in the carotid arteries of many patients with acute retinal vascular occlusion, rheologic factors seem to be of great importance in this disease.

[Surgical correction of severe astigmatism following keratoplasty].

Corneal astigmatism after keratoplasty can be treated optically or surgically. If a step forms after surgery or after removal of the sutures, resuturing is necessary. In cases of extreme astigmatism which are optically not correctable, surgical methods such as keratotomy, keratocauterization or keratoresection have been suggested. The excision of a corneal wedge, introduced in 1975, is a useful method which has now become established. The technique and the results in 8 cases are presented.

[Prevention of cystoid macular edema using indomethacin eye drops].

The effect of 1% Indomethacine solution on the development of post-operative macular edema was examined. Starting from the day of the operation, the patients received 1 drop of either the solution or the placebo 4 times daily for 3 months. Using fluorescein angiography it was demonstrated that post-operative macular edema developed with a significantly lower incidence in patients treated with the prostaglandine inhibitor Indomethacin than in the control group. There was no noticeable difference between the 2 groups with regard to side-effects.

Adenosine phosphate and glutathione levels in the regenerated corneal epithelium after abrasion and mild alkali burns.

After scraping away the corneal epithelium or after mild alkali burn, the regeneration of the cellular layers on the stromal surface was observed clinically. The average time of regeneration was 7 days after abrasion and 12 days after mild alkali burn. When the stromal surface was just covered by the newly regrown epithelium, the metabolite levels were determined. In the regenerated corneal epithelium after abrasion the ATP/ADP ratios were much higher than in the normal epithelium and much lower after the alkali burns. The GSH levels were diminished in both groups. The GSSG levels were elevated significantly only after the alkali burns resulting in a decreased redox ratio of the glutathione. The results seemed to be in accordance with morphological and enzymatical data of the regenerating corneal epithelium. Changes of the glutathione in the corneal endothelium were cautiously interpreted.

Influence of systemic ascorbic acid treatment on metabolite levels after regeneration of the corneal epithelium following mild alkali burns.

Between 6 and 16 days after mild alkali burns of rabbit eyes, the aqueous humor and corneal stroma ascorbic acid levels were decreased in the untreated animals. Systemic treatment with ascorbate 0.5 g/kg body weight injected daily subcutaneously) raised the reduced ascorbic acid levels to twice the normal values in the aqueous humor, but those of the corneal stroma increased only slightly and remained far below the normal levels. The systemic ascorbate treatment did not improve the healing rate of the corneal epithelium, but increased adenosine triphosphate and diphosphate levels as well as the ATP/ADP ratio of the glutathione was shifted to a more reduced state.

The enzymatic activities in the alkali-burnt rabbit cornea.

Alkali-burnt corneas of the rabbits with 1 N NaOH were studied periodically for enzymatic activities by biochemical methods. There was significant increase of aldolase (ALD) activity both in corneal epithelium and stroma 1, 2, 3 and 4 weeks after alkali burns. Glucose-6-phosphate dehydrogenase (G6PD) activity was significantly decreased in epithelium and was absent in stroma. Thus the breakdown of glucose would be present preferably in the Embden-Meyerhoff pathway instead of the pentose phosphate shunt. Lactate dehydrogenase (LDH) activity of corneal epithelium and stroma was significantly decreased 1, 2, 3, and 4 weeks after alkali burns and the possible pathway of glycolysis might channel to citric acid cycle, in which malate dehydrogenase (MDH) could indicate the important role in this pathway.

Enzymatic activity of beta-N-acetylglucosaminidase in the alkali-burned rabbit cornea.

Rabbit eyes burned with 1 n NaOH were studied for enzymatic activity by biochemical methods. The activity of beta-N-acetylglucosaminidase (E.C. 3.2.1.50), one of the hydrolases, was found to be significantly increased in corneal epithelium and stroma 1, 2, 3, and 4 weeks after alkali burns. The increase in enzyme activity seemed to be in correlation with the polymorphonuclear leukocytes (PMNs) infiltration of the stroma, as found by histopathologic studies. Therefore the PMNs might be the source of the increasing activity of this enzyme after alkali burns.
