Establishment and maintenance of a human glioma transplanted serially to hereditary asplenic-athymic (lasat) mice.

A tumor line in asplenic-athymic (lasat) mice designated KNS-GL-2 was established from a recurrent anaplastic astrocytoma of the cerebrum in a 15-year-old boy (NS761072). The tumor line increased in terms of the rate of tumor take and growth with serial passage, forming a nodular appearance and massive intratumoral hemorrhage at the third generation. Microscopically, tumors developed in lasat mice wee similar to the original tumor and consisted of cells with round and oval nuclei and a clear cytoplasm, which was positive for S-100 and GFA proteins by the immunoperoxidase method. Large clusters of tumor cells with dark scanty cytoplasm seen at the third generation lost their astrocytic appearance and were negative for both S-100 and GFA proteins. This was considered to be evidence for dedifferentiation or malignant transformation. There was a scattering of tumor cells with clear cytoplasm in these clusters, with a positive reaction to S-100 protein. It was possible that tumor cells dedifferentiated during serial passage but still retained the potential to differentiate into astrocytes.

Intermediate-density lipoprotein and cholesterol-rich very low density lipoprotein in angiographically determined coronary artery disease.

The relationship between the concentrations of intermediate-density lipoprotein (IDL) and other lipoproteins and the extent of coronary artery disease (CAD) was studied in 182 consecutive patients evaluated by selective coronary cineangiography. On univariate analysis, the extent of CAD correlated significantly and positively with very low density lipoprotein (VLDL) cholesterol, IDL cholesterol and low-density lipoprotein (LDL) cholesterol, and negatively with high-density lipoprotein (HDL) cholesterol. Analysis of four subgroups divided by IDL cholesterol and LDL cholesterol levels indicated that moderately increased levels of IDL cholesterol were closely associated with a high frequency of CAD. Moreover, multi-variate regression analysis demonstrated that IDL cholesterol for men, LDL cholesterol for men and women and HDL cholesterol for men were significant variables of use in the final weighting procedure. IDL cholesterol was closely associated with cholesterol-rich VLDL. This study shows that IDL and cholesterol-rich VLDL combine to contribute to the development of CAD.

Myasthenia gravis: relation between jitter in single-fiber EMG and antibody to acetylcholine receptor.

In 39 patients with myasthenia gravis, we measured jitter in the extensor digitorum communis muscle, using single-fiber electrode (SFEMG) and the serum titer of antiacetylcholine receptor antibody. Clinical severity was correlated more closely with jitter, especially the percentage of abnormal jitter pairs with blocking than with antibody titer. In patients who responded fairly well clinically after certain treatments, both the SFEMG abnormalities and antibody titer tended to improve in parallel with clinical improvement. After the plasma exchange, there was a time lag between the recovery from the defective neuromuscular transmission and the rapid decrease in antibody titer. The percentage of abnormal jitter pairs with blocking and the mean jitter value reflect defective neuromuscular transmission in generalized myasthenia gravis.

Mating behaviour of a bull and its relationship with serum LH levels in a group of oestrous cows.

A PGF-2 alpha analogue (cloprostenol) was injected into 9 cows to synchronize oestrus. The cows were placed with or without a bull in a free-stall in groups of 3 at 46 h after the injection. Sexual behaviour was observed and serum LH concentrations were measured during the next 34 h. The bull ejaculated with the cows in regular sequence and did not return to cows after an ejaculatory series was completed. The mating behaviour of the bull was closely related to the LH surge of the cows.

Congenital insensitivity to pain with anhidrosis in a 2-month-old boy.

In a 2-month-old boy with congenital insensitivity to pain and anhidrosis, the initial symptom was recurrent fever of unknown origin. After eruption of the first teeth, self-mutilation of the tongue was limited by extraction of the teeth. Unmyelinated fibers were essentially lacking, and the number of small myelinated fibers was decreased in sural nerve. This case is the youngest to be diagnosed and the first one in which nerve biopsy clearly showed the characteristic findings.

[Absorption, excretion and clinical trials of cefroxadine in the field of pediatrics (author's transl)].

A study was made with the newly developed cefroxadine (CXD) dry syrup by measuring the serum level, urine excretion and recovery rate in 10 children who were orally administered 5, 10 and 20 mg/kg at 1 hour after meals and the following results were gained. Also, its clinical efficacies and side effects were investigated in the following cases who were treated with a mean dose of 33 mg/day divided into 3 to 4 portions for a period of 9 days on the average; viz. a total of 151 cases consisting of 9 cases of pharyngitis, 39 of tonsillitis, 11 of streptococcal infection, i.e. scarlet fever, 7 of bronchitis, 6 of pneumonia, 1 of otitis media, 6 of purulent lymphadenitis, 1 of purulent parotitis, 1 of subcutaneous abscess and 3 of impetigo. 1. The serum level tends to reach its maximum level within 1 hour after administration. The mean concentrations of 5, 10 and 20 mg/kg dose in the foregoing time were 6.35, 9.12 and 21.62 mcg/ml respectively and dose response was observed. CXD showed higher concentration than CEX, CED and CFT. The mean half-life periods of the 3 dose were 72, 84 and 66 minutes respectively and variations were observed, but the drugs maintains a satisfactory serum level. 2. The time which showed highest urine excretion was mainly in the 0 to 2 hours bracket and the average concentrations of 5 , 10 and 20 mg/kg dose in the foregoing time were 381.2, 771.7 and 1,577.7 mcg/ml respectively. The dose response was more evident than in the serum concentrations. The average recovery rates within 6 hours were 93.6, 88.3 and 94.3% respectively and they were similar to those of CEX, CED and CFT. 3. The clinical effects were evaluated were in 148 cases out of the total of 151 and 136 cases, or 91.9% showed good or excellent efficacy response. 4. The daily dose groups of less than 30 mg/kg and 31 to 40 mg/kg formed the majority and there was no difference in the comparison of the clinical effectiveness in these 2 groups. Administration of a daily dose of 20 to 40 mg/kg is sufficient for the treatment of the aforementioned mild diseases except for pneumonia. 5. The clinical effects were compared between the 3 and 4 times a day treatment groups, but there was no difference between these two groups with regard to the foregoing daily dose. The 3 times a day treatment is acceptable, but the 4 times a day treatment is preferable when pharmacokinetics if taken into account. 6. The bacteriological effects in 41 cases, or 97.6% out of the 42 cases were above the efficacy base line and a high efficacy rate was achieved. 7. With regard to side effects, diarrhea developed in 4 cases and eosinophilia in 6 cases, abnormal simultaneous increases in GOT and GPT in 1 case and 1 case each for abnormal values in LDH and BUN were seen in the clinical test values. The foregoing results show that CXD has high efficacy and safety and it can be said that it is a drug required in the pediatric field.

[The ultrastructure of a gangliocytoma (author's transl)].

The tumor tissue removed surgically from the left parietal subcortical area of a 16-year-old boy was studied by light and electron microscopy. Our results were identical to cases reported previously. By light microscopy, the neoplasm was composed mostly of mature and immature ganglion cells and glial cells. Ganglion cells with large nuclei and prominent nucleoli had characteristic Nissl substance in various amounts and they were encompassed in a network of fibrous connective tissue. Glial cells were mainly oligodendrocytic. Astrocytes were small in number and did not show neoplastic growth. By electron microscopy, mature ganglion cells contained dense core vesicles in their cytoplasm and processes. The size of the vesicles ranged from 100 to 200 nm. Furthermore ganglion cells were surrounded by their own processes and also processes of glial cells. Ganglion cells were rarely found in synaptic contact with adjacent processes. Instead, oligodendrocytes were sometimes situated adjacent to these ganglion cells. These findings suggest that the tumor under study may be of hamartomatous origin. Aside from mature ganglion cells and glial cells, immature cells were present. From viewing the architecture of these immature cells, we concluded that they were also ganglion cells.

[Evaluation of tricuspid regurgitation by the ultrasonic pulsed Doppler technique from a transcutaneous approach (author's transl)].

Severity of tricuspid regurgitation was assessed by using a combined system of the ultrasonic pulsed Doppler technique and two-dimensional echocardiography from a transcutaneous approach. The study group comprised 47 patients with various heart diseases, who were clinically presumed to have tricuspid regurgitation, and 10 healthy subjects. 1) Pansystolic abnormal flow signal was detected in an area from the tricuspid valve into the right atrial cavity in 43 patients including 8 patients without definitive signs of tricuspid regurgitation. Such abnormal flow had never been detected in healthy subjects and was considered to represent tricuspid regurgitant flow. Tricuspid regurgitant flow usually exhibited a wide band spectrum of velocity component indicating a disturbed flow. In 4 patients with clinical signs of severe tricuspid regurgitation, a laminar flow was detected in the right atrial cavity, which was considered to indicate a regurgitant jet in the central part of tricuspid regurgitant flow. 2) The area where tricuspid regurgitant flow was detected was interpreted as revealing the main direction and spread of tricuspid regurgitant flow. Based on this finding, severity of TR was classified into 4 grades by the assessment on the basis of the distance reached by tricuspid regurgitant flow in the right atrium. Severity of tricuspid regurgitation was also classified into 4 grades by right ventriculography. The grade of tricuspid regurgitation assessed by Doppler technique was nearly consistent with that assessed by right ventriculography. Severity of tricuspid regurgitation was also classified into 4 grades on the basis of the extent of the area where the regurgitant flow spread, and nearly the same results were obtained as those described above. 3) Thus, the combined use of Doppler flowmetry and two-dimensional echocardiography proved to be useful for detecting tricuspid regurgitant flow and assessing the severity of tricuspid regurgitation.

[Studies of solid ovarian tumors in Tokai District (author's transl)].

The prognosis of patients with ovarian carcinoma is believed as worst among malignant diseases in the field of gynecology. Multiple factors including clinical stage, histologic type and grade may play an important role in the prognosis of patients. Because of these factors and the few cases of ovarian carcinoma seen in any one center over a short period, the ability to make valid evaluation of these factors has been limited. We organized ovarian tumor studying group in Tokai district with eleven member hospital and analyzed 453 cases of solid ovarian tumor retrospectively. 1) Cumulative survival rate was calculated according to the clinical stages. Five year survival rate of stage 1 was about 80%, that of stage 2 was about 30% and that of stage 3 and stage 4 were around 10% respectively. 2) Cumulative survival rate was calculated according to histologic types. There was minor variation of survival by histologic types. When we compared with different histologic types by same clinical stages such as stage 1 and stage 3 there was no definite differences of survival rate except slightly poor prognosis in case of mucinous cystadnocarcinoma. 3) Cumulative survival rate was calculated according to histologic grading. Histologic grade affects the prognosis of patients with ovarian carcinoma.
