Vocal cord paralysis in myasthenia gravis following anesthesia.

Our patient demonstrated two unusual features: (1) severe upper airway obstruction due to bilateral vocal cord paralysis at first manifestation of a myasthenic syndrome; and (2) precipitation of this syndrome following surgery and anesthesia with succinylcholine. In addition, this case illustrates use of ventilatory measurements through an endotracheal tube for documenting the patient's progress and response to medications, most dramatically during performance of an edrophonium test.

Intractable paroxysmal sneezing: a conversion reaction of adolescence.

A case of recurrent paroxysmal sneezing in an adolescent girl is reported with a review of the literature. The mechanisms of sneezing are described. Most cases appear to have a psychogenic etiology. We postulate that this is a conversion symptom in which the patient received secondary gain and probably represents a pre-oedipal conflict.

Thrombotic thrombocytopenic purpura in early pregnancy. Remission after plasma exchange.

A 24-year-old Chinese female developed fever, fluctuating neurological and renal manifestations, microangopathic hemolytic anemia, and thrombocytopenia during the 11th week of her first pregnancy. Therapy for thrombotic thrombocytopenic purpura (TTP), initiated during the 15th week of her pregnancy, including corticosteroids, platelet aggregation inhibitors, hysterotomy, and splenectomy was ineffective. However, dramatic improvement and remission occurred after plasma exchange of 3,560 ml was performed with the Haemonetics Model 30 Blood Cell Separator.

Pulmonary lesions in atypical measles.

Atypical measles is a well-described entity which has a high frequency of pulmonary involvement. Resolution and shrinkage of the pneumonic infiltrates into nodular mass-like lesions have been cited only once before. Three cases of atypical measles pneumonia are described which demonstrate these pulmonary nodular sequelae. In one of the cases examined a nodular residuum has now persisted for five months. Atypical measles pneumonia should be considered as a diagnostic possibility before invasive investigations are performed in a patient who has a pulmonary nodule.

Trauma as a stimulus to marrow regeneration in the osteosclerotic mouse.

Hemopoietic regeneration was studied in the tibia of mice rendered osteosclerotic by estrone injections. Trauma induced by fracture and periosteal damage was followed by a local hemopoietic response in both normal and osteosclerotic mice. Endosteal damage induced by drilling an artificial cavity in the osteosclerotic tibia was not followed by local hemopoietic regeneration, whereas in normal tibias similarly treated incomplete hemopoietic regeneration was observed. These studies indicate that trauma induced by fracture and periosteal damage is followed by a fairly uniform local hemopoietic regenerative response regardless of whether or not marrow is present in the traumatized bone. Creation of a simulated marrow cavity by drilling through osteosclerotic bone is an insufficient stimulus for hemopoietic regeneration in the mouse. New bone tissue was found to occupy the drilled shaft. This suggests that failure to regenerate hemopoietic tissue may be a consequence of insufficient bone resorption. A differential effect of trauma on endosteal and periosteal tissue is therefore postulated.

Parotid space tumors of non-salivary origin.

A review of 700 parotidectomies showed that 98 procedures were performed for parotid space tumors of non-salivary origin. The clinical presentation in this group of patients did not differ significantly from the larger group of salivary tumors except for a higher incidence (12%) of the former in the younger age group. Of the 98 cases, 54 proved to be lymph node tumors, both neoplastic and inflammatory and 44 were tumors of various somatic origins. Among the latter group, bone and joint tumors, vascular lesions and some connective tissue tumors may be recognized preoperatively. Generally, however, the entire group of non-salivary tumors can be distinguished only intra-operatively, if at all. Correct recognition may lead to variations in operative techniques and extent of resection.

Hemangiomas of the salivary glands.

Five salivary gland hemangiomas in adults are reported. The feasibility of preoperative diagnosis on clinical and radiologic grounds is emphasized. Adult hemangiomas are of the cavernous type and can be differentiated from infantile capillary hemangiomas clinically as well as histologically. Cavernous hemangiomas in the adult should be treated by surgical excision.

Metastases to the esophagus causing gastrointestinal bleeding.

A case of hypernephroma metastatic to the esophagus and presenting with massive upper gastrointestinal hemorrhage is described. The literature on metastatic esophageal neoplasm is reviewed and the methods of spread and prognosis summarized.
