Surgical removal of entrapped endocardial leads without using extracorporeal circulation.

Of 267 patients having a tined endocardial lead implanted from 1978 to December 1983, three (1.1%) developed pulse generator pocket infection. Proper treatment of this complication involves removal of the pulse generator, continued external pacing via the implanted lead, pocket drainage and administration of specific antibiotics until the infected area clears. In two patients, the electrode could not be removed by traction. A sternotomy was performed, the pericardium was opened, the endocardial electrode was located by palpation, and a purse string suture (PSS) was prepared around it on the right ventricular wall. A new myocardial electrode with its corresponding generator was then implanted to reestablish pacing. Through the PSS the myocardium was incised, the distal end of the endocardial lead was exteriorized and severed, and the PSS was tied. The remaining lead was withdrawn proximally and the surgical wounds were closed. The results of this procedure have been been excellent, allowing the removal of the entrapped leads, with continuous pacing and without the need for extracorporeal circulation.

[Vitamin D3 poisoning and irreversible sequela].

Twenty-four children with vitamin D intoxication and a follow-up of one to thirteen years old (means: four years and seven months) are reviewed. Over-dosage was prescribed by medical order in 66.6% of patients and by the mother herself in 16.6%. Intensity of clinical symptoms (renal, neurologic, digestive) were related with daily dose administered whilst final secuelae depends on duration of overdosage. Hipercalcemia was easily corrected by association of low calcium diet, corticoesteroids and/or furosemide in least than a month in 81% of cases. Two patients died during the acute fase and 22.7% remain with permanent damage (five in chronic renal failure, one in haemodialysis and three with low IC).

[Recurrent urinary infection in girls. Natural history and therapeutic response].

A group of 39 girls with urinary tract infection are studied to analyse the clinical findings, response to treatment and natural history of the disease. All of them had more than three episodes of infection per year and renal function and initial cysto-urographic study were normal. Mean age at diagnosis was 4.8 years and the mean follow-up 3.4 years. In 7.6% of cases familial incidence was observed. During infection free periods, 35.8% of patients had urinary incontinence. 25% of recurrences were asymptomatic. Only one case developed during the follow-up a unilateral polar scar associated with grave II vesicoureteral reflux. Recurrence rate/girl/year, decrease from 3.8 during the first year to 1.5 at 5th year of follow-up. Patients who received prophylaxis with nitrofurantoin showed a clear decrease in rate of recurrence.

[Peculiarities of urinary infection in 145 infants younger than 3 months of age].

Authors have analized retrospectively 145 children with urinary tract infection (UTI) within the first 120 days of life. 67% were males, 45% of patients presented an acute onset with fever and septic appearance and 55 with a chronic illness with digestive symptoms and malnutrition (95%). Breast feeding could be a protective factor against UTI. In 66% of those less than one month old, existed perinatal infectious risk factors. This fact and the presence of septic signs and positive blood cultures (71%), give rise to the possibility that the hematogenous route be the principal infectious via in this age group. The high incidence of parenchymatous participation (75%) have no relation with the presence of obstruction. At onset 29 showed no anomalies on cysto-urography, 48% vesicoureteral reflux of various degrees and 23% obstructive anomalies. The mortality rate was 1,3% due to septic shock.

[Symptomatic urinary infection in infants less than 4 months old: outcome in 129 cases].

A series of 129 children with urinary infection during the first 120 days of life, were followed-up for periods ranging from to 1 to 16 years (means = 4 1/12 year). They have been separated in two groups, depending on urocystographic findings: (I) with mild lesions and/or reflux (99 cases) and (II) with obstructive uropathy (30 cases). All the patients in group II and those with reflux of grade IV received surgical treatment. The remaining ones received chemoprophylaxis. During the follow-up, all grade I refluxes disappeared, and so did 86% of grade II and 58% of grade III. Pyelonephritic scars are present in 34% of those kidneys with grade III reflux, 56% of grade IV and 88% of those of the group II. Permanent alteration of renal function was found almost exclusively in patients with grade IV reflux or obstructive anomalies bearing no relation with the number of recurrences of infection during the follow-up.

Single bovine enamel particles examined by electron optics.

Whole enamel scrapings from teeth of embryonic calves have been separated by density gradient centrifugation in organic solvents into fractions (1.6 less than p less than 2.4 g/cm3) representing progressive stages of mineral phase maturation. Single enamel particles or their small aggregates from such fractions were examined by transmission electron microscopy, electron diffraction, and high-spatial-resolution electron probe micro-analysis. The electron optical methods demonstrated the presence of poorly crystalline hydroxyapatite as the only detectable solid phase in all fractions. Octacalcium phosphate and brushite were not identified in the fractions. Changes in electron diffraction patterns were indicative of a progressive increase in apatite crystallinity with enamel maturation. Molar Ca/P ratios were found to range from 1.48 to 1.70, with a higher mean value obtained for lower-density fractions (p less than 2.0 g/cm3). Lower-density fractions contained some particles with high ratios (approximately equal to 2.0-4.0) and a non-uniform distribution of Ca and P, as revealed by electron probe mapping. These characteristics are suggested as possibly being related to carbonate phases in early enamel.

[Xanthogranulomatous pyelonephritis in childhood: analysis of 4 cases].

A critical retrospective evaluation of four cases of xanthogranulomatous pyelonephritis (XPG) treated between 1966 and 1980. Incidence was 1.93%. Age range was from 1.5 and 24 months. Interval since first symptoms to diagnosis was between one and 14 months. Clinical presentation was nonspecific in two and urinary symptoms in the rest, abdominal mass present in all, the fourth had an onset similar to an acute leucosis. There were was a grossly dilated pelvis and calices in two patients and nonfunctioning kidney in the others, one with multiple calculi. Ureteral stenosis was present in two and three showed a vesiureteral reflux grade IV. Urinculture yielded E. coli and Proteus mirabilis. In all cases diagnosis was confirmed by macro and microscopic examination. Nephrectomy was effectively cure in all four children. All patients are well at the end of follow-up, between one and four years.

[Immunologic responses following surgical resection of gastrointestinal carcinomas].

The amount of IgG, IgM, IgA, C3, C4 and C5 in serum, and the response to delayed hypersensitivity tests with PPD, varidase, candidin and mumps antigens were estimated in patients submitted to radical surgery for gastric and colorectal cancer (group I), before and at intervals after the operation was performed, and compared with cancer patients undergoing palliative abdominal surgery (group II), and with patients submitted to major abdominal surgery for diseases other than cancer (group III). Group I showed a decrease of all the immunoglobulins (Igs) and complement fractions (F) during the first week after operation, and a significant increase after one month. Similar changes were observed in group III, the augmentation of Igs and F after one month being somewhat smaller. Group II showed a larger number of positiveness than either group I or II in both the preoperative and early postoperative periods (p = 0.001). After 2 and 3 months, the number of positive cases was larger in group I than in group II (p = 0.05). According to these results, the immune response is recovered one month after the excision of gastric and colorectal malignant tumors, being this the best moment to receive a complementary cytoreductive therapy.

Correlated physiochemical and age changes in embryonic bovine enamel.

Whole enamel scrapings from unerupted teeth of embryonic calves have been separated into fractions of varying density by stepwise centrifugation in bromoform-toluene mixtures of increasing specific gravity. Partition of enamel in this manner yields individual fractions of increasing mineral phase age and maturation. Whole scrapings and isolated fractions of the fetal bovine enamel were examined by X-ray powder diffraction, scanning electron microscopy, and atomic absorption and infrared spectroscopy to determine time-related changes in the physiochemical nature of the constituent mineral phase particles. These analyses showed poorly crystalline hydroxyapatite (HA) as the only detectable solid phase of calcium phosphate present in all fractions, its degree of crystallinity increasing with increasing density. Molar Ca/P ratios and magnesium content were highest in lowest density fractions. Carbonate vibration bands at 875 and 1420-1450 cm-1, common to mineralized tissue, were observed in intermediate and higher density fractions and in whole unfractionated enamel. Another carbonate band at approximately 705 cm-1, unusual to vertebrate calcified tissue, was detected in low density fractions and disappeared rapidly with increasing enamel maturation. Its precise relation with the enamel mineral phase has not been determined.

International collaborative study on standardization of bacterial sensitivity to fosfomycin.

An international collaborative study was undertaken involving 6 working groups to correlate the zone sizes obtained with 50 micrograms fosfomycin + 25 micrograms glucose-6-micrograms glucose-6-phosphate (G6P) and 50 micrograms fosfomycin + 50 micrograms G6P/disc with the MICs in the presence of 25 mg G6P/litre. The recommendations of the ICS for the MIC and disc diffusion methods were followed and Oxoid Mueller-Hinton agar was used. The regression lines obtained with the method of least squares show that the best correlation coefficient (r = 0.8227) corresponds to the disc with 50 micrograms fosfomycin + 50 micrograms G6P. Considering both the pharmacokinetics of fosfomycin after intravenous administration of 2 or 4 g and the distribution of sensitive bacterial populations, two breakpoints were established at MIC values of 16 and 64 mg/l corresponding to zone sizes and 18 and 22 mm, respectively.

[Anemia caused by acanthocytosis in the cirrhotic patient: diagnosis and prognostic significance. 3 cases].

The authors report the cases of 3 patients with severe alcoholic cirrhosis and spur cell anemia. The spur cell anemia must be suspected when there is haemolytic anemia, highly macrocytic with normal serum level of cholesterol and without fatty liver. The cytologic confirmation is difficult even with a phase contrast microscope. The evolution did not exceed 7 and 8 months in 2 cases; the third patient is still alive but quite sick after 1.5 year. These data lead us to point out the difficulty of the diagnosis by routine light microscopy and to remind the poor prognosis after the spur cell anemia is discovered, as documented in the literature.

[The hemophilic femoropatellar joint].

The knee is the joint most frequently involved in haemophilia. Femoro-tibial damage in chronic arthropathies has been perfectly described. By contrast, lesions of the patella (with the exception of "squaring") have been reported in only a few scanty publications. An attempt was made to define the characteristics by clinical and radiological study (including in addition to lateral films, an analysis of 30, 60 and 90 degrees axial views, even though these were often made difficult by flexion deformity and joint limitation). Femoro-patellar involvement was seen to be very frequent with various lesions: rarely "squaring", and more often patellar erosions or femoro-patellar incongruence (either pre-existing dysplasia or secondary to haemarthrosis) and sometimes actual incarceration (described as "locked patella" by anglo-saxon authors). These concepts are left to have practical consequences in the management of physiotherapy, synoviorthesis and surgery.
