Autologous mixed lymphocyte tumor cell culture in patients with renal cell carcinoma.

Cell-mediated immunity was studied by measurement of lymphocyte response to autologous tumor cells in 19 surgically treated patients with histologically proved (mixed lymphocyte tumor cell culture) renal cell carcinoma. Tumor stage was low in 9 patients and high in 10, while grade was low in 11 and high in 8. Of 8 patients in whom a positive lymphocyte response was detected 6 had high and 2 had low stage tumors (p less than 0.05), while the grade of disease was low in 7 and high in 1 (p less than 0.05). Furthermore, the more advanced and undifferentiated the tumor the more significant the decrease in lymphocyte response (p less than 0.05). Lymphocyte response was positive in 5 of 8 patients with low stage and low grade tumors but negative in 7 with high stage and high grade disease. However, no correlation between the lymphocyte response and the degree of microscopic lymphocytic infiltration in and around the tumor was found. This study confirms that the specific immunological defense mechanism of patients with renal cell carcinoma against the tumors remains well at an earlier stage of tumor development, especially in cases with well differentiated malignancy, and showed attenuation in parallel with pathological spread or in poorly differentiated tumors.

A case-control study on liver cancer with special emphasis on the possible aetiological role of schistosomiasis.

In order to elucidate the relationship between liver cancer and suspected aetiological factors, a case-control study was carried out at seven hospitals in Yamanashi prefecture, where the mortality rate of the disease was significantly higher than the corresponding national figure. This prefecture was also recognised as one of the endemic areas for Schistosoma japonicum in Japan. Sixty-two cases of liver cancer, and the same number of controls were selected during the two-year study period. A skin test for Schistosoma japonicum, serum tests for HB virus and a questionnaire study were carried out. The results of matched pair analysis revealed that liver cancer was significantly highly correlated with three factors, that is, HBs antigen (HBsAg), history of schistosomiasis, and daily intake of alcohol, and that their odds ratios were 10.0, 9.5 and 3.2 respectively. The values of percentage population attributable risk for these three factors were estimated to be 21.3%, 71.8% and 39.8% respectively. The above findings suggest that HBsAg and history of schistosomiasis are both important risk factors for liver cancer and that schistosomiasis may be much more important than HB virus in the area. The analysis of combinations of these three factors suggest that each factor might be related to liver cancer multiplicatively rather than additively.

Protein S is essential for the activated protein C-catalyzed inactivation of platelet-associated factor Va.

The prothrombin-converting activity of Factor Xa was enhanced by thrombin-stimulated Factor V-deficient platelets and supplementary extraneous Factor Va, and also by thrombin-stimulated normal human platelets. Both extraneous Factor Va and intra-platelet Factor Va were equally inactivated by a gamma-carboxyglutamic acid-containing plasma protease, activated protein C. However, a relatively larger amount of activated protein C was required for efficient inactivation of platelet-associated Factor Va as compared with the amount of activated protein C needed for inactivation of phospholipid vesicle-associated Factor Va. Protein S, another gamma-carboxyglutamic acid-containing plasma protein, increased the rate of the inactivation of platelet-associated Factor Va about 25-fold. This stimulating effect was observed only slightly with the thrombin-modified protein S. Thus, it was concluded that protein S is essential for the process of inactivation of platelet-associated Factor Va by activated protein C.

Morphological change in the corneal endothelium due to ultraviolet radiation in welders.

To clarify the relationship between morphological changes in the corneal endothelium and ultraviolet (UV) radiation, specular microscopic examinations were performed on both eyes of 118 welders and 85 controls. The results showed: a decrease in the hexagonal cells in welders (20-29 years) in comparison with the controls (20-29) (p less than 0.05); an increase in the mean cell size of the endothelium and a decrease in the hexagonal cell population with increasing age in both groups; increases in standard deviation (SD) and the coefficient of variation (CV) of the mean cell size in both groups; increases in SD and CV of the mean number of cell cell sides in both groups; and no difference in the mean cell size between the two groups. These results show that UV radiation damages not only the corneal epithelium but also the endothelium, and suggest that it causes more pleomorphic change (a decrease in hexagonal cell population) than enlargement of the mean cell size.

Spontaneous regression of pulmonary metastases after nephrectomy for renal cell carcinoma.

A case of spontaneous regression of pulmonary metastases from renal cell carcinoma after nephrectomy is presented. In a 57-year-old Japanese male who had already had pulmonary metastases at the time of nephrectomy, the metastatic lesions disappeared without adjuvant therapy 8 years after nephrectomy. He is still surviving without recurrence or any signs indicative of new metastasis at the present. The clinical aspects of this interesting phenomenon are discussed briefly.

Late recurrence of renal cell carcinoma after nephrectomy.

Late recurrence of renal cell carcinoma is unusual. Of 43 patients who underwent curative nephrectomy more than 10 years before, 2 cases (4.7%) had late recurrence. The 2 cases, both female patients, had late recurrence 12 and 19 years from the date of nephrectomy. The clinical aspects of late recurrence are discussed briefly.

Clinical experience of percutaneous transluminal coronary angioplasty.

PTCA (percutaneous transluminal coronary angioplasty) was performed on 44 patients with angina pectoris during the 18-month period from April 10, 1981 to September 30, 1982. It was successful in 36 of the 44 patients (82%), resulting in a subsidence of the symptoms and a considerable increase in exercise tolerance. Successful dilatations were achieved in 30 of 35 left anterior descending arteries (86%) and 7 of 11 right coronary arteries (64%). Five patients had complications: myocardial infarction in 2, unstable angina pectoris in 1, and dissection of the coronary artery in 2. One of these patients underwent an urgent A-C bypass operation. The patients were followed up for 0.5 to 18 months (average: 7.4 +/- 0.9 months). Stenosis recurred in 6 patients (16.7%), but repeat PTCA disclosed successful dilatations of stenotic lesions in 3 patients. No deaths occurred throughout the study period including the follow-up period. Our experience suggests that PTCA is of use in the treatment of certain groups of angina pectoris patients if case selection is appropriate.

Clinical significance of radionuclide studies in the acute phase of myocardial infarction.

Tl201 MPS and MGCBS were performed on patients with myocardial infarction in the acute phase and the results were evaluated using a scoring system. The findings obtained in this study can be summarized as follows. There was a significant difference in RNEF between Killip class I and III. Significantly high correlation was noted between RWM score and RNEF. In cases of low RNEF (less than 35%) with poor RWM (less than 7), the incidence of complications, especially malignant PVC (47%, P less than 0.01) was markedly high. There was no obvious correlation between Tl201 MPS and complications. However, adding low RNEF (35% or less) to poor Tl scores (-22 or less) malignant PVC was noted with high frequency. Tl scores on admission were not related to the number of coronary arteries involved. For patients with good Tl score and low RNEF on admission (suspected of having severe ischemia), EF did not necessarily showed a tendency toward improvement. Patients with single-, tow- or three-vessel disease showed no difference in improvement of EF at discharge. There was no marked difference in Tl score on admission among three groups of single lesion of left anterior descending artery; patients with total occlusion without collateral circulation, patients with collateral circulation and patients with recanalization.

Adverse effect of nifedipine on left ventricular obstruction detected by pulsed Doppler echocardiography.

The effect of nifedipine on the ejection flow pattern in the left ventricular outflow tract was observed using pulsed Doppler echocardiography in 6 patients with hypertrophic cardiomyopathy. After sublingual administration of nifedipine (10 mg) to 1 patient, the turbulent ejection flow pattern became more marked and increased in duration compared with the initial state. An increase in turbulence in the left ventricular outflow tract may increase the pressure gradient between the left ventricle and the aorta. Nifedipine appeared to have the potential of adverse action on left ventricular outflow obstruction.

High erythroid burst-promoting activity in sera from umbilical cord blood.

Using methylcellulose clonal cell cultures, we examined human erythroid burst-promoting activity (BPA) in sera obtained from umbilical cord blood. BPA was measured on the basis of the number of erythroid bursts (BFU-E) and hemoglobin synthesis when peripheral blood mononuclear cells were cultured with cord blood serum in the presence of 1.0 U/ml erythropoietin (Epo) at low fetal calf serum concentration (10%). Controls were cultured with adult serum. The number of BFU-E and hemoglobin synthesis increased in culture to which cord blood serum had been added, and the enhancing effects of cord blood serum on erythroid burst formation were more pronounced than in control cultures with 2.0 U/ml Epo. No erythroid bursts, however, grew in the cultures with cord blood serum in the absence of Epo. Increased sensitivity of erythroid burst formation to low concentrations of Epo was observed in cultures containing cord blood serum. Dose-relationships between the concentrations of cord blood serum and the number of BFU-E and hemoglobin synthesis were clearly observed. We therefore concluded that the stimulatory effects of cord blood serum on erythroid burst formation may be due to BPA, one of the factors required for the proliferation of erythroid precursors at an early stage.

[A case of advanced seminomatous germ cell tumor cured with PVB combination chemotherapy].

A case of stage IIB seminomatous tumor treated with the combination chemotherapy regimen of cis-platinum, vinblastin and bleomycin is presented. The patient was a 42-year-old man who was admitted to our hospital with the chief complaint of the right intrascrotal mass and edema in right lower extremity. Right radical orchiectomy was immediately performed under the diagnosis of testicular tumor. Histological diagnosis was pure seminoma. Paraaortic bulky metastasis and right hydronephrosis were found in the abdominal CT-scan. PVB therapy for initial treatment was started. After four courses of this therapy, retroperitoneal lymphadenectomy was performed. Because no live tumor cells were found histologically in any of the dissected material, complete response was considered to have been achieved by PVB therapy. Considering the poor survival and high relapse rate of advanced seminomatous tumor treated with radiotherapy, this PVB therapy should be the initial treatment for this disease.

Functional significance of coronary collateral vessels during exercise evaluated by radionuclide angiocardiography: the importance of supplying arteries.

To investigate the function of coronary collateral vessels, especially from view point of supplying arteries, radionuclide angiocardiography was performed before and during a symptom-limited ergometer exercise in 54 patients with effort angina. In single vessel disease, during exercise, left ventricular ejection fraction (EF) (%) increased in the cases with coronary collateral vessels (supplied from patent arteries) (72 +/- 7----77 +/- 8, p less than 0.025), but decreased in those without (66 +/- 10----61 +/- 10, P less than 0.001). In multivessel disease, EF decreased during exercise even in those with collaterals (supplied from stenosed arteries) (67 +/- 10----59 +/- 8, p less than 0.001). It is concluded that angiographic appearance of the coronary arteries supplying collateral vessels is a major predictive factor of the function of coronary collateral vessels and of left ventricle during exercise.

Effects of glutamic acid, kainic acid and aspartic acid on GABA release from rat retina degenerated by kainic acid.

The effects of L-glutamic acid (Glu), kainic acid (KA) and L-aspartic acid (Asp) on 14C-GABA release from the rat retina degenerated by KA were investigated. In the normal rat retina, Glu initially enhanced GABA release and subsequently inhibited it. Both KA and Asp did not have dual effects; KA enhanced GABA release, while Asp inhibited it. In the KA-degenerated retina, the stimulatory effect of Glu or KA on GABA release was markedly suppressed, while the inhibitory effects of Glu or Asp were preserved.

[Pathophysiology of asymmetric septal hypertrophy with special reference to the response to isoproterenol administration].

To investigate the pathophysiological significance of asymmetric septal hypertrophy (ASH), echocardiograms were recorded in 25 patients with hypertension (HT) (12 with ASH, 13 with symmetric hypertrophy (SH), 19 patients with hypertrophic non-obstructive cardiomyopathy (HNCM) (eight with ASH, 11 with SH), and 12 patients with hypertrophic obstructive cardiomyopathy (10 with ASH and two with SH). In eight patients with ASH, 12 patients with SH and eight normal controls, echocardiograms were also recorded during intravenous infusion of 0.02 mu/kg/min of isoproterenol. There were no significant differences in end-diastolic dimension (EDD), end-systolic dimension (ESD), percent fractional shortening (%FS), interventricular septal excursion (IVSE) and posterior wall excursion (PWE) between patients with ASH and patients with SH. Among patients with ASH, interventricular septal thickness (IVST) and its ratio to posterior wall thickness (IVST/PWT) were significantly larger in HOCM than in others. Before isoproterenol infusion, there was no significant difference in the normalized peak rate of change of left ventricular dimension during systole (pVs) between ASH and SH, and between ASH and NC. However, 5 min after isoproterenol infusion, pVs was significantly greater in ASH (7.0 +/- 2.4/sec) than in SH (4.9 +/- 1.3/sec) and in NC (4.5 +/- 1.1/sec) (p less than 0.05 and p less than 0.05, respectively). This study suggests that ASH is related to hyperfunction of the catecholamine-beta receptor system.

Responses of salt- and water-regulating hormones during a saturation dive to 31 ATA (SEADRAGON IV).

Four Japanese male subjects were studied during 3 days at 1 ATA, 3 days of compression to 31 ATA (1000 fsw), 14 days at 31 ATA, 12 days of decompression, and 3 days of postdive control at 1 ATA. The chamber was maintained at thermoneutral temperatures. During the 31-ATA exposure, urine flow increased about 500 ml/day (P less than 0.05) accompanied by an approximate 500-ml/day increase in osmotic clearance (Cosm) (P less than 0.05). Both urine flow and Cosm increases were primarily attributable to an increase in the overnight flow rates (P less than 0.01). The negative free water (-CH2O)/Cosm decreased during exposure to 31 ATA, indicating that a reduction in tubular water reabsorption may also contribute to the diuresis. Urine flow, Cosm, and -CH2O/Cosm all returned to predive values during decompression to 1 ATA. The urinary excretion rate of aldosterone increased from 2.7 +/- 0.3 micrograms/day at 1 ATA to 4.3 +/- 0.0 micrograms/day (P less than 0.01) at 31 ATA, remaining at about 3.8 micrograms/day until decompression. Urinary antidiuretic hormone (ADH) decreased from 50 +/- 7 to 33 +/- 3 mU/day (P less than 0.01) upon compression to 31 ATA and continued to decrease throughout the decompression phase. Plasma renin activity was increased by twofold (P less than 0.01) and plasma aldosterone by about 37% (NS) during exposure to 31 ATA. It is concluded that the reduction in ADH does not contribute significantly to the hyperbaric diuresis, and that the increased activity of the renin-angiotensin-aldosterone is a result of other postulated mechanisms resulting in an increased Cosm.

[Factor XIII in the treatment of postoperative refractory wound-healing disorders. Results of a controlled study].

The efficacy of factor-XIII concentrate in the improvement of postoperative wound healing disorders (suture dehiscences, fistulae) was examined in 61 patients. In an open, randomised, and controlled trial the results (decrease of wound area, signs of inflammation, wound secretion, drainage volume, contrast x-ray films, and colour photos) were evaluated twice: open judgement by the clinicians themselves and blind judgement by an independent evaluation committee. The blind evaluation of the clinical data showed relevant general improvement in the patient groups treated with factor-XIII concentrate: 61.9% (dosage: 750 IU factor XIII for 3 days) and 76.2% (dosage: 1500 IU factor XIII for 3 days) as compared to 10.5% in the control group without factor XIII substitution. The differences were significant (Mann-Whitney-U-Test, p less than or equal to 0.001). These results would suggest substituting factor XIII activity in plasma above 70% of normal value in case of wound healing disorders.
