[Indication and result of renal autotransplantation for renovascular hypertension].

Twenty patients with renovascular disease in 22 kidneys were treated surgically by the method of renal autotransplantation at our department during the 13 years from 1970 to 1982. Etiological disorders included 11 cases of fibromuscular dysplasia, 3 cases of atherosclerosis, 3 cases of aortitis syndrome, two cases of renal artery aneurysm and one case of renal artery thrombosis. Age distribution ranged from 13 to 64 years old (average 28.9 years), and male to female ratio was 1 to 3. Ex vivo surgery was carried out in 11 of these 20 cases. Angioplasty in 9 cases, endarterectomy and partial nephrectomy in one case each were performed extracorporeally. Reconstruction of the urinary tract was needed for 14 kidneys in 13 patients, 12 kidneys were treated by UCN, and PU anastomosis was performed in 2 kidneys in 2 patients. Outcome of renal autotransplantation was "cured" in 17 cases and "improved" in 3 cases during follow-up period from 8 months to 13 years. Revision was successful for the postoperative complication such as urine leakage, bladder tamponade, perirenal fluid collection and ileus.

[Cystic pontine glioma associated with von Recklinghausen's disease--report of a case].

Pontine gliomas have been considered to be out of indication for operative treatment. However, in case of a cystic type, evacuation of the cyst alone can possibly extend the survival time of the patient. Since the advent of high resolution CT the nature of the pontine tumor, whether cystic or solid, can be easily differentiated, and cystic ones subjected to operation will be increasing in number. We report a case of cystic pontine glioma associated with von Recklinghausen's disease. The patient showed a remarkable improvement in her neurological status after evacuation of the cyst. A 16-year-old girl was admitted to our clinic with complaints of tinnitus and hearing difficulty of the left ear, progressive gait disturbance and double vision. Neurological examination revealed a sensory disturbance on the left side of the face, left abducens palsy, left facial paresis, left deafness, left cerebellar ataxia, right hemiparesis and right hemisensory disturbance excluding the face. Signs and symptoms of increased intracranial pressure were absent. There were many cafe-au-lait spots and several subcutaneous nodules. CT scan demonstrated a cystic lesion with a mural nodule in the left cerebello-pontine angle. The patient underwent left suboccipital craniectomy, and a puncture of the cyst between the trigeminal and facial-acoustic nerves which were displaced dorsally yielded yellowish fluid. The content of the cyst was evacuated and its wall was widely opened. After the operation the patient showed a remarkable improvement in her neurological deficits only with left deafness remained unchanged. Histological examination showed anaplastic astrocytoma. She was discharged after irradiation of 5000 rads.(ABSTRACT TRUNCATED AT 250 WORDS)

[Anti-tumor effect of intravesical instillation of mitomycin C combined with cytosine arabinoside].

The anti-tumor effect of twice a week instillation of 30 mg mitomycin C combined with 200 mg cytosine arabinoside dissolved in 30 ml saline was studied in 37 bladder tumor patients. Endoscopic or histological disappearance of the tumor was observed in 19 cases, average instillation being 16.4 times. Small (less than 1 cm in diameter), low grade, low stage and villous-surfaced tumors were more sensitive to this therapy. Further more, in the cases showing a complete response, frequency of the intravesical tumor recurrence significantly decreased compared with the cases showing no response. The clinical results of this therapy are compared mainly with those of TUR performed at our hospital, and the usefulness and clinical limitation of this kind of management in the treatment of bladder tumors are discussed.

[A case report of infarction of the bilateral thalamus and the midbrain].

A 63 years old man was admitted because of coma of 24 hours duration. He had a history of infarction of the medulla oblongata 7 months earlier. On admission he was unconscious and had right oculomotor nerve palsy. To a painful stimulus, he responded with only a slight body movement. On the CT scan, low density areas were seen in the bilateral thalamus and the midbrain. In the next day he recovered from coma and abnormal behaviors appeared such as wondering in the ward, touching other patient's face and manipulating his stool. Verbal expression was extremely poor. Chlorpromazine was needed to suppress abnormal behaviors. Thereafter, abnormal behaviors gradually subsided and by the 23 hospital day he became bedridden even when chlorpromazine was withdrawn. At that time, he said only "yes" or "no" in response to questions and only few spontaneous movements were observed. From the 40 hospital day he suffered from pneumonia and gradually sank into the state of akinetic mutism. He died on the 50 hospital day. Pathologically, infarctions destroyed the n. parafascicularis, n. centromedianus and the intralaminar nuclei in the left thalamus, n. parafascicularis and a part of n. centromedianus in the right. In the medulla oblongata, right n. ambiguous and reticular formation were destroyed. Both thalamic lesions extended posteriorly to unite with the midbrain lesion.(ABSTRACT TRUNCATED AT 250 WORDS)

[Fibronectin--its functions and roles in tissue repair].

Fibronectin is a recently characterized 4.4 X 10(5) dalton glycoprotein consisting of two probably identical 2.2 X 10(5) dalton subunits held together by disulfide linkages. It is present as a soluble fraction in plasma at about 30mg/dl and is widely distributed as an insoluble component to various tissues, in close association with fibrillar structures of connective tissue matrices, basement membranes and certain mesenchymal cells including fibroblasts and smooth muscle cells. Fibronectin binds to fibrin and collagen reversibly and crosslinks to these filamentous proteins catalyzed by the activated blood coagulation factor XII (plasma transglutaminase). Plasma fibronectin appears to be utilized at the healing wounds, thus its concentration decreases during early postoperative days. Immunofluorescence study shows that fibronectin is closely associated with newly generated fibroblasts and fibrillar structures in the early stage of wound healing, and that it diminishes in accordance with the maturation of collagen fibers. Plasma fibronectin is shown to function to remove the colloidal debris from blood via the reticuloendothelial system and to play important roles in organizing the cytoskeleton and in the migration and spreading of interstitial cells. Thus fibronectin seems to play a pivotal role in the tissue repair via a variety of biological functions.

[Dissecting aneurysm of the anterior cerebral artery: report of a case].

A 43-year-old man suddenly experienced severe headaches and involuntary flexion-extension movements of four limbs, which were followed by hypertonic extension of the limbs lasting for a few hours. Two days later, he experienced generalized tonic seizure without loss of consciousness. After the seizures, he remained hemiparetic on the right side. His past medical history was non-remarkable, and the histories of hypertension, diabetes mellitus, head trauma and significant infectious diseases were all denied. Cerebral angiography performed 22 days after the onset showed a segmental, irregular narrowing of the left A2 segment and an aneurysmal outpouching immediately proximal to the stenosis. CT scan revealed a low density area in the left frontal lobe, corresponding to the territory of the involved left anterior cerebral artery. Cerebral angiography was repeated twice in the succeeding 6 months. Each time, the involved A2 segment showed persistence of narrowing, but its shape showed definite changes with the passage of time. A diagnosis of dissecting aneurysm of the anterior cerebral artery was reached by the characteristic angiographic features, and the patient was treated conservatively. Dissecting aneurysm of the cerebral arteries have been reported much less frequently than those of the aorta or other extracranial arteries. Recently, however, such reports are increasing in number, seemingly due to enhancement of knowledge of typical angiographic features, such as string sign, rosette sign, pearl reaction, double lumen and several others. Most of intracranial dissecting aneurysms involve the middle cerebral artery or vertebral-basilar artery, and the ones involving solely the anterior cerebral artery as in this present case are very rare.(ABSTRACT TRUNCATED AT 250 WORDS)

Comparison of intraocular irrigating solutions used for pars plana vitrectomy and prevention of endothelial cell loss.

We performed a prospective study of corneal endothelial cell loss following pars plana vitrectomy. Two different intraocular irrigating solutions were used, and the anterior capsule of the lens was removed when performing lensectomy. The central corneal endothelium was photographed with a specular microscope before and at three months after vitrectomy. Endothelial cell loss was calculated using a computer-assisted digitizer. In the 16 aphakic eyes, lactated Ringer's solution caused a significantly greater cell loss (31.1%) with resultant corneal edema when compared to BSS Plus (6.9%). All of the 17 phakic eyes showed minimal cell loss (1.0%) as well as corneal changes regardless of the kind of irrigating solution used. The results suggest that the absence of the lens is a salient factor in corneal endothelial damage during vitrectomy, and that the composition of the intraocular solution used is closely related to this endothelial damage.

Hyperbaric diuresis at a thermoneutral 31 ATA He-O2 environment.

The basic pattern of body water exchange was studied in four Japanese male divers during exposure to a thermoneutral 31 ATA (He-O2) environment for 3 d (Seadragon V). The hyperbaric chamber temperature was raised from 25 degrees C +/- 0.5 degrees C at 1 ATA (air) predive to 31.5 degrees C +/- 0.3 degrees C at 31 ATA. Both rectal and mean skin temperatures were measured every hour (including during sleep) and were maintained at the same level at both pressures. The exposure to 31 ATA induced an increase in the daily urine flow and a corresponding reduction in the insensible (and evaporative) water loss without changing the total daily water output. However, the daily fluid intake decreased by 600 ml at 31 ATA, and hence the divers developed a state of negative fluid balance, as reflected by a reduction in body weight and an increase in hematocrit. All changes in the pattern of body water exchange observed at 31 ATA were gradually reversed during subsequent decompression. As observed in a previous dive to 31 ATA (Seadragon IV) in which there was a subtle cold stress (as indicated by the 1 degree C reduction in mean skin temperature at 31 ATA), the increase in daily urine flow at pressure was almost entirely due to the increase in overnight urine flow. However, the hyperbaric nocturia observed in the present dive was a water diuresis in nature whereas that in the previous dive was an osmotic diuresis. These results indicate that the hyperbaric diuresis at 31 ATA is due to an increase in overnight urine flow and that the hyperbaric nocturia is not in any way related to the subtle cold stress attendant in many hyperbaric environments.

[Combination chemotherapy of advanced transitional cell carcinoma with cis-platinum, cyclophosphamide, adriamycin and 5-fluorouracil (CISCAF)].

Ten patients with advanced transitional cell carcinoma were treated with the combination chemotherapy (CISCAF) consisting of cis-platinum (CDDP), cyclophosphamide (CPM), adriamycin (ADM) and 5-fluorouracil (5-FU). There were 8 males and 2 females with a median age of 57 years (range 33-76). Prior to this chemotherapy, all primary lesions were resected surgically; one patient received irradiation, one interferon. CDDP (15 mg/m2) with mannitol diuresis and ADM (7 mg/m2) were given daily for 5 days. CPM (200-300 mg/m2) was given on day one, and 5-FU (200-300 mg/m2) on day 2. Courses were repeated every 3-4 weeks for 3 courses and later every 2 months. All patients were evaluable for toxicity and 9 for response. There were 3 (33%) partial and 2 minor responses. Survival difference between responders (PR + MR) vs non-responders (NC + PD) was significant during the 30-120 days of observation, but not significant throughout the whole period (generalized Wilcoxon test). The overall toxicity due to chemotherapy was generally mild, except for one irreversible nephrotoxicity and one gastric hemorrhage treated surgically.

[Two-dimensional pulsed Doppler echocardiographic estimates of cardiac output in man: sampling at the entrance to the main pulmonary artery].

To assess the feasibility and accuracy of measuring cardiac outputs (CO) by two-dimensional (2-D) pulsed Doppler echocardiography, taking samples at the main pulmonary artery just beyond the pulmonary valve, 62 cardiac patients were studied. The results obtained were compared with CO values determined by the thermodilution technique. 2-D Doppler echocardiograms suitable for computing CO were recorded in 37 (60%) of 62 patients. A significant correlation (r = 0.64) was observed between 2-D echocardiographic and angiographic pulmonary ring diameters (PRD), but the echocardiographic PRD was smaller than that of angiography by 0.4 +/- 0.2 cm. Thus, to compute the Doppler CO, the echocardiographic PRD had to be corrected by the difference between the angiographic and echocardiographic PRD. The angle between the ultrasound beam and the pulmonary blood flow was 7.8 +/- 6.1 degrees (0 approximately 18 degrees), and this was negligible for computing the Doppler CO. Even in cases with atrial fibrillation, the Doppler CO could be computed as is done in sinus rhythm. The correlation between the thermodilution and Doppler CO was highly significant (r = 0.92), and the regression equation was as follows: Thermodilution CO = 1.09 X (Doppler CO)-0.19 The difference between thermodilution and Doppler CO was 3.4 +/- 10.0% (-21 approximately +21%). Comparing the differences in CO between the thermodilution and dye-dilution techniques, those between thermodilution and the Fick methods, and those among thermodilution methods reported in the literatures, the difference in this study was not large and was acceptable. Thus, measurement of CO by pulsed Doppler echocardiography, with sampling entrance to the main pulmonary artery, can be used clinically.
