[Apical hypertrophy and catecholamine].

Catecholamine is suspected to be related to the pathogenesis of hypertrophic cardiomyopathy (HCM). To investigate the contribution of catecholamine to apical hypertrophy (APH) which is a special type of HCM, 16 normal controls, 49 +/- 4 years old; 14 patients with APH, 55 +/- 6 years old; 18 patients with other types of HCM, 53 +/- 8 years old; and 12 patients with left ventricular hypertrophy due to valvular heart diseases (VHD), 49 +/- 10 years old, were studied during treadmill exercise tests, supine ergometer tests and intravenous infusions of isoproterenol (ISP) (0.02 micrograms/kg/min for 5 min). Echocardiograms and/or electrocardiograms were obtained during the procedures. During ergometer exercise, there was no significant difference in increase of heart rates among all groups. Ergometer exercise increased the level of plasma catecholamine in the group VHD, and there was a similar but less marked tendency in the APH. During the ISP infusion, "mechanical" reaction was relatively low in the APH compared to those in the other groups. The giant negative T waves in the HCM, mostly in the APH, lessened during exercise and during the ISP infusion. Among those in whom negative T wave changes of more than 0.5 mV occurred during exercise, the wall motion of the left ventricle was more active during exercise. Giant negative T waves changed diurnally and were less deeper during daytime than at night.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical study on intact gamma-globulin (SM-4300) in surgical severe bacterial infections].

Intact gamma-globulin (SM-4300) was studied for its clinical efficacy in 13 of 23 cases of surgical severe bacterial infections. The drug was administered intravenously either 2.5 g/day for 3 days or 5 g for a day and the results were as follows. Clinical effects of SM-4300 in 13 cases of surgical severe bacterial infections were excellent in 1 case, good in 3, fair in 3 and poor in 6. SM-4300 was effective in all of 5 cases of intraperitoneal abscess. The elevation of serum IgG was observed after the administration of SM-4300 in all cases tested. As to complement, the slight elevation of serum CH50 was observed in the half of cases documented. Clinical side effects were not confirmed in any case. These results suggest that intact gamma-globulin (SM-4300) was effective in combination with antibiotics.

Corneal endothelial changes in type I and type II diabetes mellitus.

Forty-six corneas from 25 patients who had had type II (adult-onset) diabetes for more than ten years were examined by specular microscopy with quantitative morphometric analyses of individual endothelial cells. Thirty-four corneas from 21 age-matched nondiabetic subjects were examined for comparison. We also examined 31 corneas from 17 patients with type I (juvenile-onset) diabetes and compared them to 41 corneas from 23 age-matched normal volunteers. The corneal endothelium in type II diabetes showed no difference in cell density but demonstrated a significantly higher coefficient of variation, a decrease in the percentage of hexagonal cells, and a low figure coefficient compared to an age-matched nondiabetic population. Type I diabetes produced similar cell changes, but these changes occurred in the earlier decades. Moreover, we detected a significantly higher rate of cell loss in type I diabetes, resulting in a significant decrease in cell density in the fourth and fifth decades. These results clearly indicate that the diabetic endothelium is morphologically abnormal. The observed anatomic changes result in a less stable and more vulnerable cell layer, possibly explaining some of the persistent clinical changes in the diabetic cornea after surgical trauma.

Serial alterations in endothelial cell shape and pattern after intraocular surgery.

We serially examined the endothelial cellular pattern and thickness of the central cornea after intracapsular cataract extraction and penetrating keratoplasty. The endothelial cellular pattern was analyzed with a computer-assisted digitizer. We used the frequency of hexagonal cells as the index to describe the degree of hexagonality in the cellular pattern. In the intracapsular cataract extraction group (20 eyes), the frequency of hexagonal cells was 68% preoperatively. During the first four weeks this frequency decreased rapidly to a significantly lower level (52%). The corneal thickness was significantly increased within the first week (0.57 mm) and steadily decreased to preoperative values at four weeks (0.53 mm). During the period from four to 24 weeks, the frequency of hexagonal cells increased gradually to 64%. In the penetrating keratoplasty group (ten eyes), the frequency of hexagonal cells and the thickness at one month were 62% and 0.64 mm respectively. During the next six months the frequency of hexagonal cells decreased significantly to 53%, and the thickness decreased to 0.53 mm. There was a gradual increase in the frequency of hexagonal cells to 62% during the two years after penetrating keratoplasty.

The small nuclear ribonucleoproteins that react with anti-Sm and anti-RNP antibodies.

We have examined the polypeptide pattern of small nuclear ribonucleoprotein particles that react with monoclonal anti-Sm antibodies or polyclonal anti-(U1)RNP antibodies. The fresh nuclear extracts analyzed were prepared from human cells that had been pulse-chased with a mixture of 15 3H-labeled amino acids. In contrast to previous reports in the literature, the apparent molecular weights of the major polypeptides that remained in the 1 M NaCl-washed ribonucleoprotein-antibody complexes were approximately 80000, 55000, 28000, 25000, 14000 and 9000, when probed with monoclonal anti-Sm antibodies, and about 69000, 58000 and 35000, when polyclonal anti-(U1)RNP antibodies were used.

Quantitative analysis of endothelial mosaic pattern changes in anterior keratoconus.

The corneal endothelia of 21 eyes with anterior keratoconus and 15 eyes of age-matched controls were investigated with a specular microscope. Of the 21 eyes, 15 had definite keratoconus while six apparently normal fellow eyes were designated as latent. The corneal endothelial photographs were subjected to a computerized digitizer analysis of the area and shape of individual cells. The endothelium in keratoconus showed a significant increase in the extent of polymegethism (the coefficient of variation in cell area; 0.36 +/- 0.07) as compared with controls (0.24 +/- 0.03), with the mean cell area staying within normal limits. The relative frequency of hexagonal cells in keratoconus (50.5 +/- 5.7%) was significantly lower than that of controls (70.6 +/- 5.5%). Additionally, an increase of various cell shapes was noted, indicating that there was also a significant increase in cellular pleomorphism. In the latent group, alterations in cellular structure could not be detected by analysis of cell area when compared to controls. However, when these eyes were subjected to cell shape analysis and compared to fellow keratoconic eyes, both eyes showed similar pleomorphic characteristics. These results suggest that in the fellow eye in unilateral keratoconus, even when there are no ocular signs and symptoms, diagnosis can be made early with specular microscopy and computerized cell shape analysis.

Efflux of gamma-aminobutyric acid from and appearance of free arachidonic acid inside synaptosomes.

When synaptosomes were depolarized in the presence of Ca2+, or when Ca2+ was added to synaptosomes pretreated with Ca2+ ionophore (A23187), free arachidonic acid was clearly increased within synaptosomes, and at the same time an efflux of gamma-aminobutyric acid from synaptosomes was observed. Moreover, when synaptosomes labelled with [14C]arachidonic acid were depolarized in the presence of Ca2+, there was a significant decrease in the radioactivity of the fatty acid of phosphatidylinositol and phosphatidylcholine. Exogenously added arachidonic acid, but not other fatty acids, stimulated the efflux of gamma-aminobutyric acid in the absence of Ca2+. These observations suggest that the release of arachidonic acid from phospholipids is an intrinsic part of the biochemical mechanism that modulates the gamma-aminobutyric acid efflux.

Urinary procoagulant behaves as tissue factor by promoting factor VIIa-catalyzed activation of factor X.

We purified the urinary procoagulant from frozen human urine by introducing phenyl-Sepharose hydrophobic chromatography. By this method, the apoprotein of the procoagulant and the lipid-like substance were separately recovered. Upon reassociation with the lipid-like substance or exogenous crude phospholipids, the apoprotein accelerated factor VIIa-catalyzed activation of factor X, probably by forming a stoichiometric complex with the catalytic enzyme. Thus the procoagulant was confirmed to be a tissue factor by its mode of participation in the blood coagulation mechanism.

Cytoplasmic tubular inclusion in ganglioneuroma.

Unusual tubular inclusions were observed in the cytoplasm of ganglion cells of a mediastinal ganglioneuroma in a boy of 7 years. The inclusions consisted of an aggregation of about 100-nm-sized tubular structures resembling 'honeycomb-like' tubular structures in the axoplasm of rats and mice and suggested to be derived from dilated rough endoplasmic reticulum. These structures differ from profiles reported in normal and pathologic conditions and seem to be related to the neoplastic character of the ganglion cells.

Effects of the presence or absence of preceding angina pectoris on left ventricular function after acute myocardial infarction.

Left ventricular (LV) function was evaluated in 31 patients, who had total occlusion of the left anterior descending coronary artery and less than 70% stenosis of the other two major coronary arteries or any branch. Fifteen of 31 patients had a history of angina pectoris before acute myocardial infarction (AMI) and 16 of 31 patients had no history of angina pectoris before AMI. The patients with angina pectoris before AMI had a significantly better ejection fraction, percentage of abnormally contracting segment, and regional wall motion than those without angina pectoris before AMI. These data suggest that the symptom of angina pectoris before AMI could be a favorable sign in preserving LV function when the patients subsequently had AMI.

Coronary angiography during exercise-induced angina with ECG changes.

Coronary angiography was performed at rest and during bicycle exercise immediately after the onset of angina and significant ST segment elevation or depression in the ECG. Of 11 patients, six showed significant reduction of coronary lumen diameter at the site of organic stenosis; mean values of stenosis (range) before and during exercise were 55% (25% to 88%) and 98% (89% to 100%), respectively. Five patients did not have any diameter change of the organic lesion; mean values of stenosis (range) before and during exercise were 84% (74% to 89%) and 84% (73% to 92%), respectively. Excluding the areas of these stenoses, diameters of left main coronary artery, proximal, middle, and distal left anterior descending, circumflex, and right coronary artery segments were measured before and during exercise. Diameter in each coronary artery segment during exercise was not significantly changed from that before exercise, both in the groups with and without diameter reduction. Exercise provoked a localized worsening of coronary artery stenosis without changing the diameter in the remaining artery. These findings suggest that the worsening of stenosis might be caused by a regional abnormality of the coronary artery that is not necessarily related to the degree of organic stenosis.

Aneurysms of the proximal anterior cerebral artery.

The authors report eight cases of aneurysm of the anterior cerebral artery proximal to the anterior communicating artery (A1 segment). In six of these cases, the aneurysms arose from the proximal anterior cerebral artery at the origin of either a cortical branch (on case), the accessory middle cerebral artery (one case), or a perforating branch (four cases). In another case the aneurysm arose at the proximal end of the fenestration, whereas in the one remaining case no branch was present at the site of the aneurysmal neck.

Hormone receptor in renal cell carcinoma and correlation with clinical response to endocrine therapy.

Analyses of hormone receptors in cytosols from 41 renal cell carcinoma specimens were performed by the dextran-coated charcoal technique, using estradiol, synthetic progestin R5020 and synthetic androgen R1881. Binding data were calculated according to the method of Scatchard. Of 41 renal cell carcinomas estradiol receptor was detected in 11, R5020 receptor in 11 and R1881 receptor in 13. No significant correlation between histopathological findings and hormone receptors was observed. Patients were classified into those positive and negative for receptors. The clinical response of endocrine therapy for 17 with advanced residual or metastatic lesions after nephrectomy was studied in regard to the survival rates. Although there was no complete or partial regression in tumor size, the survival rate of patients with 1 or more receptors was significantly higher than that of patients negative for receptors (p less than 0.01). In conclusion, hormonal manipulation in patients with renal cell carcinoma cannot induce an antitumor effect but seems to increase survival in patients with receptors.
