[Evaluation of the clinical effect and tissue distribution of cefpimizole in the field of gynecology].

Cefpimizole (AC-1370) was administered to 5 cases with uterine myoma before hysterectomy, and tissue distribution was determined. AC-1370 was also administered to 5 cases with gynecological infections. The following results were obtained. One gram of AC-1370 was administered from 43 to 299 minutes before hysterectomy, tissue distribution of AC-1370, such as ovary, oviduct, myometrium, cervix uteri, and portio vaginalis was showed the highest level (30.0 approximately 49.5 micrograms/g) at 43 minutes after administration, and these were 39.0 approximately 64.4% of the concentration in uterine arterial blood. Tissue concentration of AC-1370 was then gradually decreased following with the decreasing of the concentration in uterine arterial blood. AC-1370 was administered to 3 cases with pyometra, 1 case with Bartholin abscess, 1 case with adnexitis. The clinical efficacy was good in all 5 cases. Bacteriological study revealed that A. faecalis and E. coli were eradicated, but B. fragilis was persisted. No side effect was observed in all cases.

Detection of renin mRNA in mouse testis by hybridization with renin cDNA probe.

Using 32P-labeled DNA complementary to mouse submaxillary gland renin mRNA, we probed mRNA gel blots from mouse testis and kidney tissues. Poly(A)-RNA from testis contained a hybridizable RNA species which was blotted onto nitrocellulose paper. The molecular size of testicular renin mRNA (approximately 1600 nucleotides in length) was not significantly different from tht of kidney renin mRNA. Densitometric scan revealed that the content of renin mRNA in mouse testis was approximately 5-fold lower than that in mouse kidney. These results support the proposal that mouse testicular cells synthesize renin.

Cloning of genomic DNA for human atrial natriuretic factor.

A human genomic DNA clone for atrial natriuretic factor (ANF) gene was isolated from a human gene library using the previously cloned cDNA for rat ANF as a probe. Partial nucleotide sequencing of the cloned DNA revealed the location of a 120-bp long intron between Lys-41 and Asn-42 of the ANF precursor. Restriction mapping also suggested the existence of at least one other intron in or proximal to the 3' -untranslated region.

Inhibitors of platelet aggregation and blood coagulation isolated from the human placenta.

We identified 2 potent proteins with inhibiting activities of platelet aggregation and blood coagulation from the human placenta. Ultracentrifugation of placental homogenate showed that these proteins are incorporated in the microsomal membrane. These proteins were solubilized in Triton X-100 by means of sonication. The inhibitors were purified by several steps of chromatography and gel filtration in a single band on polyacrylamide gel electrophoresis, having molecular weights of 450 000 and 45 000. The aggregation inhibitor was different from inhibitors reported previously such as phosphatase and prostacyclin-like substance. The coagulation inhibitor was capable of inactivating tissue thromboplastin.

[Clinical and bacteriological efficiency of sulbactam/cefoperazone in gynecological infectious diseases].

The combined antibiotics, sulbactam/cefoperazone (SBT/CPZ), was administrated intravenously to 10 patients with various kinds of gynecological infectious diseases and the efficiency of the drug was investigated clinically or bacteriologically. The results obtained were as follows. The clinical and bacteriological efficiency rate of SBT/CPZ on gynecological infectious diseases was determined as about 100 per cent and 75 per cent. Three species out of 11 isolated bacteria produced beta-lactamase. Two cases out of 3 beta-lactamase positive bacteria were disappeared in the materials obtained from the patients after the treatment of the drug. No remarkable side effect except for 1 buccal exanthema and 1 genital candidiasis was detected during the administration course of the drug.

Atrial natriuretic factor: purification of active peptides, cloning of cDNA and determination of structures of active peptides and precursors.

Four peptides possessing both natriuretic and smooth muscle relaxant activities were purified from rat heart atrium and their amino acid sequences were determined. All contained a common sequence which contains a macro-ring structure formed by 17 amino acid residues and a disulphide bridge. The major atrial peptide in the atrium was identified as that containing 31 amino acid residues. The cDNA of the atrial peptide precursor was cloned and its nucleotide sequence determined. The amino acid sequence of the precursor deduced from the nucleotide sequence contained 152 residues and a potential signal peptide sequence characteristic of secretory polypeptides.

[Effect of vitamin K2 syrup in neonatal hypoprothrombinemia].

To clarify whether vitamin-K prophylaxis in newborns is effective, a double-blind study with oral vitamin-K2 syrup was performed. There were no side-effects. Various dosages of vitamin K2 or placebo syrup were tried in 148 newborns with thrombotest results below 20%. The efficiency decreased as follows: vit-K2 6 mg greater than vit-K2 2 mg greater than vit-K1 2 mg greater than placebo. The effect of all vitamin K preparations on the coagulation system was significantly different from the placebo effect. With regard to our own results and the literature, general vitamin-K prophylaxis for newborns is not recommended. However, vitamin K 1 X 6 mg orally is helpful in newborns with thrombotest results below 20%, in infants at risk, and in all premature, dystrophic or otherwise underweight infants.

Nuclear binding and sedimentation properties of two activated forms of glucocorticoid-receptor complexes from rat mammary glands.

Mammary cytoplasmic glucocorticoid-receptor complexes, which had been exposed to a high concentration of ammonium sulfate, were found to be separated into two receptor forms, consisting of DNA-cellulose-bound and non-DNA-cellulose-bound receptors. Experiments under cell-free conditions revealed that not only the DNA-cellulose-bound receptor but also the non-DNA-cellulose-bound receptor have binding abilities to nuclei and to nuclear chromatin. In sucrose density gradient centrifugation, the non-DNA-cellulose-bound receptor sedimented at higher sedimentation coefficients than the DNA-cellulose-bound receptor under all the salt conditions tested. These data strongly suggest that in the mammary glucocorticoid receptor system there exist at least two activated receptor forms with different conformations.

The kinin-forming enzyme system in pregnancy and obstetrical DIC.

Levels of prekallikrein and HMW kininogen that had increased during pregnancy decreased with start of labor. The role of the kinin-forming system with oxytocin in the mechanism of labor was suggested from the results of decreased prekallikrein and HMW kininogen, appearance of a free kallikrein-like enzyme during labor, and from the case of arrested labor in which both prekallikrein and HMW kininogen were markedly decreased. Prekallikrein was markedly decreased in patients with acute obstetrical DIC and severe toxemia of pregnancy. The excessive activation of prekallikrein in DIC seemed to be of help for understanding such clinical signs as shock, abnormal labor, and increased permeability in obstetrical DIC.

Endocrinological and histological aspects of the process of polycystic ovary formation in the rat treated with testosterone propionate.

To elucidate the process underlying polycystic ovary (PCO) syndrome, we tried to produce experimental PCO by sterilizing rats with androgen and investigated the formation of PCO endocrinologically and histologically. Wistar-Imamichi strain female rats were given an injection of testosterone propionate 1.25 mg on 5 days of age. On 5, 10, 20, 40, 60, 80, 100, 130, 160, 200 days after the injection the ovaries were obtained. Ovarian wedge resection was also performed at 100 and 200 day-rats. Serum gonadotropins, prolactin and steroids were measured with RIA and compared with ovarian pathology. Serum prolactin was increased drastically 40 days after the injection and maintained at a constant level higher than control. Through wedge resection, the serum LH level returned to normal, but FSH slightly increased. Serum estradiol decreased more markedly than estrone did. The 40 day-ovaries showed polycystic features, lack of corpus luteum, hyperthecosis, luteinization of stroma, while no thickening of tunica albuginea. In conclusion, the rats treated with testosterone propionate had many endocrinological and histological aspects similar to those of human PCO syndrome.

[Tissue penetration and clinical effects of ceftazidime in the field of obstetrics and gynecology].

To study concentrations in the gynecological organs, ceftazidime (CAZ) was given intravenously by one shot of 0.5 g to 1 patient and of 1.0 g to 10 patients who underwent simple hysterectomy because of uterine myoma. Also, clinical effect of the drug was studied in 3 cases. The levels of CAZ in sera in uterine artery and elbow vein, and tissue concentrations in various sites of the gynecological organs obtained during 50 to 425 minutes after administration were determined by the paper-disc method with P. mirabilis ATCC 21100 strain. Concentrations of CAZ at 92 minutes after administration of 1.0 g i.v. were 39.8 mcg/ml in serum of uterine artery, 47.6 mcg/ml in serum of elbow vein, 20.5 mcg/g (tissue/serum ratio 0.43) in the ovary, 18.0 mcg/g (0.38) in the fallopian tube, 16.1 mcg/g (0.34) in the endometrium, 34.5 mcg/g (0.72) in the myometrium, 30.8 mcg/g (0.65) in the uterine cervix, 36.4 mcg/g (0.76) in the portio vaginalis and then gradually decreased time-dependently. Tissue concentrations were higher than those in serum in the endometrium, uterine cervix and portio vaginalis at 147 minutes after administration. CAZ concentrations of adipose tissue between 353 and 365 minutes after injection of 1.0 g were lower than 0.8 mcg/g. The clinical responses to CAZ in intrapelvic infections were good in all cases. Slight elevation of total bilirubin, to 1.5 mg/dl, was observed in a case. In the light of these clinical findings, CAZ appears to be a potent antibiotic effective in the clinical application.
