Preliminary report comparing piperacillin and carbenicillin for complicated urinary tract infections.

Piperacillin is a new semisynthetic penicillin with a broad spectrum of in vitro activity against common gram-negative urinary tract pathogens. We compared the efficacy and safety of piperacillin versus carbenicillin in patients with complicated urinary tract infection. A total of 56 adult patients (mean age 55 years) in stable medical condition with 1 or more structural genitourinary abnormalities entered the study. Of these patients 27 were evaluated for antibiotic efficacy. There were 20 lower tract and 7 upper tract infections, of which 17 were acute and 10 were chronic. Patients were randomized into 2 groups: 17 patients with 18 organisms received single agent treatment with 181 mg. per kg. intravenous piperacillin daily for 6 days and 10 patients with 11 organisms received 270 mg. per kg. intravenous carbenicillin daily for 6 days. Infecting organisms were Escherichia coli 45 per cent, Proteus mirabilis 14 per cent, Klebsiella pneumoniae 14 per cent. Enterobacter species 10 per cent, Pseudomonas aeruginosa 7 per cent and so forth. Antimicrobial susceptibility assessed by measurement of minimal inhibitory concentration and disk diffusion zone size demonstrated superior activity of piperacillin over carbenicillin for most micro-organisms tested. All patients responded clinically. The bacteriologic cure rate was 72 per cent at 5 to 9 days after therapy in both groups. Three patients who received piperacillin had urosepsis and were cured. No resistance emerged during therapy. Superinfections developed in 5 patients on carbenicillin (50 per cent) and in 4 patients on piperacillin (24 per cent), and none was resistant to piperacillin. Superinfections were attributed to catheterization and structural genitourinary abnormalities. The over-all incidence of adverse effects in patients on piperacillin was less than that of those on carbenicillin, 31 and 51 per cent respectively. Side effects in both groups were mild and did not require discontinuation of therapy. There were no significant alterations in fluid and electrolyte balance, or hematologic or renal function.

Immunoperoxidase localization of keratin proteins, carcinoembryonic antigen, and factor VIII in adenomatoid tumors: evidence for a mesothelial derivation.

The histogenesis of adenomatoid tumors has been a source of controversy. Although most investigators favor a mesothelial derivation, recent ultrastructural evidence suggests that some adenomatoid tumors may be vascular neoplasms. This study reports the pattern of staining of seven adenomatoid tumors with antisera to keratin, factor VIII, and carcinoembryonic antigen (CEA), using an immunoperoxidase technique. All the tumors revealed strong cytoplasmic staining for keratin with no staining for factor VIII (an endothelial cell marker) or CEA. An identical staining pattern was evident in normal and reactive mesothelial cells. These findings support a mesothelial rather than an endothelial derivation for the tumors studied.

Mapping studies on human mitochondrial glutamate oxaloacetate transaminase.

Data from six primary hybrids and twenty-two subclones have confirmed the assignment of the mitochondrial form of glutamate oxaloacetate transaminase to chromosome 16. Family studies have provided independent confirmation of this and have suggested the gene order PGP-16qh-GOT2-HP. These studies were made easier by the development of a new stain for the detection of GOT activity.

Right atrial tamponade complicating cardiac operation: clinical, hemodynamic, and scintigraphic correlates.

Persistent bleeding into the pericardial space in the early hours after cardiac operation not uncommonly results in cardiac tamponade. Single chamber tamponade also might be expected, since in this setting the pericardium frequently contains firm blood clots localized to the area of active bleeding. However, this complication has received very little attention in the surgical literature. We are therefore providing documentation that isolated right atrial tamponade can occur as a complication of cardiac operation and that there exists a potential for misdiagnosis and hence incorrect treatment of this condition. Right atrial tamponade may be recognized by a combination of low cardiac output, low blood pressure, prominent neck veins, right atrial pressure in excess of pulmonary capillary wedge pressure and right ventricular end-diastolic pressure, and a poor response to plasma volume expansion. Findings on chest roentgenogram and gated wall motion scintigraphy may be highly suggestive. This review should serve to increase awareness of this complication and to provide some helpful diagnostic clues.

Egg yolk-induced lipolysosome proliferation and fat infiltration of rat liver.

The contribution of subcellular organelles in egg yolk-induced fatty liver was investigated in liver tissue sections and fractionated liver homogenates. When 2.0 ml. of 12.5 per cent egg yolk were administered to Wistar male rats (body weight, 250 to 350 gm.), a fatty change occurred in the liver after a 4-hour period. The acute fatty liver was characterized by intralysosomal accumulations of fat. The number of fat-containing lysosomes increased as early as 30 minutes after egg yolk administration. During this period, fat infiltration was not evident elsewhere in the cytoplasm. The increase of these lysosomes in tissue sections coincided with a selective proliferation of lipolysosomes in the heavy subfraction of floating lipids of liver homogenate. Neither Golgi-derived dense bodies of very low density lipoproteins nor vesicles of the endoplasmic reticulum increased in number throughout the experimental period. These observations indicate that hepatic lysosomes are involved in the degradation of exogenous cholesterol and that transformation of lysosomes to lipolysosomes is a direct result of cholesterol overload and not simply a manifestation of fatty liver.

A solid phase enzyme immunoassay for the quantitation of serum ferritin.

A non-competitive solid phase enzyme immunoassay for the measurement of ferritin in human serum is described. This procedure involves the use of specific antibody covalently attached to derivatized hydrophilic microparticles and enzyme labeled specific antibody. Reproducible results were achieved within 4 h for ferritin in serum in the range of 4 ng/ml to 250 ng/ml. Ferritin levels as low as 0.4 ng/ml can be measured. The enzyme immunoassay and three commercially available radioimmunoassay(RIA) kits were used to determine serum ferritin levels in healthy adults. Good agreement was found between the enzyme immunoassay and the RIA methods.

A study of mitosis, meiosis, histology, and scanning electron microscopic details of spermatogenesis in an infertile male with probable 46,XY/47,XXY germinal mosaicism.

A 29-year-old infertile man with mild eunuchoid body proportions and unilateral gynecomastia with a low sperm count had normal chromosomes in lymphocytes and skin fibroblasts. Serum testosterone, urinary ketosteroid, and follicle-stimulating hormone levels were normal, but serum prolactin, luteinizing hormone, and estrogen levels were above normal. Testicular biopsy showed hypocellularity, a pacuity of maturing sperm, and increased lipid content in early spermatogonia. Cytogenetic analysis of the testicular material showed probable 46,XY/47,XXY mosaicism in spermatogonial cells. None of the cells in diakinesis and metaphase I showed the extra-X as a univalent, and 50% of the cells in diakinesis were hypodiploid with intact XY bivalents. Further analysis of spermatogenesis revealed polymorphic dyads, triads, tetrads, and uncleaved meiotic products of first and second meiosis. Sex chromatin studies of testicular cells showed that 20% of the cells were X-chromatin positive. Scanning electron microscopic study of sperm revealed a wide range of polymorphisms, with some uncleaved meiotic products maturing into double-headed and double-tailed sperm or sperm with bulbous middle pieces or protrusions. A cytogenetic basis for the origin of these abnormalities is presented.

The usefulness of NOR and RFA banding in prenatal diagnosis: a case report.

Prenatal chromosome analysis of amniotic cells showed a male fetus to carry an unusual marker D chromosome containing extra genetic material on the short arm, which could be interpreted as a possible t(D;G) or t(D;Fq) unbalanced translocation using non-branded preparations or a routine GTG band method. Analysis of this marker by NOR and RFA banding and parental chromosome studies showed the marker to be an unusual variant of the satellited area of chromosome 15, with no associated phenotypic anomalies. The usefulness of NOR and RFA banding as aids in chromosome identification in prenatal diagnosis is discussed.
