[Problems of early detection of latent pulmonary hypertension--the value of ergometric tests and the correlation between pulmonary circulation and parameters of lung function (author's transl)].

The possibility of early detection of pulmonary hypertension is rather late according to the large capacity of the lung vessels in the condition of rest. Consequently the parameters of the pulmonary circulation were measured in 89 healthy subjects and 261 men suffering from silicosis in rest and bicycle exercise by means of the microcatheter technique and statistically compared with other parameters of lung function. In conclusion it can be said there is a statistical correlation between pressure at rest and during exercise conditions, but this is not available in an individual case. Furthermore the pressure values of persons suffering from silicosis are higher than those of the healthy group at rest and the pressure rise is higher in the ill group during exercise. Statistically there is a slight correlation between the pressure behavior and the FEV1, Rtot, PO2, DCO and of minor importance to VC, RC and FRC.

[The influence of hypobaric hypoxia upon the hemodynamics of the pulmonary circulation (author's transl)].

The influence of hypobaric hypoxia at the pulmonary gaseous exchange and the hemodynamics of the pulmonary circulation was tested by 12 healthy subjects at the age from 33 to 47 years. They were submitted to a basis investigation with determination of VC, VE, VO2, VCO2, Hb, PO2A/a, pH, HR, ECG, PVd, PAp, HZV, RL. These investigations were followed by stress examination by means of the bicycle ergometry in four rectangular steps during conditions of normoxia and of hypobaric hypoxia in a low-pressure chambre at 66.5 kPa (500 Torr) each of 6 min duration. During the influence of hypoxia a PO2a-decrease in a sense of partial insufficiency could be already observed at rest during conditions of a decreased alveolar oxygen-partial-pressure. At the same time was found a significant increase of the systolic peripheral and the diastolic pulmonary arterial pressure. At the increasing step by step ergometric exercise during conditions of the hypoxia stress were growing significance evidence of different reactions of the pulmonary and cardiocirculatory parameters occurred.

Biological role of gramicidin S in spore functions. Studies on gramicidin-S-negative mutants of Bacillus brevis ATCC9999.

Gramicidin-S-negative mutants of Bacillus brevis ATCC9999 have been isolated with a remarkly higher yield after ethidium bromide or acridine orange treatment, than after N-methyl-N'-nitro-N-nitrosoguanidine treatment. Four (MIV, Smr170, R5 and EB 16) of 38 isolated mutants were characterized with respect to the lesion in gramicidin-S-synthesizing activity. The mutants sporulate to the same extent as the parental strain except mutant Smr 170 which sporulates less. However, mutant spores were more heat-sensitive and possessed a reduced level of dipicolinic acid content. No significant difference was observed in the germination time of wild-type and mutant spores. All spores germinated after 80--110 min, but the outgrowth time was different: all gramicidin-S-negative mutants grew out immediately after germination whereas wild-type spores required a lag period of 9--10 h. When the mutants were allowed to sporulate in the presence of gramicidin S, the spores were found to be heat-resistant and their outgrowth postponed to the same period as the parent spores. The addition of gramicidin also eliminated the deficiency of dipicolinic acid. A new class of gramicidin-S-negative mutant, R5, which only activates L-valine and L-leucine, is described. A possible biological function of gramicidin S in the heat-resistance and in the timing of spore outgrowth is discussed.

[Effect of hypoxia on pulmonary gas exchange and hemodynamics].

To test the influence of hypoxia upon the pulmonary gaseous exchange and the hemodynamics of the pulmonary circulation 12 healthy test-persons at the age from 33 to 47 years (mean = 36.7 years) were submitted to a pulmonary and cardiocirculatory basis-investigation with determination of VK, VE, VO2, VCO2, Hb, PO2A/a, PCO2A/a, pH, BE, HR, ECG, PVd, PAp, SV, HZV, RL. These investigations were followed by stress examination by means of the bicycle-ergometry in four rectangular steps under conditions of normoxia from 95.5 to 97.3 kPa, x = 96.5 kPa (716 to 730 torr, mean = 723.6 torr) and of hypobarohypoxia in a low-pressure chambre at 66.5 kPa (500 torr) each of 6 min duration. Under the influence of hypoxia a PO2a-decrease in the sense of partial insufficiency could be already observed at rest under conditions of a decreased alveolar oxygen-partial-pressure. At this simultaneously increases the CO2-exhalation. At the same time was found a significant increase of the systolic peripheric and the diastolic pulmonary-arterial pressure. At the increasing step-by-step ergometric employment under conditions of the hypoxia stress were growing significance evidence of different reactions of the pulmonary and cardiocirculatory parameters occurred.

[Significance of bronchial hyperreactivity (tested with acetylcholine) and ventilatory inhomogeneity for the early detection and differentiation of chronic nonspecific lung disease (CNSLD) (author's transl)].

Bronchial hyperreactivity (tested with 0.01% acetylcholine ultrasonic aerosol) and ventilatory inhomogeneity (tested with V2/V1-parameter of the flow-volume diagram) using the pneumotest apparatus were studied. Patients with high bronchial reactivity are frequently showing not only ventilatory disorders of distribution but sometimes also an additional increase due to the bronchial provocation. Bronchial reactivity testing combined with the flow-volume diagram allows to estimate the predominant site of the bronchospastic response either centrally or also in the more peripheral parts of the bronchial system. By testing bronchial reactivity persons without any symptoms and with no evident spirometric alterations can be separated in dependence on the relative increase of total airway flow resistance as well on smoking habits. That does not succeed with the appliance of the inhomogeneity parameters V2/V1. The results obtained in this study are of value for preventive measures, for early detection of risk factors in premorbid stages, for assessment of prognosis in chronic nonspecific lung disease, for an adequate therapy and also for the rehabilitation of patients in jobs with inhalation of noxious dusts.

[Effect of bronchial reactivity on the whole-body plethysmographic inhomogeneity parameter REmax/RE0 and its value in the early diagnosis of chronic nonspecific lung diseases].

Examinations performed with the help of the body-plethysmographic inhomogeneity parameter REmax/RE0 prove the close connection between the development of chronic unspecific pulmonary diseases and the appearance of disturbances of the ventilatory distribution. The functional disturbance metioned is further increased by the bronchial provocation and may partly be demonstrated in a characteristic way also in inconspicuous initial findings. The body-plethysmographic inhomogeneity parameter REmax/RE0 is, however, not suited for the early diagnosis of chronic unspecific pulmonary diseases, since with its help the verification of disturbances of the ventilatory distribution is successful only then, when the total resistance is already increased. For the final judgment of the aptitude of the bronchial reactivity test for the increase of the valency of methods of the distribution analysis in the early diagnosis further examinations are necessary.

[Possibility of the effect of a long-term activity as a welder on the development of a chronic obstructive airway syndrome].

On 62 welders and in a control group consisting of 90 test persons dynamic functional parameters of the lungs were established. In these cases welders older than 40 years after an activity lasting at least 2 decades gave references to a chronic obstructive disease of the respiratory tract, which after exclusion of other etiological factors may be regarded as a sequel of irritatively and toxically effecting welding fumes and gases.

[Reliability of the ECG analyzer EAK-2 in screening tests].

In the last years the number of derivated electrocardiograms with several approaches to the problem has permanently increased. Therefore, the computer analysis seems to be obvious. The reliability of the analogo-digital-computer Express Analyzer Cardiograph EAC-2, Medexport Moscow, was for the employment for occupational medical examinations investigated on 252 experimental persons and compared with the independent manual analysis, which was carried out by two physicians. 80 per cent of the available ECGs were automatically correctly arranged into groups and all the inconspicuous electrocardiograms were sorted out, since it occurs on no account that an electrocardiogram which was recognized as pathological by means of manual analyses, was analysed as falsely normal by the computer EAC-2. Consequently, EAC-2 at low technical interference susceptibility may be very useful for the ECG-diagnosis by means of the computer during the screening investigations.

[Evaluation of weather-dependent dyspnea by means of body plethysmographic study of bronchial reactivity].

By body plethysmographic investigations including the test of the bronchial reactivity (ultrasound aerosol of a 0.01% acetylcholine solution) was demonstrated that in dependence on the degree of the manifest disturbance of ventilation the frequency of the bronchial hyperreactivity (increase of the total resistance by at least 3.0 cm H2O/l/s compared with the initial position) increases. The constellation of findings mentioned leads to a large increase of dyspnoea depending on weather.

[Evaluation of measurement accuracy in microcatheterization of the heart].

In 192 patients with silicosis the results of the cardiac micro-catheterisation under conditions of rest were compared with those ones of a secondary examination performed two years later. This allows conclusions to the behaviour of the pressure values measured in the period observed as well as to the evidence of an individual cardiac micro-catheterisation. No essential increase of the mean pressure of the pulmonary artery was observed. The dispersion of an individual measurement is 3 Torr. A pulmonary hypertension, which is given from 20 Torr upwards, consequently may be established with a 95% statistical certainty only from an individual measuring value of 26 Torr.

[Thermographic thorax diagnostics].

Among several variants of devices the infra-red thermography has obtained a wide technical perfection allowing to measure the thermal energy emitted from the body surface without contact, quickly and dynamically, and to produce a picture for analysis. Since numerous functional and pathophysiological proceedings are connected with changes of temperature, the use of thermography for diagnosis of processes near the body surface may be recommended. Based on thermic observations in 63 patients and a control experiment in 15 persons, experiences with the thermography in the diagnosis of diseases, which are localized more profoundly in the thoracic cavity, were reported.

[Mode of action and inhibition of polygalacturonase covalently bound to polysaccharide and glass carriers].

Endo-polygalacturonase (EC 3.2.1.15.) from Aspergillus spec. is much changed as far as its mode of action and the interaction with vegetable inhibitors of pectinase (from green beans and cucumbers) are concerned when it is covalently bound to insoluble carriers (Sepharose, cellulose powder, macroporous glass and nonporous ballotinis). Whereas a 2% degradation of substrate by the soluble enzyme caused a 50% decrease of viscosity of citrus pectic acid, the comparable degradation of substrate was increased to a level of about 10% with the investigated polygalacturonase carrier complexes apparently independent of the properties of the carriers and the kind of binding of the enzyme. In contrast to this the higher degradation of substrate of 15 and 20% respectively which was further stated at a 50% decrease of viscosity is unambiguously connected with the carriers and is in direct correlation with the specific activity of the polygalacturonase carrier complexes. Contrary to the soluble enzyme the covalently bound enzyme produces more lower oligomerous galacturonic acids by an exo-mechanism or by multiple attack already at the beginning of the hydrolysis of pectic acid. During the final stage there is an enrichment of trigalacturonic acid besides mono- and digalacturonic acids independent of the state of solution of the enzyme. It could further be stated that the strong inhibition of the soluble endo-polygalacturonase by selected pectinase inhibitors which was described earlier is reduced by degrees with the enzyme covalently bound to the insoluble carriers.

[Energy expenditure on basic work-posts in metallurgy].

The paper presents results of measurements of the whole energetic cost on the work-posts recognized by the management as basic. Altogether 121 determinations of energetic cost were performed on 18 different work-posts in two basic divisions of the steelworks. The results revealed that despite a considerable technical progress there is a significant number of work-posts, on which energetic cost exceeds 2600 kcal/8 hr. On these work-posts apart from the great energetic load, other loads, connected with material work environment on character of work, occur. This means that additional factors frequently increase their harmful effects on organism. The magnitude of energetic expenditure on a given work-post establishes the hierarchy of modernizational requirements for the plant's technical services.

[Definition of limits of bronchial reactivity with aid of bodyplethysmography (author's transl)].

Investigations for dose-effect relationship of acetylcholine-ultrasonic-aerosols (apparatus "TUR" USI3, duration of inhalation three minutes, control of the ventilation parameters five minutes after termination of inhalation) led to the result that the delimination of the bronchial hyperreactivity is already possible by application of a 0.01% test-concentration. In this way the ultrasonic-aerosol-method also leads to results which are comparable with the nozzle-aerosol-method used in GDR (0.5% test-concentration, duration of inhalation one minute). As a limiting criteria for the evaluation of a bronchial hyperreactivity analogous to the decrease in the FEV1 of greater than or equal to 15% in comparison with the starting position for the bodyplethysmography yield an increase of the resistance totalis of greater than or equal to 3.0 cm H2O/1/s.
