Effect of glucagon on the double contrast examination of the stomach and duodenum.

We have evaluated the effect of glucagon on the quality of visualization of the gastric and duodenal mucosa in a routine tubeless double contrast examination of 418 consecutive cases. The patients received either 0.5 mg i.v., 0.25 mg i.v. or no glucagon, at radom. The use of glucagon improved the visualization of areae gastricae and produced slightly fewer gas bubbles in the stomach. Visualization of the duodenum was significantly improved. The frequency of adequate hypotonic duodenography was related to the dose of glucagon, with 0.5 mg it was achieved in 49% of cases. No side-effects ascribable to glucagon were observed. The routine use of a small i.v. dose of glucagon in double contrast studies is recommended.

Lymphatics filled at knee arthrography.

A review of 587 knee arthrographies revealed filling of lymph vessels in 12 cases. They occurred in connection with arthritis (4 rheumatoid), osteoarthritis, ruptured meniscus, ruptured popliteal cyst and after operation. Lymphatic filling was not related to the leakage of contrast through a ruptured popliteal cyst or to inadvertent extra-articular injection. Filling of lymphatics is more likely to occur in examinations with a positive contrast medium when a larger amount of medium is used.

Renal circulation after cardiac arrest. Angiography and microangiography.

Renal circulation following 3 to 14 min of cardiac arrest was investigated in 21 mongrel dogs by angiography, microangiography and histology. Arrest longer than 7 min caused variable degrees of abnormal angiographic findings after resuscitation: (1) segmental perfusion defects and cortical areas without nephrographic effect, (2) loss of demarcation of cortex and medulla, (3) decreased nephrography, (4) faint and delayed filling of the renal veins, (5) diffuse segmental spasm of the interlobar arteries with delayed emptying. Microangiography demonstrated non-filling of glomeruli in segmental areas of renal cortex, diffuse malperfusion and vasoconstriction as well as obliteration of afferent arterioles due to red blood cell aggregation. No tendency toward recovery within the 5-hour resuscitation period.

Contrast enhancement: diagnostic significance from a statistical viewpoint.

Contrast enhancement attenuation units were calculated from 13 tissues and organs of 20 rats at five different times after bolus 125I-diatrizoate administration. Percent distribution volume of diatrizoate was calculated from these results. The statistical significance of differences in contrast enhancement between each of 72 tissue pairs was determined at each of the five times. A similar treatment of percent distribution volume for 60 tissue pairs was determined. The time elapsed after contrast medium injection appears to be of pivotal importance in contrast enhancement diagnosis. Calculation of percent contrast distribution volume is of potential diagnostic importance, since it reveals contrast medium behavior not evident from a visual perusal of scans. The above methods can be readily adapted to computed tomography scanning.

[Computer tomography--present role and future prospects (author's transl)].

The actual CT apparatus requirements connot be estimated at present. For cranial CT, one apparatus for every 200,000 inhabitants is considered necessary. For radiotherapy, apparatus with a slow scanning rate will be sufficient. A scanning time below 1 second is required for cardiac investigations. More attention should be paid to the kinetics of organ-specific contrast media, so that better diagnostic information may become possible by effecting the roentgenograms at the most favorable time.

Intravenous bolus of 125I labeled meglumine diatrizoate. Early extravascular distribution.

A mixture of 125I labeled meglumine diatrizoate and 131I labeled human serum albumin was injected into the femoral vein of 26 anesthetized male rats. Measurements of the activities in cardiac blood and in different tissues of the lower extremity and in the testis were performed at time intervals ranging from 5 s to 5 min after injection. The determination of tissue uptake and distribution volumes of diatrizoate showed widely differing accumulation of contrast medium. Over 50 per cent of the intravenous bolus of diatrizoate was extravascular at 40 s.

Intra-arterial bolus of 125I labeled meglumine diatrizoate. Early extravascular distribution.

A mixture of 125I labeled meglumine diatrizoate and 131I labeled human serum albumin was injected into the lower abdominal aorta of 30 anesthetized, laparotomized male rats. Measurements of the activities in cardiac blood and in different tissues of the hindlimbs and tests were perfomed at six time intervals ranging from 5 seconds to 2 minutes after injection, the determine early uptake and distribution volumes of diatrizoate. Concentrations and distribution volumes were initially much greater than values obtained after intravenous injection, but these differences had considerably decreased or disappeared by 2 minutes.

Angiography of the testicular artery. III. Testis and epididymis analysed with a magnification technique.

The resolving capacity of magnification testicular angiography, as related to the known vascular anatomy of the testis and epididymis, was analysed on the basis of 9 normal angiographies and 8 cases of hydrocele with proven normal status of the testis and epididymis. The intratesticular vascular arrangement was demonstrated remarkably well, but of the epididymal vascularity only that in the head of the epididymis. The vascular anatomy of the testis as resolved by angiography may be utilized in the diagnosis of the location and nature of intra- and extratesticular mass lesions.

Microvascular structure of the human epididymis.

The microvascular anatomy of the human epididymal duct system from rete testis to vas deferens was studied using microangiography and histology. Various segments of the duct system show significant differences in the density and arrangement of the small blood vessels. The rete testis is poorly vascularized while the first lobulated segment of the epididymis, which is formed by the efferent ducts is provided with a dense subepithelial capillary bed. The more distal segments of the epididymis have less dense capillarization surrounding the epididymal duct. In the vas deferens the blood vessels form a double capillary network. The outer capillary network surrounds the smooth muscle layers and the inner is located immediately beneath the duct epithelium. The organization of blood capillaries in the human epidymis and the differences seen in different portions of the excurrent duct system follow basically the general pattern seen in some experimental animals.

Use of tissue culture to examine neurotoxicity of contrast media.

The effect of small concentrations of diatrizoate, ioglycamate, and iopanoate salts on cultured sympathetic ganglia was studied. Biliary contrast media in clinical blood concentrations caused severe degeneration of cultured neurons, but similar concentrations of diatrizoate caused no effect. This technique may be used to measure the direct neurotoxicity or general cytotoxicity of contrast media.

Extravascular contrast material: the major component of contrast enhancement.

The distribution volume of 125l-diatrizoate in rat tissues was studied 5 seconds to 5 minutes after intravenous injection and 5 seconds to 2 minutes after intra-arterial injection. Immediately after injection, diatrizoate was distributed into a larger volume than plasma. Marked temporal changes in the distribution volume of individual tissues occurred during the first 2 minutes. At 5 minutes, the diatrizoate space approached the total extracellular fluid volume, with more than 80% of the contrast agent remaining outside the blood vessels. Thus the relative magnitude of contrast enhancement of a tissue appears to be related to the volume of the rapidly equilibrating extracellular space. Intra-arterial administration can significantly overload muscle with contrast material for about 2 minutes. This and other temporal changes in contrast distribution may prove useful in contrast-enhanced computed tomography using short scanning times.

Lymphatic filling during knee arthrography.

Six cases of lymphatic filling during knee arthrography have been recorded in literature, all of which have had advanced rheumatoid arthritis. We report a case of lymph vessel filling from a popliteal cyst in a young male nonrheumatoid patient. It is concluded that abnormal synovial-lymphatic connection in the knee joint may occur also in other forms of synovial pathology and such a finding does not indicate rheumatoid inflammation.

An angiographic study of the fox testis in various stages of sexual activity.

The arteries and veins of the fox testis were studied using an angiographic technique. The postnatal development of the testis involved only minor modification of the vasculature. In the non-breeding season both the arteries and veins of the adult fox testis undergo extensive spiralization which completely disappears during the breeding season. When the sequence of morphological changes in blood vessels are related to available data on changes in testicular size, blood flow and connective tissue content, it is obvious that the vascular spiralization is passive and related to shrinkage of the organ and dose not primarily influence the testicular blood flow.
