Contrast enhancement versus vasculature of uterine tumors.

The relative contrast enhancement of normal myometrium and small or medium size uterine fibroids and clinical stage I endometrial carcinomas were correlated to their microangiographically demonstrable vascularity. In four cases small uterine fibroids showed contrast enhancement equal to normal myometrium, independent of their vascularity in microangiography. In five, endometrial carcinomas contrast enhancement was always below that of the surrounding myometrium in CT. Intratumour vasculature showed great variability in microangiography. No correlation between the topographic variations in the arrangement or density of tumour blood vessels and the degree of contrast enhancement was found. Contrast enhancement of small uterine fibroids was greater than that of small adenocarcinomas. More contrast material accumulates tissue of the fibroid than into the extravascular space of endometrial carcinoma.

Sodium taurocholate, ineffective as an adjuvant in oral cholecystography.

The effect of 6 g sodium taurocholate on the visualization of the gall bladder with calcium iopodate (Solu-Biloptin, Leiras, Schering AG) was studied on 140 patients with a control group of 99 patients. The administration of sodium taurocholate with a contrast medium did not improve the opacification of the gall bladder in oral cholecystography with calcium iopodate but induced a significant increase in side effects.

Pseudocystic pheochromocytoma associated with pneumatosis cystoides intestinalis. Case report.

The authors report a case of a large pseudocystic pheochromocytoma, which initially was operated on and histologically diagnosed as a pancreatic pseudocyst. After recurrence, a multilocular cystic tumor was found both by ultrasonography and CT. ERCP demonstrated a cut-off of the pancreatic duct. Re-operation revealed a cystic adrenal tumor anastomosed to the stomach. The operation was complicated by a hypertensive crisis and a subsequent subendocardial infarction. In the preoperative period the patient had continuous diarrhea and pneumatosis cystoides intestinalis was demonstrated by double contrast barium enema. The pneumatosis disappeared within three months but a recurrent tumor appeared. The importance of CT in the preoperative work-up of upper abdominal lesions is emphasized, as atypical cystic masses may cause differential diagnostic problems in surgery or even in localized biopsies, while CT would give a better overall view of the tumor.

Dimethylpolysiloxane as an adjuvant in double-contrast barium enema.

The value of Dimethylpolysiloxane (Simethicone) in the reduction of gas bubbles and foam formation, and in the prevention of flocculation of contrast medium during the double-contrast barium enema examination is presented. Altogether 600 unselected patients were studied, 300 with barium suspension diluted with 10 vol % of distilled water only, and 300 with the same suspension into which dimethylpolysiloxane was added. Simethicone significantly reduced gas bubbles and foam formation. It also prevented flocculation and cracking of mucosal coating, while it had no effect on the density of mucosal coating.

The posterior tracheal band: correlation between computed tomography and chest radiography.

As seen on the lateral chest radiograph, the posterior tracheal band (PTB) is useful in evaluating the retrotracheal area. The anatomical basis of its variations on the lateral chest radiograph were analyzed by comparison with computed tomography (CT). Lack of visualization or occasional thickening of the PTB is explained on an anatomical basis. Such variations are distinguishable from retrotracheal abnormalities on CT.

Contrast enhancement pharmacokinetics in experimental pancreatitis, diabetes and subcutaneous granuloma.

Contrast medium concentration in the pancreas as a function of time after intravenous bolus injection was measured in 12 rats with oil-induced acute pancreatitis, 20 rats with Streptozotocin-induced diabetes and 20 rats with subcutaneous granuloma, induced by sponge, whose pancreatic tissue was used as a control. No significant effect of diabetes was observed. Calculated distribution volume and contrast enhancement in the pancreas were increased in pancreatitis relative to diabetes and controls. The increased enhancement was due to a relatively higher accumulation into the extravascular fluid, considered to represent pancreatic oedema. An even more marked and delayed enhancement was observed with the granuloma tissue and exudate, apparently on a similar basis. There appears to be a possibility of differential diagnosis of some pancreatic lesions using contrast enhanced CT.

Posterior tracheal band in esophageal carcinoma.

The appearance and diagnostic usefulness of the posterior tracheal band (PTB) was studied from upright chest radiographs taken from 178 individuals with diagnosed esophageal squamous cell carcinoma. PTB was visualized well enough to be diagnostically useful in about 80-90% of individuals depending on the location of the carcinoma. PTB or a portion of it measured 6 mm or more in 57.1, 72.4 and 37.3% of cases of carcinoma of the upper, middle and lower third of the esophagus, respectively. While a width of 4-5 mm is frequently observed in normal patients, especially in the upper third of the PTB, a width of 6 mm or more and/or progressive widening of more than 2 mm between consecutive films is highly suggestive of pathology. Correlation between changes in PTB and operability or prognosis is poor.

Allergological risk factors as predictors of radiographic contrast media hypersensitivity.

An in-depth allergy history was obtained from 830 consecutive patients who were subjected to intravenous radiographic contrast medium (RCM) examinations. During the RCM examination 47 patients reacted with symptoms of hypersensitivity. In 26 of these (55.3%) a positive previous allergy history had been obtained. Although this finding was statistically significant, its practical value is severely hampered by the lack of selectivity, many of RCM non-reactors also giving positive allergy histories. A positive history of a previous reaction to RCM or to penicillin, however, was found to have more specificity in predicting a forthcoming hypersensitivity reaction to RCM.

Volume of distribution of contrast media in blood.

The volume of distribution of diatrizoate and iodipamide in blood in relation to hematocrit and contrast concentration was measured using 125I-labeled compounds. In concentration obtained after intravenous injection, the percentage volume of distribution of both contrast media is 100 minus hematocrit, except for high hematocrit values, which may cause uneven distribution of contrast media in smaller concentrations. No evidence of intracellular penetration was obtained.

Contrast enhancement of the pancreas in computed tomography.

Contrast enhancement of pancreas, liver, kidney, muscle, and blood was measured from computed tomography scans of 46 patients, including patients with normal pancreas (20), pancreatitis (20), and pancreatic carcinoma (6). A bolus injection of contrast agent was given intravenously. Contrast enhancement and contrast distribution volume in a single scan of the pancreas were measured at 40 sec and at 2, 5, and 15 min after the injection. There was a slight delay in the uptake of contrast material by pancreatic tissue if the disease was severe. In all other aspects, the pattern of contrast enhancement was equal in various patient groups and included great individual variations. Visually, the contrast agent distribution in severe pancreatitis was uneven. In severe pancreatitis, edema was seen both in the pancreas and surrounding tissues. It is concluded that gross anatomic changes and visual estimation are the only means of studying pancreatic pathology with an 18 sec scan time, and that contrast enhancement in such a study provides little additional diagnostic information.

The posterior tracheal band in the lateral chest radiograph.

The appearance and variations of the posterior tracheal band (PTB) was studied from 406 upright chest radiographs, taken from 293 individuals of varying age. PTB is visualized in about 80% of individuals well enough to be diagnostically useful. Its average width is 3 mm, and patient positioning can cause variations not exceeding 2 mm. Progressive widening by 2 mm or more, blurring of previously sharp margins or an overall width exceeding 5 mm should be considered suggestive of mediastinal or related abnormality.

Visualization of the areae gastricae in a double contrast examination - dependence on the contrast medium.

Visualization of the areae gastricae, a criterion of high resolution in the double contrast examination of the stomach, was studied with several barium preparation. Visualization of the areae gastricae varied among different contrast media, some gave excellent visualization while others were regularly inadequate. In general, contrast suspensions prepared from powder in the X-ray department gave better results than commercial suspensions, which retained many gas bubbles. The importance of the contrast medium selection for a high resolution double contrast examination of the stomach is emphasized.
