[Hepatic vein blood flow pattern measured by Doppler echocardiography as an evaluation of tricuspid valve insufficiency].

Evaluation of tricuspid regurgitation was attempted by analyzing the blood flow pattern in the hepatic vein using a combined system of a pulsed Doppler technique and two-dimensional echocardiography. The Doppler incident angle to the hepatic vein from the subcostal approach was approximately 0 degrees to 30 degrees, and the Doppler output was easily recognized as a narrow frequency-band pattern on the sound spectrogram. The study population consisted of 60 patients with valvular heart disease and 17 healthy subjects. Inferior vena cava dimension (IVCD), hepatic vein dimension (HVD) and the blood flow pattern in the hepatic vein were compared with the severity (negative, mild, moderate and severe) of tricuspid regurgitation (TR) assessed by right ventriculography and with right atrial and ventricular pressures. The following conclusions were derived from the study: IVCD and HVD in a group of TR severe were significantly larger than those of the other groups. The normal flow pattern of the hepatic vein was biphasic with a systolic flow greater than a diastolic flow. In cases of valvular heart disease with atrial fibrillation, three types of abnormal blood flow patterns were demonstrated; Type 1 had a slower systolic flow than a diastolic flow. Type 2 had no flow signal during systole, and had only a diastolic flow. Type 3 had a reversed systolic flow with several variations. By this hepatic flow patterns, it was possible to differentiate the TR of severe and moderate groups from the TR of mild group, because 81% of the former groups showed a Type 3 of a flow pattern. The Doppler shifts from the base line on the sound spectrogram were well correlated with right atrial pressure and right ventricular end-diastolic pressure (r = -0.72 and -0.64, respectively). The early changes of the hepatic blood flow pattern after operation were due to the improvement of TR by tricuspid annuloplasty, and also seemed to be affected by the postoperative changes of right atrial compliance and contraction.

High susceptibility of specific inbred colonies of rats to n-2-fluorenylacetamide-induced hepatocarcinogenesis.

N-2-Fluorenylacetamide(FAA)-induced hepatocarcinogenesis in three colonies of Wistar-strain rats was studied. Two specific inbred colonies of rats derived from a Wistar strain, normotensive rats of Wistar Kyoto colony (WKY) and spontaneously hypertensive rats separated from WKY (SHR), showed higher inductions of preneoplastic altered foci of basophilic cell type and hepatocellular carcinomas as compared to common closed colony rats of the same strain (CWR). After 7 weeks of 0.02% FAA-containing diet, preneoplastic foci in WKY were found 4.8 times more frequently and in SHR 2.5 times more frequently than in CWR. Most of the foci found in WKY (92.5% of the total number of foci) and in SHR (87.1%) were of basophilic cell type, whereas most of the foci in CWR (71.3%) were acidophilic. After 7 months of the diet, multiple hepatocellular carcinomas were found in most rats in WKY (87.5% of the total number of rats, 2.6/rat) and SHR (81.8%, 2.5/rat) as compared to CWR (14.3%, 0.14/rat). These data demonstrate that rats in specific inbred colonies possess even higher susceptibility to FAA than CWR, and the susceptibility is closely related to the higher incidence of basophilic foci.

Alteration of activities of delta-aminolevulinic acid synthase, delta-aminolevulinic acid dehydratase and delta-aminolevulinic acid dehydratase inhibitor in the bone marrow cells of lead poisoned rats.

A new modified method for the determination of delta-aminolevulinic acid (ALA) dehydratase activity has been developed. This modified method involves addition of 0.1 mM Zn2+ and then preheating the enzyme solution at 60 degrees C for 5 min before following commonly used procedures. The ALA dehydratase activity in the peripheral blood of lead poisoned rats, determined by this procedure, was approximately 7.5-fold higher than control values. The activity of ALA dehydratase in the bone marrow cells of lead poisoned rats increased approximately 2-fold. These increases were accompanied by a simultaneous decrease in an inhibitor specific for ALA dehydratase. Consequently, these results suggest that ALA dehydratase is induced after the reduction of ALA dehydratase activity, and that ALA dehydratase activity is controlled by a new specific ALA dehydratase inhibitor.

Changes in aminolevulinate synthase and aminolevulinate dehydratase activity in cirrhotic liver.

Delta-Aminolevulinate synthase and delta-aminolevulinate dehydratase activities were determined in liver biopsy specimens obtained from 12 patients with hepatic cirrhosis. Delta-Aminolevulinate synthase activity was determined by the incorporation of [1,4-14C]succinyl coenzyme A into delta-aminolevulinate. The mean activity of delta-aminolevulinate synthase was significantly higher in cirrhotic liver specimens (mean +/- SE, 193.7 +/- 34.5 picomoles delta-aminolevulinate per milligram protein per 30 minutes) than in controls with minimal histologic changes (32.7 +/- 13.6, p less than 0.01). Furthermore, the mean activity of delta-aminolevulinate synthase was higher in micronodular cirrhosis (281.6 +/- 58.8) than in the other types of cirrhosis (131.0 +/- 23.1, p less than 0.05). Levels of indocyanine green retention at 15 min correlated with the activity of hepatic delta-aminolevulinate synthase (p less than 0.05). The mean activity of delta-aminolevulinate dehydratase, in contrast, was significantly lower in cirrhotic liver specimens (9.4 +/- 1.3 nanomoles porphobilinogen per milligram protein per hour) than in controls (22.0 +/- 2.6, p less than 0.05). These results suggest that the extent of liver injury or the degree of portosystemic shunting, or both, influence the rate of hepatic heme biosynthesis.

[Effects of lorcainide, a new antiarrhythmic agent, on experimental cardiac arrhythmias].

Antiarrhythmic actions of lorcainide were compared with those of disopyramide on different types of arrhythmias in animal models. It was shown in dogs that lorcainide and disopyramide were approximately equipotent against the arrhythmias following coronary occlusion. In guinea pigs, lorcainide was less effective than disopyramide against the ouabain-induced arrhythmia. Both drugs showed a weak protective action against the aconitine-induced ventricular arrhythmias, but had no effect on the cardiac arrest. In the arrhythmias induced by the application of acetylcholine on the right auricle of guinea pigs, the disappearance of P-waves and the disturbance of R-R intervals were inhibited by disopyramide, but lorcainide had no influence on these events; both prevented the atrial fibrillation induced by acetylcholine application. Both drugs by themselves induced ventricular arrhythmias in guinea pigs.

Effect of vitamin E on endotoxin-induced disseminated intravascular coagulation in rats.

Experimental disseminated intravascular coagulation (DIC) can be induced by 4 hr sustained infusion of endotoxin in a dose of 100 mg/kg in rats. The experimental model of DIC in rats was used to study the preventive effect of vitamin E, alpha-tocopheryl acetate, against DIC. Before the infusion of endotoxin, 0.01, 0.1, 1.0 or 10.0 mg/kg/day of alpha-tocopheryl acetate was injected intraperitoneally for 4 successive days. The preventive effect against DIC was noted in all the parameters, such as fibrinogen and fibrin degradation products, fibrinogen level, prothrombin time, partial thromboplastin time, platelet count, and the number of renal glomeruli with fibrin thrombi, in rats treated with 1.0 or 10.0 mg/kg of alpha-tocopheryl acetate. From these results, it was shown that vitamin E, alpha-tocopheryl acetate, inhibited endotoxin-induced experimental DIC in rats.

Computed tomography of malignant tumors of the nasal cavity and paranasal sinuses.

Staging of malignant tumors of the nasal cavity and paranasal sinuses by computed tomography (CT) was studied in a total of 49 patients, 33 with squamous cell carcinoma and 16 with tumors of other histologic types. Involved sites by the tumor were studied, and clinical staging was made using CT findings alone according to AJC classification for maxillary sinus tumors. Surgical findings for comparison were available for most cases. Of 33 squamous cell carcinomas and of 16 tumors with other histologic types, the maxillary sinus was the site of origin in 29 and eight, respectively. Of these 37 maxillary sinus tumors, 11 were staged T3, 26, T4, and none was staged T1 or T2. None of these tumors were down staged, and one T3 was upstaged after surgical procedures, although all sinuses were not explored in some cases. Sinusitis due to obstruction was indistinguishable from the tumor without bone destruction. And the determination of the site of origin was difficult in some cases. Despite these, CT should be used for pretreatment evaluation of the tumors of these sites.
