Comparison of "direct" and "indirect" zirconyl-phosphate technics for carcinoembryonic antigen determination in plasma from patients having benign or malignant disease.

A simplified "direct" zirconly-phosphate gel radioimmunoassay for carcinoembryonic antigen (CEA) is compared with the "indirect" method using reagents provided in the Roche¿ CEA kit. Statistical analysis shows that (1) the methods are comparable in sensitivity and specificity; (2) the values obtained are, in most case, interchangeable; (3) the results by the two methods are comparable both among patients who have colorectal cancer and among those who have other diseases. For general use, the direct assay has proven to be an adequate alternative to the indirect assay.

[Synthesis and serum cholesterol-lowering and uterotropic effects of 15,16,17-trisubstituted oestratriene 3-methyl ethers. Part 63: On steroids (author's transl)].

The preparation of the 17 beta-carbamoyl derivatives 4 and of the 17 beta-carbonic acid esters 5 from 15 beta,16 beta-epoxy-3-methoxyoestra-1,3,5(10)-triene-17 beta-ol (3a), and the subsequent opening of the 15 beta,16 beta-epoxy group resulting in 15 beta,16 alpha,17 beta-trisubstituted oestratrienes 6 are described. The influences of several substitutents in the positions 15 and 16 and of the 17 beta-hydroxy group and derivatives thereof on the uterotropic and cholesterol-lowering activities were investigated in rats after oral application. A correlation between the cholesterol-lowering and the uterotropic activity could be established. Activity dissociation was not achieved.

Sexual abuse in children.

The purpose of this paper is to review recent English-language literature on sexual abuse in children. Most of the articles deal with incestuous behavior and this subject will be emphasized. However, I will also deal with sequelae of other sexual offenses in children.

Predictive value of plasma CEA in patients with colorectal carcinoma.

Two years follow-up of 46 patients with colorectal carcinoma resected "for cure" shows that of the nine patients with an elevated (less than 5 ng/ml) CEA plasma titer one to six months after surgery, only one, or 11%, had remained disease free. Of the 38 patients with normal (less than 5 ng/ml) plasma CEA, 27, or 71%, were free of disease two years after surgery. In another 85 patients presenting six months to ten years after resection "for cure" of their colorectal carcinoma, normal CEA levels were found in 73, and only five, or 6.4%, of these presented with disease progression when followed for two years; of the 12 patients that presented with CEA values less than 5 ng/ml, disease progression was evident in eight or 67%; in eight patients with CEA levels of greater than 10 ng/ml, the proportion of patients with disease progression increased to 87.5% (7/8). When a patient with a history of colorectal cancer, but seemingly free of disease, presents with levels not only repeatedly above normal (greater than 5 ng/ml) but above the levels found in some nonmalignant conditions (greater than 10 ng/ml), thorough re-examination of the patient to locate the site of possible disease progression is indicated. In 14 such patients further diagnostic methods showed local recurrences in four, metastasis limited to the liver in six, and other metastasis in four. In conclusion, in patients with colorectal carcinoma postoperative elevated CEA plasma levels are a sign of poor prognosis. Consistently elevated CEA levels (greater than 10 ng/ml) are a strong indication of disease progression.

Direct evidence for the involvement of prostaglandin F2 alpha in the first step of estrone-induced blastocyst implantation in the spayed rat.

The effect of prostaglandin F2 alpha (PGF2 alpha) on blastocyst implantation in spayed rats has been studied. In preliminary experiments, the first implantation sites were observed 8 - 12 hours after a single injection of estrone in ovariectomized and progesterone-conditioned rats. Intraluminal instillation of PGF2 alpha into the right uterine horn 8 - 10 h after the estrone injection increased the number of implantation sites. Even treatment with PGF2 alpha without previous estrone injection induced the first step of blastocyst implantation as shown by uterine dye site reaction (Niagara-blue test). The results are discussed with regard to the possible role of PGF2 alpha in the regulation of the blastocyst implantation processes in the rat.

[Multi-institution, multidisciplinary radiology and endoscopy trial of the diagnostic accuracy of the double contrast examination of the stomach surface].

In 12 X-ray and gastroenterology departments 1570 patients, clinically suspected to have a gastric pathology, have been examined by double contrast (surface) examination of the stomach followed by gastroscopy. Diagnostic accuracy was 93% in respect of endoscopic data, assumed correct by hypothesis. On the basis of a cost and benefit evaluation, made according to the well known criteria, a wider routine use of the double contrast technique is proposed.

Effect of intraocular irrigating solutions on intracellular membrane potentials and swelling rate of isolated human and rabbit cornea.

Isolated human and rabbit corneas were incubated in glutathione bicarbonate Ringer solution (GBR), balanced salt solution (BSS), or 0.9% NaCl solution. The swelling rate of human corneas was 25.5 micron/hr in GBR and significantly increased to 32.7 in BSS and 66.1 in NaCl. The epithelial intracellular potential of human cornea was constant at about 60 mV up to 5 hr of incubation in GBR and decreased continuously to 40 mV in BSS and NaCl. Endothelial cell potentials were stable for up to 3 hr of incubation in GBR or BSS and decreased from a control value of about 18 mV to 10 mV 2 hr after bathing in NaCl. Qualitatively similar data were obtained in the isolated rabbit cornea. The results demonstrate the advantage of GBR as an intraocular irrigating solution.

[Routine double contrast examination of the stomach: results and considerations on personal experience (author's transl)].

The results are presented on the double contrast investigation of the stomach performed during routine hospital activity. Purposes of the work are to evaluate the diagnostic accuracy of the method and to suggest the location of the diagnostic investigation. 602 consecutive patients were examined and 188 out of these underwent endoscopy. The diagnostic accuracy of the method was evaluated at gastroscopic control. Moreover the cost of the method was considered, in order to evaluate the possible routine application of the method in the hospitals.

Evaluation of a direct radioimmunoassay for carcinoembryonic antigen.

1. A radioimmunoassay for CEA using commercially available reagents (CEA-Roche), which avoids the perchloric acid and dialysis steps used in the CEA-Roche assay, has been devised. 2. Comparison of results on 746 plasma samples using our assay with those obtained using the CEA-Roche assay revealed a highly significant correlation coefficient (0.72; p less than 0.00001). 3. A further comparison of results on 373 plasma samples using our assay, but including a preincubation step with goat serum, with those obtained using the CEA-Roche assay also revealed a highly significant correlation coefficient (0.89; 0.00001). The goat serum preincubation step eliminated most of the discordant results in which our assay value was higher than CEA-Roche assay, presumably because of anti-goat antibodies in the patient plasma sample.

Relationship between uterotrophic and interceptive activities of steroidal estrogens.

The steroidal estrogens ethinylestradiol, mestranol and the synthetic "impeded" estrogen derivatives STS 456, STS 593 and J 628 were studied for their postcoital antifertility activity and estrogenicity in mice, rats and rabbits. Results of these studies suggest a positive relationship between these two biological endpoints with the exception of the findings in rats. As seen in uterotrophic--anti-implantation ratio, in view of only one species it is possible to dissociate the interceptive and the unwanted uterotrophic activities. The profound species differences in the effects of exogenous estrogens on the nidation phase are discussed.

Primary aldosteronism presenting as myoglobinuric acute renal failure.

Hypokalemia is an uncommon cause of rhabdomyolysis with acute tubular necrosis. We recently treated a patient in whom severe hypokalemia attributed to diuretic therapy antedated acute myoglobinuric renal failure by six months. After recovery, hypokalemia persisted and subsequent evaluation disclosed primary aldosteronism. This case is a unique presentation for primary aldosteronism and illustrates the importance of diagnosis before treatment in hypertension as well as the hazards of hypokalemia.
