Pathological changes of the lungs after prolonged inhalation of high concentrations of oxygen.

The pathological changes in the lungs of six patients who were treated by an artificial ventilation with a Bird or Bennett type respirator for three days to six months with oxygen concentrations of 24-100% were examined by light microscopy, studies on thick-sections, reconstruction models and vascular casts, and morphometric methods. After prolonged inhalation of high concentrations of oxygen the lungs showed thickening of the alveolar wall, marked deposition of reticulin fibers and fibroblastic proliferation in the alveolar wall, reduction in the number of capillaries, an abnormal configuration of the capillary network and hyperplasia of alveolar lining cells. These lesions are not specific to this condition, and seemed to be less marked than similar lesions in cases of chronic forms of fibrosing alveolitis, chronic interstitial pneumonia, usual interstitial pneumonia and so-called pulmonary fibrosis. Morphometric results confirm these histological observations and show not only the concentrations of oxygen but also the duration of high and pure oxygen inhalation have important roles for these pulmonary lesions. The main reasons for these lesions seemed to be repeated damage to the capillaries and loss of the normal configuration of the capillary network, accompanied by rearrangement and reconstruction of the two types of reticulin fibers.

[Clinical effect of allylestrenol on benign prostatic hypertrophy].

Allylestrenol at the daily dose of 50 mg was administered to 45 patients with benign prostatic hypertrophy. The treatment was performed for more than 12 weeks in 40 patients, and improvements, marked and moderate, were observed in 22 patients (55%) in the overall judgement. As side effects of this drug, decrease of potency was observed in 3 cases, decrease of libido in 1 case, pigmentation on breasts in 2 cases, palpitation in 2 cases, short breath in 1 case, and gastrointestinal symptoms in 1 case. However, these side effects were not serious. Our trial suggests that the treatment of benign prostatic hypertrophy with allylestrenol can be useful in urological clinics.

Dynamics of lymphocytic subpopulations in Friend leukemia virus-induced leukemia.

The in vivo roles of the immunosurveillance mechanism of the host against leukemia induced by Friend leukemia virus (FLV) were examined. The significance of T-cells in host defense against FLV-induced leukemia was indicated by the fact that thymus-deprived C57BL/6N-nu/nu mice were sensitive to FLV, although normal C57BL/6N mice were, as already reported by many authors, resistant to FLV. In relation to the role of T-cells on the onset of FLV-induced leukemia, the population dynamics of the lymphocytic subpopulations of the systemic lymphoid organs after FLV injection in FLV-resistant C57BL/6N mice were examined in comparison with the dynamics in FLV-sensitive strains, C57BL/6N-nu/nu mice and normal C3H/HeN mice. In this system, Lyt-1+2- helper T-cells in the spleen of FLV-resistant C57BL/6N mice increased in number after FLV injection. The number of immunoglobulin positive cells did not remarkably change in FLV-resistant C57BL/6N mice after FLV injection, whereas the number increased in the lymph node of FLV-sensitive C3H/HeN mice. The results indicated that a major contribution to the relative susceptibility and resistance of the host to FLV was controlled by the capacity to mobilize T-cells to the spleen in an early stage of disease, although the interaction of these T-cells with other immune cells may play an important role in mediating host resistance to FLV-induced disease.

Effects of Friend leukemia virus (FLV) inoculation in F1 mice and differentiation of FLV-induced leukemia.

Effects of Friend leukemia virus (FLV) inoculation in F1 specific pathogen free (SPF) mice were examined. Resistance to FLV was dominantly inherited both in F1 hybrid mice (BDF1) (FLV-resistant & FLV-sensitive with polycythemia) and F1 hybrid mice (B6C3F1) (FLV-resistant & FLV-sensitive with anemia). But the population dynamics of the nucleated cell components of F1 mice after FLV inoculation differed from those of FLV-resistant inbred mice. A small number of mature erythroblasts appeared in the peripheral blood of BDF1 mice. In B6C3F1 mice, erythroblastosis with splenomegaly and polycythemia occurred. However, all of these findings in BDF1 and B6C3F1 mice regressed spontaneously. In F1 mice, FLV induced an intermediate reactive pattern of the two patterns that had been induced in the parental strains. The results indicate that FLV may induce leukemia with various degrees of differentiation, according to the genetic difference of the host.

[Effects of ceftizoxime suppository rectal dosing on cecal microflora in mice].

The effect of ceftizoxime suppository (CZX-S), a new rectal preparation of ceftizoxime (CZX), against cecal microflora of mice following consecutive rectal administration was compared with that after subcutaneous administration of CZX. The numbers of aerobic and anaerobic bacteria except Enterobacteriaceae of cecal microflora were not markedly changed by the rectal administration of CZX-S at a dose of 25 mg/kg 3 times daily for 10 days. Although the number of Enterobacteriaceae was decreased, recovery was rapidly observed after completion of the administration period. In the case of subcutaneous administration, a decrease in Enterobacteriaceae was also observed, but this was no more remarkable than that occurring with rectal administration. Moreover, effects on the state of feces, i.e., diarrhea, were not observed in any of the mice. The peak level of CZX in the cecum contents when administered rectally at a dose of 25 mg/kg was 13.8 micrograms/g at 4 hours, while it was less than the determination limit (2.0 micrograms/g) when administered subcutaneously at the same dose.

[An experience of percutaneous nephro-ureterolithotomy].

Herein we report our experience of percutaneous nephro-ureterolithotomy on 18 renal or upper ureteral calculous patients between December, 1984 and May, 1985. Using an ultrasonic lithotrite and a nephroscope especially developed for the percutaneous disintegration and removal of upper urinary stones via a nephrostomy, 15 of the patients were successfully treated without open surgery.

Canada-Cronkhite syndrome associated with systemic lupus erythematosus.

A 43-year-old woman with a history of systemic lupus erythematosus manifested severe diarrhea, generalized gastrointestinal polyposis, pigmentation, and onychodystrophy, which were typical of Canada-Cronkhite syndrome. The exacerbation of systemic lupus erythematosus, presenting with hemolytic anemia and subsequently with profound proteinuria, was seen during the course of this syndrome. The patient achieved complete symptomatic remissions of both entities for more than four years following prednisolone therapy; the resolution of the gastrointestinal polyps was confirmed histologically. This case showed that this syndrome could be completely reversible in its course and that it might have an exogenous cause, as previously suggested.

Neoplastic angioendotheliosis. Immunohistochemical and electron microscopic findings in three cases.

Three cases of neoplastic angioendotheliosis (NAE) presenting with central nervous system (CNS) disease but no skin lesions are described. The histogenesis of the neoplastic cells is discussed. Microscopic examination showed accumulation of neoplastic cells in the vascular system throughout the body and their extravascular proliferation in several organs. Electron microscopic and immunohistochemical studies revealed the presence of Weibel-Palade bodies and factor VIII-related antigen in intravascular and extravascular neoplastic cells in two of the three cases. In the first case the neoplastic cells did not have any T-cell markers. However, in one case no specific markers were found in the neoplastic cells by electron microscopic, enzyme histochemical, or immunohistochemical examination. These findings, although supporting the endothelial origin of the neoplastic cells, indicate the need for further consideration of whether NAE is actually a single disease entity or several different diseases.

Red blood cell tocopherol concentrations in a normal population of Japanese children and premature infants in relation to the assessment of vitamin E status.

To determine the normal range of red blood cell (RBC) tocopherol as an assessment of vitamin E nutritional status, 261 apparently healthy children, 3 to 16 years old, including 160 males and 101 females, were investigated and this population was compared with premature infants and adults. In the children, the mean plasma tocopherol was 682 micrograms/100 ml, ranging from 309 to 1,442 micrograms/100 ml. Plasma tocopherol expressed in terms of total lipids (tocopherol/lipid ratio) was generally within accepted value (greater than 0.8). Only one had a value below 0.8. The mean RBC tocopherol was 179 micrograms/100 ml packed cells ranging from 79 to 320 micrograms/100 ml. When the cumulative percentage frequency was examined, the value of 115 micrograms/100 ml corresponded to the fifth percentile of the population of RBC. Thirteen of 90 premature infants (14.4%) less than 24-hours-old and 9 of 48 at 7 days of age (18.8%) had values of less than 115 micrograms/100 ml packed RBCs, while on the basis of the tocopherol/lipid ratio, 44.8% of the infants less than 24-hours-old and 22.2% of those 7 days old had values below 0.8. No adult was below the accepted values of either RBC tocopherol or the tocopherol/lipid ratio.

[Hyperthermic treatment with peplomycin and bestatin in superficial bladder cancer].

In order to study the effect of Bestatin on superficial bladder cancer, the drug was used in combination with hyperthermia and Peplomycin therapy. The study was made using the following 2 groups. Group 1; Hyperthermic treatment with Peplomycin was used in 6 cases of bladder cancer. Group 2; Bestatin was given in combination with hyperthermia and Peplomycin in 15 cases of bladder cancer. As a result, complete or partial regression was obtained in 1 and 5 cases in Groups 1 and 2 respectively. Therefore, the combined use of Peplomycin and Bestatin in hyperthermic treatment for superficial bladder cancer appeared to be effective. Degeneration of tumor cells, irregularity of tumor structure, interstitial edema, fibrosis and cell infiltration were observed in the surgical specimens taken from effective cases, although the mechanism of action of Bestatin on tumors was clearly demonstrated upon histological examination.

Hyperthyroidism after primary hypothyroidism. Follow-up with serial thyroid biopsies.

Hyperthyroidism occurred following primary hypothyroidism in a 46-year-old man with serologically and histologically proved chronic lymphocytic thyroiditis. Repeated thyroid biopsy specimens revealed that histologic features compatible with chronic lymphocytic thyroiditis seen at the initial hypothyroid state subsequently underwent a remarkable resolution, with titers of circulating antithyroid antibodies being reduced in correspondence with the histologic improvement. This case shows that primary hypothyroidism in adults can spontaneously evolve into a hyperthyroid state, as has been suggested previously, and that the histologic abnormalities in chronic lymphocytic thyroiditis may not necessarily be irreversible.
