[An experience of OK-432 intradermal administration in superficial bladder tumors].

A streptococcal preparation, OK432 was evaluated as the adjuvant immunotherapeutic agent for superficial bladder tumors. Twenty-nine cases were treated with complete transurethral resection and then randomized prospectively into two groups. One group had adjuvant therapy of OK432 intradermal administration and another group had no adjuvant therapy. OK432 administration significantly reduced the recurrence rates as compared to the control group. Although further studies were required for its full significance, in a short-term study, OK432 intradermal administration produced a beneficial effect in reducing the recurrence rates.

[Experience of 23 cases of cerebellar hemorrhage--indication for evacuation of cerebellar hematoma].

Twenty-three cases of cerebellar hemorrhage diagnosed by CTscan were evaluated. The subject were 13 males and 10 females and the ages of the patients were 58 years in average ranging from 24 to 83 years. Fourteen cases were treated surgically. Evacuation of hematoma by suboccipital craniectomy were performed in 11 patients and suboccipital decompression alone in one case. Two cases were underwent ventricular drainage alone for treatment of hydrocephalus. ADL assessed at discharge of the hospital was as follows: ADL 1 or 2 in 13 patients, ADL 3 or 4 in 5 patients and ADL 5 (dead) in 5 patients. The patients with small hematoma of less than 40 mm in diameter on CTscan were recovered to ADL 1 or 2 regardless of surgical or conservative treatments. The authors concluded that evacuation of hematoma is indicated for the patients with hematoma of more than 40 mm in diameter on CTscan, for the patients whose consciousness level is progressively aggravated regardless of size of hematoma and also for the patients whose CTscan shows deformity or obstruction of ambient and/or prepontine cisterns.

[Adrenal incidentaloma: report of eight cases].

Since 1983, eight cases of adrenal incidentaloma were experienced at the Department of Urology, Wakayama Medical College and affiliated hospitals. Six adrenal masses were incidentally detected by computed tomography, and two were first noted by either ultrasonography or intravenous pyelography. Surgical exploration was performed in all cases as the final diagnostic measure. Of them, three had adrenocortical adenomas, three had adrenal cysts, one had adrenal metastasis and one had pheochromocytoma. Preoperative evaluation revealed that all patients except one with pheochromocytoma had no endocrinological abnormality. The management of the adrenal incidentaloma, which will be discovered more frequently accompanied with the increasing use of computed tomography, was discussed with special references to its precise indication of surgical treatment.

[An autopsied case of pulmonary carcinoma with perforation peritonitis due to metastatic tumor necrosis at the jejunum caused by chemotherapy].

A 76-year-old man with lung carcinoma, who was undergoing both radiotherapy and chemotherapy (CDDP + ADR) suddenly died with the onset of abdominal symptoms. Autopsy disclosed that the main cause of death was suppurative panperitonitis, caused by a perforation of a metastatic lesion at the jejunum. A histological examination revealed extensive tumor necrosis in the metastatic liver lesions as well as in the jejunal lesions. Neither inflammatory nor circulatory damage was observed in the perforated lesion. Therefore, the cause of this perforation was considered to be chemotherapy that had brought on tumor necrosis. No similar reports of lung cancer deaths have been found in the literature in which the microscopic details were described. Accordingly, this case is reported for future reference with regard to cancer chemotherapy.

[Clinical effect of terodiline hydrochloride (TD-758) in patients with urinary disturbances].

We evaluated the efficacy of terodiline hydrochloride (TD-758) in 23 patients with psychosomatic bladder, chronic prostatitis and chronic cystitis. The drug was administered at a dose of 24 mg once a day for 4 weeks. In 78% of the patients the symptoms such as urinary frequency and sense of residual urine improved. Only mild side effects such as thirst were noted. We concluded that terodiline hydrochloride was effective in this trial.

Angiosarcoma of the scalp: report of two cases with fatal pulmonary complications and a review of Japanese autopsy registry data.

Two cases of angiosarcoma of the scalp were reported. The patients were elderly men and died from pulmonary complications, including pneumothorax, pulmonary haemorrhage and pneumonia, associated with metastatic tumours in the lungs. The data recorded from 95 autopsies of patients with angiosarcoma in Japan during 1980-1984 were analyzed. According to the anatomical distribution of the primary tumour, the patients could be subdivided into a scalp group and non-scalp group. In both groups, the most common metastatic site was the lung. The patients of the scalp group had more frequent pulmonary complications such as pneumonia, haemothorax, atelectasis and pneumothorax, when compared with the patients of the non-scalp group. In particular, pneumothorax was observed only in the patients of angiosarcoma of the scalp. The results indicate that angiosarcoma of the scalp tends to metastasize to the lung, especially to the subpleural or surface pleural area and these metastatic tumours are prone to necrosis, causing characteristic pulmonary complications.

Evolutionary conservation of the insulinoma gene rig and its possible function.

We have identified a gene, rig (rat insulinoma gene), that is activated in chemically induced rat insulinomas but not in normal pancreatic islets or in regenerating islets. In the present study, we have found that the insulinoma gene was activated in a BK virus-induced hamster insulinoma cell line and in a spontaneously occurring human insulinoma. From the hamster and human insulinoma cDNA libraries, rig homologues were isolated, and their nucleotide sequences were determined. In the same manner as the rat gene, both hamster and human homologues contained one open reading frame of 435 nucleotides, differing by 32- and 41-base substitutions, respectively. All the base substitutions were same-sense mutations. Accordingly, the deduced 145-amino acid sequence remained invariant in hamster, human, and rat insulinomas, suggesting that rig has evolved under extraordinarily strong selective constraints. Computerized structure analysis indicated that rig-encoded protein is a possible DNA-binding protein. The antisense oligodeoxyribonucleotide complementary to hamster rig mRNA was synthesized and injected into the hamster insulinoma cells. The antisense rig oligodeoxyribonucleotide inhibited DNA synthesis in the insulinoma cells, whereas the sense rig oligodeoxyribonucleotide or antisense insulin oligodeoxyribonucleotide had no inhibitory effect. These results strongly suggest that the activation of rig is both common and potentially significant in the oncogenic growth of pancreatic B cells of islets of Langerhans.

Primary hemangiopericytoma of the heart associated with pseudoaneurysm of the pulmonary artery--a case report.

A thirty-three-year-old male with malignant hemangiopericytoma of the right ventricular outflow tract and the pulmonary artery associated with pseudoaneurysm formation at the latter is presented. Contrast computed tomography was helpful in diagnosing the pseudoaneurysm of the pulmonary artery. The positional change of the murmur, with a tumor plop caused by the pedunculated tumor of the right ventricular outflow tract, was detected.

Cervical flexion myelopathy: a "tight dural canal mechanism". Case report.

The authors describe a case of flexion myelopathy due to specific morphological changes of the cervical cord, termed a "tight dural canal mechanism." The patient was an 18-year-old man with progressive weakness and muscle atrophy of the left arm. Neuroradiological examination revealed that the lower cervical cord was compressed during flexion of the cervical spine, but that there was no disc disease or cervical vertebral instability. The cord compression was attributed to a pinching mechanism by the posterior border of the vertebral body and the posterior component of the dura on flexion.

[Diagnostic value of digital subtraction angiography (DSA) using Toshiba Digiformer X in the cerebrospinal vascular diseases].

Using TOSHIBA Digiformer X, digital subtraction angiography (DSA) was performed in 286 patients, in whom 229 patients of cerebro-spinal vascular disease was included. The authors emphasize the usefulness of DSA in cerebro-spinal vascular disease in relation to conventional angiography. DSA taken by single small dose injection of contrast medium into the ascending aorta clearly demonstrates not only aortic arch and thoracic major vessels, but also cervical vessels and all intracranial vessels. Therefore, we could rapidly understand gross dynamics of the circulation and obtain useful informations prior to catheterization to the selective artery. This advantage of DSA was particularly useful for occlusive vascular diseases. Gradual injection of small dose of contrast medium obviously reduced recoiling of the catheter tip, which enabled the selective angiography with setting of the tip of the catheter at the entrance of cervical major vessels without its sufficient insertion into the selective vessels. This advantage is particularly beneficial for the patients with severe arteriosclerosis who was found to be difficult for selective catheterization. In our experience, demonstration of a presence of aneurysm by DSA was possible in almost all cases of subarachnoid hemorrhage, although spatial resolution of DSA is said to be inferior to the conventional angiography. Real time display of DSA decreased the time required for examination and enabled repeated angiography. This advantage of DSA is especially useful for the patients with spinal arterio-venous malformation and thoracic outlet syndrome.

[Malignant hemangiopericytoma of the right ventricular outflow tract and the pulmonary artery: a case report].

A case of malignant hemangiopericytoma of the right ventricular outflow tract and the pulmonary artery associated with formation of a pseudoaneurysm in the latter is presented. This 33 year-old man had a four month history of illness. From the surgical point of view, all non-invasive modalities including phonocardiography, M-mode and two-dimensional echocardiography, radionuclide angiocardiography and contrast computed tomography underestimated the extent of the tumor as compared with the selective cineangiographic estimation. Therefore, it was suggested that in some situations where surgery is contemplated, a combination of non-invasive methods and cineangiography is essential to obtain sufficient diagnostic information, although the introduction of catheters into the right-sided cardiac chambers containing a mass might be hazardous because of potentiality dislodging portions of a tumor or adherent thrombus. Concerning pericardial abnormalities, contrast computed tomography was the most sensitive and specific method among the diagnostic techniques used in this case.

[Studies on renal function following cardiac operations--with special reference to postoperative cardiac function].

Effect of cardiac function on postoperative renal function was studied in 54 cases. These cases were divided into two groups according to postoperative cardiac function. Group A was good cardiac function group (27 cases; the value of C.I. was below 2.2L/min/m2 less than once through all examinations after operation). Group B was low cardiac function group (27 cases; the value of C.I. was below 2.2L/min/m2 more than twice through all examinations after operation). Significant differences were found in cardiac index, mean arterial pressure, left, arterial pressure and total vascular resistance in two groups. Significant differences were not found in urinary volume and FENa in two groups. In creatinine clearance and free water clearance significant differences were found from early postoperative period, and group A showed better renal function than group B. Serum Na, serum K and urinary Na:K ratio were not significantly different between two groups. Difference of plasma renin activity was not significant, but plasma aldosterone was significantly higher in group B than in group A during postoperative period. In group B, four patients developed nonoliguric acute renal failure, eventually resulting in MOF. The author concluded that postoperative cardiac function played the most important role on the state of postoperative renal function.

[Primary hyperparathyroidism in 23 cases].

Twenty-three cases of primary hyperparathyroidism were experienced at Wakayama Medical College Hospital and affiliated hospitals between January, 1969 and December 1985. Histopathological findings were adenoma in 22 cases, hyperplasia in 1 case. On the clinical diagnosis of this disease, the most sensitive laboratory test was serum calcium level, which was elevated in 21 patients (91%), and in 87.8% of all samples measured. Serum ionized calcium and phosphorus were also sensitive measures. The positive rate of serum PTH was 67% in 18 cases. Tl-chloride scintigraphy identified preoperatively the site of the adenomas in 8 out of 10 cases, and computed tomography in 10 out of 15 cases. Both methods are non-invasive, and are beneficial for the preoperative examination of the localization of parathyroid adenoma. After parathyroidectomy, serum calcium and ionized calcium levels recovered rapidly within the normal limit in 12 hours. On the other hand, serum phosphorus level increased gradually up to the normal limit in about 1 week.

Gangliosides as markers of cortisone-sensitive and cortisone-resistant rabbit thymocytes: characterization of thymus-specific gangliosides and preferential changes of particular gangliosides in the thymus of cortisone-treated rabbits.

Neutral glycosphingolipids and gangliosides in rabbit thymus, spleen, bone marrow, and erythrocyte ghosts were analyzed by conventional chemical and enzymatic procedures and negative ion fast atom bombardment mass spectrometry (FABMS). Thymus gangliosides showed a characteristic composition. Major gangliosides comprising 75% of the total thymus gangliosides were sialosyl lacto-N-neo-tetraosyl- and sialosyl lacto-N-nor-hexaosylceramides containing NeuGc and palmitic acid. These major thymus gangliosides were not detected in spleen, bone marrow, or erythrocytes, whereas GD1a, which was not present in the thymus even in a trace amount, was present in spleen and bone marrow. In addition, the major gangliosides in rabbit thymus were preferentially reduced when an animal was given an intraperitoneal injection of cortisone acetate, as found on analysis 48 h later. The decrease was accompanied by a concomitant increase in NeuAc-containing GM3 with longer chain fatty acids.
