Antibodies to adult T-cell leukemia-virus-associated antigen (ATLA) in sera from patients with ATL and controls in Japan: a nation-wide sero-epidemiologic study.

A nation-wide sero-epidemiologic survey of adult T-cell leukemia virus (ATLV), detected es anti-ATLA (ATLV-associated antigen), was made in Japan. Sera from adult donors in 15 different locations were screened for anti-ATLA. High incidences (6 to 37%) of antibody-positive donors were found in seven regions, one in northern Japan, and the others in southwestern regions. These areas are ATLV-endemic areas corresponding to ATL-endemic areas. Examination of sera from healthy donors aged 6 to 80 years in ATL-endemic areas showed an age-dependent increase of seropositive donors with a maximum of about 30% at 40 years of age. Anti-ATLA was found in all but two of 142 patients with ATL. Anti-ATLA-positive patients with ATL were mainly found in ATLV-endemic areas, and only a few in ATL-nonendemic areas. Six patients with cutaneous T-cell lymphoma in ATLV-nonendemic areas gave a negative reaction for anti-ATLA. The geometric mean titer of anti-ATLA of patients with ATL was higher than that of healthy donors.

Gastric carcinoma with psammomatous calcification: report of a case, with reference to calculogenesis.

A case of gastric adenocarcinoma with argyrophilic property and psammomatous calcification is reported. Histologically, the psammoma bodies are found most frequently within the glandular lumina. Electron microscopy, however, reveals that calcium first appears within the cytoplasm of tumor cells. Electron probe x-ray microanalysis demonstrates calcium and phosphorus in granular or floccular osmiophilic deposits found in intracytoplasmic electronlucent zones of tumor cells. By x-ray diffractometry, the calcium component is presumed to be hydroxyappatite (Ca5(PO4)3.(OH). The findings strongly support the view of the intracytoplasmic origin of psammomatous calcification. The tumor yielded a parathyroid hormone (PTH)-like substance, and a possible relationship between this substance and psammomatous calcification is spectulated.

Histiocytic necrotizing lymphadenitis without granulocytic infiltration.

Twenty-seven cases of an unusual necrotizing lymphadenitis previously described only in Japan are reported as occurring in West Germany (23 cases), Iran (1 case), Italy (1 case), Korea (1 case) and Spain (1 case). The lesion frequently develops in the cervical lymph nodes of young women. It is characterized by infiltration of the cortex and/or paracortex by large collections of proliferating histiocytes and is devoid of granulocytes. Complete or, more often, incomplete necrosis of lymphoid tissue is seen in all cases. In cases with incomplete necrosis, the histiocytes are interspersed with pyknotic cells and nuclear debris. Based on the histological findings, the term "histiocytic necrotizing lymphadenitis without granulocytic infiltration" is proposed. Lesions to be considered in a differential diagnosis are malignant histiocytic neoplasms and necrotizing lymphadenitis with granulocytic infiltration, which is seen in lupus erythematosus and bacterial infections. The aetiology of histiocytic necrotizing lymphadenitis without granulocytic infiltration is still unclear. Some clinical and histological features indicate the possibility of an underlying viral infection.

Experimental analysis of the ill effect of anomalous pancreaticobiliary ductal union.

An ideal canine model of anomalous pancreaticobiliary ductal union, similar to a human anomaly, was devised. Direct anastomosis of the dorsal pancreatic duct and the choledochus was employed (dorsal pancreaticocholedochostomy). The ventral pancreatic duct was not manipulated. Cylindrical choledochal dilatation results in 13 out of 20 adult dogs and in 4 out of 6 puppies. Dilatation of the intrahepatic biliary tree and thickening of the choledochal wall in a hepatic direction were observed in high ratio among adult dogs. Puppies had less dilatation. The activation of pancreatic enzymes was studied in bile containing pancreatic juice. The proteolytic enzymes, trypsin and elastase were proven to be activated in the bile without the presence of enterokinase in the pancreaticocholedochostomy model. This would be the cause of the ill effects of refluxed pancreatic juice into bile in anomalous pancreaticobiliary ductal union.

[Experimental study on immunochemotherapy using schizophyllan].

Immunochemotherapy with schizophyllan (SPG) combined with chemotherapeutic agents was evaluated in two syngeneic tumor-C3H/He mouse systems. The administration of SPG alone caused a significant growth inhibition of MM 46 mammary carcinoma, the highest therapeutic effect being obtained by SPG given at the advanced stage of tumors. When combined with neocarzinostatin given 7-times every other day the simultaneous administration of SPG produced an optimal response to that therapy. In X-5563 plasmacytoma, SPG alone was ineffective. However, when combined with mitomycin C given with 1 to 5-days interval, the concurrent administration of SPG prolonged significantly the life-span of the tumor-bearing mice. These results indicated that the simultaneous administration of SPG and antitumor drugs may be a useful application method for immunochemotherapy of experimental tumors.

Neuroendocrine carcinoma (trabecular carcinoma) of the skin with ectopic ACTH production.

A case of ACTH-producing cutaneous neuroendocrine carcinoma occurring in the cheek of an 80-year-old woman is reported. The tumor was composed of undifferentiated cells which had a uniform round nucleus and scanty cytoplasm containing argyrophil granules. An ultrastructural study demonstrated neurosecretory-type granules in the cytoplasm of the tumor cells. Radioimmunoassay revealed an elevated level of ACTH in tumor tissue as well as in the plasma of the patient. This is believed to be the first documented case of ACTH-producing neuroendocrine carcinoma (trabecular carcinoma) of the skin.

Inhibition of human platelet functions by verapamil.

The effects of verapamil, a coronary vasodilator, on platelet functions was studied. Platelet aggregation induced by ADP, epinephrine or collagen was inhibited by verapamil in vitro. Calcium ionophore A23187-induced platelet aggregation was also inhibited by verapamil in a concentration dependent manner. In washed platelets, verapamil caused a dose-dependent inhibition of serotonin release induced either by thrombin or A23187 in the absence of extracellular calcium. Addition of 1 mM CaCl2 with A23187 or thrombin partially overcame this inhibition. Addition of 1 mM CaCl2 in the absence of verapamil had no effect on thrombin- or A23187-induced secretion. When verapamil was administered to the healthy volunteers at the dosage commonly used, inhibition of platelet aggregation was observed 2 hrs after the drug ingestion. It is of great interest that verapamil potentiated the anti-aggregating activity of prostacyclin in vitro. Our results may suggest a potential role for verapamil in the treatment of thrombotic disorders.

Purification of heparinase and heparitinase by affinity chromatography on glycosaminoglycan-bound AH-Sepharose 4B.

Heparinase and heparitinase were separated from an extract of Flavobacterium heparinum, induced with heparin by using column chromatography on hydroxylapatite. As the heparinase preparation contained chondroitinases B and C, chondroitinase B was removed by rechromatography on a hydroxylapatite column. Chondroitinase C was then eliminated by column chromatography on O-phosphono("phospho")-cellulose. The heparinase preparation thus obtained was free from sulfoamidase for 2-deoxy-2-sulfoamino-D-glucose (GlcN-2S), sulfatase for 2-amino-2-deoxy-6-O-sulfo-D-glucose (GlcN-6S), as well as delta 4,5glycosiduronase for the unsaturated disaccharides obtained from heparin. The remaining sulfatase for 4-deoxy-alpha-L-threo-hex-4-enopyranosyluronic acid 2-sulfate (delta UA-2S) in the heparinase preparation was removed by affinity chromatography with dermatan sulfate-bound AH-Sepharose 4B coated with dermatan sulfate. The heparitinase preparation separated by column chromatography on hydroxylapatite was purified by affinity chromatography with heparin-bound AH-Sepharose 4B coated with heparin. Sulfatase for 2-amino-2-deoxy-6-O-sulfo-D-glucose (GlcN-6S) and delta 4,5glycosiduronase for the unsaturated disaccharides obtained from heparin were removed by this chromatography. Sulfatase for 4-deoxy-alpha-L-threo-hex-4-enopyranosyluronic acid 2-sulfate (delta UA-2S) remaining in the heparitinase preparation was finally removed by column chromatography on hydroxylapatite. The recoveries of the purified preparations of heparinase and heparitinase were estimated to be 39 and 50%, respectively, from the crude enzyme fractions obtained by the first column chromatography on hydroxylapatite. The purified heparinase and heparitinase were free from all enzymes that could degrade the sulfated unsaturated disaccharides produced from heparin with heparinase.

Ultrastructure of adult T-cell leukemia/lymphoma.

Eighteen cases of adult T-cell leukemia/lymphoma (ATLL) in Japan were analyzed by electron microscopy and compared with 5 cases of B-lymphoma and well-established groups of T-lymphomas (4 cases of T-lymphoblastic lymphoma and 2 cases of Sézary syndrome). Five hundred cells in each case, categorized ultrastructurally as to the cellular size and nuclear shape, showed an essentially pleomorphic cellular distribution in ATLL in remarkable contrast with the monomorphism of B-"histiocytic", B-well-differentiated lymphocytic, and T-lymphoblastic lymphomas. Some B-lymphomas and Sézary syndrome also showed pleomorphism. Cases of ATLL were classified according to the predominant cells and degree of nuclear irregularity, which delineated a spectrum of node-based, peripheral T-cells neoplasms probably encompassing T-immunoblastic sarcoma, T-zone lymphoma, as well as multilobated T-cell lymphoma. The characteristic fine structure of ATLL, in comparison with that of B-lymphomas, included slight to marked nuclear irregularity with convoluted-shape predominance, a specked chromatin pattern of the large cells, prominent lysosomes, and glycogen accumulation in addition to the difference in cellular distribution. Although T-lymphoblastic lymphoma and Sézary syndrome shared some of these features, the ultrastructural differentiation of ATLL from them seems to be possible.

Comparative studies on the structures of highly active and relatively inactive forms of whale heparin.

Whale heparin was separated by affinity chromatography on an antithrombin III-Sepharose column into two distinct fractions. The high-affinity fraction accounted for most the anticoagulant activity of the unfractionated material, while the low-affinity fraction was relatively inactive. The yields of the two fractions were substantially equivalent. No significant difference was observed between these fractions in terms of electrophoretic mobilities on cellulose acetate membrane and analytical data except for the contents of N-acetylglucosamine and N-sulfoglucosamine. The highly active form contained more N-acetylglucosamine and less N-sulfoglucosamine than the relatively inactive form. The two fractions were separately subjected to the sequential digestion with purified heparinase and heparitinase, and the oligosaccharide fractions were isolated from the digests by DEAE-cellulose column chromatography, followed by preparative paper chromatography. The purified compounds were then characterized by routine chemical and physical methods. Compound 1, delta 4,5hexosyuronic acid1 leads to 4N-acetylglucosamine, was exclusively obtained from the highly active form, whereas compound 3a, delta 4,5hexosyluronic acid1 leads to 4N-acetylglucosamine 6-sulfate, and compound 3b delta 4,5hexosyluronic acid1 leads to 4-sulfoglucosamine, were the only ones obtained from the relatively inactive form. The yields of other oligosaccharide fractions from both forms were comparable. The present data suggest that an N-acetylglucosamine-containing oligosaccharide structure in whale heparin is essential for binding to antithrombin II.

Ovarian carcinoid composed of argyrophil and argentaffin cells.

A rare case of ovarian carcinoid tumor composed of both argyrophil and argentaffin cells is reported. The tumor cells were arranged in solid-trabecular or glandular fashion. Electron microscopically, each of the argyrophil and argentaffin tumor cells had intracytoplasmic neurosecretory granules possessing their own characteristics. Immunoflourescent and immunohistological procedures failed to demonstrate thyroglobulin, calcitonin, and CEA in the tumor. No structures suggestive of a struma ovarii were identifiable. Scattered in the tumor were mucinous cysts with APUD cells lying between the lining epithelial cells. These APUD cells are thought to be the origin of this pure ovarian carcinoid tumor.
