[Study of the clinical background and therapeutic method in patients with vulvar cancer].

Forty-two cases of vulvar cancer were reviewed with regard to clinical condition and therapeutic method, especially the efficacy of local administration of BRM. By histological examination, koilocytosis was found in 45%. Furthermore, there was 90% positive koilocytosis rate in immunohistological study using anti-human papilloma virus antibody and in situ hybridization method by HPV DNA probe (type 6, 11, 16, 18). The HPA was mostly type 16. Therefore, vulvar cancer was considered to occur in close relation with HPV type 16. Some 12% of vulvar cancer was complicated with other gynecologic cancer. In these cases, NK activity was low. Thus, cancer may be considered to occur in association with low immunological status. With regard to the therapeutic method, surgery and radiotherapy were usually used after chemotherapy in our institute. At stage 3-4, combination therapy using surgery and radiotherapy were found to be most effective. In regard to local immunotherapy using OK-432 and LAK cell and IL-2, local administration of OK-432 was effective. Therefore, surgery and radiotherapy with local immunotherapy after chemotherapy were considered to be the most effective therapeutic method for vulvar cancer.

[A case of proliferative sparganosis associated with PIE syndrome and pulmonary embolism].

A 48-year-old man was admitted because of ischialgia. The CT scanning revealed tumors in the right pelvic cavity and the gluteal muscle. The bodies of Sparganum proliferum and granulation tissues were revealed by the tumor biopsy. In this hospital course, the chest roentgenogram revealed patchy infiltrative shadows in both lungs and left pleural effusion while chest CT scanning revealed multiple nodular shadows. Eosinophils increased in the peripheral blood and transbronchial lung biopsy (TBLB) showed non-destructive vasculitis with eosinophilic infiltration. The pulmonary lesion was diagnosed as intrapulmonary migration of Sparganum proliferum and PIE syndrome caused by this worm. Thiabendazole and prednisolone were administrated, but he died of pulmonary embolism.

[Ultrasonographic evaluation of the endometrium in postmenopausal women to select the high risk group of endometrial carcinoma of the uterus].

We examined the usefulness of ultrasonography (USG) for endometrial cancer screening in postmenopausal women. 207 postmenopausal women were ultrasonographically examined. The endometrial echo was classified into 4 types: 1, anechoic; 2, hair-line; 3, linear; and 4, navicular, and was compared with histological findings. No abnormal histological findings were observed in the cases showing type 1 and 2. On the other hand, abnormal histological findings including endometrial hyperplasia, atypical hyperplasia, and endometrial cancer, were seen in 57.7% (15/26) of type 3 cases, and 65.6% (21/32) of type 4 cases. Furthermore, type 3 and 4 women had enlargement of the uterus, and an increased incidence of postmenopausal bleeding. Positive rates in the progesterone challenge test (PCT) and karyopicnotic index (KPI) were higher in type 3 and 4 cases than in type 1 and 2. It was concluded that USG is able to accurately determine the endometrial condition and is useful for identifying high-risk patients in endometrial cancer screening.

[Therapeutic embolization of renal angiomyolipoma: a case report].

A case of bilateral renal angiomyolipoma without tuberous sclerosis is reported. A 49-year-old woman was admitted to the general practitioner with a sudden onset of severe left flank pain. An excretory urogram and ultrasonogram revealed an enlargement of the left kidney. She was subsequently referred to our clinic for further investigation and treatment. Computed tomographic scan and magnetic resonance imaging using Tl-weighted image showed several tumors with a fatty, dense area in the bilateral kidney. An arteriogram demonstrated a hypervascular renal mass with aneurysms in her left kidney. Diagnosis of bilateral renal angiomyolipoma was confirmed by percutaneous needle biopsy. Superselective embolization of the tumor was successfully performed, preserving normal renal tissue. Gelatin sponges containing Carboquone (CQ sponge) were used as embolic material. Angiomyolipoma has become relatively easy to diagnose by CT, ultrasound, MRI and so on. However, there are some cases of angiomyolipoma which are indistinguishable from renal cell carcinoma using these modes of testing. Therefore, in selecting a conservative management, we indicated that percutaneous biopsy or open biopsy should be done to confirm the results of the above procedures. Moreover, therapeutic embolization for angiomyolipoma was concluded to be very useful.

[Immunosuppressive effect of culture supernatant derived from decidual cell rich fraction].

The immunosuppressive function of culture supernatant of decidual cell rich fraction (Dec. supp.) was studied with the aim of clarifying the role played by decidual tissue in maintaining pregnancy. To obtain decidual cells, decidual tissue from miscarriages was enzyme-treated and submitted to Percoll discontinuous specific gravity centrifugation in specific gravity fractions from 1,025 to 1,065 in steps of 0.01. Decidual cell rich fraction was obtained in specific gravity fractions at 1,035 and 1,045. The Dec. supp. was obtained by incubating the decidual cell rich fraction for 24 hours. The effects on cell-mediated immunity were studied with the PHA reaction of mononuclear cells (MNC) and the one-way mixed lymphocyte reaction (MLR) among normal subjects. For the PHA reaction, suppression was 19.5 +/- 7.8% and 38.2 +/- 8.9% after the administration of Dec. supp. at 20% and 50% respectively. For the MLR, conspicuous suppression of 48.0 +/- 19.1% and 61.0 +/- 31.2% was seen after administration at 20% and 50% respectively. Next, indomethacin (ind.), a synthetic suppressor of prostaglandin (PG), was added in order to determine what part PG in the culture supernatant plays as one of the immunosuppressive factors in Dec. supp. Suppression was notably weaker. The PGE density in Dec. supp. was measured and found to be as high as 840 +/- 350 pg/ml, which suggests the possible intervention of PG as one of the suppressing factors.

[Left side renal cell carcinoma removed with the spleen, pancreas body, pancreas tail, and about one third of the left diaphragm: a case report].

A 61-year-old woman was admitted with the chief complaint of left lumbodorsal pain. Left renal cell carcinoma invaded to the spleen, pancreas body, pancreas tail and left diaphragm, which was diagnosed by computed tomography. Transabdominal radical nephrectomy with en bloc resection of the spleen, pancreas body, pancreas tail and one third of the left diaphragm was performed. A similar case was first reported by Suzuki et al. in 1982. This case seems to be the second in the Japanese literature.

Association of electrocardiographic abnormalities with seropositivity to Trypanosoma cruzi in Ecuador.

In highland Ecuador (province of El Oro), electrocardiograms (ECGs) were taken and analysed from 340 subjects, of whom 154 were seropositive to Trypanosoma cruzi by indirect haemagglutination test. Abnormal ECGs were present in 40.3% of seropositive individuals and 8.1% of seronegatives, and were slightly more common in seropositive males than in seropositive females. In seropositive individuals, the prevalence of abnormal ECG progressively increased with age and reached its peak level of 64.0% in the age group over 60 years, implying that Chagas disease in Ecuador produces little mortality among seropositive individuals. The most common ECG alterations were ventricular conduction defects: 26 of 117 seropositive individuals in the age group over 40 years had complete right bundle branch block. The frequency of atrioventricular block and arrhythmias was also associated with seropositivity to T. cruzi. A further ECG examination in lowland (province of Guayas) showed that the prevalence of abnormal ECGs there was significantly lower than that in highlands. These results suggest possible differences between T. cruzi strains in the two geographical areas of Ecuador.

The relationship between severity of ulcerated lesions and immune responses in the early stage of cutaneous leishmaniasis in Ecuador.

The relationship was examined between the severity of ulcerated lesions and immune responses in 19 Ecuadorian patients in the early stages of New World cutaneous leishmaniasis. As an immunological assay, the humoral immune response was assessed by enzyme-linked immunosorbent assay (ELISA) and the cell-mediated response by delayed type skin test for leishmanial antigen (leishmanin test). There was a statistically significant correlation (r = 0.61, P less than 0.01) between the total area of ulcerated lesions and the reciprocal titre of ELISA in identical subjects. However, no significant difference was observed in the ELISA titre between patients with a single lesion and those with multiple lesions (chi 2 = 7.06, df = 5, P greater than 0.2). A correlation (r = 0.48, P less than 0.05) was observed between the area of lesions and the intensity of leishmanin test; further, the ELISA titre also correlated to the intensity of leishmanin test in the identical subjects (r = 0.71, P less than 0.01). These results suggest that the severity of ulcerated lesions relates to the activation of both the humoral and cell-mediated immune systems in the early stage of New World cutaneous leishmaniasis.
