The liver in sarcoidosis.

Neddle biopsies of the liver were performed in 121 cases of sarcoidosis. Granulomas compatible with sarcoidosis were seen in 24 percent of the cases. Liver function tests (serum alkaline phosphatase, serum aspartate aminotransferase, serum alanine aminotransferase, and bromsulphthalein clearance test) were performed on 325 patients with sarcoidosis and on 132 with non-sarcoid erythema nodosum (EN). Pathological findings were seen especially in patients with extensive EN, without any correlation with the disease responsible for the eruption. Hepatic granulomas were found more often in patients with sarcoid changes in lung parenchyma than in those with bilateral hilar adenitis only. There were no other definite correlations between hepatic granulomas and other clinical and laboratory findings. The incidence of pathological results in this study was clearly lower than, e.g., in the USA, thus reflecting the good prognosis of sarcoidosis in the Scandinavian countries.

Immediate reactions to fruits and vegetables.

Common fruits and vegetables were tested as such with the "scratch-chamber" method in 388 patients with various atopic disorders. Of the patients with hypersensitivity to birch pollen, 36% showed immediate positive responses to these natural, fresh materials. Apple, carrot, parsnip and potato elicited reactions more often than, e.g. swede, tomato, onion and parsley. On the basis of clinical data, the relevancy of the results of the skin tests was 80-90%. Both "false positive" and "false negative" responses were seen. Itching and tingling with or without oedema of the lips, mouth and tongue were the most common complaints after eating raw fruits and vegetables. In addition, laryngeal and abdominal disturbances, rhinitis and hand dermatitis were recorded. Among patients without allergy to birch pollen, reactions to fruits and vegetables were rare. The nature of the allergens could not be determined.

Hypersensitivity reactions to Gerovital.

Procaine chloride, which was a common cause of allergic reactions in earlier years, has lost importance as a contact allergen because its use for local anesthesia has ceased. Recently, we have seen hypersensitivity reactions in three patients after the use of Gerovital, which is known to contain procaine chloride. A word of warning is warranted against indiscriminate use of Gerovital, particularly of its topical preparations.

Nodules on the legs. A clinical, histological and immunohistological study of 82 patients representing different types of nodular panniculitis.

Eighty-two cases of nodular panniculitis of the legs were examined clinically, histologically and immunohistologically. Clinically the cases could be divided into four groups: typical erythema nodosum (ENty) (35 cases), erythema nodosum migrans (ENmi) (11 cases), erythema induratum (EI) (11 cases) and the remaining 25 cases not consistent with the others as "non-definite panniculitis" (NDP). The main histological categories were septal panniculitis and lobular panniculitis, the former including erythema nodosum, both typical and migrans, the latter EI and NDP. Lobular panniculitis was divided into three subgroups in which the most prominent histological features were epithelioid cell granuloma, vasculitis and palissading granuloma, respectively. Immunoglobulins in the vessel walls were found in 5 of the 46 cases of erythema nodosum, in 19 of the 36 EI and NDP cases and, in the histological groups in 4 of the 43 cases of septal panniculitis and in 19 of the 35 cases of lobular panniculitis, respectively. Fibrin was found in the walls of the papillary capillaries and deep dermal vessels in the majority of cases of lobular panniculitis. In EI and NDP the follow-up time was 40 months, on average. Twenty-two patients were treated with antituberculous drugs, 15 became symptomless, as did 5 of the 12 patients who were not treated at all.

Caustic ulcers caused by cement.

Seven similar cases with deep ulceration probably due to wet cement are presented. Within 12 hours after making a concrete floor in a kneeling position, curved ulcers developed on both sides of both patellae. The exposure time varied from 2 to 6 h. One patient got wet concrete into her boots and, consequently, ulcers developed on her feet. The high alkalinity of calcium hydroxide in wet cement was the only apparent causative agent in every case.

Skin lesions in human yersiniosis. A histopathological and immunohistological study.

Various types of skin manifestations of human yersiniosis were examined with routine histological and immunohistological methods. The biopsy material consisted of sixteen cases of erythema nodosum, eight cases of erythema multiforme, and one case of erythema figuratum. The principal histopathological changes in erythema nodosum were septal or diffuse, mild panniculitis and in seven cases also necrotizing vasculitis in small, medium-sized or large arteries. Perivascular lymphocytic infiltration without vasculitis was the most prominent feature in erythema figuratum. By using a polyvalent conjugate, immunoglobulins in vessel walls in the dermis were found in two cases of EM. The rapid course of the skin eruptions and the frequency of necrotizing vasculitis in arteries fit the changes seen in an experimental Arthus reaction, in which necrotizing vasculitis is followed by lymphocytic inflammation consistent with a delayed type of reaction.

Urticaria from alcoholic beverages.

Three cases of urticaria/angioneurotic oedema produced by alcoholic beverages are presented. In two of them, the causative agent was probably yeasts in wines. An intracutaneous test with Candida, a conjunctival provocation test with Candida, and peroral challenge tests with wines were positive in both cases. The symptoms disappeared when the patients went on a low-yeast diet. In the third case, ethanol itself and acetic acid apparently were the factors underlying urticaria. Peroral challenge tests with ethanol, brandy and acetic acid were positive, but those with acetaldehyde remained negative. The patient has been asymptomatic after giving up alcoholic drinks alone.

Allergy to ingredients of vehicles.

Common ingredients of vehicles such as perfumes, antibacterial agents, emulsifiers and other surface active agents, propylene glycol, lanolin and wool alcohols were tested in eczema patients over a three-year period. Perfume allergy was detected in 3.6% of the cases, sensitivity to thiomersal in 2%, to sorbic acid in 0.8%, to parabens in only 0.3%, and to wool alcohols in 1.2%. Reactions to emulsifiers were seen over 1% of those tested.

Photoepicutaneous testing with the chamber method.

The chamber test, so far successfully applied to epicutaneous testing, proved to be suitable for photoepicutaneous testing as well. A simple and reliable method was developed by dividing the test areas (after removing the tests from the skin) into two equal parts, one for ultraviolet irradiation and the other for dark control. Variations in the amounts of the test substances had a slight influence on the size but not on the strength of the reactions.

Contact sensitivity to emulsifiers.

Common emulsifiers were tested in over 1,200 patients with eczema. Triethanolamine stearate tested at 5% in petrolatum caused irritant reactions in 9.5% of the patients. On the other hand, non-ionic emulsifying agents tested at 10-20% produced irritation in only a few cases. Allergic reactions were found in 2.1% of those tested. Lanette, sorbitan sesquioleate, the Spans, polyoxyethylene oxypropylene stearate, polyoxyethylene sorbitol lanolin derivative, and triethanolamine stearate each elicited allergic reactions in 0.3-0.7% of the cases. The Tweens caused an allergy in only two cases, but glycerol monostearate caused no reaction at all. Five out of six patients sensitive to sorbitan sesquioleate reacted positively to the Spans as well. The patients allergic to one or more emulsifiers were also sensitive to several other substances included in our routine test series with the exception of four patients who reacted only to the emulsifying agents.
