[Changes in erythrocyte lipids in severe septic states].

In cases of major sepsis the erythrocyte lipids undergo significant change especially when there is concomitant hepatic insufficiency and this in turn lead to alteration in the blood viscosity and very probably to the genesis of disorders of coagulation.

[Study of erythrocyte lipids after perfusion of modified fluid gelatin].

Study of changes in erythrocyte lipids after the infusion of 1,000 ml of modified fluid gelatin (Plasmion) demonstrated disturbances at the level of the lipid double layer with, in particular, a decrease in phosphatidyl-ethanolamines found in the serum. No erythrocyte deformation was seen.

Beta adrenergic blockade, regional left ventricular blood flow and ST-segment elevation in canine experimental myocardial ischemia.

The effects of dl-propranolol, d-propranolol, dl-pindolol and dl-practolol on regional myocardial blood flow (assessed by means of tracer microspheres) and on ST-segment elevation in ischemic and nonischemic areas of the canine left ventricle have been investigated. dl-Propranolol and dl-pindolol, but not dl-practolol and d-propranolol, induced blood flow redistribution from the epicardium to the endocardium both in ischemic and nonischemic areas. dl-Propranolol-induced redistribution was abolished by atrial pacing at the control heart rate value. These results indicate that the redistribution phenomenon only occurs if both a bradycardia-inducing beta1 adrenoreceptor blockade and a coronary vessels beta2 adrenoceptor blockade are simultaneously achieved. All four drugs significantly decreased ST-segment elevation in ischemic areas. Under atrial pacing, this effect was abolished with dl-practolol but only reduced with dl- and d-propranol, suggesting that, besides bradycardia, membrane stabilization might be involved in protection against ST-segment elevation in ischemic areas.

[Pancreatic transplantation. Implantation of auto and allografts of the pancreas into the gastric wall of the diabetic rabbit (author's transl)].

The author's studied pancreatic autotransplants and allotransplants in the gastric submucosa of the rabbit after removal of the pancreas. Pancreatctomy causes severe diabetes with hyperkaliemic acidosis which is always fatal with 72 hours. Auto or allotransplantation cures the diabetes within two to three days, and maintains normal glucose balance for about 60 days. Early death after transplantation occurred when the graft was totally ineffective. After transplantation, the blood sugar rose progressively towards the 15th day, then suddenly and definetely at the time of necrosis of the graft towards the 15th or 21st day. The follow up in some transplanted animals exceeded two months. The interest of the procedure resides in the implantation of a large critical mass (25 to 30% of the pancreas ) in a well-vascularised area, drained by the portal system, and permitting the digestive tract elimination of the pancreatic necrosis whether of vascular or immune origin.

[Diagnosis of hepatic metastasis by enzymatic assay. Value of gamma-glutamyltranspeptidase].

Variations in serum enzyme levels were studied in 50 patients suffering from various types of neoplasm, 25 of whom had hepatic metastases. In the absence of any associated hepato-biliary or pancreatic disorder, gamma-glutamyl-transpeptidase would appear to be a reliable test in 96 % of cases. It represents a factor of assessment which merits a place in the prognostic study of all types of neoplasia.

[Hydroelectrolytic disturbances secondary to high flow lavage used for the prevention of posthemorrhagic encephalopathy in cirrhosis].

The authors carried out high flow intestinal lavage during gastrointestinal bleeding in cirrhotics in order to obtain rapid elimination of blood from the intestine and thereby reduce the risk of encephalopathy. The fluid used had been developed for preparation of the colon for surgery and was responsible for water and electrolyte disturbances with water and salt retention. Without criticising the principle of lavage in the prevention of encephalopathy, the authors express certain reserves with regard to the use of this solution and suggest the use of a sodium-free substance remaining within the intestinal lumen (hypertonic mannitol) which does not cause any water/electrolyte imbalance, is more effective and would appear to have a slight though definite action on the resorption of ascites.

Changes in carbohydrate content of surface membranes of human erythrocytes during ageing.

The carbohydrate content (neutral hexoses, fucose, sialic acid, and hexosamine) of human erythrocytes is significantly higher in young than in old erythrocytes. The differences observed in the carbohydrate content of surface glycopeptides, obtained after incubation with either Pronase or trypsin, is still more striking, suggesting important modifications of the carbohydrate moeties of the erythrocyte membrane during in vivo ageing.

[Transient acanthocytosis in a patient with liver cirrhosis. Study of red cell and plasma lipids (author's transl)].

The authors report a case of major acanthocytosis which regressed spontaneously in a cirrhotic patient. The usual lipid abnormalities: considerable rise in free red cell cholesterol and lesser rise in phospholipid, regressed concommitantly to the disappearance of acanthocytes. No change in the main liver function tests was, however, observed. The etiological mechanisms of acanthocytosis in cirrhosis are probably multiple, and their interactions are compplex. The exchange of abnormal lipoproteins between the plasma and the red cell membrane is the last theory to be considered. On the other hand, no argument could explain the disappearance of the acanthocytosis, this phenomenon is considered up to now irreversible and a sign of the gravity of hepatic lesions.
