Supratentorial astrocytic tumours of childhood: a clinicopathologic study of 41 cases.

Forty-one patients with ages from 0 to 16 years harbouring supratentorial astrocytic tumours were treated between 1964 and 1986. Twenty-four of them were located in the cerebral hemisphere or in the ventricle, and 17 were in the basal ganglia, thalamus, and/or optico-hypothalamic region. There were nine glioblastomas multiforme, four anaplastic astrocytomas, 11 fibrillary astrocytomas, 12 pilocytic astrocytomas, two pleomorphic xanthoastrocytomas, two subependymal giant cell astrocytomas and one primitive astrocytic tumour. Low grade tumours tended to occur in young subjects near the midline, and high grade one in older patients in the hemisphere. Initial treatment in most the patients was a resection or biopsy followed by radiation therapy. Eleven patients were retreated for their recurrent tumours with combinations of surgical removal of the tumour, radiotherapy, and/or chemotherapy. After a follow-up of 3 months to 17 years, 51% of the patients survived with a median survival of 7 years. Outcome of the patients with supratentorial astrocytomas correlated mostly with the histological grade of malignancy of the tumour, and to a lesser degree with the tumour location. The biological behavior and treatment of supratentorial astrocytomas in childhood are discussed.

Balloon test occlusion of the internal carotid artery with monitoring of compressed spectral arrays (CSAs) of electroencephalogram.

We used the compressed spectral arrays (CSAs) of the electroencephalogram (EEG) to monitor cerebral blood flow related events in balloon test occlusion of the internal carotid artery (balloon Matas test). Reliability of the CSAs was examined in 22 patients subjected to the test. Of 9 patients who underwent subsequent permanent carotid occlusion, in 6 there was no change on CSAs and/or in neurological conditions. In 2 patients, there was a slowing on CSAs prior to the appearance of neurological deterioration during clinical testing. CSAs transformed these EEG changes into a succinct graphic display. In 1 of the 2, for whom the Matas test was repeated 1 year later, there was a change from positive to negative and delayed cerebral infarction occurred after carotid occlusion. In the other patient, an ischemic insult occurred during the balloon occlusive procedures. The third patient had a fatal delayed ischaemic complication 3 days after surgical ligation of the cervical carotid artery, despite the negative balloon Matas test. Complications in these 3 patients were presumably related to thromboembolic mechanisms. The balloon Matas test monitored by CSAs is useful for detecting change in brain functions attributed to an inadequate blood flow. However, this test is unreliable for predicting thrombo-embolic complications.

Selective enhancement of intratumoural blood flow in malignant gliomas: experimental study in rats by intracarotid administration of adenosine or adenosine triphosphate.

We studied the effect of intravenous and intracarotid infusion of adenosine and adenosine triphosphate (ATP) on the regional blood flow of intracerebrally transplanted RG-C6 tumours in rats, using the hydrogen clearance method. The intracarotid administration of adenosine or ATP selectively increased blood flow in the tumour, but did not produce any significant change either in the regional cerebral blood flow of the extratumoural ipsilateral hemisphere or in the ipsilateral hemisphere without tumour. The intracarotid administration of ATP at a dose of 10 micrograms/kg/min produced the most effective increase in the tumour blood flow (+51.5 +/- 16.8%). In contrast, both the intravenous administration of adenosine and that of ATP failed to increase tumour blood flow. These results may possibly indicate that intracarotid administration of the adenosine or ATP might contribute in selectively enhancing the delivery of anti-cancer agents to malignant brain tumours.

Magnetic resonance imaging of jugular foramen neurinomas.

Four cases of jugular foramen neurinoma, extending from the posterior fossa into the temporal bone through the jugular foramen, are reported. The information provided by magnetic resonance imaging was compared with that by computed tomography. Due to the excellent sensitivity, the absence of bone artifacts, and the ease of imaging in any plane, magnetic resonance imaging gave better information particularly about the location of the tumour than X-CT scan did. It included the dumb-bell shaped tumour through the jugular foramen, the extra-axial nature of the tumour, and the precise anatomical relationships between the tumour and the facial and acoustic nerves or the internal carotid artery. Sagittal and coronal views clearly demonstrated the superior and inferior margins of the tumour. The information is very useful not only for preoperative diagnosis but also for planning the surgical approaches and postoperative follow-up.

Oculomotor nerve palsy in patients with cerebral aneurysms.

Twenty-six patients with oculomotor nerve palsy due to cerebral aneurysms were examined. There were six males and twenty females with a mean age of 55 years. 25 of the 26 aneurysms were located at the junction of the internal carotid and the posterior communicating artery and one was at the junction of the basilar artery and the superior cerebellar artery. Twelve patients had associated subarachnoid hemorrhage (SAH); the other 14 did not. The initial symptoms in many patients were ptosis and double vision. Twenty-one of the patients had total oculomotor nerve palsy, one had a sparing of medial rectus muscle; three patients had only ptosis and anisocoria, and one had oculomotor nerve palsy with pupillary sparing. All aneurysms, including giant aneurysms, were clipped under a microscope, and six oculomotor nerves were found to be decompressed at surgery. The follow-up periods were from six months to three years. Nine patients had a complete recovery of oculomotor function; thirteen had an incomplete recovery; and four remained unchanged. The mean interval between the onset of palsy and the time of surgery was 24 days in complete recovery cases, 42 days in incomplete recovery cases, and 119 days in unchanged cases. The recovery of oculomotor function started with the levator palpebrae muscle and followed by the medial rectus muscle. The recovery of pupillary function was, however, not consistent. Of the factors influencing recovery from oculomotor nerve palsy, the interval between the onset of palsy and the time of surgery was most important. Therefore, aneurysms with oculomotor nerve palsy should be operated on as early as possible, regardless of the presence or absence of SAH.

Clinical and laboratory findings implicating palytoxin as cause of ciguatera poisoning due to Decapterus macrosoma (mackerel).

A near fatal case of ciguatera-related intoxication following consumption of smoked Decapterus macrosoma is documented. In addition to some of the hallmark symptoms of ciguatera poisoning, the patient exhibited acute respiratory distress and severe muscle spasms. Laboratory results showed large elevations in a number of blood enzymes, indicative of muscle damage. The responsible agent was extracted from corresponding fish samples and identified as palytoxin.

Microsurgical procedures for management of giant middle cerebral aneurysm causing increased intracranial pressure.

There have not been many cases reported of giant aneurysms of the middle cerebral artery with a remarkable mass and causing increased intracranial pressure, and the successful operation for resecting an aneurysm is still low. Considering it essential that the mass of an aneurysm should be reduced and the blood flow of parent and distal arteries should be preserved, we are reporting two successful results.

Convexity meningioma with galactorrhea and hyperprolactinemia. A case report.

A case is reported of a huge convexity meningioma associated with galactorrhea and an elevated serum prolactin level of 41 ng/mL. Total removal of the tumor resulted in a return of the prolactin level to normal (9.7 ng/mL). Because of a severe mass effect, the distant tumor might have encroached upon the hypothalamic-pituitary axis and interrupted PRL inhibitory factor regulation.

Three different types of alpha-fetoprotein in the diagnosis of malignant solid tumors: use of a sensitive lectin-affinity immunoelectrophoresis.

A sensitive new technique for lectin-affinity immunoelectrophoresis was applied to samples from 28 infants and children in order to distinguish the origin of elevated alpha-fetoprotein (AFP) in sera. This new immunoelectrophoresis was successfully performed within 24 hours in sera with AFP as small as 910 ng/mL. With combined use of concanavalin A (Con A) and lentil agglutinin (LCH) binding tests, AFPs were classified into three subtypes: benign hepatic condition type (six patients), hepatocellular carcinoma type (nine patients) and yolk sac type (12 patients). AFP was of hepatocellular carcinoma type in all seven patients with hepatoblastoma, and of benign hepatic condition type in six of seven patients with elevated AFP due to conditions such as hepatitis, biliary atresia, and normal newborn. The question as to whether AFP produced in "hepatoblastoma" is of benign hepatic condition type or hepatocellular carcinoma type was first answered by the information in this present report. The differentiation between yolk sac and general hepatic AFPs was completed with the Con A binding test.

Determination of single and repeated red cell volumes by the indicator dilution method using carbon monoxide as the indicator.

The use of radioactive isotopes limits clinical applications of blood volume measurement in the ICU. We measured red cell volumes with carbon monoxide-labeled RBC in six dogs and five human volunteers. The measured values obtained on the dogs were compared with the simultaneous measurements with the 51Cr method; the ratio of the carbon monoxide to 51Cr values ranged from 0.86 to 1.17, and the mean ratio was 1.0 +/- 0.1 (SD), r = .93. We infer from these results that the carbon monoxide method has several advantages over the 51Cr method: a) the short labeling time (about 1 min), b) rapidly decreasing background levels of carbon monoxide with FIO2 1.0, and c) repeatability at intervals of several hours.

Radiation-induced osteosarcoma of the sphenoid bone.

The case of a patient who developed osteosarcoma in the sphenoid bone 15 years after radiation therapy for a craniopharyngioma is reported. Radiation-induced osteosarcoma of the sphenoid bone has not been reported previously. Reported cases of radiation-induced osteosarcomas are reviewed.

Antitumor activities of a novel 9-aminoanthracycline (SM-5887) against mouse experimental tumors and human tumor xenografts.

The antitumor effects of SM-5887, a totally synthetic 9-aminoanthracycline derivative, were evaluated in six murine experimental tumor systems (P388, Ehrlich carcinoma, sarcoma 180, Lewis lung carcinoma, B16 melanoma and colon 38) and nine human tumor-nude mouse systems (one breast cancer, two lung cancers and six gastric cancers). Characteristically SM-5887 showed excellent antitumor activities, superior to adriamycin (ADR), against human tumor xenografts, although its activities against murine experimental tumors were almost equal to those of ADR. When the human tumors were implanted sc in female athymic mice (BALB/c, nu/nu) and their volume reached 100-300 mm3, SM-5887 and ADR were injected iv. All nine human tumors tested showed statistically significant responses to SM-5887, and 7 of them were strongly suppressed in their growth by SM-5887 so that minimum T/C values were less than 30% at the maximum tolerated dose (MTD, 25 mg/kg) with a single iv injection. Compared with ADR, SM-5887 was statistically more effective in five tumors (one breast, one lung and three gastric), equal in two tumors (two gastric), and less potent in two tumors (one lung and one gastric). In addition, the 10-day-interval repeated iv treatments with SM-5887 at the MTD (25 mg/kg) resulted in remarkably potent antitumor effects (including complete regression) against human gastric cancer, 4-1ST, implanted in nude mice without enhancement of toxic effects. SM-5887 was also effective against ip-inoculated P388 by oral administration as well as iv injection.

Toxicological aspects of a novel 9-aminoanthracycline, SM-5887.

The toxicological characteristics of SM-5887 were evaluated in mice after a bolus intravenous injection, and compared with those of adriamycin (ADR). The acute toxic signs observed after SM-5887 administration were body weight decrease, ataxia, hair loss, and myelosuppression. They were qualitatively comparable to those induced by ADR. The 50% lethal dose values determined by 14-day observation after drug administration were in the range of 32 to 50 mg/kg for SM-5887 and 16 to more than 20 mg/kg for ADR in four strains of mice. The maximum tolerated doses (MTD) were estimated to be 25 mg/kg for SM-5887 and 12.5 mg/kg for ADR (no death or body weight loss of more than 3 g occurred). When 14-day survivors were further observed until 90 days after drug administration, ADR frequently and dose-independently showed delayed-type lethal toxicity at doses of more than 10 mg/kg, whereas SM-5887 did not. The myelosuppression of SM-5887 was more severe even at a half of the MTD than that of ADR at the MTD, but its recovery was more rapid than that after ADR. In addition, when the drugs were injected into the subplantar region of mouse hind paws, ADR induced a severe inflammatory reaction, whereas SM-5887 yielded only a slight one. The data suggest that toxic effects of SM-5887 are more reversible and more controllable than those of ADR.

Analysis of lectin affinity immunoelectrophoretic profiles of serum alpha-fetoprotein from patients with yolk sac tumors and carcinomas of the gastrointestinal tract: correlations with molecular structures.

Profiles of concanavalin A (Con A) and lentil agglutinin (LCH) affinity immunoelectrophoresis were compared for serum alpha-fetoprotein (AFP) from patients with yolk sac tumors and carcinomas of the gastrointestinal tract, in order to find some correlations between peaks of AFP subfractions detectable by two different lectins, and to investigate whether or not it is possible to prove that the binding of AFP to LCH is weakened to some extent if a fucosylated sugar chain has, in addition, a bisect N-acetylglucosamine (GlcNAc) attached to the beta-linked mannose. The results obtained with our improved techniques tend to indicate that a Con A-reactive AFP subfraction (peak a) corresponds to an LCH strongly reactive AFP (peak A), while a Con A-nonreactive AFP (peak b) corresponds to an LCH weakly reactive AFP (peak B). the authors consider the present data sufficient to support the above explanation.

A case of ocular reticulum cell sarcoma responded to radiation therapy.

We report a case of reticulum cell sarcoma with involvement both in the brain and the eyes. The patient was a 49-year-old man. In 1985, he suffered steroid-resistant uveitis of unknown etiology in his right eye. After that, in 1986, he was aware of a weakness in the right arm and leg and of right sensory disturbances. Histopathological study of a biopsy specimen revealed a malignant lymphoma in the left thalamus, and the lesion was effectively treated by radiation to the brain. In 1987, he complained of visual disturbances in the left eye. According to the history and ocular manifestation, the disease was diagnosed as ocular reticulum cell sarcoma. Radiation therapy to his left orbital cavity was effective and inflammatory signs in the left eye subsided.

The microsurgical anatomy of the infratentorial lateral supracerebellar approach to the trigeminal nerve for tic douloureux.

The increasing use of microsurgical decompression for trigeminal neuralgia has created a need for more detailed anatomical information about the approach. To define better this anatomy, 10 cerebellar specimens obtained at autopsy were examined, and intraoperative findings in 30 patients with trigeminal neuralgia were analyzed. Since the infratentorial subdural space on the tentorial cerebellar surface is exposed to explore the trigeminal nerve in the infratentorial lateral supracerebellar approach, attention was directed to the following: the anterolateral margin of the cerebellar hemisphere, bridging veins on the tentorial surface, superior petrosal veins, and relationships between blood vessels and the trigeminal nerve. The lateral mesencephalic segment of the superior cerebellar artery at or near the bifurcation often compressed the nerve laterally at more than one point. With this approach, the relationship of the superior cerebellar artery to the nerve could be observed from the medial side of the tentorial surface. The infratentorial lateral supracerebellar approach is discussed and compared to Dandy's cerebellar route.

Metabolism of dibucaine. II. Disposition and metabolism of dibucaine in rats.

The disposition and metabolism of dibucaine were studied in rats. After intraperitoneal administration of 3H-labelled dibucaine, the blood concentration of total radioactivity reached a maximum at 10 min and declined thereafter, with a biphasic curve having half-lives of 37.7 min and 11.2 h. Radioactivity in the tissues after administration was high in small intestine, lung, spleen, liver, kidney and stomach. Urinary and fecal excretion of radioactivity were 39.4 and 49.0% of the dose, respectively, in 3 d after administration. Biliary excretion of radioactivity in bile duct-cannulated rats was 53% of the dose within 48 h. About 40% of urinary, fecal and biliary excretion was found in the basic metabolites, and about 12% of urinary excretion was in the acidic metabolites. In addition, 9.5% of urinary excretion and 39.5% of biliary excretion were conjugated metabolites which could be extracted following enzymatic hydrolysis. Most of the conjugates were glucuronides. Remaining metabolites were highly polar and water-soluble ones which were not hydrolyzed by enzyme treatment and could not be extracted into organic solvents.

Characteristics of mutant mice (ICGN) with spontaneous renal lesions: a new model for human nephrotic syndrome.

Spontaneous nephrotic mice (ICGN mice), a new mutant strain of mouse from outbred ICR, were clinically, macroscopically, histologically and immunohistochemically studied to establish their value as a model for human nephrotic syndrome. Most of the affected mice developed proteinuria, hypoproteinaemia and hypercholesterolaemia, and some of them developed systemic oedema. A high concentration of blood urea nitrogen (BUN) and a low haematocrit value were also observed. The kidneys of severe cases showed a decrease in size and had a yellowish granular surface. These findings indicated that the mice were terminally affected by chronic renal insufficiency. Histopathology demonstrated glomerular lesions consisting of thickened basement membranes of the capillary loops with irregular spike-like protrusions and enlargement of the mesangium unaccompanied by cellular proliferation. The immunofluorescence technique revealed positive granular staining for IgA, IgG and IgM and to a lesser extent for C3 along the capillary loops in affected mice. The similarity between this spontaneous disease and human nephrotic syndrome caused by idiopathic glomerular lesions is discussed. ICGN mice may be a useful animal model for this human disease.
