[Combination chemotherapy with PAP (peplomycin, adriamycin and cisplatin) in recurrent uterine cervical cancer with pulmonary metastasis].

Six patients with recurrent carcinoma of the uterine cervix were treated with a combination of peplomycin, adriamycin and cis-platinum (PAP). The PAP regimen consisted of peplomycin at a dose of 5 mg/day, continuous i.v. drip on days 1-7, adriamycin at a dose of 40 mg/m2 i.v. on day 1, and cis-platinum at a dose of 40 mg/m2, continuous i.v. drip on day 1, repeating at five to six week intervals. Three complete responses and three partial responses were obtained in six evaluable patients. Toxicities included mild to moderate nausea and vomiting, alopecia, nephrotoxicity, and myelosuppression, were generally manageable. We have concluded that PAP regimen is one of the useful regimens for recurrent uterine cervical carcinoma with pulmonary metastasis.

[Combined chemotherapy and elemental diet in advanced lung cancer patients].

The nutritional and immune status were studied in advanced lung cancer patients receiving combined chemotherapy and an elemental diet to test the effectiveness of the latter. Parameters such as skin tests, lymphocyte count, total protein and albumin values, body weight, and nitrogen balance were improved or maintained by the administration of the elemental diet. A better response to chemotherapy was achieved in patients receiving the elemental diet. Elemental diet support is suggested as a useful addition to chemotherapy in advanced lung cancer.

A cytochemical study of lactic dehydrogenase (LDH) isoenzymes in human lung cancer.

The cytochemical and electrophoretic LDH isoenzyme patterns in cells from lung cancer tissues were examined with 2.6 M urea treatment and the correlation between the LDH isoenzyme pattern and histopathological entities of lung cancer was also studied. The zymograms of the nonlesional lung tissues indicated the main peak at LDH3, with the M/H ratio 0.74. In tumor tissues, epidermoid carcinoma, adenocarcinoma, and large cell carcinoma showed almost similar patterns of isoenzymes with LDH4 peak. Though small cell carcinomas had a peak at LDH3, they indicated lower activity of %LDH1 and %LDH2 than those in nonlesional lung tissues. Most of the tumor tissues showed a high M/H ratio, more than 1.0. In cytochemical stain for LDH, no inhibitory effect of urea treatment was seen in the nonlesional lung tissues. On the other hand, in the cells obtained from three types of lung cancer, except for small cell carcinoma, urea inhibition was noticed. Cytochemical stainability for LDH-M subunits corresponded well to the results of LDH zymogram. These results suggest that the LDH stain with urea treatment is a useful method for detecting malignancy in cytological specimens.

[Cancer of the maxillary antrum--a new multidisciplinary treatment containing neo-adjuvant chemotherapy].

During the period 1957-1982, 227 patients with maxillary carcinoma were treated. These cases were divided into 4 groups according to periods and methods of treatment. Five-year determinate survival figures for each group are as follows: I (1957-1966) 22%, 20/91, II (1967-1973) 40%, 27/67, III (1974-1978) 37%, 13/35, IV (1979-1982) 56%, 9/16 (3-year survival). The incorporation of intraarterial chemotherapy into the treatment of maxillary carcinoma has much contributed to an improvement of 5-year survival. Since 1982, a new multidisciplinary treatment containing neo-adjuvant chemotherapy has been introduced into the treatment of this type of carcinoma. Neo-adjuvant chemotherapy: day 1, cisplatin ( CPDD ) 50 mg/m2 or 80 mg/body i.a. over 2 hr., day 2-6, peplomycin (PEP) 5 mg/day i.a. over 5 hr. Two courses of chemotherapy were given with an interval of 2 weeks. Radiotherapy combined with intraarterial chemotherapy: Linac. 40 Gy/4 wks., 5-FU i.a. 250 mg/day q.d. (for 10 days). When no cancer cells were detected at the completion of this therapy, adjuvant chemoimmunotherapy was given. When an apparent tumor was still revealed by CT, radical surgery was performed. When cancer cells were detected only by histological examination, additional radiotherapy up to 60 Gy was given by Linac. The results obtained were analysed in 9 patients who completed this therapy. At the completion of CPDD and PEP treatment, complete response was achieved in 5 cases and partial response in 3 cases with a response rate of 89%. In 2 out of the 9 cases, no cancer cells were detected by histological examination after chemotherapy. No cancer cells were detected in 2 other cases after 40 Gy by Linac. combined with 5-FU i.a. infusion, and in 3 other cases after 60 Gy by Linac. combined with 5-FU. Two patients had radical surgery because of uncontrolled tumors. Periods of follow-up are too short, but it is expected that this new multidisciplinary treatment results in the higher survival.

[Clinical experience with oxendolone for treatment of benign prostatic hyperplasia. Clinical efficacy and effects on serum lipid and lipoprotein fraction levels].

Forty-three patients with benign prostatic hyperplasia were treated with weekly i.m. injections of 400 mg oxendolone for 12 weeks. The subjective symptoms were improved in 83% of these patients. Residual urine was decreased significantly and Qmax was increased by this treatment. Serum VLDL level was suppressed significantly, whereas the levels of LDL, total cholesterol, HDL-cholesterol and triglycerides were changed little. Atherosclerotic index and the ratio of (total cholesterol--HDL-cholesterol) to (HDL-cholesterol), was not influenced by the treatment. No severe side-effect was found. These findings suggest that oxendolone is the drug of choice for non-surgical treatment of benign prostatic hyperplasia.

[Effect of intravenous treatment with OK-432 on the bone marrow in patients with lung cancer].

We studied effects of OK-432 on the bone marrow and peripheral blood cells of lung cancer patients. The nuclear cell count of bone marrow increased in 5 to 7 patients upon intravenous treatment with OK-432 compared with 3 of 6 patients who were intramuscularly treated with OK-432. Serial neutrophil counts of bone marrow increased in all 7 patients treated intravenously compared with 3 of 6 patients treated intramuscularly. The mean nuclear cell count or the serial neutrophil count of bone marrow in intravenously treated patients was significantly higher than the pretreatment values (p less than 0.001). In the peripheral blood picture, the difference in white blood cells or neutrophils before and after intravenous treatment was also statistically significant (p less than 0.01). There was no change in the erythrocytic series count of bone marrow and the hemoglobin count. Our results support the superiority of intravenous OK-432 treatment over intramuscular treatment in the growth-accelerating effect on bone marrow cells, especially regarding the neutrophil series.

Identification of a glycoprotein, gp21, of adult T cell leukemia virus by monoclonal antibody.

A mouse hybridoma cell line producing monoclonal antibody, F10, was established from mice hyperimmunized with cells bearing adult T cell leukemia (ATL) virus (ATLV). F10 antibody reacted with an ATLV structural polypeptide ( gp21 ) with a m.w. of 21,000 that was glycosylated on cell surfaces. The gp21 was expressed on cell surfaces of all ATL-associated antigen (ATLA)-positive human cell lines but not on ATLA-negative cell lines nor peripheral blood leukocytes stimulated with mitogens. The gp21 was also detected with anti-ATLA-positive human serum, and the binding of F10 antibody to ATLA-positive cell surfaces was significantly blocked by pretreatment with anti-ATLA-positive human sera. Double immunofluorescence staining with F10 antibody and anti-ATLA-positive human serum caused co-capping on cell surfaces, which suggests that gp21 co-exists with other ATLV antigens expressed on cell surfaces. Immunoprecipitation studies also suggested that the gp21 is a minor component of the ATLV envelope.

[Concentration of cefotiam in the human prostatic tissue and bladder wall].

Cefotiam (CTM) was administered in 31 patients before surgery and the concentration of CTM in serum, extirpated prostatic tissue and bladder wall was measured. Additional studies on pharmacokinetic parameters were carried out. The maximum level of serum concentration, following 1 hour intravenous infusion of 1 g, was 54.0 micrograms/ml after 1 hour and T1/2 was 1.16 hours. The maximum level of prostatic tissue was 19.5 micrograms/g and of bladder wall was 29.9 micrograms/g after 1.2 hours and T1/2 was 1.14 hours for both. An appropriate administration method for single use of CTM in the patient of UTI was discussed and 1 hour infusion of 1 g CTM was suggested.

[High dose methotrexate with CF rescue therapy in patients with head and neck cancer].

A clinical trial of high dose methotrexate (MTX)-CF rescue was conducted in 17 institutions. Forty-seven patients with head and neck cancer entered this trial, of which 29 were evaluable. In this series, the patients were divided into 2 groups (Arm I, Arm II), according to physician's selection. MTX was administered 700 mg/m2 (1000 mg/body) in Arm I and 1750 mg/m2 (2500 mg/body) in Arm II by i.v. drip over 6 hours. Twenty-four hours after initiation of MTX infusion, CF rescue was started. There was no complete response in 29 patients. Four patients showed partial response with an overall response rate of 13.8%. There were 4 partial responders out of 21 patients in Arm I, with a response rate of 19%, whereas there was no partial responders out of 8 patients in Arm II. These results showed no apparent dose response. MTX concentrations in plasma were determined at 6, 24, 48 and 72 hours after initiation of MTX infusion. The assay results revealed a safe range in Arm I, but exceeded in 3 cases of Arm II. GI disturbances were seen at the rate of 78%. Bone marrow suppression was remarkable and hepatic toxicity was observed as the rate of 41%. No renal toxicity was observed. The results of high dose MTX-CF rescue therapy were not better than those of moderate dose therapy, so that we concluded that MTX should be used as one agent at low or moderate dose in combination chemotherapy as far as Japanese patients with head and neck cancer are concerned.

The trend and magnitude of ischaemic heart disease in Japan.

The secular trends and present status in Japan of the incidence and prevalence of ischaemic heart disease, particularly acute myocardial infarction, have been debated between clinicians and epidemiologists. In order to have a clearer picture, available information from national mortality statistics, cardiovascular surveys and patient surveys have been scrutinized. Also pooled autopsy records and epidemiological studies on selected population groups have been studied. The findings are still controversial, but the tentative conclusion is that morbidity from acute myocardial infarction in Japan is still low and has increased only slightly, but signs of a change can be observed. Various confounding factors in mortality and morbidity statistics are presented and discussed. There is a need to develop monitoring systems to obtain more accurate information on the magnitude and secular trends of ischaemic heart disease.

[Local administration of anti-cancer drugs].

Anti-cancer drugs in the forms of an emulsion, a microsphere, and of conjugates with high molecular dextran have been developed in our laboratories, namely a fat emulsion of anticancer drug, MMC-microsphere, or MMC-dextran conjugates. The main advantages of these forms of pharmaceutical preparation are that they give prolongation of pharmacological actions by slow release and that they are applicable for topical use because of their reduced toxicities to local tissue yet maintaining local therapeutic potency. In this study we found that the rate of sustained release of drugs and antitumor effects were enhanced when used in such modified forms of drugs for topical injections. Results of clinical trials of those drugs have been promising. However, the number of trials has been limited so far. Further studies would be required for evaluating on of their clinical utilities.

[Efficacy of intrapleural treatment with aclacinomycin combined with closed tube thoracostomy for malignant pleural effusion].

Intrapleural treatment with aclacinomycin combined with closed tube thoracostomy was used in 7 patients with malignant pleural effusion. Five patients had no recurrence of effusion 3 months after the treatment. Aclacinomycin levels were much higher in blood cells than in plasma, and metabolites were present as the active form. We posit that the local instillation of aclacinomycin is indicated in the management of malignant pleural effusion.

[Clinical studies of antimicrobial prophylaxis of recurrent urinary infection in women. Long-term, low-dose pipemidic acid for prophylaxis].

The prophylactic efficacy of long-term, low-dose antimicrobial treatment in urinary tract infection (UTI) was studied. Fifty-eight female adult patients with a history of at least two recurrent episodes of UTI in the past year were entered into this study, and the prophylactic regimen was not started until the existing UTI had been eradicated. Patients took 250 mg of pipemidic acid (PPA) daily at bedtime after voiding for 6 consecutive months. Incidence of recurrence of UTI in 48 patients with uncomplicated UTI and 10 patients with complicated UTI decreased to 0.15 and 0.29 per year, respectively, during the treatment compared with 3.5 per year before the treatment. At the end of the 6 months of prophylactic treatment, the patients were divided into two groups by the envelope method. Seventeen patients were treated for a further 6 months and 11 patients were followed up without further medication. Prophylactic efficacy of UTI was obtained in both groups, and there was no significant difference in the incidence of recurrence between the two groups. These findings suggest that the 6-month period of prophylaxis might be sufficient. Examination of the periurethral swab showed that E. coli and Klebsiella sp. were decreased during the treatment. This prophylactic treatment produced no resistant strains. Urinary levels of PPA in the morning urine of patients administered 250 mg of PPA at bedtime averaged 513 micrograms/ml. These values were about 2 times higher than those found in the evening urine after administration of the same dose in the morning.

[Remission maintenance therapy of non-Hodgkin's lymphoma with CHOP protocol].

We have retrospectively studied the problems concerning duration of maintenance therapy and chemotherapy regimens for non-Hodgkin's lymphoma treated at Shikoku Cancer Center Hospital. Four out of 15 complete remission cases had been in complete remission more than 3 years. Maintenance chemotherapy consists of CHOP protocol administering every 4 weeks and the median maintenance duration was 14 months with a median cycle of 14. Thereafter, patients have been followed up without receiving any therapy. The prospective study on maintenance therapy should be designed considering conditions such as 1) sufficient drug doses for cytocidal effects, 2) shorter maintenance period as possible, and 3) minimal side effects and complications to be expected.
