[Uric acid balance in radiotherapy].

Administration of uricostatics within the scope of radiotherapeutic treatment is indicated in all cases of manifest primary gout; in cases of latent primary gout, uricostatics should be given especially after administration of high integral doses or in case of large tumors with distinct radiation sensitivity. The decision depends on the renal function, as an increased output of substances to be eliminated by urine may induce an attack of gout in cases of latent gout, or, otherwise, may cause renal lesions followed by acute uric acid nephropathy.

[Classification and immunosuppressive therapy of systemic lupus erythematosus: long-term follow-up of 31 patients (author's transl)].

The course of systemic lupus erythematosus (SLE) in 34 patients receiving prednisone and immunosuppressive (cytotoxic) drugs is described. 31 patients are in continuous ambulatory care, 27 of them 3 to 17 years. Only two were hospitalized for therapeutic purposes: one patient because of exacerbation of lung tuberculosis; the second patient, who discontinued prednisone therapy despite active SLE, because of severe autoimmune haemolytic anaemia. The retrospective analysis of symptoms demonstrated that only 22 of the 31 patients manifested 4 or more of the prelimievious reports. Two patients with severe lupus glomerulonephritis and azotaemia at the time of initial evaluation did not benefit from combined therapy. Nine patients with mesangial and focal glomerulonephritis have normal kidney function up to 12 years after diagnosis (in two cases kidney function improved after therapy).

[Thrombolytic therapy in arteriosclerotic disease (authors transl)].

The treatment of acute arterial occulsion, especially emboli and also thromboses associated with a complete ischaemia is the domain of the surgeon. Subacute or chronic arterial occlusion -- uni- or myltilocular --are suited for a trial of thrombolytic therapy, with an approximate success rate of 40%. When thrombolysis is contraindicated, then transluminal dilatation according to Dotter can be attempted in cases of short stenosis. A new and successful indication for thrombolysis is in the therapy of cases of late reocclusion after vascular reconstructive operations. The preliminary results in this field are very good, especially in the aortoiliac region. The indications, the practical details, the results and complications of thrombolytic therapy with streptokinase are discussed.

[The diagnosis of cholestasis: lipoprotein X (LP-X) (author's transl)].

The diagnostic specificity of a new method to detect obstructive jaundice by determination of lipoprotein X (LP-X) was tested in 144 patients with different kinds of hepatic diseases and compared with the usual chemical "obstructive jaundice specific" tests, such as bilirubin, SGOT, SGPT, alkaline phosphatase, LAP and gamma-GT. The LP-X test was performed by using all-in test kit LP-X Rapidophor" low-voltage electrophoresis of Immuno AG/Wien. The results were correlated with the histological classification of the liver biopsy specimen. In 82% of the histologically verified cases of obstructive jaundice the result of the LP-X test was positive, whilst in 98.5% of the histologically negative cases the result of the LP-X test was negative. Hence, this LP-X method proved superior to chemical methods in providing a clear-cut positive or negative answer to the presence of cholestasis. Furthermore, the LP-X test was suitable for long-term follow-up investigation of patients with obstructive jaundice.

The effects of sodium, potassium and ATP on a developmental puff sequence in Drosophila salivary glands in vitro.

Salivary glands of late third instar larvae of Drosophila melanogaster were isolated at a developmental stage when the release of ecdysone had already taken place. They were then incubated in a chemically defined medium. An ecdysone-dependent developmental puff sequence was measured in vitro and influenced by adding various substances or by changing iso-osmotically the sodium or potassium content of the medium. Trinactin, valinomycin N-ethylmaleimide and KCN blocked the puff sequence, i.e. the regression of the early and the induction of the late ecdysone-dependent puffs probably by increasing the Na+ influx and depleting the ATP content of the cell. A medium that contained Na+ as the only monovalent cation decreased the size of the late ecdysone-dependent puffs and increased the size of other puffs, such as 50 CD. Addition of tetrodotoxin to the normal medium had the opposite effect, i.e., it increased 63 E and inhibited 50 CD. Na+ free medium, inhibition of K+ flux by tetraethylammonium chloride, and application of ouabain did not considerably influence the size of the puffs measured. It is concluded from these results that Na+ in particular has an inhibitory effect on the induction of late ecdysone-dependent puffs. Na+ (and perhaps also K+) may act by modulating the effect of proteins that are involved in gene control mechanisms.

A new method for producing a chronic E. coli pyelonephritis in rabbits.

Chronic pyelonephritis, especially that caused by the most common invader E. coli, corresponding satisfactorily to the pathophysiology of the human disease is difficult to reproduce. We have developed a pattern of infection with which we succeeded reliably in producing a chronic E. coli pyelonephritis. This was achieved by inserting a plastic catheter into the renal pelvis. A suspension of E. coli was injected via the catheter into the renal pelvis of 32 rabbits. This pattern of infection takes a chronic progressive course and, in all cases, results in typical macroscopic and histological changes in the kidney. The bacterial excretion in the urine remains unchanged over a period of months. With the experimental procedure described, the pathogenesis and pathophysiology of chronic E. coli pyelonephritis can be clearly studied. Drugs currently used on patients can be tested for their effectiveness in long-term application.

Proliferation of ineffective erythropoiesis with nuclear abnormalities and megaloblastoid appearance in preleukaemia.

Erythropoiesis of two patients in the early stage of acute leukaemia and two patients with refractory anaemia and hypercellular bone marrow (preleukaemia?) was studied with the cytophotometric-autoradiographic method. Megaloblastoid erythroblasts show a decreased proliferative activity in comparison to the morphologically normal cells and are in the maturation stage of E4 (early polychromatic normoblasts) mainly in the G1-phase; they are therefore largely comparable to the megaloblastoid erythroblasts in erythroleukaemia. Erythroblasts in preleukaemia with nuclear abnormalities are found in a high percentage in the G2-phase or are unlabelled with a DNA content of between diploid and tetraploid value. They show a similar proliferative behaviour to the megaloblasts in pernicious anaemia. Early polychromatic erythroblasts arrested in G2-phase can differentiate without mitosis into tetraploid mature erythroblasts (E5). They can divide elsewhere endomitotically, produce binucleated E5 or take up the DNA synthesis and become polyploid. E4 with nuclear abnormalities do not proliferate and are mainly found in the premitotic phase.
