[Comparison of scintigraphic and neurosurgical findings in brain tumors].

In 120 cases of operated brain tumors the neurosurgical findings are compared with the results of brain scintigraphy with 99mTc-pertechnetate. The study comprises only the frequent types of brain tumors such as meningeoma, astrozytoma, glioblastoma, oligodendroglioma, neurinoma and metastases. From the neurosurgical findinds were evaluated the localisation and size of the tumor, the extent of vascularity, of cysts and necroses. These data were related to the type or radioangiography and the degree of uptake in the scintigraphy. The tumors could be demonstrated by scintigraphy in 105 of the cases. Radioangiography was obviously superior to late scintigraphy in the diagnosis of the type of tumor. The size of the tumor appeared in the meningeomas greater and in the other types of tumor smaller than found by operation. The uptake in scintigraphy showed a correlation to vascularity. Otherwise there was found no correlation between vascularity and type of radioangiography in glioblastomas. This surprising result needs further investigatiion. Half of the cases with cysts and necroses showed an inhomogeneity in the uptake by the tumor.

[New drug combination for medical management of hyperlipemia: clinical study].

Sixteen patients with different kinds of hyperlipidemia were treated with Atroplex (Mg-Chlorphenoxyisobutyrate 350 mg, Mesoinositol-Hexanicotinate 250 mg), a new serum lipid decreasing drug. Within an interval of 60 days' standard treatment of 3 tablets of Atroplex, the serum level of cholesterol was reduced by 16-20% and that of triglycerides by 36-49%. This significant effect was obvious over the whole period of treatment. According to the compound of Clofibrate, Atroplex interfered with the anticoagulant treatment with phenprocoumon. In spite of this an AC-therapy is possible during the Atroplex medication, if the AC doses are reduced by 30-50%.

[Acute stomach lesions and changes in histamine level of various organs in immobilization stress].

In the glandular part of the stomach, the histamine content was significantly increased during stress by immobilization. Following 8 hours of restraint these alterations were observed only in lung tissue, but not in other organs investigated. Although the maximum of stress ulcer formation did not correspond with the maximum increase in gastric histamine content as far as the time course is concerned, a role of histamine in stress ulcer disease cannot be excluded.

[Clinical pathology and treatment of upper urinary tract urothelial tumors--a 10-year review (author's transl)].

Fifty-four patients with upper urinary tract urothelial tumors were treated from 1966 to 1975 at the Urologische Klinik und Poliklinik der Universität Erlangen-Nürnberg. The neoplasms were multicentric in 13 patients (24%). All of the tumors were classified according to the WHO classification and the recommendation of the UICC. The correlations between histologic grade of malignancy and local spread are statistically significant. Lymph node metastases were found primarily with grade 3 tumors and infiltration into outer muscle layer. Nephroureterectomy with radical lymphadenomectomy via transabdominal approach and frozen-section histology of marginal nodes are recommended. The overall 5-year survival rate was 65% (+/- 16%) in all patients with malignant disease.

[Agranulocytosis after antiarrhythmic therapy with ajmalin (author's transl)].

A case of agranulocytosis (min. leucocyte count 980/mm3) after 4 weeks of treatment with an ajmalin derivate is reported. Recovery could be achieved by taking away the antiarrhythmic drug. It appears that these agranulocytoses only happen after a dosage of more than 300 mg/die, and that they have an intermediate position between allergic and toxic agranulocytosis (phenothiazine type).

[Treatment of primary chronic fissure-in-ano by anal dilatation versus sphincterotomy (author's transl)].

A double-blind, randomized, controlled trial in the treatment of primary chronic fissure-in-ano by digital stretching of the anal canal or by a modified lateral subcutaneous sphincterotomy is reported. Whereas there was no difference in the postoperative course of both groups, by checking 65 of 66 patients 6 months postoperatively, we found significantly more lacks of continence after stretching. There was no difference regarding recurrence rate. Pressure monitoring of the anal canal showed significantly higher values at rest in both groups before operation. No correlation between results of monitoring anal pressure and defects of anal continence has been seen. These results point out that in treatment of primary chronic fissure-in-ano the lateral subcutaneous sphincterotomy is superior to anal dilatation regarding postoperative defects of anal continence. The recurrence rate in the present study does not force to prefer one of these two methods. Manometric investigations show that the resting pressure in anal canal seems to be elevated in primary chronic fissure-in-ano.

[Refractory anemia with hypercellular bone marrow and preleukemia (author's transl)].

In refractory anemia with hypercellular bone marrow and in preleukemia the primary cellular defect probably is located at the level of the morphologically not definable stem cell. This defect causes reduced proliferative activity and ineffectivity of the morphologically definable stem cell and consequently is accompanied by peripheral cytopenia. The disarrangement of cellular metabolism first of all manifests itself in morphological anomalies of bone marrow hemopoietic cells. Using 17 own observations and in reference to previous literature the manifold manifestations of this disturbance are described.

[The therapeutic management of consumption coagulopathy in progressive liver cirrhosis: low-dosage heparin therapy (author's transl)].

The high incidence of consumption coagulopathy in active liver cirrhosis prompted us to introduce low-dosage heparin therapy (LDHT) in the management of this condition. An investigation was carried out on 109 patients with clinical and biochemical evidence of progressive liver cirrhosis, which was designed to evaluate whether in addition to basic LDHT, the administration of either vitamin K1, human fibrinogen or partial prothrombin complex (Prothromplex 500) enhanced the results obtained with LDHT alone. The normotest, PTT, thrombin coagulase activity, fibrinogen and platelet count were determined at regular intervals. A significant increase in fibrinogen and platelet count was obtained within 14 days of LDHT in about 75% of the patients and the consumption coagulopathy was halted. Additional treatment with vitamin K1 did not bring about any further increase in the prothrombin complex. Substitution therapy with factors II, IX, X and fibrinogen combined with LDHT brought the expected results. The results reported in the literature and the aims of, and indications for LDHT are discussed.

[Acute hepatic coma. Experimental study on the predictive value of clinical-chemical findings for the prognosis of acute hepatic coma].

In order to determine the validity of clinical-chemical parameters for the prognosis of hepatic failure, 28 pigs were subjected to liver ischemia for 40--160 minutes duration. The following parameters were studied: GOT, GPT, gamma-GT, LAP, LDH, GlDH, AP and isoenzymes, total bilirubin, potassium, sodium and chloride. In a statistical comparison in the surviving animals, an unexplainable increase in GlDH activity was observed. In the other clinical-chemical parameters none was seen to be of use for the prognosis for either life or death in acute hepatic failure.

Myelopoietic cell proliferation in granulocytopenia of refractory anaemia and preleukemia states with hyper-plastic bone marrow.

Myelopoiesis in healthy subjects and in 8 cases of refractory anaemia or preleukemia states was studied by the combined cytophotometric-autoradirgraphic method. Granulocytopenia was present in 7 of 8 cases. The results can be summarized as follows: 1. Mature myelocytes of healthy persons are a mainly differentiating cell population with a low labelling index (3-10%) and a high incidence in G1-Phase. 2. The proliferating immature granulopoietic cells of the patients with neutropenia showed a reduced labelling index with 3H-thymidine as a result of an arrest of these cells in G1 (6 of 8 cases). 3. A similar result was obtained in the study of the hyperplastic myelopoiesis of an other patient with preleukemia and normal granulocyte count in peripheral blood, with the disturbance of cell proliferation preceding the symptom neutropenia. 4. Finally in only one case an increased frequency of myelopoietic cells in G2-phase was found suggesting an arrest of cell proliferation in the premitotic phase.

Proliferative activity of undifferentiated cells (blast cells) in preleukaemia.

Undifferentiated cells (blast cells) in preleukaemic states show labelling indices comparable to those of leukaemic blast cells. Since this cell population can remain constant over months or even years, a steady state must exist between the production of blast cells and cell death. It is more likely, however, that in preleukaemic conditions, blast cells may differentiate to morphologic 'normal' haemopoietic cells. The fact that the latter show an impaired cell proliferation supports the view that they derive from an affected stem cell compartment.

A cross-national epidemiological study of mania.

An epidemiological study of the first admission rate for mania was carried out in London and Aarhus. The case registers in these two centres were used to conduct a retrospective study of case notes covering several years, and a screening procedure was used for the prospective collection of new cases over the course of one year. The annual incidence of mania was found to be virtually identical in both centres. The retrospective study gave a figure of 2-6 per 100,000 population in both Aarhus and London. But the London sample was found to contain 45 per cent of immigrants in contrast to the Aarhus sample in which only a negligible proportion were born outside Denmark. Male West Indians, in particular, were over-represented in the London sample.
