Patient handling of a multidose dry powder inhalation device for albuterol.

BACKGROUND: Studies have suggested that novel inhaling devices may be misused by patients and medical personnel in the clinical setting soon after their introduction. to assess the adequacy of patient handling of a newer multidose dry powder albuterol inhaler, we conducted a comprehensive survey of use in primary and respiratory practices. METHODS: Patients 6 years of age and older and requiring inhaled beta 2-agonist for the treatment of stable obstructive airways disease were recruited from the practices of primary care and specialist physicians. After instruction from their physician or clinic staff in the use of a multidose dry powder albuterol inhalation device (Diskhaler), patients used the device in unblinded fashion for 2 weeks instead of their usual beta 2-agonist. After 2 weeks, patient use of the device was assessed at a return clinic visit. RESULTS: A total of 4,529 patients with reversible airways obstruction participated in a 2-week assessment of the device and compared it with their previous beta 2-agonist delivery system. Significantly more patients preferred the dry powder device (54 percent) over their previous device (29 percent) (p < 0.001), with 17 percent expressing no preference. After instruction, 98.5 percent of patients could demonstrate adequate use at the initial visit. Performance difficulties at the initial visit resulting in exclusion from the trial were infrequent but more common in the elderly (3 percent vs 0.2 percent for all other age groups combined, p < 0.001). Despite adequate use at the initial visit, at the conclusion of the trial, incorrect use of the device was noted in 10.2 percent of the elderly vs 3.2 percent of all other age groups combined (p < 0.001). Albuterol delivered by Diskhaler was well tolerated in all patient groups. CONCLUSIONS: A majority of patients are able to use a multidose dry powder albuterol inhaler device in the clinical setting, and prefer the device to previously used beta 2-agonist inhalation devices. However, follow-up shows that problems with device handling may become apparent in as little as 2 weeks after initial instruction, thereby mandating the need for periodic follow-up and reinstruction.

An evaluation of a school-based suicide awareness intervention.

This study employed a self-report questionnaire in a Solomon four-groups design to assess the efficacy of suicide intervention classes in achieving their instructional objectives. Because adolescents are often the first to know of a peer's suicidal thoughts or plans, the goal of the classes was to increase the likelihood that students who come into contact with potentially suicidal peers can more readily identify them and will be consistently inclined to take responsible action on their behalf. Students who participated in the classes as compared to controls showed significant gains in relevant knowledge about suicidal peers and significantly more positive attitudes toward help seeking and intervening with troubled peers. Results of this study will be used to strengthen components of the lessons aimed at enhancing the likelihood of performance of responsible interventions.

Assessment of three different methods of tissue dilation for ceramometal restoration.

Tissue dilation is one of the key factors in obtaining accurate duplication of subgingival finishing lines and hence excellent marginal adaptation of the restoration. Elastic impression materials do not displace the tissue. Therefore, the tissue must be displaced away from the prepared teeth or a small ribbon of the free gingiva must be removed to expose the finishing line before making the impression.

Stabilization of standard platelet concentrates and minimization of the platelet storage lesion by a prostacyclin analogue.

Platelet concentrates were pretreated with a stable synthetic prostacyclin analogue (Iloprost) at two different concentrations before the second centrifugation step (pelleting step) of preparation. This resulted in loss of platelet sensitivity to aggregating agents. To mimic the situation after transfusion and to assess the reversibility of platelet inhibition, platelets were washed during and after storage and resuspended in fresh-frozen autologous plasma. The Iloprost-treated and washed platelets exhibited an increased sensitivity to the aggregating agents, compared with the control platelets (p less than 0.01). Post-storage recovery of the synergistic aggregation was more than 80% of prestorage aggregation. Beta-thromboglobulin (beta TG) release and thromboxane B2 (TXB2) formation were significantly inhibited in Iloprost-treated platelets (p less than 0.01). After the second centrifugation step, beta TG release was 0.7% +/- 0.3%, compared with 2.7% +/- 0.9% for the controls. TXB2 was 99 +/- 91 pg/ml, compared with 495 +/- 356 pg/ml for the controls. Platelet morphology and ultrastructure were well preserved during 5-day storage. In addition, Iloprost exerted a cytoprotective effect, as evidenced by the significant reduction in lactate dehydrogenase leakage. Post-storage LDH was 378 +/- 159 and 415 +/- 239 U/l respectively by the two Iloprost concentrations, compared with 1180 +/- 937 U/l for the control platelets. The inhibitory and cytoprotective effects of Iloprost were sustained throughout storage, in contrast to the effect of PGE1 (Prostin) which was limited to the early phase of storage.

Incrimination of Anopheles aconitus Donitz as a vector of epidemic malaria in Bangladesh.

In 1989-91, post-monsoon epidemics of vivax malaria occurred in the central flood plain near Dhaka. Anopheles philippinensis, the usual vector in the paddy field habitat, was not present, but 1.4% of parous An. aconitus were infective. This is only the second time An. aconitus has been incriminated as a vector in Bangladesh. We speculate that the surprising increase in lowland malaria may have been caused by environmental change that favored the survival of An. aconitus.

Adolescents' experience with and response to suicidal peers.

The present study investigated a sample of 325 suburban high school students' knowledge of suicidal peers, whether they had ever talked to a suicidal peer, and, if so, what they actually did in that situation. Sixty-eight percent of the females and 42.5% of the males reported knowing a teen who had committed or attempted suicide. Ninety-seven students reported having talked to a peer who was definitely considering suicide; of these, 63% talked to their peer about his or her concerns, 24.7% told an adult, and 12% did nothing in response to the encounter. Ninth graders were significantly more likely to do nothing as compared to eleventh graders. A mixed pattern of results was found as to the relationship of the response of youth to suicidal peers and their general experience with suicidal peers. The results confirm the importance of adolescents themselves for the prevention of youth suicide, and the need to convince adolescents to report at-risk peers to an adult.

Visceral leishmaniasis and its control in Bangladesh.

Visceral leishmaniasis, which is also known as kala-azar, reappeared in Bangladesh during the 1980s, approximately 7-8 years after large-scale use of DDT had been abandoned by the malaria eradication programme in the country. Pabna, Mymensingh and Rajshahi were the regions most affected with kala-azar. The article presents a historical review and information about the present status of leishmaniasis in Bangladesh together with control strategies and a proposed plan of operation.

Thrombotic thrombocytopenic purpura and haemolytic uraemic syndrome in three siblings.

Two brothers and one sister had three variants of thrombotic thrombocytopenic purpura and haemolytic uraemic syndrome (the 'TTP-HUS' complex). The sister had a chronic fatal variant of thrombotic thrombocytopenic purpura with severe neurological manifestation. One brother had a chronic relapsing disease but the kidneys were not affected, and the other brother had haemolytic uraemic syndrome. This occurrence in one family supports the hypothesis that haemolytic uraemic syndrome and thrombotic thrombocytopenic purpura are actually two variants of the same disease.

Infection with hepatitis A and B viruses in French volunteers working in tropical Africa.

The risks of developing hepatitis A and B virus infections were studied in 233 French volunteers working for 18 to 35 months in the field in West and Central Africa. During their stay in Africa, the seroconversion rates to antibody to hepatitis A virus of the immunoglobulin G (IgG) class and hepatitis B marker(s) were 48.1% and 10.5%, respectively; 21.5% of the volunteers developed jaundice, and 78.0% of the jaundice cases were associated with hepatitis A seroconversion, 14.0% with hepatitis B seroconversion, 4.0% with seroconversion to both, and 4.0% without seroconversion. Male health workers were at lower risk than other male occupations for hepatitis A infection. Female health workers were at higher risk than other female occupations for hepatitis A infection. Health workers were at a lower risk of hepatitis B infection, but the significance was borderline. Populations such as these volunteers should receive immunoprophylaxis against hepatitis A and B infections.
