Treatment of uncomplicated cases of rhegmatogenous retinal detachment with an expanding gas bubble.

We reviewed 55 consecutive cases of rhegmatogenous retinal detachment treated with pneumatic retinopexy as a primary procedure and followed up for a minimum of 1 year. 40 eyes were phakic, 11 aphakic and 4 pseudophakic. 0.6 ml of sulfur hexafluoride were injected into 42 eyes, while 0.3 ml of perfluoropropane were introduced into 13. Transconjunctival cryotherapy was performed in 51 eyes, while light coagulation was used as retinopexy after the retina was reattached in the remaining 4 cases. Of the 55 eyes undergoing treatment, 46 (83.6%) retinas were reattached with one operation. The benefits, complications and disadvantages of the procedure are reported.

[A comparative evaluation between streptokinase and r-TPA in limiting the myocardial damage during an acute myocardial infarct. An echocardiographic study].

Once the experience of GISSI I was concluded, the as yet unresolved problem of the choice of one fibrinolytic agent in preference to others emerged. In the present study, the authors used echocardiography in order to compare streptokinase with r-TPA in order to limit myocardial damage in acute myocardial infarction in 33 patients, average age 64.6 years. No statistically significant difference between the two drugs was observed as far as left ventricular contractility and extent of the infarcted region was concerned. This was in accordance with the findings of the Italian PAIMS study.

[Prevalence of anti-HIV-2 antibodies in a population at risk attending the drug-dependence center of the Rho hospital].

We have evaluated the prevalence of anti HIV-2 antibodies in a population of 139 individuals at risk for AIDS, attending the center for drug addiction of the Rho hospital. 79.85% of our patients were positive for anti HIV-1 antibodies. 51 patients (36.69%) were positive for anti HIV-2 antibodies in an immunoenzymatic method, but only 16 patient (11.51%) have been confirmed in Western Blot. We stress, therefore, the necessity of confirming the positivity for anti HIV-2 antibodies with the Western Blotting method. 14.41% of our anti HIV-1 positive patients were also anti HIV-2 positive, but no one showed an isolate positivity for anti HIV-2 antibodies.

Increase of serum lipid-associated sialic acid concentration in dogs with neoplasms.

Serum concentrations of lipid-associated sialic acid (LASA) were determined in sera obtained from 26 apparently healthy dogs (group 1), 56 dogs with nonneoplastic disorders (group 2), and 100 dogs with neoplasms (benign [group 3], malignant [group 4]), using a simple, reliable, rapid spectrophotometric procedure. The LASA concentrations were significantly increased (P less than 0.01) in group 4 dogs (malignant neoplasms) when compared with those in dogs from groups 1, 2, and 3. The sensitivity of the test was between 71.5% and 100%, and specificity was 92.3% for group 1, 67.8% for group 2, and 80.6% for group 3. A preliminary study concerning variations of serum LASA values as a marker after surgical treatment of tumor showed a good correlation between effectiveness of therapy and serum LASA concentrations.

Humoral immunity and lymphocyte subpopulations in patients with dilated cardiomyopathy.

The incidence of serum organ and non-organ specific autoantibodies, the peripheral blood lymphocyte blastogenic response to phytohaemagglutinin, concanavalin A and pokeweek mitogen, and the surface markers of peripheral T and B lymphocytes were studied in 15 patients affected by coronary artery disease and in 21 patients with dilated cardiomyopathy. In the latter group there was a significantly impaired blastogenic response to concanavalin A with respect to both the normal control group and patients with coronary artery disease (P less than 0.01). The percentage of peripheral blood lymphocytes with cytotoxic/suppressor activity was also reduced in patients with dilated cardiomyopathy in comparison to normal subjects and patients with coronary artery disease (P less than 0.01 and P less than 0.05, respectively). These data may reflect an in vivo defect in suppressor cell function in patients with dilated cardiomyopathy.

Depression of lymphocyte reactivity by serum from patients with dilated cardiomyopathy.

We have investigated the presence of serum factors (serum inhibitory factor and rosette inhibitory factor) which inhibit the blastogenic response to phytohemagglutinin and E-rosette function of normal lymphocytes in patients affected with dilated cardiomyopathy. We found them to be present in significantly higher levels with respect to a "control disease" group (P less than 0.05) and normal subjects (P less than 0.02). Furthermore, serum inhibitory factors were significantly correlated to the functional class of cardiac failure as evaluated according to the New York Heart Association functional classification (P less than 0.05). There was no correlation between serum factors and other immunological parameters investigated (serum IgG, IgA, IgM, C3 and C4 concentrations, alpha-2-macroglobulin levels). Serum inhibitory factors may affect lymphocyte subpopulations, accounting for a T-suppressor functional defect correlated with greater severity of the disease.
