[Usefulness of the combination of the dynamic electrocardiogram with esophageal pH measurement in the differential diagnosis of chest pain].

Several diseases of the gastrointestinal tract can cause angina-like chest pain. Differential diagnosis can be extremely difficult, especially when pathological gastroesophageal reflux is present. We present 10 cases, 2 males and 8 females, in which invasive and non invasive cardiological techniques were unable to indicate a clear diagnosis. We have therefore used a new technique which combines dynamic electrocardiography with 24 hours monitoring of esophageal pH, to correlate pain symptom with electrocardiographic changes and/or gastroesophageal reflux. Using this approach we have diagnosed the presence of isolated pathological gastroesophageal reflux in 3 patients, isolated ischemic heart disease in 2 cases, both conditions in 3 patients and no signs of either condition in the remaining 2 cases. We think that this technique is of special value in subjects suffering from both conditions (ischemic heart disease and gastroesophageal reflux). It provides objective documentation of the role of each disease in the genesis of pain which is essential in developing appropriate therapy.

Decreased corneal complications after no-reflux, low-dose 5 fluorouracil subconjunctival injection following trabeculectomy.

In order to reduce corneal complications in 27 glaucomatous eyes that underwent trabeculectomy and postoperative subconjunctival injection of 5-fluorouracil we tried 1) a low-dose administration of 5-fluorouracil and 2) a modified subconjunctival injection technique. The mean total 5-fluorouracil dose was 18.0 +/- 6 mg. A life-table analysis showed a success rate, at 15 months, of 78% (+/- 7 S.E.). Four of the 27 eyes (13%) had corneal complications, limited to punctate keratopathy, with no corneal epithelial defects and/or abrasions. Our study shows that corneal complications decreased when the contact between 5-fluorouracil and the ocular surface was reduced.

Contact sensitization to corticosteroids: increased risk in long term dermatoses.

Patients affected by chronic dermatoses are at high risk for the development of sensitization to corticosteroids. This study was carried out to evaluate contact hypersensitivity to corticosteroids in a selected group of patients affected by longlasting cutaneous dermatoses. Sixty subjects underwent the GIRDCA series. The Italian GIRDCA series we applied had the following substances in addition to the European standard series: imidazolidinyl urea 2% pet, thiomersal 0.1% pet, disperse yellow 31% pet, disperse red 1% pet, 4'4-diaminodiphenylmethane 0.5% pet, ammoniated mercury 1% pet. The patients were tested with our corticosteroids series and some of them also with their own steroid products. Allergic reactions to corticosteroids were observed in 8/60 (13.3%) patients; budesonide was the main sensitizer (7 positives) followed by hydrocortisone-17-butyrate and betamethasone-17-valerate (2 and 1 positives, respectively). One patient also reacted to methylprednisolone aceponate contained in her own cream. 4/8 positives were certainly related to previous drug-exposure. This study showed a very high percentage of sensitization to corticosteroids. In our opinion, this value may be due both to our selection-standards and to the wide employment of a well known sensitizer like budesonide in our country. Although a corticosteroids series like ours seems to be adequate for the detection of sensitized patients, patch tests with individual compounds and/or steroids corresponding to local prescription habits are recommended, especially in unresponsive chronic dermatoses.

Conservative surgical treatment in infertile patients with endometriosis.

In the period 1985-1988 23 infertile patients with pelvic endometriosis underwent conservative surgery with microsurgical technique. 8.7% (only 2 women) of the patients were in 2nd stage, 43.5% in 3rd stage and 47.8% in 4th stage of the American Fertility Society classification (1985). Associated pathology was found in 52.2% of the patients. Surgical techniques and results are presented. We obtained 13 (56.5%) full term pregnancies. The postoperative birth rate was 50% in 2nd stage, 80% in 3rd stage and 36.4% in 4th stage. Associated pathology was present in 69.2% of pregnant patients. Ten women (43.5) did not conceive: 10% were at 2nd, 20% at 3rd and 70% at 4th stage. Associated disease was present in 30% of women that did not become pregnant.

[Prevalence of hyperkinetic ventricular arrhythmias in an ambulatory population of nonprofessional athletes. A study controlled by ambulatory electrocardiography].

Hyperkinetic ventricular arrhythmia being the main cause of electrical destabilization and hence fatal tachy-arrhythmias, we have studied, by Holter test, the prevalence of these arrhythmias in ambulatory non-professional athletes and compared it with a homogeneous group of sedentary subjects in order to evaluate if sports, even if not in athletic competitions, represent a potential risk for sudden death. The results show the prevalence of hyperkinetic ventricular arrhythmias to be similar in the two groups and not related to sports.
