Diagnosis and localization of intraocular foreign bodies by computed tomography.

Computed Tomography (CT) is the only imaging modality whereby bone and anatomical soft tissue structures are clearly defined. Detection of intraocular foreign bodies (FBs) can be easily obtained by scanning in the axial plane. For the precise localization of FBs, information in two planes is necessary; this is usually achieved by adding a CT study in the coronal plane to the axial plane study. When using a superimposed grid with millimeter calibration on a zoom orbital scan, the accurate location can be calculated. This contribution is of the utmost importance in deciding upon the appropriate approach when surgical removal is indicated.

Flurbiprofen pretreatment in argon laser trabeculoplasty for primary open-angle glaucoma.

We examined the influence of topically administered flurbiprofen sodium 0.03%, a potent prostaglandin inhibitor, on the inflammatory response and intraocular pressure following argon laser trabeculoplasty. Our investigation included one eye of each of 118 patients with primary open-angle glaucoma in whom laser trabeculoplasty was indicated. Sixty-three patients received flurbiprofen and 55 received a vehicle during the two-hour period immediately preceding laser therapy and for one week postoperatively. No significant differences in postoperative IOPs were noted between groups. However, when we considered conjunctival erythema at 24 hours or control of inflammation at the termination of the investigation, the eyes receiving flurbiprofen had less inflammation than did the vehicle-treated eyes.

Preliminary study of hydrophilic hydrogel intraocular lens implants in cats.

Soft hydrophilic intraocular lenses (IOLs) made of an aminopolyamide (Duragel) and a hydroxyethylmethylmethacrylate (HEMA) hydrogel polymer were implanted in cats and followed for six weeks. During surgery, a 45-second purposeful contact between the lens surface and the corneal endothelium was made. During the course of the experiment, all implanted eyes maintained clear IOLs and manifested a continuing mild iritis. Scanning electron microscopy of the corneas revealed numerous microvilli but no damage to cell membranes. Scanning electron microscopy of the aminopolyamide IOLs revealed fibroblast growth on the surface. No fibroblast growth was noted on the surface of the HEMA IOLs. These experiments suggest that soft hydrogel IOLs may be better than conventional hydrophobic acrylic IOLs in minimizing endothelial contact damage. However, the observation of corneal microvilli may indicate a mild adverse reaction to the hydrogels, possibly from impurities. Fibroblast growth on the aminopolyamide may also be due to an ocular inflammatory response to the material or impurities. Hydrogel IOLs may be of interest because of their semirigid physical properties and greater safety on contacting the endothelium. However, considerable attention must be given to polymer composition and polymer purification.

The scotopic threshold of the human retina during static physical effort.

The scotopic threshold of 15 subjects was measured with a 1 degree circular white stimulus light placed at 15 degrees below the fixation level during 45 min at rest followed by 20 min constant pedalling on an ergometric bicycle with the head stationary. Loads were adjusted individually so that the effort extended by each subject required 60-75% of his maximum oxygen uptake. Similar results were obtained under both resting and effort conditions, indicating that the dark adaptation threshold remained unaffected by vigorous static effort.

Scotopic threshold of the human retina during dynamic physical effort.

The scotopic threshold of 7 subjects was measured with a 1 degree circular white light located 15 degrees below fixation during 45 min at rest followed by 10 min of running on an exercise treadmill. The slope of the treadmill was adjusted individually so that the effort extended by each subject required 60-75% of his maximum oxygen uptake. Thresholds showed a sudden elevation of about 0.55 nLa (logarithmic scale) at the onset of the exercise period, and remained at this level throughout the following 10 min. It was concluded that the head movements and consequent eye instability imposed by the run were the main cause for the increase in scotopic threshold.

Topical treatment of periocular basal cell epithelioma with solcoderm.

25 patients with 29 histologically confirmed periocular basal cell epitheliomas were treated by topical application of Solcoderm, a solution of organic and inorganic acids and ions. The cure rate of the 29 lesions, which were followed for a mean period of 32 months, was 80%. The overall cure rate following second Solcoderm treatment of the 6 recurrent lesions was 96.7%. The cosmetic and functional results were excellent and there were no serious complications and side effects.

An unusual "morphologic" variant of BF S.

In the course of family studies of haplotypes of the alleles of the sixth chromosome loci HLA-A, C, B, D/DR, BF, C2, C4A, C4B, and glyoxalase I, we encountered an unusual BF variant. Its mobility was similar to BF F but it appeared to have a lesser intensity after straining with Coomassie Blue, and it was demonstrated by crossed immunoelectrophoresis to be present in lower concentration. It was therefore designated BF FQL. This variant was found on the haplotype HLA-A1, B17, DR7, BF*FQL, C2*C, C4A*6, C4B*1, GLO2. All other haplotypes of this type so far identified carry the BF variant BF S. Following activation of serum samples with zymosan, BF was analyzed by both agarose electrophoresis and isoelectric focusing and immunofixation. On both treatments, serum with BF SFQL produced a Ba pattern identical to that of a sample which was BF S. The Bb pattern for F and S are similar but differ from those of the rare variants BF F1 and BF S1. The Bb pattern of BF FQL was, thus, as expected, the same as BF F or BF S. Hence, we conclude that the variant is a mutant from BF S with mobility similar to BF F. The mutation seemed also to have resulted in a lower concentration of product than normal.

Meridional variations of the scotopic sensitivity in the human retina.

The absolute threshold of the dark-adapted retina for a 1 degree circular white light stimulus located at 15 degrees eccentric to the fovea was measured on four meridians. In each of the 11 tested subjects the sensitivity was found to be higher on the 90 degrees suprafoveal vertical meridian, on average by 0.3 log nLa compared to the subfoveal vertical meridian. On the two oblique 45 degrees and 135 degrees meridians of the upper retina the sensitivity was only very slightly lower compared to the 90 degrees meridian.

Corneal abscesses in silicone and soft contact lens wearers.

Fourteen cases of corneal abscesses in wearers of contact lenses were seen in our department during the last 3 years. The hospitalization period varied from a few days to seven weeks. In one case, it was necessary to perform penetrating keratoplasty in order to restore good vision. Cultures taken from the corneal abscesses were positive in six cases. Pseudomonas was found in five cases and Enterobacter in one case. During the three years of the survey, no hard contact lens wearers with corneal abscesses were hospitalized. Wearers of silicone contact lenses comprised 21.4% of the cases with corneal abscess. This is a high percentage considering the fact that these lenses are very rare among contact lens wearers in Israel.

Anterior uveitis as a first sign of primary orbital lymphosarcoma.

A 47-year-old woman had a painless anterior uveitis in her right eye. Later, it appeared to be secondary to a developing extraocular orbital tumor, which also caused marked proptosis. Histopathologic examination of the tumor revealed lymphocytic lymphosarcoma. The tumor regressed rapidly after irradiation and cytological treatment.

Protein glaucoma as a possible mechanism in a case of glaucomatocyclitic crisis and periphlebitis.

A case of glaucomatocyclitis crisis with recurrent episodes of increased I0P, abnormal appearance of the angle structure and periphlebitis was presented. Some of these episodes involving first one eye and later occurring bilaterally were characterized by absence of flare in the anterior chamber. It is suggested that protein leakage following retinal periphlebitis or an inflammatory reaction involving the ciliary body might be the underlying mechanism for the elevated ocular pressure. The abnormal angle structure in our patient favored this protein glaucoma. A permanent increase in I0P observed at the latter stages of the disease indicate that the trabecular meshwork might be damaged irreversibly by protein glaucoma.
