[The blood-flow in hands with Dupuytren's contracture and Raynaud disease: functional and morphological studies (author's transl)].

Concerning the question whether a correlation between the vasospasm and morphological changes in the neuro-myo-arterial glomerular (Hoyer-Grosser-Organ) exists also in Dupuytren disease, a series of 18 patients with Dupuytren and 5 patients with Raynaud disease were studied by finger venous occlusion plethysmography and finger tip biopsies. No typical histological changes were found in Dupuytren's Contracture, particularly none in the 13 cases (70%) which had a latent vasospasm and none in the 5 cases (30%) which had a normal plethysmogram, whereas in all the patients with Raynaud disease the typical morphological changes of wall, lumen and morphometric counts of the "Suquet-Hoyer" canals were found in correspondance to the abnormal plethysmogram. According to these results the vasospasm of Dupytren and Raynaud disease seems to be pathogenetically different.

[The bacterial contamination of epidural catheters with areobe and anaerobe germs in obstetrics (author's transl)].

Following anaesthesia the tips of 105 epidural catheters were investigated for aerobe and anaerobe bacteria. In one case aerobe germs (staphycoccus epidermidis) and in three cases anaerobe germs (propioni-bacteria were isolated. In two further cases aerobic growth was suspected but not confirmed in sub-cultures. The contamination rate was therefore 0.9% aerobe and 4.8% by anaerobe bacteria. Even in the face of a clinically uncomplicated course the presence of bacteria on the catheter tip is considered to be a potential risk for infection. As a preventive measure the skin disinfection procedure was changed and the disinfectant is left for sic minutes instead of three minutes.

Acid lipase: a histochemical and biochemical study using triton X100-naphtyl palmitate micelles.

Hydrolsis of a-naphtyl palmitate dispersed with the detergent Triton X-100 at acid pH was studied by a histochemical diazocoupling technique in both fixed sections and cultures of primate tissues as well as by a biochemical assay employing the same chromogenic substrate. Evidence for the exclusive hydrolysis of this artificial fatty acid ester substrate by acid lipases was gathered from (1) comparison of isoelectric focusing zymograms developed with different substrates, (2) kinetic analysis of enzyme activity in the presence or absence of inhibitors, including a natural substrate of acid lipase, trioleylglycerol, (3) specific localization of marked enzyme activity in certain tissues, and (4) absence of detectable enzyme activity in a case of human acid lipase deficiency (Wolman's disease). Histochemically, acid lipase activity was most readily detected in cells active in the uptake and processing of neutral lipids, i.e., the phagocytes of the reticuloendothelial system, the adrenal cortex and the lipid-storing cells in the athero-sclerotic plaques of arteries.

[Development of a pediatric amino-acid solution for premature and newborn infants following pharmacokinetic principles].

The blood level kinetics of an amino-acid solution following short-term infusions was measured in 25 premature and 5 hypotrophic newborns. From the calculated transfer rates it was possible to compose an amino-acid solution capable of avoiding imbalances. Clinical tests showed that the solution was tolerated well. Balance studies in 10 premature infants demonstrated a positive nitrogen balance from the first day of life on with a 55% retention of administered amino-acid nitrogen.

[Community psychiatry. An empirical study].

Aim of the present study was the analysis of the situation of fulltime and partime stationary psychiatric services, as well as clinic-administered out-patient services for a geographically defined population group in a large city (Hannover) over a period of serveral years. For this purpose an analysis was made of the clinical, social and demographic characteristics of all patients from a particular part of the city (sector), which had been admitted for treatment into the psychiatric hospitals of Wunstorf, Ilten, the municipal clinic in Hannover-Langenhagen and the psychiatric department of the Medical University of Hannover.

Chemotherapy of disseminated breast cancer. Current status and prospects.

Chemotherapy once relegated to end stage patients has markedly improved with the use of combinations. Response rates with single agents have improved from 15 to 35%, to 50 to 70%, using combinations with an increase in complete response rates to about 25%. A series of four studies completed by the Eastern Cooperative Oncology Group over the past eight years typifies the improvement in response rates achieved by combinations as compared to single agents. Survival gain can be demonstrated for responders vs non-responders; however with current combinations, there is an apparent plateau in response rates (55 to 60%), durations of response (eight months) and survival for responders (18-22 months) as compared to survival of non-responders (six to eight months). Further improvement in response rates may occur by searching for new agents, combining hormonal and immunostimulation with chemotherapy or by sequencing non-crossresistant combinations. However, since most patients with breast cancer present with local or regional disease but go on to die of disseminated cancer, major improvements in survival are most likely to occur by treating this neoplasm as a systemic disease through cobmining effective local therapy with systemic treatments.

Ketogenic response to medium-chain triglyceride load in the rat.

We studied ketonemia induced in rats by a single oral load of medium-chain triglycerides (MCT) (C8:0 50.5%, C10:0 48.0%, C12:0 1.0%). Medium-chain fatty acids, rather than being incorporated into the lipids synthesized by the liver, are oxidized there, with high production of ketone bodies. Severe and long-lasting hyperketonemia developed rapidly. With increased MCT loads, ketonemia also increased, although not linearly. The level of the hyperketonemia seemed equal in the two sexes. Ingestion of MCT by fasting rats caused an additional rise in ketonemia. Long-chain triglycerides were not ketogenic, since their constituent fatty acids are incorporated into lipids and are thus less subject to oxidation. Lipids induce less severe ketonemia in genetically obese rats than in normal-weight rats.

Macroglobulinemia Waldenström and motor neuron syndrome.

One patient suffering from macroglobulinemia Waldenström developed a neurological disease which may be a previously unrecognized paramalignant phenomenon in this disorder. The clinical symptoms and signs indicate a motor neuron syndrome and autopsy revealed degeneration of ventral and lateral funicles in the spinal cord, loss of ventral motor neurons, degeneration of ventral nerve roots and muscular atrophy. The rather low incidence of macroglobulinemia and motor neuron disease suggest some causal relationship rather than a sporadic occurrence of two disorders in the same patient.

[Surgical treatment of malignant skin epitheliomas. Principles, methods, results].

From personal experience in the treatment of 655 patients with malignant epitheliomas of the skin, treated from 1966 to 1973 at the Department of Plastic and Reconstructive Surgery at the University Hospital, Innsbruck, the basic principles, methods, and results of treatment are reported. An adequate resection and a primary reconstruction is the method of choice. The clinical data and follow-up observations are analyzed and special types of treatment and indications are discussed. With it the 5-year-cure-rate of basal-cell carcinomas was 92%, of squamous-cell carcinomas 83%.

[Indications and experiences in saving of the lower leg following injuries and tumors].

In the primary treatment of skin and soft tissue defects of the lower leg, after injuries or tumor excision, split skin grafting is the method of choice. But in respect to the late results, frequently additional plastic reconstructive measures, preferrably skin flap procedures, must be considered. Follow-up over decades and if necessary, skin transplantation, are mandatory to prevent interference of growth and function of the leg, as well as malignant degeneration of the scars.
