Acral (volar-subungual) melanoma.

Acral melanoma occurs in the volar surface of the hands, feet, fingers, toes and subungual sites. Recently it has been recognized as a distinct entity with characteristic clinical and pathological features. Of our 340 patients with malignant melanoma, 24 (7 per cent) had acral melanoma. Sixteen were in the plantar skin, two in the palms and six in the nailbed. The delay in diagnosis was 6 months to 5 years and most of the patients presented with large neglected tumours. Fourteen lesions had histological features consistent with acral lentiginous melanoma - a unique pattern to this area. Fourteen patients were in clinical stage I at diagnosis, seven in stage II and three had distant metastases (stage III). The treatment was mainly surgical. Palmar-plantar lesions were widely excised. One patient underwent below-knee amputation. Lesions of the digits were treated by amputation of the affected toe. Fourteen of the patients underwent either prophylactic or therapeutic lymph node dissection. In 9 of them, regional metastases were found. In patients with advanced disease chemotherapy was added. Three patients had isolated limb perfusion. Fourteen patients died of metastatic disease within 1-5 years. Four are alive but have metastatic spread. Six patients are disease-free, one to 4.5 years following diagnosis.

Effect of (+)-cyanidanol-3 on chronic active hepatitis: a double blind controlled trial.

Forty patients with biopsy proven chronic active hepatitis were studied, 22 received (+)-cyanidanol-3 in a dose of 3 g daily and 18 placebo. Side effects related to cyanidanol were fever (four patients), haemolysis (one patient) and urticaria (one patient). All side effects subsided on discontinuation of the medication. Cyanidanol had an effect no better than placebo on symptoms, laboratory tests, and histological findings on liver biopsy.

Granulosa-theca cell tumor in a one-year-old infant.

A case is presented of a 12-month-old female infant, with sexual precocity due to a Granulosa-Theca cell tumor which presented with vaginal bleeding, breast development and public hair growth. The 17 beta-estradiol levels prior to operation were 300 pg/ml. Levels of gonadotropins were undetectable. Postoperative estradiol levels were measured and found to be 10 pg/ml. This tumor is very rate in infants, and there have been only four previously reported cases in children under 2 years of age.

Life-threatening clavicular osteomyelitis in two debilitated patients.

Two cases are presented of immunologically compromised patients who developed acute pyogenic clavicular osteomyelitis from direct spread of a contiguous focus. Each patient's condition declined despite antibiotic therapy drainage procedures until survival was in doubt. Both showed striking reversal of their conditions and recovery when an aggressive surgical debridement (total claviculectomy) was performed. Acute pyogenic osteomyelitis of the clavicle is uncommon and rarely reported. The special implications in our patients of this disease of the adapted therapy are discussed.

Unusual presentation of carcinoma of the vermiform appendix: a report of two cases.

Two unusual presentations of perforated mucoid adenocarcinoma of the appendix are described. One patient presented with a large periappendicular abscess that extended into the gluteal region, the other with a retroperitoneal abscess and abscesses in the right flank and groin. Recognition of the source of the abscess may be difficult but is essential for the prevention and elimination of the sepsis which may be life-threatening. The presence of mucin in the drained pus is highly suggestive for perforated bowel carcinoma. A high index of suspicion and the performance of prompt diagnostic procedures may bring early surgical treatment and better results.

Hepatic granuloma in polymyalgia rheumatica.

Hepatic granuloma occurs rarely in the polymyalgia rheumatica syndrome. A case was presented of a 62-year-old woman with polymyalgia rheumatica, elevated concentrations of hepatic enzymes, and granuloma of the liver. Low doses of corticosteroids produced a marked improvement in the patient's condition and a return of liver function tests to normal. This is apparently the first case of polymyalgia rheumatica associated with liver granuloma that has been reported in Israel.

[Sarcomatoid carcinoma of the kidney].

Sarcomatoid renal carcinoma is an uncommon renal malignancy with an aggressive biological behaviour and very poor survival rate. We treated lately 4 patients, three of which died shortly after the operation. The case reports are presented and the histology and the clinical implications are discussed.

Endoscopic removal of a giant duodenal polyp.

A 50-year-old female with a giant duodenal polyp is described. An uneventful endoscopic removal of the polyp was performed. An aggressive endoscopic approach to such tumors seems to be justified regardless of their size.

Endometriosis of the umbilicus.

Endometriosis is a common phenomenon. Most cases reported are pelvic endometriosis which cause typical symptoms. Cutaneous endometriosis is rare, usually appearing in surgical scars or following surgical procedures. Umbilical endometriosis can occur after surgery but is generally spontaneous. Six cases of spontaneous umbilical endometriosis are presented.

Familial juvenile colonic polyposis with associated colon cancer.

A mother and son were found to have multiple juvenile polyps of the colon; later a metastatic adenocarcinoma of the large bowel developed in the mother. In both patients, histologic examination of the polyps did not reveal adenomas, but some of the juvenile polyps contained adenomatous-like elements. Because of the mother's history of colon cancer and the adenomatous features in some of the son's juvenile polyps, he underwent an elective subtotal colectomy. This family may represent a phenotype variation with features of both the juvenile and adenomatous polyposis syndromes and the presence of dysplasia of the glandular epithelium could be a marker of potential malignancy in patients with juvenile polyposis coli.

Acute complications of metastatic melanoma to the gastrointestinal tract.

The gastrointestinal tract is a favoured site for metastatic melanoma. Complications such as obstruction, bleeding or perforation should be alleviated by surgery in order to improve the patient's condition for further systemic therapy. Six patients with melanoma involving the gastrointestinal tract are presented: 3 were operated upon for metastatic lesions in the small bowel causing intussusception, 2 patients were treated for perforation of the small bowel and 1 patient with massive bleeding from metastatic melanoma in the stomach underwent gastrectomy. Two of the patients are alive 6 months and 4 years respectively after these operations. The other 4 survived between 6 months and 2 years.

Multisystem presentation of eosinophilic fasciitis.

A 20-year-old man experienced the onset of progressive scleroderma-like skin changes with eosinophilia and hypergammaglobulinaemia after strenuous military activity. A biopsy showed faciitis. Concomitant splenomegaly, polysynovitis, restrictive lung function, myositis, decrease hepatic clotting factors and proteinuria were documented. He responded well to corticosteroids, relapse upon their discontinuation and had a complete remission of both skin and systemic features while on penicillamine and azathioprine. He is new in good health and has received no medication for the past two years.

Endoscopic removal of colonic lipoma.

A 71-year-old female with symptomatic colonic lipoma is described. An uneventful colonoscopic removal of the lipoma was performed. Colonoscopic removal of symptomatic pedunculated submucosal colonic lipoma may be recommended.

Leukaemic infiltration of the prostate.

A biopsy of the prostate should be done in lymphoma/leukaemic patients suffering from urinary obstruction. Patients with a lymphomatous infiltration of the prostate should be irradiated and not subjected to surgery. Two such cases successfully irradiated are described and the present literature is reviewed.

Retrograde urography with a new antiseptic contrast contrast material. An experimental study.

A new antiseptic contrast material, composed of sodium diatrizoate-meglumine diatrizoate and PVP-iodine solution, was prepared and tested for retrograde cystography. It was tested in vitro and in dogs, showed satisfactory radiological features and rapid wide bactericidal action without detectable side effects. Septic complications following radiological demonstration of the urinary tract may be reduced.
