[Clinical assessment of body hair growth in Japanese women. The relationship between a grade of hirsutism and the menstrual status].

Three hundred and sixty-nine volunteer women and 308 outpatients were studied to clarify the criteria of hirsutism and the relationship between a grade of hirsutism and menstrual status in Japanese women. The body hair growth was assessed according to the modified score system described by Ferriman and Gallway. The menstrual status was classified into six groups based on basal body temperature. The hair growth score calculated in volunteers tended to be lower than that of western women in most sites. Therefore one criterion of hirsutism in Japanese women should be a score of more than 6 points in our system. There was a close correlation between body hair growth and menstrual status. As a result, it can be considered that the systematic assessment of body hair growth is useful in diagnosing and treating some irregular menstrual patients.

Selective enlargement of the third ventricle found in chronic schizophrenia.

Selective enlargement of the third ventricle was observed when the area and the VBR (ventricle-brain ratio) were measured in 37 chronic schizophrenics (DSM-III criteria) as compared with 37 controls. However, the width or the width to IS (maximum inner skull width) ratio was not significantly different between the two groups. There was no correlation between any measure of the third ventricle and any of the clinical variables. Five patients had VBRs of the third ventricle that were larger than the mean + 2 SD of the control, but there was no significant difference between these five schizophrenics and the others in any of the clinical variables.

Chemistry and inhibitory activity of long chain fatty acid oxidation of emeriamine and its analogues.

Emericedins A, B, and C, new betaines having inhibitory activity of long chain fatty acid oxidation, were isolated from the culture broth of Emericella quadrilineata IFO 5859. Their structures were determined by spectroscopic analyses as (R)-3-(acylamino)-4-(trimethylammonio)butyrate (acyl: A, acetyl; B, propionyl; C, n-butyryl). Structural confirmation and assignment of absolute configuration were made by chemical synthesis from L-asparagine. Deacylation of emericedin gave a potent derivative, (R)-3-amino-4-(trimethylammonio)butyrate, designated as emeriamine. In order to study the structure-activity relations, various analogues of emeriamine, including a stereoisomer, were prepared. Among them, N-palmitoyl and N-myristoyl derivatives showed much stronger inhibition of fatty acid oxidation than emeriamine.

Carpal tunnel syndrome caused by aberrant lumbrical muscles associated with cystic degeneration of the tenosynovium: a case report.

A 7-year-old boy who suffered from carpal tunnel syndrome caused by aberrant lumbrical muscles associated with cystic degeneration of the tenosynovium of the left hand is described. Judging from the literature studied, this seems to have a very rare origin. Symptoms subsided, and no recurrence has been noticed after operative removal of these structures.

Role of sex-hormone-binding globulin and free sex steroid hormones in hyperandrogenic anovulation.

To clarify the mechanism of hyperandrogenic anovulation, the binding capacity of sex-hormone-binding globulin (SHBG-BC), the levels of free steroid hormones, and the levels of basal serum testosterone (T), estradiol (E2), estrone (E1) and gonadotropins were determined in 42 hyperandrogenic anovulatory women. The mean levels of basal T (0.74 +/- 0.04 ng/ml, p less than 0.001), free T (3.07 +/- 0.51 ng/dl, p less than 0.01) and free E2 (2.26 +/- 0.27 pg/ml, p less than 0.05), basal luteinizing hormone (LH) (33.4 +/- 2.7 mIU/ml, p less than 0.001), responsiveness of LH (180.7 +/- 21.5 mIU/ml, p less than 0.001) after luteinizing hormone releasing hormone (LHRH) administration, and the basal LH/basal follicle stimulating hormone (FSH) ratio (3.42 +/- 0.25, p less than 0.001) in the patients were significantly higher, the SHBG-BC level (1.76 +/- 0.33 micrograms DHT bound/dl, p less than 0.05) significantly reduced, and basal levels of E2 (38.0 +/- 3.2 pg/ml) and E1 (110 +/- 13 pg/ml) unchanged compared to the controls. These findings suggest that increased T in hyperandrogenic anovulatory women results in decreased SHBG-BC and increased free T and E2 which might be the cause of inappropriate gonadotropin secretion followed by chronic anovulation.

[Urinary trehalase activity during normal pregnancy: tubular dysfunctions].

By measuring urinary trehalase which is localized in renal brush borders, we examined whether tubular dysfunction occurred or not during and after normal pregnancy. Urinary trehalase activity in 1st trimester, 2nd trimester, and 3rd trimester was increased significantly as compared with the controls (=non-pregnant women) (p less than 0.01). After delivery, urinary trehalase activity in puerperal 5th day was decreased to the level of the 1st trimester, and the activity of puerperal on the 30th day was in the range of the control value. Urinary beta 2-microglobulin (beta 2-MG) in the 1st trimester and 2nd trimester was not increased significantly as compared with the controls. Urinary beta 2-MG in the 3rd trimester, however, was significantly higher than that in the controls. After delivery, the value for beta 2-MG in puerperal on the 5th day was not decreased significantly as compared with the 3rd trimester, and in puerperal on the 30th day, urinary beta 2-MG was decreased to the control value. Urinary N-acetyl-beta-D-glucosaminidase (NAG) in the 3rd trimester was significantly higher than that in the controls (p less than 0.01) and in the 1st trimester (p less than 0.02). After delivery, a significant decrease was observed in urinary NAG between 5th puerperal day and the 3rd trimester. Moreover, urinary NAG on the 30th puerperal day was the level of the controls. On the basis of these results, it is inferred that tubular dysfunction is observed during normal pregnancy.

[Mode of left ventricular diastolic filling in hypertrophic cardiomyopathy as studied by pulsed Doppler echocardiography and multigated blood pool scan].

The clinical significance of the mode of left ventricular (LV) diastolic filling in hypertrophic cardiomyopathy was studied by the LV inflow velocity patterns (LVIF) of pulsed Doppler echocardiography and LV early diastolic filling rates (V2) of radionuclide (RI) angiography. The relationship between the deceleration time (DT) obtained from LVIF and the V2 was evaluated in 34 patients with nonobstructive hypertrophic cardiomyopathy (HCM) and in nine with obstructive hypertrophic cardiomyopathy (HOCM), and the results were compared with those of 10 patients with dilated cardiomyopathy (DCM), of two with restrictive cardiomyopathy (RCM), and of 19 normal subjects. HCM was subdivided into the following groups according to V1-DT relationships: Group 1 with prolonged DT and decreased V1, Group 2 with normal or short DT and normal V1, and Group 3 with normal or short DT and decreased V1. There were significant negative correlations between V2 and DT in Groups 1 and 2 of HCM, normal and HOCM. However, there were significant positive correlations in Group 3 of HCM, DCM and RCM. Nearly all patients in Group 3 had decreased LV ejection fraction and % fractional shortening, distinct B-B' step formation of the mitral valve echogram, and huge A wave of the apexcardiogram. These findings suggested that the LV rapid filling interval gradually became shorter because the LV contraction is decreased as myocardial fibrosis develops in HCM, and that we must pay attention to the diagnosis of such abnormalities, similar to those of DCM or RCM.
