[Imaging diagnosis of head and neck cancer--diagnosis of laryngeal cancer, hypopharyngeal cancer and cancer of the cervical esophagus by CT x-ray].

Fifty-six patients with laryngeal cancer were examined. The rate of correct diagnosis of clinical staging was 64.3%. Whereas the rate of correct diagnosis of staging by CT was markedly improved, 89.3%. In particular, CT facilitated correct diagnosis of T4. Supraglottic T3 cancer is diagnosed on the basis of deep infiltration or fixation of the hemi-larynx. Disappearance of the pre-epiglottic space or ulcer formation was recognized as a CT finding of deep infiltration. Disappearance of the paraglottic space and dislocation of the arytenoid cartilage were used as the standard for CT findings of laryngeal T3 cancer. CT allowed diagnosis to be made in 84.4% of cases which were pathologically determined to be T4. It can be said that CT is a very effective method for staging diagnosis in patients with laryngeal cancer. Fifty-seven patients with hypopharyngeal cancer and cancer of the cervical esophagus were also examined. When 40 post-surgical patients were compared according to pathological classification of T, the rate of correct diagnosis of clinical staging was 77.5% while that of staging diagnosis by CT was 82.5%. The rate of correct diagnosis by postoperative CT was 80.0% compared with the pathological determination of depth of invasion. The diagnosis of depth of invasion by CT on initial examination is thus an effective means for the determination of prognosis.

Conservative surgery of infertile patients with pelvic endometriosis--factors affecting the pregnancy rate and expectancy.

Conservative surgery was used in the treatment of 95 infertile patients with endometriosis who desired to have a child; of these 95 patients, 44 (46.3%) became pregnant after treatment. We studied the postoperative pregnancy rate and the expectancy value for pregnancy and the factors affecting them. The postoperative pregnancy rate was significantly lower in the patients with an infertility period of more than 5 years. As the age of the patients at the time of operation increased, the pregnancy rate decreased; the expectancy value was also significantly lower in patients over 30 years of age. Endometriotic lesions were, in the ovary (77.9%), in the peritoneum (42.1%), and in the uterosacral ligament (42.1%). The rates of pregnancy in patients in Beecham's stages II (43.2%) and III (53.3%) were not statistically significant. The rate was significantly lower in stage IV. Intrapelvic complications were usually myoma of the uterus and cystoma of the ovary. Both pregnancy rate and expectancy value were significantly lower in the patients with pelvic complications. Hormonal therapy combined with conservative surgery gave a higher rate of pregnancy and expectancy value than surgery alone. Thus, the duration of infertility, the age of the patient at the time of operation, intrapelvic complications, and the mode of treatment were key factors affecting the postoperative rate of pregnancy and expectancy.

Spironolactone therapy for hyperandrogenic anovulatory women--clinical and endocrinological study.

Seventeen patients with irregular menstrual periods and androgenic symptoms such as excessive hair growth, acne, and oily skin were treated with spironolactone. The basal levels of serum testosterone (T), dihydrotestosterone (DHT), estradiol (E2), and luteinizing hormone (LH) were significantly higher in the patients than in the controls. After a 4-week treatment with spironolactone, serum T and DHT showed significant decreases. Before treatment, the serum sex hormone binding globulin (SHBG) levels were significantly lower in the patients than in the controls. After initiation of the spironolactone treatment, the SHBG levels tended to increase but not significantly. Spironolactone treatment resulted in the improvement or disappearance of acne in 4 of 5 patients who suffered from this ailment. Improvement of hirsutism was obtained in 2 of 10 patients. Although 4 oligomenorrheic patients and 6 amenorrheic patients showed anovulatory bleeding but did not ovulate, 2 patients in the amenorrheic group ovulated. Two patients complained of tiredness and weakness, and 2 others of polyuria and polydipsia. Both groups showed only temporary symptoms, so treatment was not interrupted. Spironolactone has beneficial effects on ovarian function and few side effects, so it may prove to be an effective anti-androgen, especially for hyperandrogenic anovulatory patients.

Pentylenetetrazol-induced seizures produce an increased release of IR-Met-enkephalin from rat striatum in vitro.

In this work we analyzed the immunoreactive-methionine-enkephalin (IR-Met-enkephalin) levels in several brain regions of rats sacrificed during the tonic extension, induced by acute treatment with pentylenetetrazol (PTZ). The results show an increased of IR-Met-enkephalin content in striatum but not in amygdala, hypothalamus, septum, hippocampus and cortex. To characterize whether this elevation of enkephalin levels in striatum corresponded to the releasable pool, we studied the in vitro efflux of this peptide in striatal slices of rats sacrificed during the seizures, in acute PTZ and in PTZ-kindled rats (kindling group I). In addition, PTZ-kindled rats were analyzed 24 h after the last stimulus (kindling group II). The striatal slices of acute group and kindling group I displayed a significant increase in the evoked release of IR-Met-enkephalin. However, no significant changes occurred from striatal slices of kindling group II animals. In vitro superfusion of GABA (100 microM) produced a approximately equal to 63% decrease in IR-Met-enkephalin released from striatal slices in both saline and acute PTZ-treated rats. Several studies suggest that opioid peptides may be released in the ictal phase of seizure in order to mediate some transient postictal behavior. Our results suggest that of several brain regions tested, only the striatal IR-Met-enkephalin may be released during the ictus to mediate postictal behavior in the acute PTZ treated and in PTZ-kindled rats. This effect may be regulated by the GABA system.

Effects of acetaldehyde and/or ethanol on neutral amino acid transport systems in microvillous brush border membrane vesicles prepared from human placenta.

At 20 mM of acetaldehyde, the activities of three transport systems of L-alanine distinguished by the differences in their cation dependence, namely 1) Na+-specific, 2) Li+-dependent, and 3) Na+-independent systems, were significantly reduced in a similar manner. Only the Li+-dependent system was selectively inhibited at toxic concentrations of acetaldehyde and ethanol.

Emeriamine, an antidiabetic beta-aminobetaine derived from a novel fungal metabolite.

Emeriamine [(R)-3-amino-4-trimethylaminobutyric acid], derived from a novel fungal metabolite "emericedin" [(R)-3-acetylamino-4-trimethylaminobutyric acid], was proved to be a strong and specific inhibitor of carnitine-dependent oxidation of long chain fatty acid (IC50; 3.2 X 10(-6)M) and its main inhibition site was shown to be carnitine palmitoyltransferase I located on the outer-surface of the mitochondrial inner membrane. Emeriamine also showed hypoglycemic and antiketogenic activities in a dose-dependent manner (1 - 10 mg/kg) when administered orally to fasted normal and diabetic animals.

A sensitive fluorometric assay for serum monoamine oxidase with kynuramine as substrate.

The detailed procedure of a new sensitive fluorometric assay for human serum monoamine oxidase (MAO) with kynuramine as substrate is described. The data on its reproducibility, stability, correlation with another method, and serum MAO levels for 150 healthy subjects and 205 patients with various diseases, are presented to support the usefulness of this method. Since our method is much simpler and more sensitive than other methods conventionally used, we can recommend it for routine clinical investigation.

CT scan as a diagnostic tool to detect cancer invasion in the carotid artery.

The presence of cervical lymph node metastasis in head and neck tumors is usually determined by manual neck palpitation. However, this method cannot ascertain whether carotid artery invasion is present or not. Actually, fixed lymph node metastasis means nothing but deep fascia invasion of the metastasis. Contrast CT scan, on the contrary, can present exact information about the carotid artery. We investigate the comparison between CT scan results and surgical results in this paper.

Hazard to endotracheal tubes by CO2 laser beam. Experimental report.

Ignition or combustion of the endotracheal tube during CO2 laser microlaryngo surgery may cause fatal lung disorders. We undertook an experiment on ignition and combustion of endotracheal tubes by CO2 laser beam and came to the following conclusions: (a) the silicon endotracheal tube was nonflammable; (b) the spiral endotracheal tube was also nonflammable; (c) a pulse wave was recommended and 3-second intervals were necessary; (d) four sheets of moist gauze were necessary to prevent cuff trouble, and (e) it was advisable to set the O2 concentration below 25% when using the silicon endotracheal tube.

Isolation and characterization of bulgecins, new bacterial metabolites with bulge-inducing activity.

Bulgecins A, B and C, new bacterial metabolites which induce the formation of bulges by cooperation with beta-lactam antibiotics, were isolated from the culture broth of Pseudomonas mesoacidophila. The three components, separated by column chromatography on QAE-Sephadex A-25, are water-soluble acidic compounds containing a sulfate group in the molecule. Acid hydrolysis showed that D-glucosamine and a new proline derivative are common constituents of the three components. In addition, taurine and beta-alanine are constituents of bulgecins A and B, respectively.

Cyclic therapy resulted in pregnancy in premature ovarian failure.

A successful pregnancy in a patient with premature ovarian failure is described. Increased serum gonadotropin titers, decreased serum estrogen levels, and lack of ovarian follicles on histologic examination confirmed the state of ovarian failure. The patient conceived after several courses of cyclical treatment with estrogen-progestin. A possible relationship between the estrogen treatment and subsequent pregnancy is discussed.

[Stress pulsed Doppler echocardiography in the evaluation of diastolic left ventricular function].

The effect of sublingual nifedipine on left ventricular (LV) function was assessed by analyzing LV inflow velocity patterns using pulsed Doppler echocardiography (PDE) and radioisotope (RI) angiography in 23 patients with hypertrophic cardiomyopathy (HCM), 13 with hypertensive heart disease (HHD) and 24 with ischemic heart disease (IHD). The results were as follows: LV inflow velocity pattern and M-mode echocardiography after the administration of nifedipine (Study-I). The deceleration time (DT) of the diastolic rapid filling wave was shortened after nifedipine in HCM and HHD, but not in IHD. The acceleration time (AT) of the diastolic rapid filling wave and the A/D ratio (amplitude of an atrial contraction wave/amplitude of a diastolic rapid filling wave) were not significantly changed in all groups. Peak negative Vcf was significantly improved after nifedipine in HCM, but unchanged in the other groups. Fractional shortening and peak Vcf did not change in all groups. LV inflow velocity pattern and RI angiography after nifedipine (Study-II). The mean LV rapid filling rate (V1) based on the LV volume curve by multi-gated blood pool scans increased during nifedipine in HCM and HHD, but other diastolic indices, such as the peak filling rate and 1/3 filling fraction, were unchanged. The DT-V1 relationship during nifedipine shifted toward the right and downward in HCM and HHD. In IHD, however, this relationship shifted toward the left and downward in cases with reduced LV function (V1 less than 1.0 EDC/sec), but toward the right and downward in patients whose LV function was maintained (V1 greater than or equal to 1.0 EDC/sec). Nifedipine therapy was associated with an improved LV filling in nearly all cases of HCM and HHD, and in some cases of IHD. PDE and RI angiography are useful for evaluating LV diastolic function during sublingual nifedipine in various heart diseases.

Plastic unification of a double uterus and the outcome of pregnancy.

Habitual abortion and premature delivery are occasionally associated with a bicornuate or septate type of uterus when the abnormality is severe. A considerable improvement of reproductive performance was seen in patients who underwent plastic operation of the uterus. The procedures introduced by Strassman, Jones and Tompkins were used. Although it is still difficult to compare the postoperative reproductive history of these three surgical procedures, the procedure used by Tompkins may be technically more simple and provides for an easier entrance into the endometrial cavities in order to incise the common lower uterine segment. Primary sterility still remains a controversial indication for metroplasty and the postoperative reproductive history for this is poor.

[Genesis of protodiastolic extra heart sound in mitral stenosis: a phono-, apex-, echo- and cineangiographic study].

To clarify the genesis of a protodiastolic extra heart sound (S3') which was occasionally recorded at about the beginning of a diastolic rumble in mitral stenosis (MS), phono-, apex- and echocardiography were performed for 33 patients with MS, and left ventricular (LV) cineangiography was performed for eight of them. The patients were classified as S3'(+) and S3'(-) groups, according to whether they had S3'. Results were as follows: The S3' was synchronous with the rapid filling (RF) wave of the apexcardiogram (ACG). Its amplitude was proportional to the size of the RF wave. The RF wave was significantly sharper in the S3'(+) group as compared with that of the S3'(-) group. The S3' always appeared after onset of dispersion of dots in the velocity pattern of blood flow at the mitral valve orifice according to pulsed Doppler echocardiography. There was no significant difference between the S3'(+) and S3'(-) groups in the region of the mitral valve orifice according to two-dimensional echocardiography, and at the peak rate of change of the LV dimension during diastole as determined by M-mode echocardiography. The peak rate of change of the long-axis dimension of the LV during diastole as determined by cineangiography was significantly greater in the S3'(+) group than in the S3'(-) group. However, there was no significant difference between the two groups regarding the peak rate of change in the short-axis dimension of the LV during diastole as determined by cineangiography. The amplitude of the early diastolic dip of the interventricular septum (IVS) was significantly greater in the S3'(+) group as compared with that of the S3'(-) group. The amplitude of the S3' and the size of the RF wave correlated positively with the amplitude of the early diastolic dip of the IVS in pts with atrial fibrillation. Fractional shortening of the LV ascertained by M-mode echocardiography was significantly greater in the S3'(+) group than in the S3'(-) group. The end-systolic dimension of the LV tended to be less in the former than in the latter group. In conclusion, the S3' in MS was considered to be a third heart sound. Expansion along the long-axis of the LV and its sudden change in early diastole may account for the genesis of the S3', and this expansion may be accentuated by restoring force and active diastolic suction of the LV, and by velocity, direction and spread toward the cardiac apex of the stenotic mitral jet flow.
