Cardiovascular anomalies produced by nimustine hydrochloride in the rat fetus.

The cardiovascular teratogenicity of nimustine hydrochloride (ACNU) was studied in rat fetuses. This drug is a nitrosourea derivative anticancer agent and produces alkylation of DNA. Pregnant Donryu rats were treated with single doses of 10, 11 or 13 mg/kg of the teratogen at various stages during gestation. Examination of the hearts was performed by microdissection after sacrificing the animals on the 20th day of gestation. The highest frequency of cardiovascular anomalies was found in the groups treated on the 8th day of gestation, but there was no difference in the rates induced by the three dosages of ACNU administered. The most common cardiovascular anomalies observed were ventricular septal defect (76.8%) and double outlet right ventricle (10.3%). A considerable number of affected fetuses (37/263) showed complex cardiac anomalies with atrioventricular (AV) malalignment and other AV valve anomalies. These anomalies include: double inlet left ventricle, straddling AV valve, atresia or stenosis of the AV valve, and dysplastic AV valve. ACNU appears to be a useful teratogenic agent for inducing complexes of cardiac anomalies which include AV malalignment.

QS pattern in the right precordial leads of a child with extensive myocardial fibrosis following myocarditis.

A QS pattern in the right precordial leads of the electrocardiogram was noted in a 9-year-old asymptomatic boy. Echocardiography revealed paradoxical movement of the interventricular septum, which showed interstitial myocardial fibrosis in the biopsy specimen and a defective uptake of thallium-201. Extensive myocardial fibrosis was probably a sequela of an attack of myocarditis in infancy.

Environmental mycological studies on the causative agent of summer-type hypersensitivity pneumonitis.

Environmental mycological studies were carried out in 22 homes of patients with summer-type hypersensitivity pneumonitis and in 195 homes of control subjects. In 10 patients' homes, indoor sampling was performed by open-plate culture, house dust culture, and swab culture (group 1), but in the other 12 patients' homes, sampling was only by house dust culture (group 2). We isolated 302 strains of yeasts from the 22 patients' homes and 962 strains of yeasts from the homes of control subjects. The incidence of yeasts, except genera Trichosporon, was not significantly different between homes of patients and control subjects when homes were assessed by three culture methods. T. cutaneum was isolated from seven of 10 patients' homes in group 1, and their colonizing places were revealed by the swab culture method. In group 2, the cells were isolated from four of 12 patients' homes. No T. cutaneum, however, was isolated from the control subjects' homes. Among the isolated yeasts from patients' homes, 23 strains were reactive to the patients' sera at 1:128 or higher in indirect fluorescent antibody titers; 10 yeasts were T. cutaneum, isolated from 10 homes of 14 patients, but the other 13 yeasts were each a different species isolated from 10 different homes. Furthermore, inhalation challenge with the culture-filtrate antigen prepared from T. cutaneum was performed on the nine patients of six homes in group 1 and the two asymptomatic family members. Of the nine patients, six were positive, one was probable, and two patients were negative. Neither of the two asymptomatic family members responded. These results suggest that T. cutaneum is a major causative agent of summer-type hypersensitivity pneumonitis.

Mutagenicity of monochlorodibenzofurans detected in the environment.

Recently, monochlorodibenzofuran, which is formed by the reaction of dibenzofuran with residual chlorine, has been detected in tap water in one region of Japan. The mutagenicities of 1-, 2-, 3-, and 4-chlorodibenzofuran were tested using Salmonella typhimurium TA98 and TA100. 1-Chlorodibenzofuran and 4-chlorodibenzofuran proved to be practically non-mutagenic, while 2-chlorodibenzofuran was weakly mutagenic. Unlike these three isomers, 3-chlorodibenzofuran was markedly mutagenic, and the intensity of its mutagenicity in TA98 was about one-fifth and in TA100 about one-twentieth of that of benzo[a]pyrene.

Endotoxin, cerebral blood flow, amino acids and brain damage in young rabbits.

The effects of bacterial endotoxin (lipopolysaccharide; LPS) on the cerebral blood flow (CBF), amino acid levels and brain histology were studied in young rabbits. The CBF was slightly decreased in the cerebral cortex and markedly decreased in the cerebral white matter at 60 and 120 min after LPS administration. Histological examination revealed only slightly pyknotic neurons around small vessels at 24 hours, and multifocal necrosis in the deep cerebral cortex and white matter at 72 hours. Some amino acids were increased in the plasma and brain regions at 24 hours after LPS administration, most of which were essential amino acids. GABA in the cerebral white matter was decreased at 24 hours. At 72 hours, most non-essential and glucogenic amino acids were decreased. These results suggest that the brain histological changes are related mainly to hypoperfusion and vascular damage in the brain. The amino acid changes may also be related to inappropriate amino acid metabolism associated with brain cell damage.

Potentiating effects of verapamil on cardiovascular teratogenicity of phenobarbital in the chick embryos.

Teratogenic to subteratogenic doses of phenobarbital were administered to young chick embryos together with a calcium-blocking agent, verapamil, at doses where it alone induces no cardiovascular malformations. The cardiovascular teratogenicity of phenobarbital was significantly potentiated by verapamil (5 X 10(-11), 1 X 10(-10) mol). The alteration of calcium movement into the cell and/or depressed cardiac function following impaired calcium transport by verapamil are suggested as modus operandi in the potentiation of phenobarbital-induced cardiovascular malformations.

Current surgical treatment and results for aneurysm of the thoracic aorta in elderly patients.

During the period from January, 1983 to December, 1986, a total of 199 patients with thoracic aortic aneurysm underwent surgical treatment in our Cardiovascular Surgical Service. During this period, criteria for surgical indications were established and general surgical principles and techniques were standardized. As a consequence, surgical results appeared to stabilize during this period. It was clearly established that the surgical result for patients over 65 years was poor with high mortality compared to the younger age group (38.6% vs 7.2%). This tendency was in sharp contrast to that of abdominal aortic aneurysm surgery where low mortality (3%) was obtained regardless of age. Other factors influencing high mortality were as follows: 1) atherosclerosis as an etiological background, 2) aneurysm situated at the aortic arch, 3) Urgency for surgery, 4) pre- and postoperative status of respiratory and renal function. Late follow-up results showed that 25% of patients died, while 66% are in fair condition.

C1q and C3 in bronchoalveolar lavage fluid from patients with summer-type hypersensitivity pneumonitis.

Immune complexes have been thought to participate in the pathogenesis of hypersensitivity pneumonitis, but the role of complement components is not defined. In our study of nine patients with summer-type hypersensitivity pneumonitis (summer-type HP), C1q in bronchoalveolar lavage fluid (BALF) was strikingly increased (mean 3.7, range 0.4 to 10 micrograms/ml). The value of C1q/albumin was several to 20 times greater in BALF than in serum samples from individual patients. In contrast, BALF samples from control subjects (ten patients with sarcoidosis and nine normal subjects) contained an undetectable amount (less than 0.02 micrograms/ml) of C1q. C3 in BALF also increased in the summer-type HP patients. Furthermore, C1q (as well as specific IgG and IgA antibody activities to Trichosporon cutaneum antigen) in BALF correlated with clinical symptoms and diffusing capacity (DCO), while the BAL lymphocytosis or the change of OKT4/OKT8 ratio did not. These findings are indicative of local secretion or concentration mechanism of C1q and C3, supporting the involvement of immune complexes in the respiratory tract of the patients.

Idiopathic isolated pulmonary arteritis with chronic cor pulmonale.

Multiple obstructions of the proximal pulmonary arteries were found in a patient with pulmonary hypertension and chronic cor pulmonale without any systemic arterial lesions. An operation to relieve severe stenosis at the right main pulmonary artery provided great clinical improvement. Pathologic examination of the biopsied specimen revealed a nonspecific panarteritis with giant cells consistent with idiopathic isolated pulmonary arteritis.

Streptococcus acidominimus infections in a human.

We described the first case of pneumonia, pericarditis and meningitis due to Streptococcus acidominimus, who had no underlying disease. The organism is very rarely obtained in clinical bacteriology. The identifying characteristics of this isolate were discussed. There are no reports in which this organism caused such complicated human infection as this case in the English literature.

[A case of bilateral spermatocele].

A case of bilateral spermatocele is reported. A 46-year-old man was admitted to our hospital complaining of bilateral scrotal swelling. We obtained a colorless, opalescent fluid which contained numerous spermatozoa. The fluid volume, pH and gravity obtained from right spermatocele were 85 ml, 6.8 and 1.005, respectively, and those obtained from left side were 40 ml, 6.8 and 1.006, respectively. Spermatocelectomy was done under lumbal anesthesia. Both spematocele were found near the body of the epididymis. The wall of spermatocele had on epithelial lining of cuboidal cells. Twenty two cases of spermatocele reported in Japan since 1951 are reviewed.

[Clinical evaluation of cefpodoxime proxetil in the treatment of skin and soft tissue infections. A double blind comparison of cefpodoxime proxetil and cefaclor].

In order to objectively evaluate the effectiveness, safety and usefulness of the new oral cephem cefpodoxime proxetil (CS-807, CPDX-PR) for the treatment of skin and soft tissue infections, a double-blind comparative study was undertaken using cefaclor (CCL) as the control drug. CPDX-PR and CCL were administered for 7 days at daily doses of 400 mg (divided into 2 portions) and 750 mg (divided into 3 portions), respectively. A total of 243 patients (118 in the CPDX-PR group and 125 in the CCL group) was treated in this study. The effectiveness, safety and usefulness were evaluated in 222 (106 in the CPDX-PR group and 116 in the CCL group), 234 (113 in the CPDX-PR group and 121 in the CCL group) and in 223 patients (107 in the CPDX-PR group and 116 in the CCL group), respectively. There were no differences in patients' backgrounds between the 2 groups, except for the presence or the absence of surgical treatments. The results we obtained are summarized below: 1. In the evaluation of clinical efficacy by the subcommittee, excellent, good, fair and poor efficacy were observed in 36, 43, 17 and 10 patients in the CPDX-PR group, respectively; the efficacy rate was, therefore, calculated to be 74.5%. As for the CCL group, respective results were observed in 50, 39, 17 and 10 patients, indicating an efficacy rate of 76.7%. There was no significant difference between the 2 groups. Improvement rates judged by physicians in charge were 80.2% in the CPDX-PR group and 88.8% in the CCL group. Moreover, no significant difference in diseases or severity were found between the 2 groups. 2. As for the bacteriological efficacy, the 2 groups showed high elimination rates, as 90.1% and 91.6% of the disease causing bacteria were eliminated in the CPDX-PR group and in the CCL group, respectively. Elimination rates in single infections with Staphylococcus aureus were determined to be 85.7% in the CPDX-PR group and 85.0% in the CCL group. 3. Although 6 patients in the CPDX-PR group and 2 patients in the CCL group developed side effects, which were mainly gastrointestinal symptoms, there was no significant difference in the incidence of side effects between the 2 groups. Abnormal laboratory values were found in 5 patients in the CPDX-PR group and 1 patient in the CCL group. 4. Fatal Yersinia enterocolitica septicemia complicated by unique hemosiderosis. A case report.

A fatal case of Yersinia enterocolitica septicemia which was complicated by unique hemosiderosis is reported. On admission, the patient had diabetes and showed unusual hyperferritinemia. Postmortem examination revealed that the liver was studied with abscesses, and Yersinia antigen was expressed in foamy macrophages within these abscesses. Moreover, the cadaver showed generalized hemosiderin deposition, which was mainly observed in the liver and, to lesser degrees, in the pancreas, spleen, lymph nodes, brain, thyroid and kidneys. Since there was no apparent cause of the hyperferritinemia and generalized hemosiderosis, consideration was given to possible primary hemochromatosis. However, no liver or pancreatic fibrosis was demonstrated. Kupffer cells were also loaded with hemosiderin, and therefore it was considered that these cells had lost their ability to phagocytize the microorganism adequately, leading to major liver involvement. Interestingly, hemosiderin deposits in the kidneys were mostly present in glomerular epithelial cells. To our knowledge, this unique presentation has not been previously described in humans.
