[Nutritional assessment of severely retarded children and adults].

The nutritional status was assessed with physical and biochemical parameters on 39 severely retarded children and adults, 17 males and 22 females, ranging from 6 to 40 years of age. In the anthropometric measurements, a majority of the cases showed short stature, light body weight, decreased triceps skinfold thickness and decreased mid-upper arm muscle circumference. The analysis of serum amino acids showed decreases of the sum of essential amino acids, the sum of branched chain amino acids, the ratio of the sum of essential amino acids to the sum of nonessential amino acid, and the ratio of the [methionine + tryptophan] to [valine + lysine + threonine]. The levels of prealbumin and transferrin in the serum were within normal limits. Other biochemical parameters were lower limits, including total protein, albumin, total cholesterol, triglyceride, phospholipid and total lipids in the serum. In addition, the level of fasting blood glucose tended to be low, and levels of serum free fatty acid were markedly different between before and after breakfast. These results suggest the presence of nutritional disturbances in most of severely retarded children and adults.

[A case of chronic myelogenous leukemia complicated with nephrotic syndrome].

An 81-year-old woman was admitted, complained general malaise, and edema on face and lower extremities. In the peripheral blood, leucocytosis (17,220/mm3), microcytic hypochromic anemia (RBC 348 x 10(4)/mm3, Hb 9.6 g/dl, Ht 29.2%), and thrombocytosis (130 x 10(4)/mm3) were present, and many myeloid cells containing of myeloblasts, promyelocytes and so on were observed. Bone marrow aspiration revealed increment of the myeloid series without hiatus leukemia . The Neutrophil Alkaline Phosphatase score and rate was low, and on bone marrow scintigram using indium chloride, liver and extremities were shown. On admission, proteinuria (21.5 g/dl) and hypoalbuminemia (2.5 g/day) were pointed out, and the renal biopsy specimen showed membraneous proliferative glomerulonephritis (MPGN), so we diagnosed this case that chronic myelogenous leukemia (CML) complicated with nephrotic syndrome. At first, she was treated with prednisolone, but proteinuria was not entirely improved, then busulfan was given, myeloid cells in peripheral blood were disappeared and proteinuria was gradually decreased. From this coarse, the causality between CML and nephrotic syndrome was verified.

[Cardiac surgery of tetralogy of Fallot associated with DiGeorge syndrome: a case report].

A case of 7-year-old boy with tetralogy of Fallot associated with DiGeorge syndrome was reported. He had been diagnosed as cardio-facial syndrome with hypoparathyroidism because of hypocalcemia, convulsion, cardiac defect and the characteristic face. But immunologic study showed hypofunction of cellular immunity and cardiac catheterization and angiogram revealed the Tetralogy of Fallot. So we diagnosed as DiGeorge syndrome and performed operation. The thymus was not found and we close the ventricular defect with patch and reconstructed right ventricular outflow tract with transannular patch. We often measured serum ionized calcium level and corrected with calcium gluconate intravenously. Peritoneal dialysis was done due to oliguria and much inotropic support was necessary.

[Pathogenesis of pulmonary involvement in HTLV-I-associated myelopathy (HAM): studies on T-cell function].

It has been shown that there are pulmonary involvements in patients with HTLV-1-associated myelopathy (HAM). Pulmonary lesions found in HAM are characterized by T-lymphocytosis and increased levels of soluble IL-2 receptor in bronchoalveolar lavage fluid. Peripheral blood lymphocytes from HAM patients proliferated spontaneously when cultured in vitro. Lymphocytes proliferated were positive for CD3 and HLA-DR staining. There were CD4+ cells and CD8+ cells in proliferating T-lymphocytes 5 days after cultivation. Culture supernatants of proliferating cells showed an increased soluble IL-2 receptor. These results suggest that activated T-cells play an important role in developing pulmonary lesions in HAM.

[A case of pulmonary alveolar proteinosis accompanied with the elevation of CEA (carcinoembryonic antigen) in bronchoalveolar lavage fluid].

A 44-year-old man was admitted with increasing dyspnea and butterfly shadow on his chest roentgenogram. Both the bronchoalveolar lavage and transbronchial lung biopsy (TBLB) led to a diagnosis of pulmonary alveolar proteinosis. CEA level in his bronchoalveolar lavage fluid (BALF) (216.8 ng/ml) was markedly elevated more than that in his serum (50.6 ng/ml). The localization of CEA on TBLB specimen stained by peroxidase-antiperoxidase technique using anti-CEA antibody was investigated and it was found that some alveolar epithelial cells and the material filling the alveoli were positively stained. Functions of alveolar macrophages in BALF were also studied, that is, adherence to glass, cell spreading, the nitroblue tetrazolium reduction, phagocytosis of immunobeads, and the activity of lysosomal enzymes, and found that all of those functions were lower than that of normal smokers. It was suggested that type II alveolar epithelial cells were those which produced and secreted CEA into the alveoli, as they do surfactant-like material in pulmonary alveolar proteinosis, and that the dysfunction of alveolar macrophages was secondary to the overingestion of the proteinaceous material in the alveoli.

[A case of retroperitoneal abscess accompanying pregnancy].

A 40-year-old pregnant woman, who suffered a continuous episode of fever and anemia since the 26th week of pregnancy, was referred to our clinic for evaluation of her left flank pain and tumor. As retroperitoneal tumor was suspected by retrograde pyelography, sonography and CT, translumbar tumorectomy was performed under general anesthesia. The histological diagnosis was retroperitoneal abscess.

[Human flash visually evoked cortical potentials under different levels of halothane anesthesia].

Flash visually evoked cortical potentials (FVECPs) were recorded in 13 surgical patients under different levels of halothane anesthesia, altering the rate of stimulus frequency from 0.5 Hz to 10 Hz. VECPs were obtained in the awake state under premedication with atropine sulphate, and at end-tidal halothane concentrations of 0.38%, 0.75%, 1.13% and 1.5%. The peak latency of P1 (the major positive component with a latency of about 100 msec) progressively increased from 116.7 +/- 3.3 msec (mean +/- SE) in the awake state to 124.3 +/- 6.3 msec, 136.5 +/- 5.2 msec, 151.5 +/- 5.2 msec and 166.3 +/- 8.7 msec with increasing halothane concentration. Significant reductions in P1 amplitude were observed to 2 Hz, 5 Hz and 10 Hz stimuli, whereas there were no significant changes in amplitude to 0.5 Hz and 1 Hz stimuli. P1 amplitude plotted against stimulus frequency had a maximum at 10 Hz in the awake state. This curve gradually and preferentially attenuated at higher frequencies as halothane concentration increased.

[A case report of aneurysm of the sinus of Valsalva due to infectious endocarditis with ASR].

A case (56 year-old, female) of the aneurysm of sinus of Valsalva due to infectious endocarditis with aortic stenosis and regurgitation was described. The patient was successfully operated upon by patch closure of the orifice of the aneurysm and aortic valve replacement with Medtronic Hall 23A. Post operative course was uneventful and discharged from the hospital at 37 post operative days in good condition.

[Usefulness of serum adenosine deaminase activity in the early diagnosis of Mycoplasma pneumonia].

It is sometimes difficult to clinically diagnose mycoplasma pneumonia at an early stage before the rise of titer of antibody to Mycoplasma pneumoniae. Mycoplasma pneumonia may be related with T-lymphocyte activity, and its inflammatory process is different from that of bacterial pneumonia. Adenosine deaminase (ADA) activity is a predominant T-lymphocyte enzyme, and its plasma activity is high in diseases in which cellular immunity is stimulated. In this study, we have investigated 31 cases with pneumonia (11 cases of mycoplasma pneumonia and 20 cases of bacterial pneumonia) within 10 days after onset to evaluate the usefulness of ADA measurement in the diagnosis of mycoplasma pneumonia. Serum ADA activity in the group of mycoplasma pneumonia was 32.1 +/- 12.0 U/l (63.9-18.8 U/l), that of non-mycoplasma pneumonia 12.5 +/- 3.3 (4.6-18.6) U/l, and that of normal control 14.0 +/- 3.4 (6.3-19.3) U/l. ADA activity of the group of mycoplasma pneumonia was significantly higher than those of non-mycoplasma pneumonia and normal control (p less than 0.001). This results suggest that serum ADA activity in patients with acute pneumonia may be useful for the early diagnosis of mycoplasma pneumonia.

[T-lymphocytosis and increased levels of soluble IL-2 receptors in bronchoalveolar lavage fluid in cases of HTLV-I-associated myelopathy with pulmonary involvement].

HTLV-I-associated myelopathy (HAM) is endemic in southwest Japan and frequently shows T-lymphocyte alveolitis. In the present study, we investigated 13 patients with HAM to determine if they showed increased levels of soluble IL-2 receptors, a marker of lymphocyte activation, in bronchoalveolar lavage (BAL) fluid. BAL studies showed that there was an increased proportion of lymphocytes in BAL fluid from HAM patients. The increased BAL lymphocytes consisted mainly of CD3+ cells and CD4+/CD8+ ratios were similar to those of normal controls. Serum levels of soluble IL-2 receptors were significantly elevated in patients with HAM as compared to normal controls (685 +/- 210 vs 286 +/- 49 U/ml, p less than 0.01). BAL levels of soluble IL-2 receptors were detectable in patients with HAM and these levels were approximately 13 times higher on average than those in serum (166 +/- 103 vs 18 +/- 13 U/mg albumin, p = 0.001). BAL Il-2R levels in patients with HAM correlated well with the number of T-lymphocytes and CD4+ cells in BAL fluid. These results suggest that T-lymphocytes infiltrating the lungs of HAM patients are activated locally to produce IL-2 receptors. In conclusion, immunological mechanisms seem to be responsible for the development of pulmonary lesions in patients with HAM.

[Surgical treatment of atherosclerotic aneurysms of the thoracic aorta].

To determine the problem of the surgical treatment of atherosclerotic aneurysm of thoracic aorta, we reviewed our experiences with 113 consequtive patients who underwent thoracic aortic aneurysmectomy from January 1983 to December 1985. Fifty-two patients had atherosclerotic aneurysms and 61 patients had aneurysm with other etiologies. Hospital mortality was 19% in former group and was higher than in the latter group (6.5%). Especially, the mortality was the highest (50%) among the patients who required the intraoperative clamping of the arch vessels. Many complications related to atherosclerosis, such as atheroemboli, bleeding due to wall weakness and etc, was found among the patients died soon after the surgery. Also, age over 70 and renal or respiratory dysfunction were major risk factors to determine the operative death. On the other hand, asymptomatic ischemic heart disease and cerebrovascular disorders of three months after the onset were not significant determinant. It was concluded that the surgical indication should be contemplated with extreme care specifically among the patients with such risk factors.

Crystallization and some properties of acetylpolyamine amidohydrolase from Mycoplana bullata.

During the course of investigations on the catabolism of acetylpolyamines by microorganisms, we found that acetylpolyamine amidohydrolase was produced by Mycoplana bullata FERM BP-1845 and isolated the enzyme from the cell-free extract in crystalline form. The enzyme had an apparent molecular weight of 67 kDa and was composed of two identical subunits. The enzyme activity was inhibited by o-oxyquinoline and the crystalline enzyme contained one zinc atom per each subunit. The enzyme had an optimal pH around 8.0 with acetylputrescine as substrate and showed broad substrate specificity and high affinity towards various acetylpolyamines, such as acetylputrescine, acetylcadaverine, acetylspermidine, and acetylspermine.

Generation of hypophosphatemia in rats by continuous oral administration of cadmium.

To obtain further information on the negative calcium balance caused by cadmium (Cd), the factors associated with serum calcium and phosphorus homeostasis other than inhibition of intestinal calcium absorption were studied by using parathyroid hormone (PTH) and 1 alpha-hydroxycholecalciferol (1 alpha-OH-D3). In rats exposed to Cd for 30 or 90 days, the concentrations of serum calcium after treatment with PTH, parathyroidectomy (PTX) or 1 alpha-OH-D3 showed almost the same patterns as those of control animals. It was considered that the mechanism of regulation of calcium in Cd-exposed rats was normal. The continuous oral administration of Cd generated hypophosphatemia in rats. On the other hand, in 30- and 90-day-treated rats, the low concentration of serum phosphorus caused by Cd was further decreased by administration of PTH, whereas it was increased by PTX. The hypophosphatemia found in rats exposed to Cd for 30 days, but not for 90 days, was reversed by treatment with 1 alpha-OH-D3. From these results, it was concluded that the hypophosphatemia caused by long term oral administration of Cd resulted from secondary hyperparathyroidism, due to inhibited calcium absorption from the intestine, as was demonstrated previously.

Differences in neuronal lipid composition between superior cervical ganglia and nodose ganglia of the rat.

The lipid content and composition of rat superior cervical ganglia containing sympathetic motor neurons and nodose ganglia containing parasympathetic sensory neurons were studied for the first time to elucidate the mechanism of the different effects of exogenous gangliosides on these neurons in the culture medium. The ganglioside content of the superior cervical ganglia was almost 3-times that of the nodose ganglia. Although both ganglia contained GM3, GD3, GD1b and GT1b as major gangliosides, the nodose ganglia additionally contained a significant amount of sialosyllactoneotetraosylceramide LM1 (10% of total sialic acids). Contrasting with nodose ganglia, vagus fiber and dorsal root ganglia of rats, superior cervical ganglia had a higher content of sulfatide than galactosylceramide. The phospholipid content was lower in superior cervical ganglia than in nodose ganglia. Superior cervical ganglia contained less ethanolamine plasmalogen and more phosphatidylcholine than nodose ganglia. Sphingomyelin in superior cervical ganglia contained mainly medium-chain fatty acids, while that in nodose ganglia contained mainly longer-chain fatty acids. Differences in the fatty acid composition of glycerophospholipids were also observed. The results indicate that the properties of neuronal cell membranes from superior cervical ganglia and nodose ganglia are quite different, and that the differences may reflect the physiological roles of these ganglia.
