Comparison of four weeks' treatment with fenoterol and terbutaline aerosols in adult asthmatics. A double-blind crossover study.

Fenoterol and terbutaline, two long-acting beta 2-adrenoceptor agonists in aerosol form, were compared in an 8-wk randomized double-blind crossover study in 22 mild to moderately severe asthmatics. Patients completed diary cards, recorded peak expiratory flow rate (PEFR) twice daily, and attended a clinic for measurement of PEFR, 1-sec forced expiratory volume (FEV1), and forced vital capacity (FVC) twice during each treatment period. Fifteen patients completed the study; 5 dropped out while using fenoterol, and 2 while using terbutaline. At clinic attendances, the patients had a significantly higher mean PEFR after 4 wk on terbutaline (385 L/min) than after fenoterol (316 L/min) (p less than 0.001). Similar results were found on analysis of the morning and evening PEFR recordings. On comparing each individual's PEFR recordings during the 2 treatments, it was found that there was no significant difference among the treatments in 3 patients, while 9 patients had a better response to terbutaline, and 3 patients had a better response to fenoterol. While similar number expressed a subjective preference for each treatment, the lung function data suggested that the effectiveness of fenoterol appeared to decline during the trial period.

Samaritan contact among 325 parasuicide patients.

With the aim of aiding primary prevention of parasuicide by Samaritans, a study of 325 parasuicide patients admitted to hospital was undertaken to discover the extent of knowledge about Samaritans, the proportion of patients who had contacted Samaritans and the reasons why the remainder had not done so. Ignorance about Samaritans was found in 28 per cent, such ignorance being significantly more common among teenagers than in other age groups. Only 1.4 per cent had sought help from Samaritans immediately before the present parasuicidal act, though a further 13 per cent had contacted Samaritans previously. Suggestions are made for future public education programmes about Samaritans.

Mn2+ ions pass through Ca2+ channels in myoepithelial cells.

1. Intracellular recordings were made from the myoepithelial cells of the proventriculus of the marine polychaete worm Syllis spongiphila. Over-shooting responses were elicited either by carbamylcholine added to the bathing medium or by directly applied intracellular current pulses. 2. In control artificial sea water (ASW) directly applied current pulses elicited regenerative responses of 68-119 mV in amplitude and 70-1800 ms in duration; these responses were associated with contractions of the myoepithelial cells. 3. Both pharmacologically and electrically elicited responses were reversibly abolished in Ca-free ASW and were unaffected by TTX or low-sodium solutions. Regenerative responses were elicited by direct intracellular stimulation in calcium-free ASW containing 1 mM-Ba2+ or 10 mM-Sr2+. Directly elicited responses were blocked reversibly in ASW containing calcium and 15-20 mM-Co2+ or 2.5-10 mM-Ni2+; they were blocked irreversibly in ASW containing calcium and 10 mM-La3+ or 100 microM-Zn2+. 4. Regenerative responses were elicited in Ca-free solutions containing 10-50 mM-Mn2+; these responses were not associated with contractions, were consistently of longer duration than responses elicited in control ASW, and were blocked by 20 mM-Co2+ or 10 mM-La3+. The overshoots of Mn2+ responses elicited in both Na-free and Na-containing, Ca-free solutions increased as the external concentration of Mn2+ was increased, with a slope of about 27 mV per 10-fold change in concentration of Mn2+. In Ca-containing solutions the slope was reduced to about 15 mV per 10-fold change. 5. The results indicate that the myoepithelial cells generate Ca-spikes and that Mn2+ ions, in addition to Sr2+ and Ba2+ ions, pass through the Ca2+ channels of the myoepithelial cell membranes. Although Mn2+ can replace Ca2+ in generating spikes, it apparently cannot replace Ca2+ in initiating contraction, and it many compete with Ca2+ in activating repolarization of the cell.

Enzyme immunoassay for measuring lipoprotein lipase activator in milk.

A method for determining the relative amount of lipoprotein lipase activator in milk was developed. The activator was measured in arbitrary units which were based on a standard curve for high density lipoprotein. Activator varied between cows not only in whole milk but also in skim milk and milk serum. Most of the activator was in the skim milk, and the amount of activator in milk serum varied between 32 and 91% of that in skim milk. In seven of eight cows, free fatty acids of milk increased with increasing units of activator.

Treatment in an alcoholism and drug addiction unit.

Estimates of the number of alcoholics in New Zealand indicate that a highly selected minority actually receive treatment for their alcoholism. This report examines who refers these patients to an alcoholism and drug dependence unit, the utilisation of compulsory treatment, and the demographic and social characteristics of the patients. A one year sample of these patients was followed for 12 months to assess treatment outcome. One year after discharge 11 percent had adopted an abstinent life-style. Forty-two percent had been rehospitalised.

A microscopical study of the innervation of flight muscles in the tsetse fly.

The gross morphology of the nervous system supplying the dorsal longitudinal flight muscles of tsetse flies is described. Electron microscopical investigation of the nerves reveals that the dorsal longitudinal muscles are innervated by branches from four main axons. A detailed description of the neuromuscular junctions in the flight muscle is presented.

Posterior spinal fusion with Harrington instrumentation using "balanced" anesthesia.

In our experience nitrous oxide-oxygen-morphine-curare anesthesia has proved preferable because of the need for swifly awakening patients after instrumentation to ascertain neurologic status. Adjunctively, doxapram has been used to awaken patients in a predictably short time span. To prevent massive blood loss, hypotension has been attained by pentolinium, trimethaphan camsylate, and sodium nitroprusside.

Coagulation and fibrinolytic activity of cerebrospinal fluid.

Fibrin/fibrinogen degradation products (fragments D and E) were detected in cerebrospinal fluid in 23.4% of 252 patients admitted to a neurological/neurosurgical unit. Other coagulation proteins of low molecular weight (plasminogen and factor IX) were also present but larger proteins (fibrinogen and factor V) were not. These findings are consistent with protein leakage across a blood-CSF barrier damaged by inflammatory, vascular, or neoplastic disease. Fibrin/fibrinogen degradation products in cerebrospinal fluid after subarachnoid haemorrhage may not, therefore, be a reliable index of increased fibrinolytic activity in the subarachnoid space and may be misleading when selecting patients for fibrinolytic blockade.
