Thorotrast-induced hepatic angiosarcoma with 39 years latency. A pathologic and immunohistochemical study.

An autopsy case of hepatic angiosarcoma which occurred in a 64-year-old man who had been examined by Thorotrast angiography 39 years previously, is reported together with the pathologic, radiologic and immunohistochemical findings. Thorotrast deposits were found in the spleen, bone marrow, right elbow joint and lymph nodes as well as the liver. The angiosarcoma was accompanied by markedly distended sinusoids with hypertrophic and hyperplastic endothelial cells, which were considered to be closely associated with angiosarcoma. The results of immunostaining indicated that only a very limited number of tumor cells and not many sinusoidal endothelial cells showed a positive staining reaction to anti-factor VIII-related antigen (anti-F VIII-RAG). On the other hand, endothelial cells lining blood vessels in the portal areas were distinctly positive for anti-F VIII-RAG. This would suggest that production of F VIII-RAG by tumor cells was dependent on the degree of cell dedifferentiation or on the heterogeneity of endothelial cells.

Characteristics of transmural potential changes associated with the proton-peptide co-transport in toad small intestine.

1. Ionic dependence and kinetic properties of the peptide-evoked potentials across everted toad intestine were investigated with eighteen dipeptides and four tripeptides. All peptides evoked saturable increases in the mucosal negativity regardless of the presence of Na+. 2. The peptide-evoked potentials recorded in the absence of Na+ were sensitive to external pH (pHo); lowering pHo from 7.4 to 6.5 and 5.5 caused stepwise increases in their amplitude. 3. Loading epithelial cells with 9-aminoacridine or acetate caused a significant increase or decrease in amplitude of the Gly-Gly-evoked potential, suggesting intracellular alkalinization or acidification also has a great influence on the peptide-evoked potential. 4. Kinetically, Na+-independent peptide-evoked potentials conformed to simple Michaelis-Menten kinetics, and lowering pHo caused a decrease of the half-saturation concentration (Kt) for Gly-Gly without changing the maximum potential difference increase. Similar affinity-type kinetic effect was also seen for Gly-Gly influx. 5. Simultaneous measurements of Gly-Gly-induced increase in short-circuit current and Gly-Gly influx revealed that the coupling ratio of H+ and Gly-Gly flows was 1.78 +/- 0.12, suggesting the stoichiometry of the H+-peptide co-transport being 2:1. 6. Kinetic analyses of the peptide-evoked potentials indicated that all glycyl-dipeptides tested (Gly-Gly, Gly-Pro, Gly-Sar, Gly-Leu, Gly-Phe) and other dipeptides (Ala-Ala, Ala-Phe, Phe-Ala) shared a common carrier. Gly-Gly-Gly and Ala-Ala-Ala were also found to share the same carrier, while Phe-Phe, Leu-Leu and Phe-Leu appeared to be transported by a different carrier. 7. Kt values for di- and tripeptides, which apparently shared a common carrier, fell in a narrow range (0.5-2.2 mM). There was no clear correlation between 1/Kt value and molecular weight.

Tumor response to thermoradiation therapy: use of CT in evaluation.

Seventy-two tumors, mostly in deep locations, were studied retrospectively with computed tomography (CT) before and after thermoradiation therapy. Hyperthermia was administered locally or regionally with radio frequency capacitive heating equipment after irradiation (total dose, 20-70.2 Gy) for 40-60 minutes per session in a total of four to 13 sessions. Sixteen of the 72 tumors showed complete regression. Tumor regression was not related to the average maximum or minimum tumor temperature. CT scans obtained after thermoradiation therapy demonstrated a clear low-density area in 32 of 56 tumors that did not regress completely. Histopathologic examinations, performed in 14 of the 32 tumors, showed the low-density area to be massive coagulation necrosis. The low-density area was classified into three types according to its percentage area in the tumor: type 3 (more than 80%), type 2 (50%-80%), and type 1 (less than 50%). The type correlated with the average maximum and minimum tumor temperature, and follow-up observations and histopathologic examinations showed great effects in most of the type 3 tumors.

Transposition of target information from the magnetic resonance and computed tomography scan images to conventional X-ray stereotactic space.

A technique to apply reconstructed X-ray computed tomography (CT) and magnetic resonance imaging (MRI) for target determination in stereotactic Bragg peak proton beam therapy of intracranial lesions was developed. Twenty-one benign intracranial tumors and vascular abnormalities were managed using this technique. Clinical features of these lesions, as well as targeting problems associated with the MRI and CT image interpretation, are presented.

Inhibition of hypertension and salt intake by oral taurine treatment in hypertensive rats.

Effects of oral treatment with taurine on fluid intakes produced by renin were assessed in spontaneously hypertensive rats of the Okamoto strain (SHR). Renin injected into the preoptic area increased water intake and evoked salt (2.7% NaCl solution) intake, and angiotensin II injected into this area increased water intake, but not salt intake, in both SHR and control normotensive Wistar-Kyoto rats (WKY). The salt intake elicited by renin, but not water intake produced by renin or angiotensin II, was potentiated in SHR. These effects of renin and angiotensin II on fluid intakes were antagonized by previous administration of taurine or gamma-aminobutyric acid into the cerebral ventricles in both strains. When SHR received water containing 3% taurine from 32 to 105 days of age, development of hypertension was inhibited. Renin administered into the preoptic area at 105 days of age caused an increase in salt intake, but the increase was markedly inhibited by the oral administration of taurine as well. These results show that salt appetite produced by centrally administered renin is exaggerated in SHR and that development of hypertension as well as renin-induced salt appetite in SHR is inhibited by dietary taurine.

[Effect of terodiline hydrochloride on the function of urinary bladder in rats].

The effect of terodiline on the function of urinary bladder was investigated in anesthetized rats. When saline was infused continuously into the urinary bladder of rats, terodiline (1-10 mg/kg, i.v.) prolonged the time to micturition in a dose-dependent manner. When enough saline was infused into the urinary bladder to induce the voiding contraction in urethra-ligated rats, terodiline (1-10 mg/kg, i.v.) and verapamil (1 mg/kg, i.v.) abolished the contraction, of which amplitude and frequency were partially inhibited by atropine (1 mg/kg, i.v.). Efferent discharge from the pelvic nerve on the micturition reflex was inhibited by terodiline (3 mg/kg, iv.v.). Both of the bladder contractions evoked by the electrical stimulation of the peripheral or central cut end of the pelvic nerve were dose-dependently inhibited by terodiline (1-10 mg/kg, i.v.). At 3 mg/kg or more, terodiline significantly inhibited the contraction, and the effects were long lasting. The effect of atropine (1 mg/kg, i.v.) was similar to that of terodiline (3 mg/kg, i.v.). Increase in frequency of urination and decrease in total urinary volume per micturition after the bilateral transection of the hypogastric nerve were improved after on oral administration of terodiline (1-10 mg/kg).

[Effect of terodiline hydrochloride in isolated rabbit detrusor].

Effect of terodiline on isolated rabbit and guinea pig detrusor was investigated in comparison with that of flavoxate and oxybutynin. Terodiline (10(-6) M) parallelly shifted the dose-response curve for carbachol in rabbit detrusor to the right, and high doses of terodiline (3 X 10(-6)-3 X 10(-5) M) inhibited the maximal contraction. Flavoxate (10(-5) M or more) also inhibited the maximal contraction. Oxybutynin (10(-8) M or more) shifted the dose-response curve to the right, but did not affect the maximal contraction. At 3 X 10(-6) M or more, terodiline dose-dependently inhibited the Ca-contraction of rabbit detrusor. While the contraction of rabbit detrusor induced by electrical field stimulation was inhibited by atropine (3 X 10(-7) M) or nifedipine (3 X 10(-6) M) by 35% or 73%, respectively, the combination of atropine (10(-7) M) and nifedipine (10(-6) M) abolished it. Oxybutynin (3 X 10(-7) M) inhibited it by about 30%; terodiline (10(-6) M or more) and flavoxate (10(-5) M or more) dose-dependently inhibited it, and abolished at 10(-4) M and 3 X 10(-4) M, respectively. Terodiline inhibited the 1-quinuclidinyl-[phenyl-4-3H]-benzilate binding to the microsomal fraction of guinea pig urinary bladder, brain, atria and ileum dose-dependently, and it had similar affinity among these fractions. Terodiline (3 X 10(-6) M or more) inhibited the 45Ca uptake to minced guinea pig urinary bladder dose-dependently, but did not influence the 45Ca efflux even at 10(-4) M. Flavoxate (10(-4) M) only slightly inhibited the 45Ca uptake.(ABSTRACT TRUNCATED AT 250 WORDS)

The complete cancellation of abnormal Q waves due to an old anteroseptal infarction following subsequent acute posterior myocardial infarction.

A rare case of multiple infarction is described. An abnormal Q wave due to an old anteroseptal infarction was completely masked by a new contralateral posterior myocardial infarction. Coronary angiogram revealed multiple stenoses of segments 1 (100%), 7 (99%), 12 (90%) and 13 (99%). Left ventriculography also showed akinesis of segments 2, 3 4 and 6, and reduced wall motion in segments 1, 5 and 7. These findings, together with electrocardiographic changes from before the present attack reinforce the above interpretation.

Lung phospholipids during recovery from oxygen toxicity are altered by hydrocortisone.

Corticosteroids affect type II alveolar epithelial cells, increase the lung's content of lipids associated with pulmonary surfactant, and have been used to treat the adult respiratory distress syndrome. We used hyperoxia to produce diffuse alveolar damage in rats and then determined the contents of lung lipids and the incorporation of precursors into these lipids while the rats recovered in air and received either hydrocortisone or saline injections. With saline injections (control) the total lipid content after hyperoxia and 48 hours recovery in air was nearly twice the lipid content of lungs never exposed to hyperoxia; many lipids, including phosphatidylcholine (PC), shared in this increase. With hydrocortisone injections the total lipid content, when expressed per lung, was not significantly greater than with saline injections but when expressed per mg DNA several lipid subgroups, including phosphatidylcholine, were significantly greater. Palmitate incorporation into most lipids of lung slices was not significantly different by 48 hours of recovery, and hydrocortisone had a modest effect after 96 hours. In contrast to the incorporation of palmitate, the incorporation of glycerol and of lysophosphatidylcholine (LPC) nearly doubled in most lipids during the recovery phase. Hydrocortisone injections were associated with greater glycerol incorporation after 96 hours of recovery in nearly all lipids and LPC incorporation was increased primarily into PC. We conclude that lung lipids increase markedly during recovery from severe oxygen toxicity and that hydrocortisone leads to additional increases of saturated and unsaturated PC.

[Current status of diagnosis and treatment of metastatic liver cancer].

Clinical usefulness of liver function tests, tumor markers including CA 19-9, CEA and serum ferritin and image diagnosis was discussed in regard to early diagnosis. Liver function tests were not useful, and tumor markers were highly positive, i.e., CA 19-9 was positive in 70.7% and CEA in 66.7%. However, these were low positive rate, in the early stage. Imaging techniques were very useful in detecting liver metastasis; US positive in 92.5%, CT 87.2% and angiography 93.1%. In the treatment of metastatic liver cancer, arterial chemotherapy and transarterial embolization are the major modality.

[Present status of hypoxic cell sensitizers and PLDR inhibitors].

Clinical and basic studies in hypoxic cell radiosensitizers and potentially lethal damage (PLD) repair inhibitors were reviewed. Most clinical trails on misonidazole (MISO) show that the toxicity of MISO is a major reason for failure in the trials and an adequate selection of tumors is needed for the success. Phase III studies on SR-2508 and RO 03-8799, less toxic and more effective sensitizers, are in progress. AK-2123 and RK-28 developed in Japan are drugs with similar clinical applicability of MISO. Thiol depletion of tumors is one of the way to increase the radiosensitization. The measurement of intercapillary distance on histopathological microslide is a valuable method to select the tumors containing much hypoxic cell fractions. Stable and effective chemicals as MISO in hypoxic cell sensitization must be developed to investigate the clinical applicability of PLD repair inhibitors.
