[Percutaneous nephroureterolithotomy in two city hospitals: review of 102 cases].

The authors experienced 102 cases of percutaneous nephroureterolithotomy (PNL) in the urological clinic at two city hospitals between May, 1984, and July, 1986. Percutaneous access for renal pelvis was performed under interventional ultrasound and the disintegration of stones was mainly performed by an electric drill system. Stones were successfully removed in all the cases, although residual stones were recognized in 16 cases (15.7%). Complications were rather minor and were treated conservatively without open surgery. PNL is a very effective and promising method not only for university hospitals but also for the urological clinic in city hospitals.

Molecular studies on thiaminase I.

The thiaminase I gene of Bacillus thiaminolyticus was cloned on a 1.6 kb DNA fragment (enzyme molecular weight 42,000), and was expressed in both Escherichia coli and Bacillus subtilis. When a selection drug was absent, the plasmid was maintained stably for approx. 100 generations in wild-type E. coli. Instability of the thiaminase gene was demonstrated in the thiamin pyrophosphate-requiring mutant of E. coli from which the plasmid was deleted rapidly. Wild-type E. coli accumulated the enzyme in its periplasm. A method for the detection of thiaminase I enzyme in SDS-polyacrylamide gel was developed. Thiaminase I of B. thiaminolyticus was found to exist in two sizes, 44 and 42 kDa, among different strains. Moreover, thiaminase of 42 kDa became approximately 41 kDa after a long-term culture, most likely because of the action of proteinases. Thiaminase expressed in E. coli from a thiaminase-positive recombinant plasmid was 42 kDa, and showed the same mobility on SDS-polyacrylamide gele electrophoresis as the enzyme isolated from the young culture of the parent strain of B. thiaminolyticus used for cloning. This value was, therefore, considered to represent intact thiaminase that had escaped from the attack of bacilli proteinases.

Potentiation of pressor response to angiotensin II at the preoptic area in spontaneously hypertensive rat.

Cardiovascular responses to angiotensin II(AII) at the preoptic area (POA) were compared between normotensive Wistar Kyoto rat (WKY) and spontaneously hypertensive rat(SHR) by measuring blood pressure and heart rate under unrestrained, conscious state via a catheter implanted chronically into the abdominal aorta and by injection of drugs into POA through a chronic guide cannula. AII injected into POA at doses of 0.3 ng and 1 ng produced a dose-dependent pressor response, accompanied with a slight decrease of heart rate, in both WKY and SHR. However, in SHR, the pressor response to AII was more than 2 times greater than that in WKY and was quick in onset and lasted about 30 min. When AII in combination with [Sar1, Ile8]-angiotensin II (0.5 microgram), an AII receptor antagonist, were simultaneously administered to POA, the pressor response to AII was strongly inhibited in both WKY and SHR. The results suggest that the pressor response to AII due to its receptor stimulation at POA is markedly potentiated in SHR.

Radiofrequency capacitive hyperthermia for deep-seated tumors. I. Studies on thermometry.

The thermometry results of radiofrequency (RF) capacitive hyperthermia for 60 deep-seated tumors in 59 patients are reported. Hyperthermia was administered regionally using two RF capacitive heating equipments which the authors have developed in cooperation with Yamamoto Vinyter Company Ltd., (Osaka, Japan). Intratumor temperatures were measured by thermocouples inserted through angiocatheters which were placed 5 cm to 12 cm deep into the tissues. Tumor center temperatures were measured for 307 treatments in all tumors; thermal distributions within tumors and surrounding normal tissues were obtained for 266 treatments of 53 tumors by microthermocouples. Thermometry results obtained were summarized as follows. A maximum tumor center temperature greater than 43 degrees C and 42 degrees C to 43 degrees C was obtained in 23 (38%) and 14 (23%) of the 60 tumors respectively. The time required to reach 43 degrees C in the tumor center was within 20 minutes after the start of hyperthermia in 87% of tumors heated to more than 43 degrees C. Temperature variations within a tumor exceeded 2 degrees C in 81% of tumors heated to more than 43 degrees C. The lowest tumor temperature greater than 42 degrees C was achieved in six of the 53 tumors (11%). Of 42 tumors in which temperatures of the subcutaneous fat, surrounding normal tissues, and the tumor center were compared, 24 (57%) showed the highest temperature in the tumor center and ten (24%) in the subcutaneous fat. When the heating efficacy was assessed in terms of a maximum tumor center, it great deal depended on the treatment site, tumor size, thickness of subcutaneous fat, and tumor type. Tumors in the head and neck, thorax, lower abdomen, and pelvis could be heated better than tumors in the upper abdomen. Greater heating efficacy was shown in patients with large, hypovascular tumors, and with the subcutaneous fat measuring less than 15 mm thick. The predominant limiting factor for power elevation was pain associated with heating. Systemic signs including increases in pulse rate and body temperature were not serious and seldom became limiting factors for power elevation. Our thermometry results indicate that the advantages of deep RF capacitive heating are its applicability to various anatomic sites and negligible systemic effects. The disadvantages are that its primary usefulness is limited to patients with thin subcutaneous fat and with large or hypovascular tumors.

Radiofrequency capacitive hyperthermia for deep-seated tumors. II. Effects of thermoradiotherapy.

Clinical effects and safety of radiofrequency (RF) capacitive hyperthermia in combination with radiotherapy were evaluated in 40 patients with locally advanced deep-seated tumors. Hyperthermia was administered regionally with an 8-MHz or a 13.56-MHz RF heating device, once or twice a week after irradiation, four to 13 sessions total. Radiotherapy was delivered in fractions of 170 to 200 cGy a day, 5 days a week to 30 to 70 Gy to 33 patients, whereas the remaining seven patients received a total dose of 28 to 60 Gy in fractions of 400 cGy, twice a week. Six of the 40 tumors treated showed CR (100% regression), 6 PRa (80%-100% regression), 13 PRb (50%-80% regression), and 15 NR (less than 50% regression) when assessed by tumor size on computerized tomography (CT) scan. The tumor size before treatment was significantly smaller in CR + PRa tumors than in PRb + NR ones. TDF Time-dose fractionation (TDF) and number of heat treatments, however, did not differ significantly between the both tumors. Greater regression was observed in tumors heated to 41 to 43 degrees C in the maximum temperature than in tumors heated to below 41 degrees C or above 43 degrees C. The minimum tumor temperature was not related to the tumor regression. Posttreatment CT scan revealed remarkable low-density areas in 18 of the 34 tumors that did not regress completely. Histopathologic examinations demonstrated the low-density area to be massive coagulation necrosis and no malignant cell was observed in two tumors examined thoroughly. The types of low-density areas, which were classified according to its percent area in the tumor, correlated with the maximum and minimum tumor temperature. Most of the type III tumors (more than 80% low density) did not regrow in follow-up studies. Complications consisted of subcutaneous fat necrosis in four patients, local edema in four patients, and one abdominal abscess in one patient, all of which eventually resolved. These clinical results strongly suggest the usefulness of RF capacitive hyperthermia combined with radiotherapy for the treatment of refractory deep-seated tumors, and that intratumor low-density areas which appear on posttreatment CT seems to be a good parameter for assessing the tumor response to thermoradiotherapy.

Identification of a family of high molecular weight tumor-associated glycoproteins.

The monoclonal antibody (MAb) designated DF3 was prepared against a human breast carcinoma metastatic to liver. This MAb reacts with a high molecular weight glycoprotein detectable in human breast carcinomas and human milk. In contrast, MAb F36/22 was prepared against the MCF-7 breast carcinoma cell line, MAb 115-D8 against human milk fat globule membrane (HMFGM) and MAb Ca1 against the HEp-2 human laryngeal carcinoma cell line. These MAb have similar patterns of reactivity with normal tissues and tumors based upon immunoperoxidase staining. In the present study we have monitored reactivity of these MAb against DF3 antigen purified from human breast carcinoma cell lines (MCF-7, BT-20) and HMFGM. Solid phase immunoassays and immunoblotting demonstrate that MAb DF3, F36/22, 115-D8, and Ca1 all react with the same purified DF3 antigen. Furthermore, immunoblot analysis indicates that the DF3 antigen reactive with these MAb differs structurally in preparations from breast carcinoma cells and HMFGM. We also demonstrate that MAb F36/22 completely inhibits MAb DF3 binding in competitive blocking assays. In contrast, the results indicate that MAb 115-D8 and Ca1 only partially block MAb DF3 reactivity and the extent of this inhibition varies with DF3 antigen purified from breast carcinoma cells and HMFGM. Taken together, these findings with multiple MAb prepared against a variety of immunogens suggest that existence of a family of related but not identical high molecular weight tumor-associated glycoproteins.

Nucleoside-triphosphatase and hydrolysis of thiamin triphosphate in Escherichia coli.

A membrane-bound nonspecific triphosphatase of E. coli was solubilized and purified to a homogeneous SDS-acrylamide gel electrophoresis band. It was found to be a single polypeptide of 16 kDa requiring no Mg2+, with an optimal pH at 6.5. The substrate specificity was broad and a nonspecific Mg2+-independent ribonucleoside-triphosphatase (NTPase) activity was expressed together with thiamin-triphosphatase activity. The molecular size and characteristics were clearly different from the known NTPase (EC 3.6.1.15). Using the purified thiamin-triphosphatase II, ATP:thiamin-diphosphate phosphoryl transferase (EC 2.7.4.15) activity was demonstrated with an optimal pH of approx. 5.3. Considering its kinetic parameters and other characteristics, however, the thiamin triphosphate synthesizing activity was not thought to take part in cellular thiamin triphosphate synthesis. The possibility that thiamin-triphosphatase II plays a part in the hydrolysis of thiamin triphosphate to control its cellular level is suggested.

Anatomical observation of six calves affected with segmental aplasia of the spinal cord.

Four male and two female Holstein-Friesian calves with segmental aplasia of the spinal cord were examined macroscopically and radiographically, and in some cases also histologically. External symptoms included inability to stand, deep depression of the body near the middle of the back but without any blemish in the skin, and slight reduction of the body length. Both hind limbs were almost normal. In every case segmental aplasia of the spinal cord was observed between the caudal thoracic and the cranial lumbar region. Owing to this defect, the vertebrae of that limited area consisted only of the body and the ribs fused on both sides. The sternum showed abnormal ossification at its caudal part. Other defects were observed such as kyphoscoliosis (six cases), polycystic kidney (one case), cryptorchidism (one case), and XX/XY chimerism (one case). From these findings it was apparent that this anomaly was uniquely different from the other two main spinal cord anomalies, spina bifida and perosomus elumbis. One conceivable pathogenesis of this rare anomaly, it was conjectured, is as follows: This anomaly results primarily from a segmental disorder of the neural tube, probably due to vascular problems, whereby a part of the spinal cord fails to develop properly from its normal genesis. Anomalous shapes of the vertebral column and ribs are perhaps secondary defects caused by aplasia of the spinal cord.

Determination of chinoform in biological fluids and nervous tissues of the dog by gas chromatography-mass spectrometry.

A sensitive gas chromatographic-mass spectrometric method for the determination of 5-chloro-7-iodo-8-hydroxyquinoline (chinoform, clioquinol) in biological fluids and nervous tissues is described. Chinoform was converted into the pentafluorobenzyl ether, which was separated on a 10% Dexsil 300GC column and determined by the use of chinoform-d4 as an internal standard. The clean-up of chionoform in plasma and urine was efficiently achieved by extracting with benzene, while the drug in the tissue was pretreated successively by extraction with 12.5% v/v pyridine-benzene, separation on a Clin-Elut cartridge and adsorption on alumina. The quantitation limit of chinoform was 100 pg, and the recovery rates of chinoform added to plasma and tissue were 98% and 92%, respectively. The chinoform levels in biological fluids and tissues in dogs after prolonged administration of the drug at a dose of 400 mg/kg/day were measured by the proposed method. The plasma level and tissue distribution of chinoform are also discussed.

Prostatic biopsy guided by transrectal ultrasonography using real-time linear scanner.

Prostatic needle biopsy guided by transrectal ultrasonography using a real-time linear scanner was performed on 257 cases in our outpatient clinic and mass screening program for prostatic diseases. The success rate was 99.2 per cent, and complications occurred in only 2 cases (0.8%) after biopsy. This newly developed method is recommended as being safer and more accurate than conventional ones.

Regional hyperthermia combined with blockade of the hepatic arterial blood flow by degradable starch microspheres in pigs.

The benefit of hepatic arterial microembolization by degradable starch microspheres (DSM) was investigated in regional hyperthermia of the liver. Hyperthermia with and without blood flow blockade of the hepatic artery using degradable starch microspheres was performed on six pigs. Heat was given for 30 min in each treatment by 8 MHz radiofrequency capacitive heating equipment. To maintain blood flow blockade during hyperthermia, 10 mg/kg of degradable starch microspheres was administered into the hepatic artery as an initial dose and 5 mg/kg of the drug was added periodically under the measurement of hepatic arterial blood flow by an electromagnetic flowmeter. To evaluate the effect of degradable starch microspheres, the temperature increase in the liver and rectum was compared between the treatment with and without DSM. All pigs showed a larger increase in intrahepatic temperature when heated in combination with degradable starch microspheres than without. On the other hand, temperature increase in the rectum as a result of hyperthermia to the liver was suppressed by DSM as compared with hyperthermia alone. These results indicate that hepatic arterial embolization by degradable starch microspheres potentiates radiofrequency capacitive heating of the liver. Although this study was not made with liver tumors, regional hyperthermia may be effective in the control of liver tumors when heat is given after the blockade of the hepatic artery by DSM.

Chemical form-dependent induction of hepatic zinc-thionein by arsenic administration and effect of co-administered selenium in mice.

Four arsenic compounds (m-arsenite, arsenate, methylarsonate (MMA), and dimethylarsinate (DMA) were administered to mice, either orally or by intraperitoneal injection, and the amounts of induced hepatic zinc-thionein (Zn-Th) were determined to investigate the chemical form dependency in metallothionein induction by arsenic. Several mice died within 24 h following intraperitoneal injection. The toxicity estimated from the lethality was in the order of m-arsenite greater than arsenate much greater than DMA approximately equal to MMA. Although all 4 arsenic compounds more or less induced Zn-Th following intraperitoneal injection, the dose of inorganic arsenic compounds observed to cause induction was one order lower than that of organic arsenic compounds. The induction of hepatic Zn-Th was also observed following oral administration, and inorganic arsenic compounds were again found to be more effective inducers via this route. To study the interactions of m-arsenite and selenite with regard to the amounts of Zn-Th induced, the two compounds were co-administered intraperitoneally. The amount of hepatic Zn-Th induced by the co-administration was one-fifth of that observed after selenite administration, and was almost the same as that after m-arsenite injection. Namely, when co-administered m-arsenite had the effect of lowering the level of Zn-Th induced following selenite administration, but the converse phenomenon was not observed.

Evaluation of various types of new hypoxic cell sensitizers using the EMT6 single cell-spheroid-solid tumour system.

Eleven new hypoxic cell sensitizers representative of those developed in Japan between 1980 and 1985 were evaluated in vitro and in vivo in comparison with misonidazole (MISO), SR-2508, Ro 03-8799, and ANT (2-amino-5-nitrothiazole). The new compounds included 2-nitroimidazole nucleoside analogues, nitrotriazoles and other nitroaromatics, non-nitro compounds, and electron-affinic compounds that readily intercalate DNA. The sensitizing activity in the EMT6 single cells correlated not only with the reduction potential but, for some compounds, also with the reactivity with non-protein sulphydryls. The sensitizers were also tested using the EMT6 spheroids and solid tumours. The patterns of changes in sensitizer enhancement ratios (SERs) for single cells, spheroids, and solid tumours were classified into two types: (1) SERs for the three testing systems were similar; and (2) SERs decreased in the order of: single cells, spheroids, and solid tumours. Only nitroimidazole and nitrotriazole derivatives belonged to the former type. RK-28 and RK-29, 2-nitroimidazoles with sugar analogue components, had in vivo effects almost equal to those of MISO. Also 3- and 4-nitrotriazole derivatives had definite in vivo effects.

Three cases of secondary leukemia. Clinicopathologic findings.

Three cases of secondary leukemia developing after chemotherapy and/or radiotherapy for myeloma, mycosis fungoides, and non-Hodgkin's lymphoma are reported. The first case was a 51-year-old man with IgG-lambda myeloma, treated with melphalan and prednisolone, who developed acute myelomonocytic leukemia 54 months after the diagnosis of myeloma. The second case was a 54-year-old woman with mycosis fungoides treated with radiation, predonine, and cyclophosphamide, who developed acute megakaryoblastic leukemia 298 months after the diagnosis of mycosis fungoides. The third case was a 35-year-old woman with stage IV non-Hodgkin's lymphoma treated with VEMP who developed acute myelogenous leukemia 26 months after the diagnosis of malignant lymphoma. All cases showed pancytopenia and two of three cases had morphologic abnormality in several hemopoietic cell lineages in the leukemic stage. There is a possibility that second malignancies are an increasingly recognized complication in the patients treated with a large amount of chemo-radiotherapy.
