Effects of step-up and step-down heating combined with radiation on murine tumor and normal tissues.

Radiosensitizing effects of step-up heating (SUH) and step-down heating (SDH) on the tumor and skin were studied by using mammary adenocarcinoma transplanted to the foot of the C3H/He mouse. The tumor and skin responses were assessed by the tumor growth delay method and the skin reaction scoring method, respectively. Neither SDH (44 degrees, 10 min----42 degrees, 30 min) nor SUH (42 degrees, 30 min----44 degrees, 10 min) alone caused a substantial tumor or skin response. When the heat treatment was given immediately after irradiation, the thermal enhancement ratio (TER) was higher in SDH than in SUH for tumors as well as the skin. A therapeutic gain factor (TGF) of 1.2 was obtained in SUH, while no therapeutic benefit was found in SDH. SDH was applied at various times (0 to 3 hr) before or after irradiation. When SDH was given before irradiation, the TER was consistently high to almost the same degree for tumors and the skin regardless of the time interval, resulting in minimal or no therapeutic gain. With SDH after irradiation, the TER for the skin decreased with increase in the time interval, while the TER for the tumor was moderately enhanced. Therefore, the TGF increased with increase in the time interval and reached 2.2 when SDH was given 3 hr after radiation. SUH is slightly advantageous over SDH in terms of the TGF, and SDH should be given 3 hr after irradiation when selective tumor heating is not possible.

[A personal point of view to EC-IC bypass surgery in completed stroke after the co-operative study].

The role of EC-IC bypass surgery has become a very controversial issue recently. EC-IC anastomosis was performed in 132 patients of stroke in our institute. Thirty six of them which could be followed up completely had a completed stroke and ten of these showed a significant clinical improvement by the next day of surgery. We studied the angiograms, CT scans and dynamic CT studies of all 36 patients to determine the common factors responsible for a significant clinical improvement in these ten patients. It was found that patients with occlusion of middle cerebral arteries and with good collateral circulation fared better than those with other lesions and good cross circulation. Site of low density on CT revealed patient with watershed low density did well. Characteristic of low density area was found to be a significant factors. If the low density area was homogeneous and well defined in an arterial territory, it was invariably associated with no improvement after surgery. Patients showing an improvement had scattered or irregular low density areas. Dynamic CT studies were not found to be significant.

Pharmacodynamic effects of the sleep inducer zopiclone.

Pharmacodynamic effects of [6-(5-chloro-2-pyridyl)-6,7-dihydro-7-oxo-5H-pyrrolo[3,4-b]pyrazin -5- yl]-4-methyl-1-piperazine-carboxylate (zopiclone, RP-27267), chemically unrelated to benzodiazepines and a potential new sleep inducer, on the peripheral system were investigated in several species of animals. The drug was dissolved in the vehicle of 0.01 mol/l HCl solution for intravenous administration or for addition to the bath medium and was suspended in 0.25% carboxymethylcellulose solution for oral administration. In unanesthetized rabbits, zopiclone, 0.5 mg/kg i.v., exerted no action and at 1 mg/kg slightly decreased respiration and heart rate without affecting blood pressure and ECG. Zopiclone at 10(-6) g/ml had no action in the isolated guinea-pig atria but at 10(-5) g/ml it produced a gradual and slight decrease in heart rate without affecting the contraction. In the isolated small intestine of rabbits and guinea-pigs, zopiclone at 10(-6) g/ml had no action but produced a slight inhibition in a dose of 10(-5) g/ml. Zopiclone, 10(-5) g/ml, did not affect the stimulatory effects of acetylcholine, serotonin, histamine and barium in the isolated guinea-pig intestine. Zopiclone, 1, 5 and 10 mg/kg i.v., exerted no action on rabbit intestinal movement in vivo. Zopiclone, 5, 10, 20 and 50 mg/kg p.o., had no effect on the propulsive motility of the mouse intestine. Zopiclone, 10(-5) g/ml, did not affect contractile movement of the uterus isolated from rabbits and did not influence the contractile response to epinephrine.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of extramedullary plasmacytoma].

The patient is 51-year-old male. A tumor arose on the upper eye lid at the age of 44 years and 6 years later, a tumor arose on the forearm. The tumor recurred 3 times on the forearm and were resected each times. When he was 51 years old, 3 tumors arose on the upper arm. Tumors were found in the cutis and subcutis of the jaw, back, forearm and thigh. A tumor was also found on the palatine tonsil. Bence Jones protein was negative in serum and urine. There were no particular findings in serum immunoelectrophoresis, bone X ray films, bone marrow aspiration and bone scintigram. The Borrmann 4-like pattern was revealed on the stomach by barium examination. By chemotherapy and radiotherapy, the tumors decreased markedly in size. Excisional biopsy showed that the tumor was composed of atypical plasma cells and gamma-chain and lambda-chain were immunohistologically detected.

[Intraoperative radiotherapy].

Intraoperative radiotherapy, which delivers a large single dose of electrons to a tumor intraoperatively, is applied for locally advanced and/or difficult to cure malignancies which are rather difficult to treat by conventional external beams. Although this method was initially developed in Japan, increasing numbers of institutes have recently been working with IORT on a world-wide basis because of the increased availability of medical accelerators and the better results obtained. The malignancies for which IORT is indicated are cancers of the stomach, liver, biliary tract, pancreas, colo-rectum, urinary bladder, prostate and uterus, which are surrounded by normal tissues or organs with low tolerance doses, and also recurrent or difficult to cure malignancies of the brain, lung, mediastinum, bone and soft tissues are becoming candidates for IORT treatment.

[A study of lymph node metastasis in gastric cancer--a new method for fixing resected specimens with methylene blue formalin].

Lymph node metastasis in 171 cases after a radical resection of the gastric cancer have been studied by means of examining specimens of these lymph nodes fixed with methylene blue formalin. The number of the resected lymph nodes in a single case was 50.3 on the average. By the Appleby method this number was 82.8. The rate of metastasis (the number of cases with lymph node metastasis measured against the number of patients) was 43.9%. The frequency of metastasis (the number of metastasized nodes versus the number of dissected nodes) was 11.3%. When classified by location no difference was noted, but in the group which showed an infiltration of a macroscopic type or an undifferentiated adenocarcinoma, the rate of metastasis and the frequency of metastasis was found to be higher than in the group with a non-infiltration type or a well-differentiated adenocarcinoma. Although the metastatic rate of perigastric nodes (No. 1-6 was higher than in the other lymph node group (No. 7-11), no difference was recognized between both groups in the frequency of metastasis.

The radiation response of SCCVII tumor cells in C3H/He mice varies with the irradiation conditions.

The radiation response of SCCVII tumor cells in C3H/He mice under various irradiation conditions was studied using an in vivo-in vitro assay. When tumor-bearing mice were irradiated without anesthesia or physical restraint, the tumor had a hypoxic fraction of 5.4%. Both anesthesia and immobilization of the tumor-bearing leg with adhesive tape produced significant increases in the hypoxic fraction (23 and 28%, respectively). Restraining the mouse in a jig without immobilizing the tumor-bearing leg also increased the hypoxic fraction (13%).

Hydrogen ion-coupled transport of D-glucose by phlorizin-sensitive sugar carrier in intestinal brush-border membranes.

In rabbit intestinal brush-border membrane vesicles, Na+-independent D-glucose uptake in the presence of an inside-negative transmembrane potential was found to be stimulated by an imposed pH gradient. Na+-independent, pH-dependent and phlorizin-sensitive D-glucose-evoked potentials could be recorded from isolated toad intestine. The obtained data suggest that phlorizin-sensitive D-glucose carriers of intestinal brush-border membrane can interact with H+ when Na+ is absent.

Multi-institutional studies on hyperthermia using an 8-MHz radiofrequency capacitive heating device (Thermotron RF-8) in combination with radiation for cancer therapy.

A joint clinical trial of hyperthermia using a newly developed 8-MHz radiofrequency (RF) capacitive heating device (Thermotron RF-8; developed in cooperation with Yamamoto Vinyter Co. of Osaka) was performed under collaboration of seven institutions. Radiation with 4 Gy twice a week for a total of 40 Gy or 2 Gy five times a week for a total of 50 Gy was delivered. After irradiation, hyperthermia at 42.5 degrees C +/- 0.5 degree C for 40 to 60 minutes was given twice a week for a total of 10 times. Tumors examined in this trial were located in various depths in the body, and included those which were considered refractory to conventional treatments or radioresistant such as malignant melanoma and soft tissue tumors. Of the 63 tumors treated, 52.4% showed complete regression (CR); 19.0% more than 80% regression (PRa); 20.6%, 80% to 50% regression (PRb); and 8.0% no regression (NR). Our joint clinical trial demonstrated that hyperthermia with the use of the Thermotron RF-8 is safe and effective in the treatment of radioresistant tumors located in superficial, subsurface, and in some cases deep regions, if the surface cooling is properly managed by the temperature-controlled saline pad and electrodes of appropriate size are paired.
