Establishment and characterization of an Epstein-Barr virus negative B-cell lymphoma cell line and successful heterotransplantation.

An EBV-negative B-cell lymphoma cell line designated as HBL-1 was established from the pleural effusion of a patient with malignant lymphoma, diffuse, large cell. Surface marker studies revealed that HBL-1 cell possessed the same phenotypes of a mature B-cell as the original lymphoma cell from pleural effusion. HBL-1 closely resembled morphologically to the original lymphoma cells observed in the lymph node biopsy and pleural effusion. Chromosome analysis revealed that HBL-1 cells showed chromosomal aberrations of a 14q+ marker chromosome as the result of a translocation between chromosomes 14 and 16. Heterotransplantation to subcutaneous tissue of an athymic nude mouse has succeeded, but intraperitoneal heterotransplantation has failed. Tumors were localized in the subcutaneous tissues and no metastasis was found. This newly established cell line may facilitate immunological and oncogenic studies for human B-cell lymphomas.

Conditions for thiamin assay by cyanogen bromide oxidation.

Optimum conditions for the assay of thiamin were studied using a cyanogen bromide (BrCN) oxidation method. The adopted procedure included neither pre-purification of samples through an ion exchanger nor extraction of the thiochrome into an organic solvent. The 0.25 M BrCN (the concentration before the addition of alkali) and the final NaOH concentration of approx. 1% gave the highest yield of thiochrome by a laboratory-prepared BrCN. To obtain the highest intensity of fluorescence, a concentrated BrCN (1.8 M) was introduced in place of the conventional BrCN (0.11 M), obtaining 300% or more intensity of fluorescence. For the oxidation of thiamin diphosphate, 0.15-0.2 M of laboratory-prepared BrCN gave the highest intensity of fluorescence instead of the 0.25 M for free thiamin. For simultaneous oxidation of free thiamin and thiamin diphosphate, therefore, 0.23-0.24 M of laboratory-prepared BrCN was deduced to give the best yield of fluorescence. With a solution of commercially obtained solid CNBr, optimum concentrations for the oxidation of thiamin were about 0.04 M for CNBr and about 0.16% for NaOH. When the sample contained in inhibitor of oxidation, such as ascorbic acid, the percentage of inhibition decreased inversely proportional to the concentration of the sample in a rough approximation. The degree of inhibition was not reduced by the increased amount of BrCN reagent. Thus the possibility was indicated that thiamin in an ascorbic acid-contaminated sample could be determined accurately by extrapolating values for serially diluted samples.

[Treatment with degradable starch microspheres (DSM) in malignant hepatic tumors-clinical experience of implantable drug delivery system (Port-A-Cath)].

Chemoembolization with degradable starch microspheres (DSM) was performed in twelve cases of hepatocellular carcinoma, one case of cholangioma, and one case of metastatic liver cancer. The following results were obtained. (1) Tumor regression of over 50% was observed in eight of fourteen cases. Tumor regression of over 25% was obtained in ten of fourteen cases. (2) Half of the cases had fever, pain and leukocytopenia, but they were slight and transient. (3) Implantable drug delivery system (Port-A-Cath) was applied for intra-arterial chemoembolization in four cases. The longest implantation period was 662 days and 33 infusions were made. These results suggest that chemoembolization with DSM can be effectively used in the treatment of malignant hepatic tumors.

Peripheral T-cell lymphoma with helper T-cell phenotype (Leu 3a+ Leu 8-).

Eleven cases of Leu 3a+ Leu 8- peripheral T-cell lymphoma (PTCL), excluding adult T-cell leukemia/lymphoma, were studied by immunostaining with monoclonal antibodies and enzyme histochemistry in order to clarify the histogenesis of PTCL. Seven of the eleven cases had varying degrees of polyclonal hypergammaglobulinemia. All cases were histologically characterized by neoplastic proliferation of clear cells and some cases showed a histologic background similar to IBL or AILD lesions with proliferation of immunoblasts or plasmacytoid cells and vascular proliferation. Immunohistologic analysis of PLP-fixed frozen tissues revealed that neoplastic clear cells expressed a Leu 3a+ Leu 8- phenotype (helper T-cell subset). The distribution of Leu 3a+ Leu 8- neoplastic cells corresponded closely to that of DRC-1+ cells, which are localized in the lymphatic follicles, but hardly at all with that of beta-glucuronidase+ vessels, termed PCV or HEV, which are usually present in T-cell areas. One case only progressed from Leu 3a+ Leu 8- IBL-like T-cell lymphoma (IBL-T), with proliferation of immunoblasts or plasmacytoid cells and vascular proliferation, to diffuse lymphoma of the large cell type showing none of these lesions. From these observations it is suggested that IBL-T might progress to T-cell-type monomorphous diffuse lymphoma.

[Effects of OK-432 on UFT activation].

An experiment to gauge the effect of OK-432 on UFT activation under a combined administration of UFT and OK-432 at a therapeutic dose has been pursued clinically by determining the blood and intratumoral 5-FU concentrations after administration of UFT. UFT was administered orally to a group of AH-130-bearing rats at a dose of 12 mg/kg or 8 mg/kg. In another group that was given combined treatment with OK-432, the OK-432 was administered subcutaneously at a dose of 0.1 KE/kg together with UFT. The intratumoral 5-FU concentration at 4 hours after UFT 12 mg/kg administration showed higher values in th single UFT treated group than in the combined treated group, but at 8 hours no significant difference was found between these two groups as the concentrations were 0.133 +/- 0.027 microgram/g and 0.128 +/- 0.021 microgram/g, respectively. Further, no significant differences were noticed in the blood and intratumoral 5-FU concentrations between both groups after administration of UFT at a dose of 8 mg/kg. Clinically, the blood FT-207 and 5-FU concentrations after UFT administration showed no significant difference between the single UFT treated group and the combined OK-432 treated group, nor was there a significant difference in the intratumoral concentration. In summary, it is considered that OK-432 has little effect on UFT activation when UFT and OK-432 are administered in combination.

[Evaluation of adjuvant chemotherapy of osteosarcoma with special reference to adriamycin (final report)].

Adjuvant chemotherapy mainly with ADR performed in 117 patients (pts) with primary osteosarcoma of the extremity for the purpose of preventing pulmonary metastasis after radical ablative surgery. The mean follow-up period for 117 pts was 51.7 months (range: 3 to 137), for 53 survivors, 90.1 months (range: 60 to 137) and for 64 decreased, 20.5 months (range: 3 to 73). ADR was administered intravenously with 0.6-0.8 mg/kg/day for 3 consecutive days at monthly intervals after surgery until reaching 600 or 500 mg/m2 of the total cumulative dose. Five-year overall and disease-free survival rate of all pts was 50.2% and 39.4%, respectively. Thirty-seven pts (multi-drug group) with the combination of ADR and HDMTX had a higher survival rate (63.1% in 5-year overall survival rate and 47.8% in 5-year disease-free survival rate) than that of 80 pts with ADR alone (ADR group) (44.4% in 5-year overall survival rate and 35.6% in 5-year disease-free survival rate). Five-year survival rate for 65 pts administered the greater than 500 mg of ADR was 59.3% compared to 36.9% for 52 pts the less than 500 mg (p less than 0.05). In 65 pts administered the greater than 500 mg of ADR, 5-year survival rate (76.5%) of the multidrug group (17 pts) showed superiority to that 52.1%) of the ADR group (48 pts) (p less than 0.01). Even in the multi-drug group, 5-year survival rate (76.5%) of 17 pts administered the greater than 500 mg of ADR was higher than that (41.3%) of 20 pts given the less than 500 mg (p less than 0.01). Distant metastases were recognized at lung in 52 pts (lung group), lung + extrapulmonary organs in 14 (+ extragroup), and only extrapulmonary organs in 3 (extra group). Five-year survival rate of 66 pts with pulmonary metastasis was 17.1% and 21.2% in the lung group compared with 0% of the extra group (P less than 0.01). Five-year survival rate for 23 pts treated with thoracotomy was 43.5% compared to 2.6% for 43 without it (p less than 0.01).

[Developments in the experimental methods of evaluating hypoxic cell sensitizers].

Recent developments in the evaluation methods for hypoxic cell sensitizers were reported with emphasis on the following 3 points. (1) A new sensitizer-screening system consisting of the EMT 6 single cell, spheroid, and solid tumor was proposed by us and was found to be a useful, sophisticated system. (2) In the in vivo experiments, irradiation conditions for mice and the drug dose level influenced the hypoxic fraction of tumors significantly. Investigators must always take these facts into consideration when doing in vivo experiments. (3) Three new compounds recently developed in Japan proved to be superior to misonidazole.

[Clinical experience in the combined use of hyperthermia and radiotherapy].

The experience of thermoradiotherapy of superficial and deep-seated tumors with Thermotron RF-8 in the Kyoto University Hospital indicates the effectiveness of RF capacitative heating. CT scanning plays an important role in the assessment of thermoradiotherapy as it permits the detection of low density zones in a heated area. In such cases even in the absence of tumor regression, local tumor control is observed. It is confirmed by the retardation of tumor growth and histology findings.

Significance of extra 18q- chromosome in Japanese t(14;18)-positive lymphoma.

Karyotype evolution of t(14;18)-positive lymphoma was studied in 13 Japanese patients. The extra 18q- chromosome, found in six of ten patients with complex karyotypes, was the most common change subsequent to a t(14;18)(q32;q21) chromosome translocation. The additional change was interpreted as being a duplication of an 18q- derived from a t(14;18). The six patients had transformed histology of follicular small cleaved cell lymphoma or diffuse large cell lymphoma, and five of them had extranodal expansion associated with a poor prognosis. These findings indicate that the extra 18q-, together with other chromosome abnormalities, is closely associated with the advanced grade disease of t(14;18)-positive lymphoma, and the extra chromosome is evolutionally comparable with the second Philadelphia (Ph1) chromosome often found in the blastic phase of chronic myelocytic leukemia carrying a t(9;22)(q34;q11). In addition, since the extra 18q- is rarely found in American patients with t(14;18)-positive lymphoma, there appears to be a difference in the karyotype evolution between Japanese and American patients.

[Results of thermoradiotherapy of deep-seated tumors and problems in the evaluating its effects].

The results of thermoradiotherapy for deep-seated tumors indicated that 15% of the affected patients achieved CR and that the response rate (CR plus PR) was 58%. However, in some of the tumors which did not regress sufficiently, no regrowth was observed during follow-up. Furthermore, post-treatment CT scan revealed low-density areas occupying as much as 80% of the tumor area in 4 of 13 PRb tumors and 5 of 15 NRs. The appearance of these remarkable low-density areas on CT might be specific to thermotherapy, since it is uncommon following radiation therapy alone. Fifteen PR or NR tumors were resected and subjected to histopathological examination. It was found that cancer cells remained mainly in the tumor periphery in 12 of the 15 tumors while none were present throughout the entire tumor in the remaining 3 (2 PRbs and one NR). It was concluded from these results that it was possible to achieve local tumor control following thermoradiotherapy not only in CR tumors but also in some PRs or NRs which might remain unchanged in size but show prominent low-density areas on CT scan during follow-up. Accordingly, this suggests that the effects of thermoradiotherapy for deep-seated tumors should be evaluated on the basis of not only tumor regression but also the appearance of these low-density areas on post-treatment CT.
