A histogenesis of malignant lymphoma, small cleaved cell of the B cell type and intermediate lymphocytic lymphoma (mantle zone lymphoma). An immuno- and enzymehistochemical study.

We have studied the histogenesis of malignant lymphoma (ML), small cleaved cell of the B-cell type and intermediate lymphocytic lymphoma (mantle zone lymphoma) by comparing immunophenotypes and ALP-activity of neoplastic cells with those of germinal center cells (follicular center cells) and mantle zone (MZ) cells of secondary follicles in non-neoplastic lymphoid tissues. The neoplastic cells in 3 cases of ML, follicular, small cleaved cell and 1 case of ML, small cleaved cell expressed the phenotypes similar to those of germinal center (GC) B lymphocytes (SIgM+, B1+, B2+, CALLA+, SigD-, IL-2R-, Leu-1- and ALP-). The neoplastic cells in 2 cases of ML, follicular, small cleaved cell and 12 cases of ML, diffuse, small cleaved cell displayed the characteristic phenotypes of MZ B lymphocytes (SIgM+, SIgD+, BA-1+, IL-2R+, Leu-1+ and ALP+). The phenotypes of 2 cases of mantle zone lymphoma were closely comparable with those of MZ B lymphocytes. These findings indicate that the histogenesis of ML, small cleaved cell of the B-cell type is heterogeneous and can be divided phenotypically into 2 types (GC B lymphocyte origin and MZ B lymphocyte origin). It is also apparent that intermediate lymphocytic lymphoma (mantle zone lymphoma) is derived from MZ B lymphocytes of secondary follicles.

Phasic EEG activities associated with rapid eye movements during REM sleep in man.

Human cortical potentials associated with rapid eye movements (REMs) during REM sleep were recorded in 6 normal young adults by using the averaging technique. The potentials were compared with those recorded during voluntary saccades in the waking state in a dark or an illuminated room. A positive potential of relatively long duration with peak latency of about 200 msec and a wide spread bilateral distribution and of highest amplitude in the mid-parietal area appeared only during REM sleep. It is suggested that this positive potential is phasic EEG activity appearing in association with REMs during REM sleep in man. Comparison of this positive potential with P300s induced by task-irrelevant visual stimuli disclosed that they resemble each other. The possibility was discussed that this positive potential occurring in association with REMs during REM sleep is indeed a P300 appearing in response to visual images in dream.

Gamma-aminobutyric acid and taurine antagonize the central effects of angiotensin II and renin on the intake of water and salt, and on blood pressure in rats.

Antagonism by neuro-amino acids of the central effects of angiotensin II and renin in rats was investigated. Angiotensin II (100 ng), injected into the preoptic area, stimulated the intake of water but not salt, to a lesser extent and with a shorter duration as compared with that induced by renin (5 mU), injected into the preoptic area. This angiotensin II-induced intake of water was markedly inhibited by [Sar1, Ile8]-angiotensin II, an angiotensin II receptor antagonist, but not by captopril, an angiotensin-converting enzyme inhibitor, previously administered through the same cannula. The angiotensin II-induced intake of water was also inhibited in a dose-dependent manner by gamma-aminobutyric acid (GABA) (50-100 micrograms), muscimol, a GABA agonist, (100-200 ng), taurine (100-200 micrograms) and hypotaurine (100-200 micrograms), administered into the cerebroventricle and by GABA (5-10 micrograms), muscimol (10-20 ng) and taurine (10-20 micrograms) injected into the preoptic area in smaller doses. Renin (5 mU), injected into the preoptic area, elicited a marked increase in the intake of water and salt, which lasted for about 3 days. The effect of renin was inhibited by [Sar1, Ile8]-angiotensin II (10 micrograms) and was eliminated by captopril (25 micrograms) injected into the preoptic area. This effect of renin was not influenced by the peripheral administration of captopril. The effect of renin was also inhibited by GABA, muscimol or taurine injected into the cerebroventricle, in larger doses, or into the preoptic area in smaller doses.(ABSTRACT TRUNCATED AT 250 WORDS)

Cytogenetic characterization of a T-cell line, ATN-1, derived from adult T-cell leukemia cells.

A T-cell line, ATN-1, was established by culturing peripheral blood mononuclear cells derived from a patient with adult T-cell leukemia/lymphoma (ATL/L). Identities of the patterns of chromosomal abnormalities, cell surface phenotypes, morphologic findings, rearrangement patterns of T-cell receptor beta chain gene, and an integration site of human T-cell leukemia virus I proviral genome indicated that ATN-1 was derived from original leukemic cells. Both ATN-1 and the original leukemic cells showed a variety of patterns of chromosomal abnormalities that include 3q-, 6q-, rearrangements involving 2q31, 7q11.2, 8q11, 8q24, 19p13.3, and also 14q11 and 14q32, where genes for the T-cell receptor alpha chain and the immunoglobulin heavy chain are located. Availability of a genuine ATL/L cell line with these chromosomal abnormalities may greatly facilitate the biologic analysis of ATL/L.

Japan gastric trials in intraoperative radiation therapy.

Based upon our clinical results indications of intraoperative radiotherapy (IORT) for gastric cancer were summarized as follows: (a) The primary tumor must be surgically removed. (b) There must be no metastases to the liver or peritoneum. (c) Serosal invasion must be limited to the posterior wall of the stomach. IORT is not adaptable to patients in whom there is direct invasion of the peritoneum beyond the anterior wall because of the ease of peritoneal dissemination. (d) All unresectable lesions must be encompassed by a single radiation field. (e) No significant difference between cumulative survival of patients with Stage I gastric cancer who were treated by IORT or surgery alone was found. Therefore IORT may be of no benefit to the prognosis of patients with Stage I gastric cancer. As for the IORT dose, it is recommended that for clinically undetectable lesions a single dose of 28 Gy be delivered. For macroscopic remnants 30-35 Gy should be delivered depending upon the residual tumor size. The electron energy is selected so that the entire lesion is included by the 90% isodose line. When IORT is applied to a curative operation, the radiation field is positioned toward the lymph node groups around the celiac axis, which are hard to eliminate by a surgical procedure.

A new in vitro method using fura-2 for the quantification of endothelial cell injury.

Endothelial cell injury was determined by a new method based on measuring a specific release of fura-2, a fluorescent calcium indicator. We examined the cytotoxicity of 15-hydroperoxyeicosatetraenoic acid (15-HPETE) on cultured endothelial cells using the fura-2 assay. The new assay was compared to traditional assays for cytolysis such as measuring the release of 51Chromium (51Cr) or lactate dehydrogenase (LDH). The fura-2 method were able to detect minor and early phase membrane damage in endothelial cells due to the hydroperoxide. Significant release of fura-2 was detected by 5, 10, 25 and 50 mu/ml of 15-HPETE (5, 10 and 25 micrograms/ml p less than 0.01; 50 micrograms/ml, p less than 0.001) as early as 1 hour after the start of the 15-HPETE treatment. The fura-2 assay was so sensitive to the endothelial cell injury that the damage of the cells due to 15-HPETE at the dose of as low as 5 mu/ml could be detected in as early as 30 min. On the other hand, in the traditional 51Cr or LDH release assay, only the cell damage caused by high dose (50 mu/ml) of 15-HPETE could be detected in as late as 6 hours after the start of 15-HPETE exposure. The new method may be useful for the approach to searching injurious substances or their antagonists to several kinds of cells.

Simultaneous expression of activated lymphoid cell-associated and granulocytic cell-associated antigens on Hodgkin's and Reed-Sternberg cells in Hodgkin's disease.

The surface antigens of Hodgkin cells and Reed-Sternberg cells (H- and R-S cells), including lacunar cells, were analyzed with a large panel of monoclonal and polyclonal antibodies by an immunohistochemical method and an immunoelectron microscopic technique. H- and R-S cells in each histologic subtype of nodular sclerosis, mixed cellularity and lymphocyte depletion were stained similarly with anti-Leu-M1, anti-Leu-11b, TG8, anti-HLA-DR, anti IL-2R, RSC-1 (Ki-1) and anti-alpha-1-antitrypsin, but not with other antibodies examined. These findings suggest the following: (1) H- and R-S cells of nodular sclerosis, mixed cellularity and lymphocyte depletion are not heterogeneous, at least in terms of surface antigen expression, and (2) H- and R-S cells may be lymphoid cells which simultaneously express activated lymphoid cell-associated antigens (e.g., HLA-DR, RSC-1 and IL-2R) and granulocytic cell-associated antigens (e.g., Leu-M1 and TG8).

Treatment results of intracranial germinoma as a function of the irradiated volume.

Between 1962 and 1986, 70 patients were treated with radiation for confirmed or suspected intracranial germinoma at our hospital. The diagnosis was based on histology in 30 cases, cerebrospinal fluid (CSF) cytology in 12 cases, and on clinical and radiological findings in the remaining 28 cases. The target of radiation was the primary tumor site in 34 cases (Group A), the entire neuraxis in 22 cases (Group B), the whole brain in 4 cases (Group C), and the ventricle plus spine in 6 cases (Group D). Four patients were not included in the above groups for various reasons. The average radiation dose was 50-55 Gy to the tumor, 30 Gy to the whole brain, and 24 Gy to the spinal axis. The 5- and 10-year survival rates of the 68 primary cases in which radiotherapy was completed were 86% and 79%, respectively. The survival and relapse-free survival rates for the above 4 groups did not differ significantly, although slightly better results were seen in Groups B and C. Five cases in Groups A and D developed intracranial recurrence, 4 adjacent to the primary site but 1 distant from it, whereas no intracranial recurrence was found in the whole-brain-treated groups (B and C). One patient in Group B developed spinal metastasis, which was possibly due to inadequate radiation fields, and another in Group B developed abdominal metastasis via the shunt tube. Craniospinal irradiation should be administered to the patients with demonstrated meningeal seeding or with a positive CSF cytology. For cytology-negative cases with no evident metastasis, irradiation of the tumor plus a wide margin is usually sufficient, but craniospinal irradiation should be considered when the disease extends along the ventricular walls or is present in both pineal and suprasellar regions.

Microangiographic and histologic analysis of the effects of hyperthermia on murine tumor vasculature.

The effects of hyperthermia on murine tumor vasculature were studied by microangiography and histological examination. The tumors used were SCC VII carcinoma and mammary adenocarcinoma of syngeneic C3H/He mice. For the quantitative analysis of microangiographic changes, the percent (%) vascular area, which was defined as the percentage of opacified tumor vessel area to the entire tumor area, was determined in each microangiogram. The % vascular area after heating in a water bath at 44 degrees C for 30 min was minimized 24 hr after heating in both types of tumors. The histologic study revealed that the initial decrease of the % vascular area was due to congestion, thrombosis, and rupture of tumor vessels, and its subsequent increase was due to angiogenesis. SCC VII was more heat sensitive than mammary adenocarcinoma in terms of tumor growth delay, and tumor vessels of SCC VII were more vulnerable to heat than those of mammary adenocarcinoma. Histological examinations showed a marked difference in the architecture of vessels between the two types of tumors. Tumor vessels of mammary adenocarcinoma were supported by a connective tissue band, whereas those of SCC VII consisted of a single endothelial cell layer. Our findings suggest that the tumor vessels supported by a connective tissue band are less sensitive to heat than those without such support. The vascular damage of SCC VII was temperature dependent, and the critical temperature at which dramatic vascular damage appeared was between 42.7 degrees C and 43.7 degrees C.

Isolation and sequencing of a cDNA coding for the human DF3 breast carcinoma-associated antigen.

The murine monoclonal antibody (mAb) DF3 reacts with a high molecular weight glycoprotein detectable in human breast carcinomas. DF3 antigen expression correlates with human breast tumor differentiation, and the detection of a cross-reactive species in human milk has suggested that this antigen might be useful as a marker of differentiated mammary epithelium. To further characterize DF3 antigen expression, we have isolated a cDNA clone from a lambda gt11 library by screening with mAb DF3. The results demonstrate that this 309-base-pair cDNA, designated pDF9.3, codes for the DF3 epitope. Southern blot analyses of EcoRI-digested DNAs from six human tumor cell lines with 32P-labeled pDF9.3 have revealed a restriction fragment length polymorphism. Variations in size of the alleles detected by pDF9.3 were also identified in Pst I, but not in HindIII, DNA digests. Furthermore, hybridization of 32P-labeled pDF9.3 with total cellular RNA from each of these cell lines demonstrated either one or two transcripts that varied from 4.1 to 7.1 kilobases in size. The presence of differently sized transcripts detected by pDF9.3 was also found to correspond with the polymorphic expression of DF3 glycoproteins. Nucleotide sequence analysis of pDF9.3 has revealed a highly conserved (G + C)-rich 60-base-pair tandem repeat. These findings suggest that the variation in size of alleles coding for the polymorphic DF3 glycoprotein may represent different numbers of repeats.

Embolism of massive hepatic tissue in inferior vena cava and right atrium after closed liver injury.

Embolism from liver tissue in a major vessel in coincidence with a severe injury to the liver is a rare phenomenon. We report a case of a 4-year-old boy who was involved in a traffic accident and died shortly after the accident. An autopsy revealed that an intrahepatic inferior vena caval injury was accompanied by a severe closed injury to the liver; the crushed liver fragment was driven through the lacerated wall into the inferior vena cava and almost completely occluded its lumen. The crushed liver flap further extended over into the right atrial cavity. Careful microscopic examination of the lungs showed no emboli of hepatic tissue.

A novel troponin T-like protein (calponin) in vascular smooth muscle: interaction with tropomyosin paracrystals.

Calponin is a smooth muscle tropomyosin-binding protein which is antigenically related to troponin T. The fine localization of calponin and troponin T on the polar paracrystals of smooth muscle tropomyosin were studied by electron microscopy. Both calponin and troponin T bound to the filamentous tropomyosin at a site about 17 nm away from the C-terminal end of tropomyosin molecule. This calponin-binding site corresponds to the domain which is highly conserved in the amino-acid sequences among the tropomyosin isoforms from skeletal muscle and smooth muscle. The result suggests that calponin, in collaboration with tropomyosin, may play an important role in regulating contractile apparatus.

Alkaline phosphatase-positive B cell lymphomas.

Alkaline phosphatase (ALP) activity of 70 cases of non-Hodgkin's lymphomas of the B-cell type was studied. ALP activity was found in malignant lymphoma (ML), follicular, small cleaved cell (1/5 cases); ML, diffuse, small cleaved cell (3/13 cases); and mantle zone lymphoma (intermediate lymphocytic lymphoma) (2/2 cases). The ALP-positive neoplastic cells simultaneously displayed the characteristic immunophenotype of mantle zone (MZ) B lymphocytes of secondary follicle (SIg D+, BA-1+, IL-2R+ and Leu-1+). All other B-cell lymphomas, including ML, follicular, mixed small cleaved and large cell (9 cases); ML, follicular, large cell (4 cases); ML, mixed small and large cell (7 cases); ML, diffuse, large cell (27 cases); and ML, small noncleaved cell (3 cases), were consistently negative for ALP. The present study indicates that ALP-positive lymphomas including follicular lymphomas and diffuse lymphomas may be neoplastic counterparts of ALP-positive MZ B lymphocytes.

Somatostatin-secreting islet cell tumor (somatostatinoma): suppression of growth hormone (GH) release induced by GH-releasing hormone.

A patient with a somatostatin (SRIH)-secreting islet cell tumor, whose only symptoms were dyspepsia and anemia, is described. The diagnosis of somatostatinoma was based on high plasma SRIH concentrations and immunocytochemical findings. The pancreatic exocrine response to secretin was decreased, whereas the insulin and/or glucagon responses to glucose and arginine were normal. Although the basal plasma GH concentration was normal, the plasma GH response to GHRH was subnormal. Gel permeation chromatography studies indicated that SRIH-14 was the predominant form of SRIH in plasma as well as in tumor tissue.

Capillary permeability and boron distribution in ethylnitrosourea-induced rat glioma.

The vasculature and capillary permeability of gliomas induced by ethylnitrosourea in Sprague-Dawley rats were studied with horseradish peroxidase and Evans blue dye. The distribution of the boron-10 compound, Na2(10)B12H11SH, which is now in clinical use for boron neutron capture therapy (BNCT) for brain tumors, was investigated quantitatively using neutron-induced alpha-autoradiography. The vasculature and the degree of capillary permeability varied widely, depending mainly on the size of the glioma, and were often heterogeneous even in the same tumor. The distribution of boron-10 also varied, correlating to capillary permeability. The boron-10 concentration and the tumor:blood concentration ratio in large and medium-sized gliomas were adequate for successful BNCT. This study suggests that the vasculature and capillary permeability of the target brain tumor exert an important influence on the therapeutic efficacy of BNCT.
