Value of the minimum bactericidal concentration of antibiotics in the management of a case of recurrent Streptococcus bovis septicaemia.

After prostatectomy a 60-year-old man developed Streptococcus bovis septicaemia. The patient did not respond to treatment with a combination of ampicillin and erythromycin and in vitro antagonism between these antibiotics was demonstrated by minimum bactericidal concentration (MBC). The value of determining the MBC of antibiotics used in the treatment of septicaemia is emphasised.

Perioperative infarction: effects of cardiopulmonary bypass on collateral circulation in an acute canine model.

Significant intracoronary collateralization [circumflex (Cx) to anterior descending (AD)] can be established acutely by selective hypertensive perfusion of the circumflex (through left main) after acute occlusion of the AD in working canine hearts. Intracoronary collateral flow can be estimated by a collateral circulation index (CCI) = distal coronary diastolic pressure x 100/aortic diastolic pressure. The effects of 60 to 90 min of normothermic, total cardiopulmonary bypass (TCPBP) on such collaterals were evaluated in four dogs with CCI > 80%, and compared to six control dogs with CCI < 80% and without TCPBP. Three hours after AD ligation, control dogs had minimal myocardial injury despite significantly lower CCI. In contrast, the four study dogs placed on TCPBP experienced an abrupt fall of the CCI to 30% (P < 0.03), severe ischemia, and infarction in each; two became pump dependent. Thus, ventricular pressure work seems to be an important determinant of flow through collateral channels. These results suggest that preservation techniques for myocardium supplied by collaterals during coronary surgery may require considerations other than intermittent perfusion via existing coronary inflow.

Unicystic ameloblastoma: a prognostically distinct entity.

The occurrence of unicystic ameloblastoma has been studied in 20 patients presenting with unilocular cystic lesions whose clinical, radiographic and gross features were those on non-neoplastic cysts. In the majority of these, the lesion mimicked dentigerous cyst. The rate of recurrence for this group of lesions as determined by long term follow-up observation available for the majority of patients was distinctly lower than that associated with multicystic character of the lesion throughout its course, is indicative of a much less aggressive variety of neoplasm. The adequacy of simple enucleation as a modality of treatment in the majority of patients with this type of lesion is suggested. It was not possible to answer the question whether or not the ameloblastoma began in antecedent non-neoplastic cyst.

Multinucleated giant cell neoplasm of pancreas: light and electron microscopy features.

The morphological features, as demonstrated by electron microscopy, of a rare pancreatic neoplasm indistinguishable from giant cell tumor of bone by light microscopy is presented. These findings are compared with those of the only other case studied by electron microscopy and reported. Features were noted in both studies that are strongly suggestive of the epithelial origin of this neoplasm. However, there were similarities between the material from the current case and giant cell tumor of bone. Degree of differentiation seems to be the most likely determinant of the degree of similarity of the pancreatic neoplasm to the skeletal lesion.
