[Analysis of female infertility structural factors].

The study was aimed to evaluate, within the population of infertile women, the interrelations between such characteristics as patient's age, duration and type (primary, secondary) of infertility, its etiologic factor as well as the treatment outcome (pregnancy rate). Retrospective analysis was performed in 176 women aged 20-43 years (mean age 29.5 years). Primary infertility was less common in older women (above 35 years). Duration of primary infertility was independent of age, on an average 1.5 years shorter compared to secondary infertility. Participation of all causal factors in the structure of infertility was steady--11.6-20.2%. The most frequent reasons of secondary infertility were unexplained infertility 55.3% and tubal pathology--19.1%. The pregnancy rate was 30% in all treated women. In the studied population of women, pregnancy rate was not significantly influenced by the patients's age, type of infertility and its cause.

[The naloxone test in the degenerative changes of seminiferous tubules].

AIM: Are there any changes in hypophyseal and gonadal hormones levels during the naloxone test in the degenerative changes of the seminiferous tubules? MATERIAL AND METHODS: The naloxone test (0.4 mg i.v.) was performed in 13 patients with degenerative changes of the seminiferous tubules. The plasma FSH, LH, prolactin, testosterone and estradiol levels were determined before and 30, 60, 90, 120 minutes after the administration of the drug. RESULTS: In the examined group of patients the plasma FSH, prolactin and estradiol levels were significantly higher, testosterone levels significantly lower and LH levels were in the same range compared with the patients with azoospermia and normal spermatogenesis. The plasma FSH, prolactin and testosterone levels did not change significantly their values during the test. The plasma estradiol levels decreased significantly and plasma LH levels increased significantly during the test. CONCLUSION: Elevated plasma FSH, prolactin and estradiol levels, diminished testosterone levels and the decrease of plasma estradiol levels and the increase plasma LH levels during the naloxone test indicate the degenerative changes of the seminiferous tubules in the cases of azoospermia.

[The naloxone test in patients with varicocele].

AIM: Is there any influence of naloxone administration on plasma levels of hypophyseal and gonadal hormones in varicocele patients? MATERIAL AND METHODS: The naloxone test (0.4 mg i.v.) was performed in 15 varicocele patients The plasma FSH, LH, prolactin, testosterone and estradiol levels were determined before and 30, 60, 90, 120 minutes after the administration of the drug. RESULTS: The prolactin levels decreased significantly, and FSH, LH, testosterone and estradiol levels did not change their values. The similar results were obtained in patients with azoospermia and normal spermatogenesis. CONCLUSION: The treatment of varicocele patients with anti-opioid drugs is not recommended because the naloxone did not increase LH and testosterone plasma levels.

[Doppler flowmetry measurements in testicular artery of aging men].

AIM: Testicular artery Doppler flow measurements in aging males were performed for the evaluation of eventual disorders of vascular flow in vessels supplying the testes. MATERIAL AND METHODS: 40 men aged between 51 and 80 years (studied group) and 40 healthy men aged below 40 (control group) were examined. The volume of testes and qualitative flow indices (S: D, PI, RI) in terminal section of testicular artery were measured. The difference between particular values obtained in both groups and the correlation of those parameters with testes volume and patients' age was determined. RESULTS: PI and RI values were significantly higher in studied group when compared to the control one. The correlation coefficient between values of flow indices and testes volume as same as patients' age appeared to be unsignificant in all cases with the only exception in case of S: D and testes volume in studied group. CONCLUSION: The values of qualitative flow indices indicate the increased vascular resistance in testicular artery of aging males.

[Computer-assisted analysis of cardiotocographic recordings in the course of fetal hemolytic disease].

OBJECTIVES: The aim of this study was to assess the usefulness of computerized cardiotocography in the diagnostics of fetal hemolytic disease. STUDY DESIGN: 50 invasive diagnostic and therapeutic procedures proceeded by computer assisted CTG recording were analysed. The values of particular parameters of CTG were compared with adequate values obtained in uncomplicated pregnancy. The correlation between both cardiotocographic and hematological parameters was estimated. The common relationship between all analyzed features was examined using multidimensional analysis of variancy. RESULTS: The most significant correlation was determined in case of short-term variation and hematocrite of fetal blood. The five-feature algorhythm, containing short- and long-term variation, number of accelerations, fetal movements and gestational age, was established for the prognosing of the most probable level of fetal hematocrite. CONCLUSION: Computerized cardiotocography seems to be the useful diagnostic method which enable to decrease the number of invasive intrauterine procedures in case of fetal hemolytic disease.

Lipids in serum of patients with malignant ovarian neoplasms.

OBJECTIVE: In numerous papers, different associations of serum lipids and lipoproteins with neoplastic diseases were found. The aim of our work was to find out associations between levels of 11 serum lipids, lipoproteins and ovarian cancer. METHODS: 25 healthy women and 25 patients with ovarian cancer underwent examinations. Uni- and multivariate analysis of variance and discrimination analysis were used to analyze our results. RESULTS: The analysis demonstrates that ovarian carcinoma is associated with a significant reduction of total cholesterol and its esters in serum and in high density lipoprotein fractions compared to controls. CONCLUSION: It could be shown that using multivariate analysis of variance it is possible to find the optimal set of serum lipid parameters, containing serum esterified cholesterol, serum total cholesterol and high density lipoproteins esterified cholesterol, to differentiate healthy persons from patients affected by ovarian cancer.

Comparison of Hysterosalpingography with Laparoscopy and Chromopertubation

We compared two methods of determining tubal patency. Between January 1, 1993, and December 31, 1994, 55 women were evaluated for infertility by laparoscopy and chromopertubation and either traditional hysterosalpingogram (HSG; group 1, 25 patients, 16 primary, 9 secondary infertility) or a mixture of Hy-Co-Sy with Echovist (group 2, 30 patients, 14 primary, 16 infertility). In group 1 HSG showed 64% bilateral patency, 8% unilateral patency, and 28% bilateral occlusion; laparoscopy and chromopertubation showed 72%, 8%, and 20%, respectively. Identical findings were present in 92% of women, with method sensitivity and specificity of 100% and 89.4%, respectively. One case of allergic reaction occurred. In group 2, rates with Hy-Co-Sy and Echovist were 63.3% bilateral patency, 23.3% unilateral, and 13.3% bilateral occlusion; laparoscopy and chromopertubation results were 83.3%, 10%, and 6.7%, respectively. Identical findings were present in 86.7% of patients, with method sensitivity and specificity of 100% and 84.9%, respectively. No side effects were noted. The high sensitivity of both methods suggest that either one is of value in primary assessment of tubal patency, as neither requires general anesthesia and both may precede laparoscopy of hysteroscopy.

Pregnancy and delivery after liver transplantation.

The first case of pregnancy in a patient after an orthotopic liver transplantation (OLT) in Poland is presented. A 21-year-old woman was liver grafted 3 years prior to the pregnancy. Before having conceived the patient's graft function was stable. The woman was on immunosuppressive therapy with cyclosporine A and prednisolone. During pregnancy no significant changes in biochemical tests of liver function and liver blood flow were noted. Starting at the second trimester, a slight anemia occurred and the quantity of blood platelets continued to decrease, the latter having been observed immediately after the transplantation. The intrauterine growth of the fetus was monitored by ultrasound and the assessment of blood flow to the placenta was made. No abnormality was observed. In the second and third trimester the presence of HCV-RNA in the serum was found. In the 41-st week of pregnancy labor commenced. The threat of intrauterine infection indicated a cesarean delivery. The newborn weighed 4180 g and had an Apgar score of 10. The cesarean section, as well as puerperium, was normal. The immunosuppressive therapy was continued, and antibiotics were administered for prophylactic reasons. During the first month the infant was treated with antibiotics because of pneumonia and the suspicion of meningitis. Nine months after the delivery, the patient's health is satisfactory and the baby is making normal progress.

[Clinical effectiveness of Marvelon].

Clinical results of a new contraceptive pills--Marvelon (Organon) were studied. 32 women, ages 30 +/- 5 years, participated in the study during at least 6 cycles. Marvelon was good tolerated and subjective side-effects were very low. The cycles were regular, duration of withdrawal bleeding was 3-5 days and the amount of the blood lost were unchanged or milder. Marvelon didn't affect body weight and blood pressure. Favourable lipid profile--increased a mean level of HDL-Cholesterol and decreased a mean level of LDL-Cholesterol were noticed. Pearl Index was 0,0.

[Occurrence of Bacteroides fragilis strains in full term and post term pregnancies].

Bacteroides fragilis (B.f.) is an aetiological agent of gynecological and obstetrical infections. The aim of this study was to determine occurrence of B.f. in women genital tract. A total number of 118 women in labor between 38 and 43 gestation age and not demonstrating clinical symptoms of infection was examined. B.f. strains were isolated from 9 women (15.9%). More often B.f. strains were isolated from women delivering post-term and staying previously in Pregnancy Pathology Ward. It was observed that infant mean weight on the 3rd day of life was higher in the vagina. The following features of isolates were compared: presence of the capsule and agglutination of sheet erythrocytes. Differences in haemagglutination and susceptibility to antibiotics were observed.

[Clinical treatment results of treating androgenization symptoms in women with preparation Diane-35].

26 women with androgenization: hirsutism, acne, seborrhea, aged 18-30 were treated with Diane 35 during at least 12 cycles. In investigated group of patients acne was in remission in 82.4% after 3 cycles, seborrhea was reduced in 85.5% after 3 cycles and in 96.2% after 6 cycles. Hirsutism was reduced only in 19.2% women after 12 cycles. Diane 35 was well tolerated and effective in contraception all investigated women.

[Clinical observations using the new contraceptive preparation containing gestagen].

31 women aged 26-35 were observed during hormonal contraception with Femovan (in Poland registered as Femoden). The aim of the study was an analysis of effectiveness, influence on menstrual cycles and adverse effects in Femovan users. The conclusions of the study indicated that Femovan is very effective in contraception (Pearl index = 0). It provides good cycle control. It reduced in 50% women nausea and breast tenderness which occurred before starting of contraception. There were not body gain and elevated blood pressure in any women. Acne was reduced in 40% women after 3 months of using.

[Toxoplasmosis as a risk factor in pregnant women after renal transplantation].

Toxoplasmosis is an anthropo-zoonosis caused by the protozoa Toxoplasma gondii. The frequency of infection in the population of adults is estimated at 30-50%. Acquired toxoplasmosis takes a differentiated clinical course. It is usually symptomless or the symptoms are indistinct. In case of immunosuppressive treatment of the host severe Toxoplasma gondii infections can occur. There also is a possibility of transmission of the protozoa via transplant from the donor to the recipient as well as the possibility of reinfection of the acquired infection. In perinatal medicine toxoplasmosis plays a particularly important part due to the possibility of the transmission from mother to foetus. It seems therefore that women who are chronically treated with immunosuppressive eg after a renal transplant deserve special medical attention.

[Results of treating endometriosis with nafarelin].

50 women with endometriosis diagnosed during laparoscopy or laparotomy were given six months treatment of nafarelin 400 mcg/day. Before the treatment and after its completion endometriosis was classified according to the AFS scale. Symptom Severity Score numerical scale was also used. After the completion of nafarelin treatment a decrease in AFS scores from 19.6 to 10.2 was seen. The complete withdrawal of clinical symptoms was seen in 52% of patients, and partial withdrawal in 42% of patients. The mean of the Symptom Severity Scores (dysmenorrhoea, dyspareunia, lower abdominal pain and pelvic exam abnormalities) has decreased from 7.1 to 2.1 points which is a 70% decrease. These results show the high efficacy of nafarelin in endometriosis treatment.
