[Ovarian function in women after kidney transplantation].

The full recovery of reproductive age patients suffering from chronic renal failure due to a successful transplant should include the restoration of normal reproductive functions. In the study data are presented concerning the resumption of menstruation and the evaluation of the ovarian function of renal transplant recipients. After a successful renal transplant the ovarian function improves considerably but isn't always fully restored which can be attributed to the renal efficiency grade or result from the administered immunosuppressive treatment. Approximately 40% of the patients have ovulatory cycles with a normal length of the luteal phase. 40% have also ovulatory cycles but the luteal phase is shorter and the progesterone values are lower. The remaining patients have anovulatory cycles with low estrogen values and a high FSH and LH concentration.

[Relationship between decreased activity of NK cells in umbilical cord blood and infection incidence in children].

In our study the natural cytotoxic activity of the umbilical blood of healthy newborns born in 1987 was examined. All mothers of the examined children were healthy and pregnancy was normal. Blood was drawn from the umbilical vein immediately after the delivery to the heparinized tubes. Mononuclear cells of the blood were isolated using the modified Boyums method. The mean activity of the NK cells was 8.6 +/- 1.5 (SE), and was lower than in the studies by other authors. Three years after the delivery all mothers of the examined children got the questionnaire about the health of their babies. After the analysis of the results, the correlation between lowered NK cells activity in the umbilical blood and the morbidity of the children was shown.

[Clinical analysis of patients surgically treated for cancer in the body of the uterus].

107 patients, aged 40 to 87, after surgical intervention with diagnosis of uterine corpus cancer were clinically examined. 68.1 percent of them were 51 to 70 year old women. 20.5 percent had been never pregnant, and 22.47 been pregnant for one time. In 93.2 percent, corpus cancer was revealed after menopause. Among risk factors, there was observed: diabetes mellitus in 7.4%, hypertension in 35.5%, and obesity in 78% of cases. It was stated interdependence between the depth of uterine infiltration, parametrium metaplastic focuses, adnexa metaplastic focuses, cervix infiltration and decrease of adenocarcinoma maturity. The concord between biopsy and clinical stage I degree was stated in 86 percent of cases.

[Usefulness of hysteroscopy in the diagnosis and treatment of intrauterine adhesions].

During hysteroscopy intrauterine adhesions were found in 96 patients in a group of women with disturbances of fertility and menstruation. The diagnosis and treatment were carried out with a Wolf hysteroscope using CO2 as the expanding gas. The purpose of the study was evaluation of the effectiveness of treatment of these adhesions by means of hysteroscopy. Normal menstrual bleeding after this treatment was achieved in 75% of the treated patients, and 51% of the patients conceived. Hysteroscopy was useful in the diagnosis, treatment and posttreatment control in cases of Asherman adhesions.

[Results of treatment with ofloxacin for inflammatory conditions in male reproductive organs].

The results are presented of the clinical and microbiological studies of the patients of the Andrological Outpatient Clinic in Warsaw, who were examined for explaining the causes of marital infertility or for elucidation of the cause of testicular pain. Thirty-two men aged 21 to 42 years were chosen, in whom semen investigation demonstrated presence of leucocytes in numbers exceeding 10 per field of vision and with clinical signs of inflammatory process in the male reproductive organs (10 patients) and without clinical signs (22 patients). The patients in whom semen cultures demonstrated presence of bacteria sensitive to ofloxacin were treated with the preparation Tarivid Hoechst for 10-20 days in daily doses of 2 x 200 mg. The clinical signs of inflammation of the reproductive organs regressed in 100% of cases, and in 64% of cases no leukospermia was found after the treatment.

[Ambulatory laparoscopy as an alternative to hospitalization].

Laparoscopy is a new way of activity in gynecological diagnostics and therapy, but it is still hardly acceptable in our country. It is caused by long-lasting habits of performing traditional and more invasive treatment. One of directions in development of gynecological diagnostics is performing laparoscopy in out-patient conditions, or in conditions close to hospital in out-patients units. Our study shows the range of indications to performing this kind of procedures. Possible late complications after laparoscopy are described, which in author's opinion should not be a contraindication to performing these procedures.

[Usefulness of hysteroscopy in diagnosing sterility].

Sterility is a severe social problem. Using hysteroscopy, which is possible thanks to development of medical technology, seems to be an important improvement in diagnostics and therapy of sterility. The aim of this study is evaluation of the usefulness of hysteroscopy in diagnostics of sterility. There was a group of 106 women with diagnosed primary sterility and 82 women with secondary sterility. Wolf and Storz hysteroscopes were used during examinations, whereas the uterine cavity was dilated by CO2 using Wolf Metrometer 2121. During this examination a normal uterine cavity was found in most cases (32.9%). The next findings were: endometrial polyps (27.6%), intrauterine adhesions (23.4%), sub-serous myomas (14.3%), and congenital uterine abnormalities (1.5%). Comparing outcomes of hysteroscopy in both groups of sterile women, the high rate of intrauterine adhesions in the secondary sterility (43.9%) was detected. Performing hysteroscopy in patients with sterility increases considerably the range of informations about reproductive organs. Also the possibility of eye-controlled biopsy, deliberation of intrauterine adhesions or removing of endometrial polyps during endoscopic procedure increases the value of hysteroscopy.

[Diagnosis of fetal hemolytic disease and therapeutic possibilities].

The possibility of fetal blood sampling and direct blood transfusion into fetal circulation began the new direction in diagnostics and treatment of fetal haemolytic disease. Early diagnostics and possibilities of intrauterine treatment of the fetus enabled effective treatment also in severe hydrops cases. In the article diagnostic and therapeutic scheme introduced in the I Department of Obstetrics and Gynaecology and Central Rh-disease Unit in Warsaw was presented.

[Analysis of congenital malformations in newborns].

We analyzed data from 194 newborns with congenital malformations of central nervous, cardiovascular, gastro-intestinal, skeletal, genito-urinary systems and Down's syndrome. We detected that the highest frequency of congenital malformations occurred from may 1986 to december 1986. Congenital malformations were detected more frequently in female newborns and newborns delivered by multipara. The percentage of CNS malformations was constant but at the same time the number of other malformations were significantly changed. We made an attempt of statistical analysis of frequency and type of malformations taking into consideration possible influence of radiation during analyzed period.

[Evaluation of the diurnal rhythm of secretion of chorionic gonadotropins in the first trimester of normal pregnancy].

The authors evaluated the diurnal rhythm of the secretion of hCG in patients in the first trimester of normal pregnancy. The examination was carried out in a group of 22 patients (chosen at random) who came to hospital to undergo abortion (for social indications). The patients stayed in normal hospital conditions and were not given any drugs. All of them were informed of the aim and method of the examination and agreed to it. The blood to be examined was taken every two hours during 24 hours. The determination of hCG was made by means of the radioimmunological method. The statistical analysis was made by means of the Halberg cosinor method. In 14 patients the authors found the existence of the diurnal rhythm (p less than 0.05), in 4 cases the rhythm was circadian (p less than 0.1), in 4 remaining patients no rhythm was found. For the values obtained from 14 patients with a clear diurnal rhythm the authors drew an ellipse of errors by the Halberg method. The ellipse does not cover the centre of the cosinor circle, which confirms the compatibility of the results obtained. The medium value of the amplitude in this group was 20.72% of mesor and acrophase appeared at 16(01).

[Nausea and vomiting in pregnancy].

Nausea and vomiting are frequent disturbances in the first trimester of pregnancy, sometimes they may take the form of hyperemesis, even life-threatening. The authors survey the concepts of the aetiology and pathogenesis of the condition: hormonal, metabolic, psychological. Principles of management using simple methods reducing the ++ill being of the pregnant patients are presented, together with pharmacological management of hyperemesis. The suggested therapeutic methods seem to be relatively safe for the fetus and practically eliminate the necessity of pregnancy interruption for medical indications.
