[Effects of oral administration of estrogen replacement therapy in surgical menopause].

OBJECTIVE: The aim of this study was to analyze effects of oral administration of conjugated estrogens in women after surgical menopause. MATERIAL AND METHODS: A prospective study that included 41 women after hysterectomy with bilateral oophorectomy, was performed. All patients received conjugated estrogens (Premarin) 0.625 mg daily from second or third days after surgery. Plasma level of estradiol, FSH and several homeostatic parameters were evaluated before and after 2, 5 days and 6 weeks of hormone therapy. The symptoms of climacteric syndrome were estimated based on the scale of Kuppermann. RESULTS: A decrease in plasma estradiol levels in early postoperative days and an increase up to 100 pg/ml in estradiol concentration after 6 weeks of estrogen therapy were observed. Changes in platelet number, plasma concentration of coagulation factors were transient and there were no significant thrombophilic effects after 6 weeks of Premarin administration. CONCLUSIONS: The postoperative oral administration of conjugated estrogens did not cause thromboembolic complications among women after surgical menopause. Early beginning of estrogen replacement therapy after hysterectomy may prevent symptoms of climacteric syndrome occurrence.

[The atherogenic lipid profile parameters and fibrinogen serum concentration in climacteric women under various routes of hormone replacement therapy administration].

OBJECTIVES: Influence of various routes of HTR on atherogenic lipid profile parameters and serum fibrinogen concentration was investigated. DESIGN: In 85 women in four groups receiving HRT transvaginally, transdermally, orally and intramuscularly, the total cholesterol, HDL-cholesterol, triglycerides, apoB, apoA, LpA and fibrinogen serum concentration was assessed before treatment and after six month. RESULTS: Most of tested parameters: total cholesterol, LpAI, apoB, LDL cholesterol, non HDL cholesterol, changed favorably (lowered concentration) in group using HRT orally and intramuscularly. CONCLUSION: Orally and intramuscularly route of administration HRT have better cardioprotective effects.

Granulocyte chemiluminescence activity, antibody production and percentage of CD4(+) and CD8(+) lymphocytes in peripheral blood of offspring of salbutamol-treated pregnant C3H mice.

Preterm delivery is one of the most important problems in obstetric care. One of commonly used treatment of such high risk cases is salbutamol-beta(2) adrenoceptor agonist. The aim of present study was to determine if such treatment causes any changes in neonatal immune system and therefore should be considered in newborn care. The experiments were performed in 4-5- and 6-7-week-old female and male offspring of salbutamol treated C3H inbred mice. In the present study chemiluminescent activity of peripheral blood granulocytes, percentage of CD4(+) and CD8(+) lymphocytes and antibody production were evaluated. A lower number of peripheral blood granulocytes in 6-7-week-old offspring of salbutamol treated mothers was observed, while in the case of younger mice's lymphocytes count in both groups, the differences were not significant as compared to control group. In 4-5-week-old mice a lower percentage of CD4(+), CD3(+) and CD8(+) was evaluated, while in older offspring the percentage of CD4(+) and CD3(+) was higher in the case of the progeny of salbutamol treated mothers. As far as chemiluminescent activity was concerned no differences were found in any of experimental groups. We showed higher IgM production both in male and female offspring of the experimental group and no changes in IgG levels in mice sera. Alterations observed in progeny of salbutamol treated mice might have influence on their further immune system development and function.

Thymus and lymph node cell CD4(+) and CD8(+) marker expression and their angiogenic activity of offspring of salbutamol-treated pregnant C3H mice.

Salbutamol, beta(2)-adrenoceptor agonist is a first choice drug in preterm delivery treatment. The aim of the present study was to determine whether salbutamol can cause any alterations in neonatal immune systems and therefore should be considered in newborn care. The experiments were performed on 4-5- and 6-7-week-old female and male offspring of salbutamol-treated C3H inbred mice. Thymus and lymph node weight, cellularity and lymphatic organs, lymphocytes phenotypes and their angiogenic activity were evaluated. We observed lower thymus weight in 6-7-week-old progeny of salbutamol-treated mothers and in the same time lower thymus cell number in both age groups. Lower lymph node weight was developed in older progeny while cellularity was diminished both in 4-5- and 6-7-week-old offspring of salbutamol-treated mothers. We have not detected any changes in percentage of CD4, CD8, CD3 and CD4CD8 positive lymphocytes in progeny of salbutamol-treated mothers. As far as lymph node lymphocytes phenotype is considered we showed in both age groups lowering of CD4 and CD3 positive cells in experimental groups. In the LIA test (lymphocyte-induced angiogenesis) we showed lower lymph node cell angiogenic activity of salbutamol-treated mothers' progeny in both age groups. In the case of thymus lymphocytes we have not observed any alterations in their angiogenic activity. The differences in histological examination of thymus and lymph nodes were not detected in experimental and control groups.

[Feto-maternal immunization and fetal hemolytic disease: invasive diagnostics and therapy in the Warsaw center of serological collision in years 1992-1999].

OBJECTIVE: The objective of this study was the assessment of intrauterine procedures, which are performed during the diagnostic phase and therapy of fetal hemolytic disease in the Warsaw Center of Serological Collision. MATERIALS AND METHODS: The study covered 146 women, who were patients of the Center from 1992 to 1999. Analyzed were the time of commencement of diagnostic and treatment, type of operations, duration of therapy, time and manner of delivery after the past intrauterine procedures, the medical condition of infants born as a result of these deliveries and the necessity for blood treatment in the early neonatal stage. RESULTS: 451 intrauterine procedures were carried out, of which 159 were diagnostic punctures of the umbilical vessel and 292 were fetal transfusions. The diagnostics and treatment was performed between the 19 and 38 week of pregnancy. The number of punctures for each patient ranged from 1 to 4, and 1 to 10 for intrauterine transfusions. The therapy, which on average lasted 42 days, significantly improved the fetal blood morphology results. During the therapy 5 intrauterine demises were noted--3 as a result of advanced fetal hemolytic disease and 2 related to operation complications. The average pregnancy duration in the analyzed group was 36.4 for treated women and 37.4 weeks for diagnosed women. 52.7% of deliveries were vaginal, 47.3% were performed by cesarean section. The general condition of 87% of infants was good. However, 77% of infants required blood treatment during the early neonatal period. CONCLUSIONS: Invasive methods of fetal hemolytic disease diagnostics and intrauterine therapy assure high efficiency in the guiding of serological collision and fetal hemolytic disease conflicted pregnancies. At the same time, these methods are safe and carry a low risk of complications.

[Utero-ovarian flow velocity waveforms in patients after renal transplantation].

The aim of this study was to estimate the utero-ovarian flow velocity waveforms in patients after renal transplantation. The study material consisted of 24 women aged 28-44 years which underwent renal transplantation and 26 healthy controls. All women had normal menstrual cycles. Using transvaginal approach PSV, RI and PI were measured both in uterine and intraovarian arteries. There were no differences in PSV in both groups. The group of ranal transplants had significantly decreased PI and RI measurements on the side of renal placement when compared to the other side or controls. We conclude that renal transplantation influences blood flow in pelvic vessels by lowering resistance on the side of transplantation.

[The effectiveness of prenatal steroid and ambroxol therapy for the prevention of respiratory distress syndrome in newborns of diabetic mothers].

The aim of the study was to determine the effectiveness of antenatal steroids and ambroxol in reduction of respiratory distress syndrome (RDS) in newborns of diabetic mothers. A retrospective chart review was done on 101 diabetic gravidas who were hospitalised at 1st Department of Obstetrics and Gynaecology Medical University in Warsaw between January 1989 and December 1999. We studied the neonatal outcomes for women with diabetes mellitus who delivered before 37 gestational weeks depend on exposition to antenatal steroids or ambroxol. Both groups were compared with unexposed neonates for respiratory distress syndrome. Frequency of occurrence and severity of RDS were similar in all study subgroups. We did not found reduction of respiratory distress syndrome in newborns of diabetic mothers exposed to antenatal steroids or ambroxol.

[The influence of prenatal glucocorticoids on glycemic control among pregnant women with type I diabetes].

The purpose of this study was to determine the impact of glucocorticoids treatment, administered for a severe risk of premature delivery. The study material consisted of 30 diabetic mothers treated with intensive insulin therapy. The effects of steroid treatment on glucose level two days prior, one day prior, during 1-, or 2-, or 3-days therapy and the first 2 days following treatment were determined. No case of ketoacidosis was reported.

[Results of clomiphene citrate ovarian stimulation vs laparoscopic electrocoagulation of the ovaries in infertile women with PCOS].

OBJECTIVE: To assess the results of clomiphene citrate ovarian stimulation vs laparoscopic electrocoagulation of the ovaries in infertile women with PCOS. DESIGN: 47 women with PCOS were included into the study. 25 of them had ovarian stimulation using clomiphene citrate (100 mg for 5 consecutive days, during 6 cycle) and 22 underwent laparoscopic electrocoagulation of the ovary (6 cycle follow-up) No significant differences were detected between age and body mass index. RESULTS: In the first group ovulatory cycles were confirmed in 68% of women, pregnancy rate was 28%. In the second group ovulations were observed in 90.9% of women and pregnancy rate was 63.6%. There were statistically significant differences. CONCLUSION: In summarising, the results showed lower, statistically significant differences in pregnancy rate in the treatment of infertility associated with PCOS. It seems that the much more successful result of the PCOS therapy may be achieved after laparoscopic ovarian diathermy in comparison with stimulation of the ovaries with CC.

[Serum VEGF (vascular endothelial growth factor) concentration in patients with endometriosis].

The theory of Sampson that endometrial cells and fragments desquamated during the menstrual period are transported through fallopian tubes into the peritoneal cavity where they implant, proliferate and develop into endometriotic lesions is generally accepted. Accumulating data suggest that deficient immunity against retrograde endometrium during menstruation may be involved in the pathophysiology of endometriosis. Recent studies in women with endometriosis demonstrated functional changes in several immunologic components in the peritoneal fluid as well as in sera of those patients. Among others it was shown that a wild pattern of cytokines take part in events occurring during endometrial cells implantation, proliferation and forming of endometriotic lesions. One of them VEGF seems to play a very important role in neovascularisation and implantation of ectopic endometrial lesions. In present study we evaluated the concentrations of VEGF in serum of patients with endometriosis and showed negative correlation between AFS score and VEGF concentration in peritoneal endometriosis. Above results do not confirm former observations indicating the role of VEGF in endometriosis pathogenesis.

[Electroresectoscopy in submucous fibroids, intrauterine adhesions and uterine malformation treatment].

182 hysteroscopic operations with electroresectoscope use performed in Ist Department of Obstetrics and Gynaecology (Warsaw) between 1996-1999 were analyzed. Myomectomy was performed in 117 patients, resection of uterine septum in 25 and deliberation of intrauterine adhesions in 40 patients. Abnormal bleeding and infertility were the main indications for hysteroscopy. In 70% of patients normalization of menstrual bleeding was obtained and 24% of patients got pregnant. In 3 cases uterine wall perforation was noted, increased uterine bleeding was observed in 7 patient and there was only one case of fluid overload.

[Testicular artery Doppler flow evaluation in men with degenerative changes of seminiferous tubules].

AIM: The aim of this study was to assess the eventual impact of degenerative changes of seminiferous tubules on Doppler flow parameters in testicular vessels. MATERIAL AND METHODS: There were 12 men with degenerative changes of seminiferous tubules (diagnosed by testicular biopsy) in study group and 24 healthy men with normal semen parameters in control group. Patients' age in both groups was respectively 32.22 +/- 5.31 and 29.64 +/- 3.17 (ns). Sonographic biometry of testes volume was performed and the values of qualitative flow indices in terminal section of testicular artery were evaluated. The both groups results were compared with themselves by T-test. Also the values of qualitative flow parameters in testis after biopsy and contralateral one were compared. Additionally the correlation between vascular flow parameters and selected hormone levels (FSH, LH, T, E2 and Prl) was determined. RESULTS: The mean testes volume in the study group was 10.81 +/- 3.77 cm3 and in the control one 14.42 +/- 5.42 cm3 (ns). The values of particular flow indices in both study and control group were respectively: S:D 3.20 +/- 0.95 and 2.92 +/- 0.96 (ns), PI 1.46 +/- 0.46 and 1.23 +/- 0.41 (ns), RI 0.69 +/- 0.09 and 0.66 +/- 0.08 (ns). There were no differences between flow parameters in testis after biopsy and contralateral one. Testosterone was the only hormone correlating with analyzed flow indices (S:D, PI, RI) and the correlation coefficient value was respectively 0.51, 0.49 and 0.52 (p < 0.05). CONCLUSIONS: The significant influence of degenerative changes of seminiferous tubules on vascular flow in testicular artery was not observed. There were no differences between flow parameters in testis after biopsy and contralateral one. The testosterone was the only hormone significantly correlating with analyzed flow indices.

[The efficacy of low estrogen transdermal therapy in climacteric symptoms management].

The aim of the study was to evaluate the efficacy and tolerability of low doses estrogen transdermal system treatment in management of climacteric symptoms. On the basis of performed researches it seems to be reasonable to start hormonal replacement therapy with the very low dosage of estrogen to achieve sufficient results and avoid side effects.

[The effect of selected factors on the birthweight of the newborns in gestational diabetes].

Fetal macrosomia is commonly associated with gestational diabetes mellitus (GDM) which may lead to various complications. We attended to establish maternal constitutional and metabolic risk factors responsible for the genesis of macrosomia. 219 GDM mothers were divided into two groups treated by diet or insulin. This study shows that maternal glycemic status and maternal nutritional status before pregnancy are important determinants of fetal macrosomia.

[Factors affecting birthweight in insulin-dependent diabetes mellitus pregnancy].

Fetal growth is affected by various maternal factors. Metabolic disturbances often present in pregnancy complicated by insulin-dependent diabetes mellitus may have important effect on birthweight. The aim of this study was to evaluate the influence of maternal morphometric and metabolic factors on birthweight parameters in women with IDDM. Data were analyzed from 124 diabetic women who delivered in I Department of Obstetrics and Gynaecology University Medical School in Warsaw between 1989-1998. Maternal morphometric factors, glycemic control, insulin requirement, glycosylated hemoglobin and fructosamine levels were correlated with birthweight and birthweight ratios. Glycemic control in first weeks of second part of pregnancy as well as maternal height and weight gain during pregnancy affected birthweight in women with insulin-dependent diabetes mellitus.

[The impact of selected factors on birth weight of newborns whose mothers were treated with fenoterol due to imminent preterm labor].

Various maternal genetic, morphometric and environmental factors may have important effect on birth weight. The aim of this study was to evaluate the influence of maternal pregravid weight, height, body mass index, weight gain during gestation and the length of fenoterol therapy on birth weight in case of imminent preterm labor. The study was performed in the group of 148 pregnant women who delivered after 36th week of gestation as a result of successful tocolytic treatment. Analyzed parameters were correlated with birth weight index and birth weight ratio, also in relation to parity. It was established that birth weight was significantly correlated with maternal morphometric factors but not with fenoterol therapy.
