Priapism--conservative treatment versus surgical procedures.

Fourteen patients with priapism have been treated over the last 12 years. Two patients had a recurrence 2 and 5 years after the primary incident. Abuse of alcohol (often combined with psychotropic drugs) and the use of antihypertensive drugs were the most prominent aetiological factors. Three out of 9 patients treated conservatively and 5 out of 6 patients treated with a shunt procedure have preserved their erection ability. Our results indicate that conservative treatment is not justified and that a shunt procedure should be performed with the least possible delay.

Doppler evaluation of blood flow in therapeutic arteriovenous fistulas.

The flow velocity in 77 patent arteriovenous fistulas was measured with a 5 MHz continuous wave directional Doppler (Parks 906). Ultrasound measurement of the diameter of the brachial artery feeding 56 of the fistulas were at the same time making possible measurements of the flow volume. Comparison between 5 MHz and 10 MHz Doppler demonstrated about 10 per cent lower flow values with the latter equipment. A normal range of flow velocity values (30--70 cm/s) in patent arteriovenous fistulas is suggested.

Percutaneous cytodiagnosis of abdominal masses by ultrasound guided fine needle aspiration biopsy.

A review was made of 168 ultrasound guided fine needle biopsies on 145 patients. Fifty-seven biopsies were from the pancreas, 40 biopsies from the liver, 28 from the kidney, 26 from the abdomen and 17 from the retroperitoneum. The total accuracy rate was 66 per cent varying from 50 per cent in abdominal lesions to 83 per cent in hepatic lesions. The differences of the results are discussed as are the indications and the role of ultrasound guided fine needle puncture compared with other modalities.

Antegrade spermatic phlebography in the diagnosis of metastases from testicular tumors.

Antegrade spermatic phlebography was performed in 18 patients in connection with orchiectomy because of testicular expanding lesions and was compared with other diagnostic methods. Fifteen of the cases were teratomas or seminomas. In 6 patients metastases were demonstrated at phlebography and later confirmed. In one case no abnormality was found at phlebography, nor at CT, but at lymphography metastases were found in not enlarged lymph nodes. Due to technical error at phlebography one case was falsely considered abnormal. The technique is described and its use as a screening procedure for metastases is advocated.

Ultrasound examination of the kidney after Politano-Leadbetter ureteroneocystostomy.

Ultrasound was used in infants and children operated upon with together 62 ureteroneocystostomies to detect complicating obstructive uropathy in the early postoperative period. It was compared with isotope examination and urography. Ultrasound was reliable and accurate in appreciating urinary stasis and should thus be preferred as examination method.

Ultrasound examination of lesions in the thorax.

Thirty-five patients with intrathoracic lesions were examined once or several times during one year with B-scan grey scale ultrasound. The difference between encapsulated fluid, organized exudate or empyema and solid tumor is discussed, as is the procedure of the examination and the possibility of using ultrasound for diagnostic punctures.

Ascending lumbar veins. Catheterization technique and radiographic anatomy.

The normal anatomy of the ascending lumbar veins and a catheterization technique used to demonstrate these veins at phlebography are reported. The use of this examination when dealing with malignant disease in the abdomen and retroperitoneal region is not warranted, on account of the wide range of anatomic and phlebographic variation.

Quantitative Doppler and ultrasound measurements in surgically performed arteriovenous fistulas of the arm.

Quantitative Doppler flow velocity determinations were made combined with ultrasound estimation of the arterial diameter of the feeding vessel in arteriovenous fistulas of the arm used both for dialysis administration of cytostatics and parenteral nutrition. In 83 patients flow velocity alone was determined while the volume flow was calculated in 63. The effect of the length of the dialysis period and age of the fistula on the different parameters is discussed.

Heparinized catheters for long-term intraarterial infusion of 5-fluorouracil in liver metastases.

A heparinized catheter was used for the regional infusion of 5-fluorouracil in seven patients with liver metastases. The hepatic artery was catheterized from the left brachial artery. The period of treatment varied from one to 13 weeks. Pull-out angiograms, obtained after eight catheterizations, revealed complete occlusion of the brachial artery in five and partial occlusion in three, although no patient had ischemic symptoms in the hand. Thus, heparinization of the catheter did not prevent thrombosis during long-term infusion therapy.

Impotence, smoking, and beta-blocking drugs.

Four patients complaining of erectile dysfunction and using tobacco or propranolol were examined with penile blood pressure measurements and Doppler recordings, calculating the penile acceleration ratio (PAR). After a change in regimen, erectile capacity was restored, PAR returned to normal, and blood pressure measurements revealed increased systolic penile blood pressure in three patients. The changes, especially in PAR, indicate a marked penile vascular reaction during smoking and beta-blocking treatment in these men.

Treatment of liver cancer with regional intraarterial 5-FU infusion.

The results of a retrospective three year study of forty-six patients with cancer of the liver treated with regional intraarterial infusion of 5-FU are reported. No primary mortality was noted. Oblective overall remission rate was 43 per cent. Overall median survival from onset of treatment was six months. The one year survival rate was 33 per cent and the two year survival rate 11 per cent. Patients with an objective response had a significantly prolonged survival as compared with nonresponders, especially in the colorectal group: sixteen months versus four months. Survival was not related to tumor size and involvement of the liver. During treatment 42 per cent of the patients developed extrahepatic metastases. Quality of life was improved in 63 per cent of the patients. The results indicate that infusion therapy induces reasonable response and palliation but is inadequate for the control of extrahepatic tumor growth.

Arterial changes during treatment with intrahepatic arterial infusion of 5-fluorouracil.

Changes in the liver arteries ranging from minimal irregularities in occlusions and development of aneurysms were investigated in 27 patients who received intra-arterial infusion of 5-FU in the hepatic artery. Variations in the arterial caliber peripheral to the hilum of the liver were noted in 13 of the patients. Further investigation of such arterial changes may be important in controlling the growth of liver metastases.
