beta-Glucuronidase release from human monocytes induced with aggregated immunoglobulins of different classes.

We studied beta-glucuronidase release from human monocytes induced with aggregated immunoglobulins of the nine different human classes and subclasses. Release was induced in a time and dose-dependent manner by all aggregated IgG subclasses. Aggregated IgA1 caused a greater beta-glucuronidase release than aggregated IgM, IgD, and IgE, but the difference was not statistically significant. Release of beta-glucuronidase was not phagocytosis dependent since inhibition of phagocytosis by cytochalasin B or dihydrocytochalasin B did not diminish enzyme release. On the contrary, cells incubated with cytochalasin B prior to addition of aggregated IgG released approximately twice as much enzyme compared to untreated controls. Enzyme release induced by latex particles, a non-Fc receptor mechanism, was decreased by cytochalasin B. Monomeric IgG1, IgG2, IgG3, and IgG4 inhibited aggregated IgG1 enzyme release in a dose-dependent manner. The ability of monomeric IgG to inhibit beta-glucuronidase release correlated with previous reports describing the binding affinities of monomeric IgG to monocytes, i.e., IgG2 was relatively ineffective compared to the other subclasses. Monomeric IgA, IgE, and pentameric IgM were unable to diminish IgG-induced enzyme release. The data indicate that normal peripheral blood monocytes express predominantly Fc receptors for IgG and that all four IgG subclasses induce the release of the lysosomal enzyme beta-glucuronidase.

Cardiovascular effect of sparteine in anaesthetized dogs with and without blockade of cardiac autonomic nerves.

L-Sparteine sulfate (sparteine, 3 mg/kg) was injected intravenously over 3 min in 12 anaesthetized dogs in order to evaluate the inotropic effect of this antiarrhythmic drug on the heart in intact organisms. There was a discrete decline in left ventricular dP/dtmax which was transient within 9 min. No influence could be detected on right ventricular pressure rise velocity. Aortic pressure increased by about 15% and a parallel increase in left ventricular enddiastolic pressure was observed. Heart rate was slightly diminished. Pharmacological blockade of cardiac autonomic nerves did not influence considerably the effect of sparteine. Pretreatment with captopril and phentolamine did not abolish the vasoconstrictive property which was also present in a reserpine pretreated dog. Sparteine shows no pronounced inotropic effect in the heart in situ. It increases arterial blood pressure by direct vascular constriction.

The impact of morphological features on the prognosis of patients suffering from breast cancer.

A group of 156 patients suffering from breast cancer with axillary metastases was studied for connections between the morphological structure of the tumor and the spreading of the disease on the one hand and duration of remission on the other hand. The statistical analysis shows that the summary WHO-grading proves to be the most indicative prognostic factor next to the number of affected lymph nodes and tumor size. Definition of a prognostic index from these factors leads to a more precise prognostical distinction of the collective.

A patient self-disclosure instrument.

Four studies in which a patient self-disclosure questionnaire was developed and tested are reported. The instrument was designed to include only content judged by both patients and clinicians as important. Three areas of patient difficulty in disclosing to a health care provider were measured: personal problems and feelings, responses to health care, and life style. Patient difficulty in disclosure was assessed by 21 7-point rating scales, 7 for each disclosure area; ratings were summed to obtain scores. A correlation matrix of the ratings of 216 patients was subjected to a three-factor principal components analysis with varimax rotation; the solution accounted for 59% of the total variance. High internal consistency and test re-test reliability for the total scale and subscales were obtained; Cronbach's alpha .82-.83, n = 214; r = .81-.87, n = 66. In separate tests of construct validity patient self-disclosure was shown to be influenced by locus of control and perception of clinician empathy, and to covary with heart rate changes.

Surgical decision in glaucoma simplex.

The indications for 220 trabeculectomies performed by six surgeons at three different centres were studied. 82% of the decisions to operate were based on a non-progressive but high risk situation. The 18% of cases showing preoperative deterioration were characterised by early presentation and low preoperative residual pressures. There was a distinct similarity in the incidence of individual indications at the different centres.

Cyclosporin improves Graves' ophthalmopathy.

Cyclosporin given to two patients with Graves' ophthalmopathy produced a rapid objective improvement in both, including restoration of ocular-muscle function and visual acuity, improvement in exophthalmos, and reduction in orbital muscle swelling judged by computed tomographic scan. Thyroid autoantibody levels fell with treatment, and T-cell subset abnormalities returned towards normal. In one patient an absent thyrotropin response to thyrotropin-releasing hormone became normal after treatment. These results suggest that cyclospirin treatment should be considered in patients with the congestive ophthalmopathy of Graves' disease.

[Can the diagnosis "stress incontinence" be reliably made even without urodynamic clarification?].

Anamnestic data and gynaecological findings of 1036 incontinent women without previous incontinence surgery were examined with the aim in mind to safely establish a diagnosis of stress incontinence. The information given by 234 women (initial group), whose answers were recorded on two different questionnaires, were compared with those of 802 women (control group). By one single information, namely, that urine can be retained for more than three hours, it is possible to predict stress incontinence with an accuracy of 85% in 46% of all incontinent women of the starting group. In a sub-group defined by three data (40% of all women) stress incontinence can be assumed with an accuracy of 90%. As a result, the indication for incontinence surgery in 40% of all examined women could be made with an error rate of 10% only, compared with 26% in the starting group.
