Chelating drugs and zinc.

Essential trace metals participate in a wide range of biochemical processes. Administration of drugs containing nucleophilic atoms, primarily N-, O- and S-ligands, may result in formation of organometallic non-ionic complexes or chelates. The stability of the complex depends on the position of the metal in the first transition series, except for Cu2+, which forms stronger bindings than Zn2+ does. In nature, a wide range of chelates occur, e.g. as vitamin-metal complexes (Zn-thiamine, Zn-pyridoxamine and Zn-biotin). Phytate in unleavened grains forms unsoluble Zn-complexes. In the body, complex-formation is essential for the transport of Zn with circulating plasma albumin. Some examples of chelating drugs are given. Zn-pyrithione is used for local treatment of dandruff and pityrosporum folliculitis. Ethambutol is a strong metal-binding drug used for tuberculosis therapy. The mechanism of action is possibly related to inactivation of Zn and Cu in the pathogenic organisms. 8-hydroxyquinolines enhance Zn absorption, which is the reason for the successful treatment of acrodermatitis enteropathica. Effects of D-penicillamine, phenytoin, tetracycline and disulfiram on Zn metabolism are discussed.

Serum alkaline phosphatase and serum zinc levels in the diagnosis and exclusion of zinc deficiency in man.

In the present study we monitored serum zinc (Zn) and serum alkaline phosphatase (AP) levels during Zn supplementation in (A) a young zinc depletion syndrome (ZDS) patient with severe Zn deficiency, (B) three acrodermatitis enteropathica (AEP) patients with mild Zn deficiency and (C) 7 elderly and 3 younger patients without Zn deficiency. In (A) serum Zn and serum AP values were low, but following parenteral Zn the parameters rose to normal levels (r = +0.79, p less than 0.001). In (B) serum Zn and serum AP levels decreased when oral Zn was stopped, but shortly returned to normal upon reinitiation of Zn. In (C) Zn therapy caused a rise in the serum Zn concentration whereas the serum AP activity decreased (r = -0.01, p greater than 0.1). The results suggest that serial determinations of serum Zn and serum AP during Zn supplementation may be a valid tool in the diagnosis of severe and mild Zn deficiency as well as in the exclusion of the diagnosis.

Human T-lymphotropic retrovirus type III (HTLV-III) in Danish homosexuals.

Sera from 119 patients attending a venereal disease clinic in Copenhagen during May 1983 were screened for antibodies to the newly characterized and isolated retrovirus HTLV-III. Ten out of 45 (22%) homosexual patients and two out of 74 (2.7%) heterosexual patients were found to have antibodies against HTLV-III. The homosexual group (except one case that developed AIDS and died during one year observation period) contained otherwise healthy men some of which had no contact with individuals from high-risk areas and only a few sexual partners. These results strongly suggest that Danish homosexuals with only few contacts are also at risk and that sero-epidemiological studies should also include the healthy homosexual group and probably the heterosexual group attending venereal disease clinics.

The ultrastructure of treponema pallidum isolated from human chancres. Morphologic variations from Nichols' strain.

The ultrastructure of treponema pallidum obtained directly from human chancres by biopsy was studied by electron microscopy. The treponemes were enveloped by a trilaminar cytoplasmic membrane and a trilaminar periplastic membrane. The central part of the periplastic membrane corresponds to the protective mucoid layer. In undamaged organisms bunches of axial filaments were seen to entwine the whole cytoplasmic body without any disruption or overlapping. The number of axial filaments varied between three and four. Identical nose pieces were demonstrable in both ends of the treponemes. Axial filaments and nose pieces seem to differ from those of Nichols' strain.

Pityriasis rosea eruption in secondary syphilis: an isomorphic phenomenon?

A 41-year-old man was referred because of a slightly pruritic eruption, and a classic case of pityriasis rosea was diagnosed on the first examination. Results of routine tests for syphilis were strongly positive, and on repeated examination the eruption had grown further. Skin biopsy specimens showed plasma cells and Treponema pallidum. All symptoms disappeared following antisyphilitic treatment. The possibility of an isomorphic response is discussed.

Bone lesions in early syphilis detected by bone scintigraphy.

We report a case of early syphilis with multiple bone lesions which all resolved after treatment with penicillin. We discuss why bone lesions may be more prevalent than generally believed and why 99m-Tc-MDP-bone scintigraphy is more sensitive than radiography in detecting syphilitic periostitis.

Pseudo-Kaposi sarcoma of the feet: an electron microscopic investigation.

A case of pseudo-Kaposi sarcoma of the feet (acroangiodermatitis) is reported. The patient showed clinical and light-microscopic signs closely resembling Kaposi's sarcoma, but by electron microscopy of skin biopsies definite signs of degeneration of vascular walls and infiltrating cells were demonstrated. Electron microscopy should be applied when conventional histology is inconclusive or fits poorly to the anamnestic information and/or clinical appearance of the lesions.

[Hematogenous contact eczema following oral administration of neomycin].

Systemic contact dermatitis may occur when a previously cutaneously sensitised patient is given the hapten orally or by inhalation or injection. Some patients will develop a distinctive clinical picture with eczematous lesions on the neck, in the axillae and in the genital area. Further, a pompholyx on hands and feet might appear. The disease entity is illustrated in a neomycin-sensitive patient who was accidentally given an overdose of the hapten.

Xanthogranuloma juvenile: a case report.

A case of juvenile xanthogranuloma in a female newborn infant is reported. Red-brown, flat, well-demarcated lesions were present at birth on her scalp, face and trunk. During the following year of observation the eruptions regressed slowly and spontaneously. Light and electron microscopy revealed histiocytes and Touton giant cells dominating the dermal cell infiltrate. The Touton cells contained bizarre invaginated nuclei, lysosomes, a granular endoplasmic reticulum, and cholesterol clefts in their cytoplasm. No fat droplets were seen.

[On the practical value of microscopic examination of urethral and cervical smears in gonorrhoea].

A one-year material of patients with gonorrhoea was made up by 150 men and 91 women. In 104 men a microscopic examination of urethral smears was performed by which a correct diagnosis was made in 88 cases. There were 84,6% correct positive, 11,5% false negative and 2,9% false positive microscopies among the men. In the female group 30 microscopies of urethral and cervical smears were performed of which 7 were diagnostic. There were 23,3% correct positive and 73,3% false negative microscopies. Men had significantly more microscopies done than females and a correct diagnosis was significantly more frequently established in men than in women. Men had significantly more often symptoms of gonorrhoea than women.

[Treatment of condyloma acuminatum with 0.5% 5-fluorouracil-solution, A double-blind clinical trial].

30 males and 29 females with condyloma acuminatum took part in a double-blind randomized trial with 0,5% 5-fluorouracil (5-FU) salicylic acid containing varnish for warts versus the varnish without 5-FU. The females were treated twice weekly at the clinic, the males were treated once daily at home by themselves. The 5-FU solution was found significantly superior to placebo (P less than 0,01). 60% of the 5-FU treated patients were healed with a median healing time of two weeks in the males and three weeks in the females. In the placebo group 27,6% healed with a median healing time of 2,5 weeks (males) an 3,5 weeks (females). Blood tests obtained before and after the trial were normal. Patch tests with 0,5% 5-FU revealed no sensitization after the therapy.

Microbial eczema of the feet.

Twenty-seven patients were investigated for eczema of the feet which mainly affected the medial interdigital spaces and the dorsal areas of the medial toes. The lesions tended to spread to the dorsum of the foot, rather than the plantar surface as seen in tinea pedis, which typically affects the lateral interdigital spaces. In all patients S. aureus and/or haemolytic streptococci were cultured, whereas scrapings for fungi were negative. All responded well to treatment aimed at eradicating bacteria and dermatitis, but relapses occurred in 33% of the cases. We believe this clinical entity is due to infective dermatitis in which the presence of bacteria provokes an eczematous response in the host, probably due to pre-existing sensitization rather than direct infection.
