Recurrent genital herpes on the buttocks: "herpes disciformis".

We report a clinical variety of recurrent genital herpes on the buttocks of seven women ranging in age from thirty-four to eighty-one years. The lesions had recurred for one to nine years and had produced a characteristic cicatricial rounded area within the S2 to S3 dermatomes on one buttock. Herpes disciformis is believed to be an anatomical variant of the common genital herpes infection. We have only noted it in middle-aged or older women with no history of genital herpes.

Syphilis in HTLV-III infected male homosexuals.

Out of 100 male homosexual persons screened for AIDS, 54 were found to be HTLV-III antibody positive. Of these, 34 (63%) had a positive history of syphilis, as compared to 16 (35%) of the 46 HTLV-III antibody negative persons (P less than 0.01). In the HTLV-III antibody positive group 12 (22%) had had more than three syphilis infections as compared to none in the negative group (P = 0.02). The frequency of a previous primary, secondary, and latent syphilitic infection was different in the two groups. During the 1970s and early 1980s the incidence of syphilis in the U.S.A. and in Denmark was increasing, but from 1982 the incidence decreased markedly, possibly a consequence of the fear of AIDS, which appeared in 1981-82. It is speculated that the high frequency of syphilis among homosexual men might have been a co-factor for the acquisition of the HTLV-III infection.

Serum zinc levels during oral glucocorticoid therapy.

The course of serum zinc (S-Zn), plasma albumin (P-Alb), urinary zinc, serum alkaline phosphatase, and plasma alpha 2-macroglobulin levels was monitored in 14 adult hospitalized patients receiving oral glucocorticoid therapy, about 40-50 mg prednisone daily for various skin diseases. Within 3 days S-Zn decreased slightly from 12.6 +/- 2.3 mumol/liter (mean +/- SEM) to 11.1 +/- 2.5 mumol/liter. Then the level rose to about 14-15 mumol/liter and remained elevated, but within the normal range for the next 2 weeks. The P-Alb level showed parallel fluctuations although less pronounced. The S-Zn/P-Alb ratio increased from 0.024-0.029. No consistent patterns could be seen in the fluctuations occurring in the additional parameters studied. The possible role of ACTH on the S-Zn regulation is discussed.

Acute HTLV III infection associated with exanthema, diagnosed by seroconversion.

We report three homosexual men who developed malaise, fever and a roseola-like exanthema which disappeared over 2 weeks. During 3 months follow-up, all three patients felt well and had no symptoms. HTLV III seroconversion was detected several weeks after the acute HTLV III infection. In two of the three cases the Western blot technique was able to detect HTLV III antibodies before they were detectable by the ELISA technique.

Neurosyphilis after treatment of latent syphilis with benzathine penicillin.

Two patients developed neurosyphilis after treatment of latent syphilis with intramuscular benzathine penicillin. The cases confirm recent investigations that show that neither benzathine penicillin nor procaine penicillin in the standard doses results in treponemicidal concentrations of penicillin in cerebrospinal fluid. Neurosyphilis can be treated effectively by intravenous benzylpenicillin infusions of 18 to 21.6 g daily for 15 days.

Treponema pallidum in macular and papular secondary syphilitic skin eruptions.

The ultrastructure of biopsies from dry macular and papular secondary syphilitic skin lesions of 10 patients were studied by electron microscopy. In all biopsies few diffusely distributed treponemes were observed. This may explain the difficulties in demonstrating treponemes by darkfield examination of tissue fluid from dry secondary syphilitic skin lesions. The outlines of treponemes were less distinct as compared to those of primary syphilis. The periplastic membranes were almost invariably absent and the cytoplasmic membranes appeared in close contact with an enclosing layer of irregularly demarcated, electron dense amorphous substance. This substance may be a manifestation of the immune reaction of the host cells to the treponemes. Degenerations were noted in both unmyelinated and myelinated nerve tissue. This accounts for the fact that skin lesions in secondary syphilis are usually without symptoms. Also in the vessel walls treponemes were demonstrated. The vascular endothelial cells were proliferating and the basement membranes were multilaminated and split.

Erythema elevatum diutinum and pre-AIDS.

Erythema elevatum diutinum (EED) is a chronic disease with symmetrical persistent erythematous nodules and plaques primarily in an acral distribution. EED is often associated with infections, especially of streptococcals. An immunological reaction has been proposed as pathogenetic mechanism. We describe a patient, who developed EED secondary to a LAV/HTLV III positive lymphadenopathy syndrome. Immunological investigation of a skin lesion and a lymph node biopsy is described.

Treponema pallidum in human chancre tissue: an electron microscopic survey.

In biopsies obtained from syphilitic chancres of varying ages in 10 patients, a total of 766 ultrathin sections of Treponema pallidum were studied by electron microscopy. The course and number of axial filaments observed reveal that one bunch of 3-4 filaments without interruption entwine the whole cytoplasmic body. In 9.2% of the sections a trilaminar or a fragmentary trilaminar periplastic membrane was observed outside the cytoplasmic membrane and the axial filaments. The occurrence of the periplastic membrane decreased with advancing ages of the chancres. A protective function of the membrane is discussed. A peritreponemal fine reticular halo demonstrable in most fragments is supposed to be due to fixation induced shrinkage of treponemal hyaluronidase-influenced semifluid glycosaminoglycans. Peritreponemal reticular halos were also observed in collagen tissue. A destructive effect of the treponemes on collagen fibres could explain how the organisms penetrate through the collagen rich meninges into the central nervous system. A surface associated narrow border of electron dense amorphous substance, probably originating from the host organism, yields to tangentially cut treponemes a spiny caterpillar-like appearance.
